Description
ENDOPROTHESE VASCU AIRE EN PLUSIEURS ÉLÉMENTS POUR ANÉVRISMES INTRACRANIENS VASCUARY ENDOPROSTHESIS IN MULTIPLE ELEMENTS FOR INTRACRANIAL ANEVRISMS
The subject of the present invention is a multi-element stent for intracranial aneurysms.
It is intended to be placed surgically in an artery to locally prevent the risk of internal bleeding caused by the bursting of an aneurysm, and is in particular designed to be used in cervical vessels of very small section.
Arterial aneurysm is a particularly formidable condition which occurs mainly in elderly men and must be followed by a rapid therapeutic sanction: resection of the dilated segment, or occlusion by the surgical route using an internal prosthesis. Indeed, the rupture of the aneurysm constitutes a dramatic accident which can lead to extremely serious irreversible lesions.
Aneurysms are generally the consequence of atherosclerotic lesions which destroy the elastic fibers of the middle coat of the vessel and make the arterial wall fragile. They slowly increase in volume and can rupture, giving rise to a frequently lightning internal hemorrhage or be the cause of thromboses leading to cerebral attacks in the case of arteries supplying the brain.
Le brevet international N° WO 92There are a number of internal prostheses for the treatment of aneurysms. For example, US Patent No. US 4,878,906 filed by MM. Lindermann and Haughton describes a flexible thin-walled plastic sleeve molded with reinforcements and intended to be placed using a catheter and an inflatable balloon. European patent No. EP 221 570 by M. Julio Palmaz and application No. PCT / FR 95/01 460 filed by the author of the present invention state similar devices, however, using malleable metal sleeves. International patent N ° WO 92
Les anévrismes traités à l'aide des trois derniers dispositifs sont occultés au moyen d'un tube ou d'un film en matière synthétique recouvrant l'organe expansible, ce dernier jouant le rôle d'armature.06 734 in the name of M. Young SONG describes a process consisting in compressing a self-expanding prosthesis to introduce it into a tube, the elasticity of the constituent material ensuring its radial extension as soon as it is released. The aneurysms treated using the last three devices are concealed by means of a tube or a film of synthetic material covering the expandable member, the latter playing the role of reinforcement.
These techniques, well mastered for large vessels, are however difficult to apply in the case of cervical arteries because of their small internal diameter. The conventional means used to date to cause occlusion or embolization of intracerebral aneurysms consists in filling the lesion in an attempt to seal it. For this purpose, neurosurgeons use tiny shape memory spirals called "coils", intended to fill the aneurysms and embolize them to avoid their rupture. It is necessary to plan up to ten spirals for an aneurysm. The installation of these devices requires very delicate handling and very long duration, because they must be introduced one after the other, which leads to interventions that can take several hours. In addition, these spirals can be entrained by the blood flow or cause an aneurysm to burst and migrate into the cervical tissue with unpredictable consequences.
The author of the present application has filed, under the number FR 98/01 117, a PCT describing a vascular prosthesis intended in particular for the treatment of intracranial aneurysms and having the shape of a cylindrical sleeve and constituted by a thin metallic ribbon and elastic wound in a spiral whose outside diameter at manufacture is substantially greater than the internal diameter of the vessel to be treated and capable of being compressed by increase in its number of turns to be introduced into a catheter allowing its positioning, the expansion occurring by unwinding of the ribbon when the sleeve is expelled from the catheter by means of a pusher provided for this purpose and sliding in the catheter.
This device can, however, hardly be used for the very small vessels numerous in the brain, and its expansion can be difficult to control rigorously due to its design. In addition, this prosthesis is completely sealed against blood over its entire length and all its periphery and therefore closes the collateral vessels located near the aneurysm.
The invention aims to allow the realization of an internal vascular prosthesis relatively simple to manufacture, usable for vessels of very small section and letting blood pass to the collateral vessels near the aneurysm or coming from these vessels.
The prosthesis consists of a main element formed by a split head ring secured to one or more longitudinal bars, and a series of independent split rings, these rings, all of the same size and whose diameter at the manufacturing is substantially greater than the internal diameter of the vessel to be treated, being intended to be placed contiguously at the location of the aneurysm by means of a catheter tube, the longitudinal bar or bars being arranged across it and serving as support for independent rings.
In the appended drawings, given by way of nonlimiting examples of embodiments of the subject of the invention: FIG. 1 represents, in axono-metric view, the elements of the vascular prosthesis, FIG. 2 is an axial section of a section of blood vessel comprising an aneurysm concealed by a prosthesis according to the invention, FIG. 3 represents a variant of the main element of the prosthesis whose longitudinal bar is offset towards the outside , Figure 4 is an enlargement of the detail D of Figure 3, Figures 5, 6 and 7 show three variants of the main element seen flat, Figure 8 is an axonometric view of a variant of a main element comprising two longitudinal bars, FIG. 9 shows the form taken by the same vascular prosthesis in vessels of different sections and FIG. 10 shows, at the end, a main element disposed in a catheter tube.
The device, Figures 1 to 9, consists on the one hand of a main element 1 formed of at least one longitudinal bar 2 secured to a split head ring 3 transverse and, on the other hand, split rings 4 independent formed of a thin bendable band and having the same dimensions as the head ring.
The stent is assembled in situ by first placing in the vessel 5 the main element 1 and positioning it so that the longitudinal bar 2 is located across the aneurysm 6, then in introducing several independent rings 4, the middle part of which can then bear on the longitudinal bar, the assembly being held in place by the elasticity of the split rings 3, 4 which will be arranged contiguously so that the implant is blood-tight (Figure 2).
La longueur de 1 ' anneau de tête 3 et des anneaux indépendants 4 est déterminée pour q'ils soient ouverts une fois en place dans le vaisseau sanguin, afin de permettre le passage du sang des éventuels vaisseaux collatéraux pouvant se trouver à proximité de 1 'anévrismeThe length of the head ring 3 and of the independent rings 4 is determined so that they are open once in place in the blood vessel, to allow the passage of blood from any collateral vessels that may be near the aneurysm
The longitudinal bar 2 has a length greater than that of the aneurysm 6 to be treated, so that its two ends rest on a healthy part of the wall of the vessel 5.
Selon le procédé de fabrication employé, ce décalage pourra être obtenu par pliage ou par tout autre moyen approprié (figures 3 et 4).In order to allow proper positioning of the independent rings 4, the internal surface of the longitudinal bar will advantageously be offset towards the outside with respect to the internal surface of the head ring 3, by a distance equal to the thickness of the constituent material, so that the independent ring following the head ring can be in the exact extension of the latter. Depending on the manufacturing process used, this offset can be obtained by folding or by any other appropriate means (Figures 3 and 4).
Dans ce cas, les anneaux indépendants 4 peuvent être supprimés (figures 6 et 7).The head ring 3 is preferably located at the end of the longitudinal bar (s) 2, the main element 1 seen flat having the appearance of a tau (FIG. 5). However, it may possibly be arranged substantially in the middle of the length of the longitudinal bar (s), the main element, seen flat, then having the appearance of a cross. In this case, the independent rings 4 can be omitted (Figures 6 and 7).
The longitudinal bar 2 may have rectilinear parallel edges or be in the form of a diamond or an oval so as to ensure better sealing of the aneurysm 6 (FIGS. 5 to 7).
As a variant, the main element 1 may comprise two longitudinal bars 2 arranged symmetrically on the split head ring (FIG. 8), or several very thin bars folding down behind the independent rings 4 so as to obstruct the vessel 5 or to slow the blood flow. The main element 1 and the independent rings 4 can be produced by laser EDM from a metal block or strip, by laser engraving or machining of a thin-walled metal tube or by stamping and placing in shape. The material used will advantageously be of the shape memory type.
The main element can be manufactured in one piece or by assembling by welding or any other means the longitudinal bar and the head ring.
The implantation of the prosthesis is carried out by folding the split rings 3, 4 on themselves so that they have a very small section so as to be able to introduce them inside a catheter tube 7 consisting of a sheath comprising an axially movable pusher, fixed to the end of a flexible operating rod sliding inside the catheter. The components of the prosthesis are successively introduced into the blood vessel 5 using the catheter and brought to the right of the aneurysm 6 to obstruct and then expelled from the catheter by means of the movable pusher. The latter will advantageously consist of a hollow tube and will have a groove in which the longitudinal bar 2 will be housed. The components of the implant can be made integral with the movable pusher by mechanical attachment, by an electromagnetic system, by bonding or by any other suitable means allowing these components to be released at the chosen time, for example by means of an electrical pulse causing electrolysis or a rise in temperature.
The split rings 3, 4 have a shape and an elasticity allowing them to deploy more or less as soon as they are released to press against the internal wall of the vessel 5 and against the longitudinal bar 2 so that the prosthesis is maintained firmly in place and that the aneurysm 6 is completely obstructed.
In order to allow precise positioning of the stent, the end of the catheter 7 will include advantageously radiopaque markers visible by X-rays made up of layers of a heavy metal such as tantalum, titanium or gold deposited electrolytically or fixed by bonding. It may in particular be two circular markers disposed respectively at the two ends of the longitudinal bar (2) and an elongated mark, parallel to the axis of the catheter for checking the position of the prosthesis.
The positioning of the various constituent elements gives the object of the invention a maximum of useful effects which had not, to date, been obtained by similar devices.
