Abstract

ELEKTROSTIMULATOR SRCA SA ULTRAZVUČNIM DOPLER SISTEMOM, ugradnjom aksijalno polariziranog i jednostrano usmjerenog ultrazvučnog pretvarača na elektrodni kateter za stimulaciju srca, omogućuje se mjerenje protoka krvi oko elektrodnog katetera u smjeru njegove osi pomoću dopler elektroničkih krugova spojenih na spomenuti pretvarač. Željena polarizacija i usmjerenost ultrazvučnog snopa postiže se odgovarajućim ultrazvučnim lećama i reflektirajućim odnosno apsorbirajućim podlogama montiranim uz pretvarač ili više njih. Ako se mjeri krvni protok kroz trikuspidalnu valvulu, tada valni oblik protoka pokazuje val ranog diastoličkog punjenja i valatrijskog punjenja. Elektrostimulator može detektirati pojavu atrijskog vala punjenja te nakon odredjenog intervala kašnjenja stimulirati ventrikul sinkrono sa detektiranim atrijskim valom. Takodje, elektrostimulator može detektirati nestanak atrijskog vala uzrokovanog atrijskom fibrilacijom kao i nestanak vala diastoličkog punjenja izazvanog ili ventrikularnom aritmijom ili gubitkom odgovora na stimulirajući impuls. Vremenski interval od impulsa stimulacije do pojave vala diastoličkog punjenja jest senzor za regulaciju frekvencije u frekvencijski adaptivnom načinu stimulacije. Omjeri vršnih brzina kao i omjeri vremenskih integrala valova punjenja služe za detekciju ishemije izazvane prebrzom stimulacijom.HEART ELECTROSTIMULATOR WITH ULTRASOUND DOPPLER SYSTEM, by installing an axially polarized and unidirectionally directed ultrasound transducer on a pacemaker electrode catheter, it is possible to measure blood flow around the electrode catheter in the direction of its axis using Doppler electronic converter connected circuits. The desired polarization and orientation of the ultrasonic beam is achieved by appropriate ultrasonic lenses and reflective or absorbent substrates mounted next to the transducer or more. If blood flow through the tricuspid valve is measured, then the waveform of the flow shows a wave of early diastolic filling and valatry filling. The electrostimulator can detect the occurrence of an atrial charge wave and after a certain delay interval stimulate the ventricle synchronously with the detected atrial wave. Also, the electrostimulator can detect the disappearance of the atrial wave caused by atrial fibrillation as well as the disappearance of the diastolic filling wave caused by either ventricular arrhythmia or loss of response to the stimulus impulse. The time interval from the stimulation pulse to the occurrence of the diastolic charging wave is the sensor for frequency regulation in the frequency adaptive stimulation mode. Peak velocity ratios as well as ratios of time integrals of charge waves serve to detect ischemia caused by too rapid stimulation.
