Description
JUGULAR VENOUS CATHETER FIXING DRESSING APPARATUS
Technical Field
The invention is related to a jugular venous catheter fixing dressing apparatus, which is designed to prevent problems that may occur as a result of the dislocation of the catheter in patients undergoing jugular venous catheterization in the healthcare sector, has a support surface that prevents falling of the lumens connected to the catheter in the neck of the patient, consists of a transparent section where the entry of the catheter into the vein can be observed, and has the feature of adhering to the skin from its edges.
The State of the Art
Central veins should be used for rapid fluid replacement or administration of hypertonic fluids. For central venous catheterization; the subclavian vein, internal jugular vein, and femoral vein can be used. Internal jugular veins usually collect blood from the areas fed by the branches of the arteria carotid communis and from the organs found in these areas, namely the brain, eye, nasal cavity, oral cavity, face, and neck. The internal jugular veins begin in the fossa jugularis and extend in the spatium parapharyngeum with the arteria carotid interna, nervus vagus, glossopharyngeus and accessorius, and below with the arteria carotid communis and nervus vagus. It slides outwards under the larynx and joins the vena subclavia in the angulus venosus. Expansions called bulbus jugularis superior and inferior are observed in the beginning and end parts of the internal jugular veins.
Catheter-related bloodstream infections are common in intensive care patients and are an important cause of morbidity and mortality. Optimal approaches for the prevention of infection are presented in the guidelines. During the catheter attachment and care, maximal precautions should be taken in the presence of guides. It has been determined that the use of chlorhexidine antiseptic solutions and chlorhexidine gluconate impregnated transparent gel dressings reduces entry site infections, and the use of silver nitrate iv connectors reduces the entry of microorganisms from the catheter lumen and reduces the infection rate.
Dressing materials such as sterile gas compress and Transparent Polyurethane Dressing (TPD) are used in the care of the jugular vein catheter. The sterile gas compress is closed to completely cover the catheter site after the catheter care is performed, and fixed on the skin with a plaster. Although it is frequently used because it is a simple and inexpensive method, the opaque nature of the sterile gas compress does not allow for observing the catheter site. While TPD is more advantageous than sterile gas compresses because it has a transparent structure, allows the catheter location to be observed, and needs to be changed less frequently, its high cost limits its use. In addition, the currently used dressing materials only protect the catheter entry and do not have any features for the fixation of the catheter tips. On the other hand, closing the catheter tips with sterile gas compresses to prevent contamination creates extra weight and volume at the catheter tips, which causes the catheter to hang down, crushing and bending the catheter, and sometimes dislocation of the same. Dislocation of the catheter can threaten the patient's life by creating complications such as vessel damage, infection in the catheter site, and the development of thrombus in the lumens due to catheter damage. Dislocation of the catheter interrupts the patient's current treatment, which is an important obstacle for the patient to perform their vital functions. In addition, it causes the repetition of jugular vein catheterization, which is very difficult to apply and requires experience, causing the patient to experience the risks it brings, and negatively affecting the patient's compliance with the treatment process. Since the jugular vein catheterization restricts the neck movements of the patient due to its location, the patient has difficulty performing daily activities. In particular, the inability to fix the catheter lumens creates a risk of dislocation in the event of a possible error in the patient's activities such as bathing, dressing, and sleeping, causing the patient to be uneasy, and this reduces the comfort of the patient regarding the care process.
Many studies have been carried out and new structures have been developed in order to facilitate the dressing process by fixing the catheter. One of these studies is the invention that is the subject of the utility model application numbered CN201920577515U titled "Venous catheter dressing." The invention is related to an intravenous catheter dressing. The intravenous catheter dressing includes a support layer, wherein the transparent adhesive layer is arranged sequentially on the supporting layer in a perpendicular direction to the supporting layer; the attachment area and the non-adhesive area lie in the plane of the release layer. The non-adhesive area is arranged on at least one side of the attachment area and the attachment area is larger than that of the release layer. Non-adhesive area; the adhesive tape is arranged
on at least one of the release layer and the support layer, and the protrusion of the adhesive tape on the plane in which the release layer is located is at least partially or completely over the joint area. According to the venous catheter dressing provided by the utility model, stability and easiness can be increased for fixing the catheter.
Another study is the invention which is the subject of the utility model application with the number CN202021485734 and title “A new dressing for central venous permanent catheter”. The invention discloses a new dressing for a central venous permanent catheter on which a release paper is arranged on an adhesive layer at the inside of the dressing body as well as a placement bag for placing a reserved catheter on the same. The placement bag and the dressing body are integrally formed; a zipper is arranged on the storage bag for opening the storage bag; the bandage is arranged on the two sides of the dressing body, and the bandages are fastened with a velcro fastener. A waterproof cover is arranged on the zipper; the waterproof cover is attached to the dressing body via a velcro fastener. According to the utility model, the risk of catheter infection caused by wetting the catheter dressings during daily skin cleaning while the patient wears the catheter for a long time is effectively avoided, the infection rate of the patient's permanent central catheter and irritant dermatitis around an incision are reduced.
As a result, the presence of the need for a jugular venous catheter fixation dressing apparatus, which eliminates the disadvantages of the present art, and the insufficiency of the existing solutions made it necessary to make an improvement in the related technical field.

Detailed Description of the Invention
In this detailed description, the jugular venous catheter fixing dressing apparatus (10) of the invention which is designed to prevent problems that may occur as a result of the dislocation of the catheter (11) in patients (20) undergoing jugular venous catheterization (11) in the healthcare sector, has a support surface that prevents falling of the lumens (13) connected to
the catheter (11) in the neck of the patient (20), consists of a transparent section (17) where the entry (12) of the catheter into the vein can be observed, and has the feature of adhering to the skin from its edges is described only as an example for the better understanding of the subject and in the way that creates no limiting effect whatsoever.
The jugular venous catheter fixing dressing apparatus (10) of the invention which is shown in Figure-1, has been developed in order to fix the lumens (13) and prevent problems that may develop due to the dislocation of the catheter (11) in patients (20) undergoing jugular vein catheterization. The purpose of the invention is to reduce the complications such as vascular injury, pain, infection, and thrombus formation that may develop due to the dislocation of the catheter (11), and to prevent the interruption of the patient's (20) treatment, thanks to the safe fixation of the lumens (13) with the jugular venous catheter fixation dressing apparatus (10). In addition, the detection of lumens (13) from the exit point of the catheter (11) reduces the possibility of dislocation that may occur during the neck movement of the patient (20). In addition, in the treatments to be applied to the patient (20) through the lumens (13), the lumens (13) are prevented from hanging down, thus providing convenience to the nurses and patient relatives who will care for the patient (20). On the other hand, problems such as the lumens forcing the exit site of the catheter (11) and causing the dislocation of the catheter (11) in case of a possible failure of the lumens (13) while the patient (20) is performing the daily activities such as putting on and taking off clothes, have been resolved. Another purpose of the invention is to increase the freedom of movement and comfort in daily life by reducing the patient's (20) anxiety by fixing the patient's (20) catheter (11) area with a more durable and safe material.
The jugular venous catheter fixing dressing apparatus (10) shown in Figure-2 includes the dressing (18) which is fixed to the neck region of the patient (20) and has a transparent compartment (17) on which the catheter entry site (12) and the catheter entry site (12) can be observed. The catheter (11) is connected to the lower part of the said catheter entrance (12) and the lumen fixer (19) is connected to the upper part. At the other end of said lumen fixer (19), the lumen (13) is connected. The clamp (15) is positioned on the said lumen (13), while the catheter tip (14) and the cover (16) are positioned at the tip. The dressing apparatus (18), which extends to the said catheter (11) area and further towards the lumens (13), is made of a hydrocolloid material, unlike the dressing apparatus available in present applications. Said hydrocolloid material is generally used for wound care as a dressing, but not for the catheter
site. The said hydrocolloid dressing supports the catheter (11) area better thanks to its adhesive feature and prevents the development of infection in the area by keeping the catheter (11) area dry due to its liquid absorbency. Since said lumen stabilizer (19) fixes each lumen (13) one by one, it supports the lumens (13) inside the jugular venous catheter fixing dressing apparatus (10) without being separate from the jugular venous catheter fixing dressing apparatus (10). Lumens (13) do not hang down, thanks to the lumen stabilizer (13), which fixes the lumens (13) extending at the entrance (12) and the continuation of the catheter in a way that separates them.
