Abstract

Invention concernant un implant inter-vertébral composite destiné à être placé entre les apophyses épineuses et formé d'une coque métallique en deux parties pouvant s'emboîter l'une dans l'autre et d'un noyau en textile synthétique.

Description

IMPLANT INTEREPINEUX A COQUE METALLIQUE DOUBLE
Le dispositif objet du présent brevet vise à stabiliser deux ou plusieurs vertèbres consécutives en les fixant entre elles deux par deux au moyen d'un implant inter-épineux à coque métallique bivalve à noyau textile synthétique. On a proposé pour fixer les apophyses épineuses entre elles des cales interépineuses semi-rigides avec des dispositifs d'amarrage incorporés de mise en place facile.
Ce dispositif n'apporte pas d'a mortissèmen .
De ce fait, on peut craindre qu'il ne génère des vibrations entrainant à la longue des fissures au niveau de l'implantation sur les apophyses épineuses.
L'implant objet du présent brevet est composite.
Il comprend un noyau en matière synthétique tressé qui a un rôle de comblement dont la taille varie en fonction de l'espace interépineux à appareiller et une coque métallique faite de deux parties, qui contiennent le noyau en matière synthétique tressée et évitent son fluage.
Ces deux parties métalliques sont réalisées de manière à ce que la pièce caudale puisse s'emboîter dans la coque craniale et coulisser légèrement à l'intérieur de cette dernière, réalisant ainsi un amortissement lors des mouvements réciproques des vertèbres.
Sur les deux pièces métalliques se fixent des appendices métalliques, deux sur la partie craniale et deux sur la partie caudale, formant des tenons destinés à recevoir et à contenir les apophyses épineuses correspondantes, formant les mortaises.
L'implant est formé de trois parties :
A) Le noyau textile tressé est enroulé sur lui-même de manière à s'adapter à la forme de la pièce métallique craniale dans laquelle il s'emboîte.
B) Une partie métallique supérieure en forme de demi-cylindre creux comportant à décrire :
* une face supérieure en forme de demi-disque dont le coté aplati est antérieur et le coté en demi cercle est postérieur. L'union entre les deux cotés est arrondie de manière à éviter tout angle vif. Sur la partie craniale de cette face s'implantent deux tenons destinés à s'ajuster au bord inférieur de l'apophyse supérieure.
* une face postérieure arrondie en demi-cylindre.
1 * une face antérieure plane se raccordant sans angle vif à la partie postérieure.
* le demi-cylindre ainsi réalisé est évidé à sa partie caudale de manière à recevoir le noyau textile de remplissage. C) Une partie métallique inférieure en forme de plateau formant un demi-disque avec un coté aplati antérieur et un coté semi- circulaire postérieur se raccordant à angle mousse. Ce demi- disque reçoit à sa partie craniale une tige métallique perpendiculaire qui est destinée à se fixer à l'intérieur du noyau textile dans un orifice ménagé à cet usage.
De l'autre coté du demi-dique sont fixés deux appendices formant un tenon destiné à contenir le bord cranial de l'apophyse épineuse de la vertèbre inférieure.
La taille du demi-disque inférieur lui permet de coulisser à l'intérieur de la partie métallique craniale.
La mise en place de l'implant se fait par un abord chirurgical, par voie postérieure, de la colonne vertébrale. L'espace inter-épineux est débarrassé des ligaments inter-épineux et des débris périostes. * les apophyses épineuses sont préparées à l'aide de râpes reproduisant en creux la forme des tenons supérieurs et inférieurs.
* quand la préparation osseuse est suffisante, une famille de fantômes, reproduisant l'encombrement total pour chaque taille d'implant, est utilisée pour choisir la bonne taille de l'implant,
* puis l'implant choisi est ajusté entre les apophyses épineuses à frottement dur,
* la fixation peut être complétée par l'utilisation d'attaches en textile synthétique, passées dans les espaces inter-épineux supérieurs et inférieurs.

REVENDICATIONS
REVENDICATION 1
Dispositif formant une cale inter-épineuse, destiné à stabiliser les vertèbres entre elles et formé d'un noyau en matière synthétique et d'une coque métallique divisée en deux parties pouvant coulisser l'une dans l'autre.
REVENDICATION 2
Dispositif selon la revendication 1 caractérisé en ce qu'une partie de la coque métallique est creuse, destine à recevoir le noyau synthétique et l'autre partie plate fixée dans le noyau textile au moyen d'une tige est destinée à coulisser dans la partie précédente.
REVENDICATION 3 Dispositif selon les revendications 1 et 2. Matériel de pose formé de râpes permettant de préparer les bords des apophyses épineuses.
REVENDICATION 4 Dispositif selon les revendications précédentes. Fantômes reproduisant la taille des différents implants permettant de choisir la bonne taille de l'implant inter-épineux.
