Abstract

本發明係關於一種蘿蔔硫素前驅物一種能夠將該蘿蔔硫素前驅物轉換成蘿蔔硫素之酵素、一種酵素增強劑(enzyme potentiator),以及一種奶薊萃取物或奶薊粉末之組合。本發明亦與一種蘿蔔硫素或蘿蔔硫素之一種衍生物及一種奶薊萃取物或奶薊粉末之組合有關。本發明亦與一種青花菜萃取物或青花菜粉末及一種奶薊萃取物或奶薊粉末之組合有關。本發明提供與該些組合有關之組成物及方法。
天然物的使用在人類及寵物間越來越廣泛。一些天然物會與膳食補充品及醫療用食品結合。本發明所屬技術領域對於可作為化學保護
劑及/或抗氧化劑之補充品有所需求。此外,對於可用在麩胺基硫(glutathione)相關疾病及失調之醫藥組成物及膳食補充品,本發明所屬技術領域亦有需求。為預防或減少影響人類及家養動物之許多疾病發生,透過使用天然物以提供化學保護(chemoprotection)正逐漸成為一種安全、有效、價廉、容易取得且實用的方式。目前已知,可在分子等級對細胞造成損害的致癌物,經常以無毒前驅物的形式被攝取及吸入。這些無毒前驅物之後在體內會轉換成致癌物質。化學保護劑,例如可活化去毒酵素或其輔助因子的天然物質,可抵消致癌物之作用及允許致癌物之清除。這些天然物質亦可增強其他現有的自然防禦機制,例如免疫系統。
某些天然物具有抗氧化活性。在老化、神經退化性疾病之進程,以及諸如局部缺血之生理創傷中,氧化壓力(oxidative stress)扮演了主要角色。抗氧化劑可減少或抑制重要維生生物分子的氧化,亦可在治療、預防或減少氧化壓力相關疾病發生等方面發揮其作用。
某些天然物有助於增進肝臟健康。在美國,非酒精性脂肪肝疾病(Non-alcoholic fatty liver disease,NAFLD)是最常見的肝臟疾病,大約有30%的人口受其影響。NAFLD亦稱肝脂沉積(hepatic lipidosis)。在有NAFLD疾病的人中,10%會發展成非酒精性脂肪變性肝炎(non-alcoholic steatohepatitis,NASH),當中有25%會發展成肝硬化(cirrhosis)。肝硬化患者中,有10~25%可能發展成肝癌(hepatocellular carcinoma)。按照此速度及目前沒有有效治療方式的情況下,預計2030年前肝癌將成為美國診斷罹癌排名第一位。NAFLD的發生是因脂肪在肝臟中沉積(脂肪變性,steatosis),而非由於酒精濫用。NAFLD與慢性炎症、胰島素阻抗(insulin
resistance)、糖尿病及肥胖症有關。NAFLD幾乎沒有或完全沒有症狀,經常是在常規血液測試結果有異常(亦即血清ALT及AST水平升高)後才發現,然後經超音波或組織切片後確認。NAFLD目前無法以手術或藥物治療。控制NAFLD的建議包含調整生活方式,例如健康的飲食、運動、減重、降低膽固醇及控制糖尿病。
NAFLD經常與麩胺基硫(glutathione)水平降低有關。麩胺基硫是一種三肽(tripeptide),其在半胱氨酸之胺基與穀胺酸側鏈之羧基間有一γ肽鍵。麩胺基硫在體內扮演重要角色,因其可作為一種抗氧化劑、解毒劑及免疫調節劑。麩胺基硫可與代謝物及毒素,例如前致癌物(procarcinogens),共軛結合,以從身體排出。在病人體內,麩胺基硫的水平可能因幾種原因而下降,包括不健康的飲食、污染、曝露在毒素及/或某些藥物下、壓力、創傷、老化、感染及輻射。低水平之麩胺基硫會使病人易受氧化壓力、疾病及癌症所影響。舉例而言,麩胺基硫的水平降低與肝臟、前列腺、腦部、肺臟、腎臟、大腸、乳房、食道、胰臟、卵巢等等相關疾病有關。與麩胺基硫低水平及麩胺基硫缺乏有關聯之病症包含但不限於:NAFLD、癌症(肺癌、前列腺癌、大腸癌、乳癌、腦癌、肝癌、卵巢癌、食道癌、胰臟癌、鼻咽癌、骨肉瘤)、白血病、囊性纖維變性、人類後天免疫不全病毒(HIV)、麩胺基硫合成酵素缺乏(glutathione synthetase deficiency)、阿滋海默氏症、帕金森氏症、亨丁頓舞蹈症、肌萎縮性側索硬化(ALS)、Friedreich共濟失調、多發性硬化、纖維肌痛(fibromyalgia)、慢性疲勞、自閉症,以及糖尿病。
蘿蔔硫素(sulforaphane)是被認為具化學保護及抗氧化特
性的天然物之一實例。蘿蔔硫素是一種有機硫化合物,又稱1-isothiocyanato-4-methylsulfinylbutane。蘿蔔硫素前驅物,即蘿蔔苷(glucoraphanin),可從十字花科蔬菜中獲得,例如青花菜、抱子甘藍及甘藍。但要獲得產生化學保護所需的水平,必須食用非常大量的蔬菜。蘿蔔苷係由稱作芥子酵素(myrosinase)的一種硫代葡萄糖苷酵素(thioglucosidase enzyme)轉換成蘿蔔硫素,該轉換發生於各種外因性來源(例如十字花科蔬菜)及內因性之腸道菌相中。然而,不是所有動物都能在攝取蘿蔔苷後將其轉換成蘿蔔硫素,原因很可能是微生物相及整體健康的變化。此外,在諸如胃等酸性環境中,蘿蔔苷可能被轉換成惰性代謝物。活性代謝物蘿蔔硫素能夠誘發核轉錄因子nuclear factor erythroid-2-related factor(Nrf2),Nrf2會轉而上調(upregulate)第二階段去毒酵素(Phase II detoxification enzymes)及細胞保護酵素(cytoprotective enzymes)的產生,例如麩胺基硫轉移酵素(glutathione S-transferases)、氧化還原酵素NAD(P)H:quinine oxidoreductase(NQO1),以及第一型血基質氧化酵素(heme-oxygenase-1,HO-1)。蘿蔔硫素已被認為可誘發這些酵素產生而不會顯著改變細胞色素P-450酵素的合成。第二階段酵素的上調被認為在各種生物活性中均發揮了作用,包含保護腦部不受細胞毒性影響、保護肝臟不受脂肪蓄積之毒性作用影響,以及其他各種組織之去毒。

奶薊(Silybum marianum)是一種菊科植物。奶薊含有水飛薊素(silymarin),水飛薊素由數種成分組成,包含但不限於黄酮木脂素(flavonolignans),例如水飛薊賓(silibinin,亦稱silybin或silibin)、異水飛薊賓(isosilibinin)、水飛薊亭(silichristin)、水飛薊寧(silydianin)、檞皮素(kvercetin)、脫氫水飛薊賓(dehydrosilybin)、去氧水飛薊亭(deoxysilycistin)、去氧水飛薊寧(deoxysilydianin)、silandrin、silybinome、silyhermin及neosilyhermin。水飛薊素之成分具有若干生物效應,包括抑制自由基形成、結合自由基群組(free radical species)、防止細胞膜脂質過氧化反應、增加麩胺基硫之水平,以及鐵之螯合作用。水飛薊賓為水飛薊素之主要活性成分,水飛薊賓被認為具有護肝特性。有關水飛薊素之討論見美國專利7,563,779號,該專利之全部內容茲以此述及方式納入本說明書。
Zhang等人在Proc.Natl.Acad.Sci.,(1994),Vol.91,pp.3147-3150之文章中討論了一項對SD大鼠進行之試驗,以測定蘿蔔硫素及與其結構相關之合成降莰基異硫氰酸酯(norbornyl isothiocyanates)之抗致癌活性。該試驗測定出,給予服用蘿蔔硫素在阻斷乳腺腫瘤形成方面有效果。

Munday等人在Cancer Res,(2008),Vol.68(5):pp.1593-1600之文章中討論了一項對大鼠進行之試驗,該試驗係關於青花菜芽之一種冷凍乾燥水萃取物對膀胱癌進展之效應。該試驗發現,給予服用該青花菜芽萃取物可顯著誘發膀胱中的麩胺基硫轉移酵素及氧化還原酵素NAD(P)H:quinine oxidoreductase(NQO1),該些酵素係對抗氧化物及致癌物施以防護活性之酵素。
Aghazadeh S等人在Exp Toxicol Pathol.(2011)Sep;63(6):569-74之文章中討論在實驗性脂肪變性肝炎之治療中,奶薊之抗凋亡及抗發炎作用。該研究發現,相較於僅以甲硫胺酸與膽鹼缺乏(methionine and choline deficient,MCD)飲食飼養之控制組大鼠而言,給予用MCD飲食所飼養以誘發非酒精性脂肪變性肝炎之大鼠服用一種奶薊萃取物,可改善其體內AST及ALT之活性並增加麩胺基硫之含量。
歐洲專利申請案2213280號揭露了包含硫代葡萄糖苷(例如蘿蔔苷)及芥子酵素之配方,其中該配方係封入膠囊或以膜衣包覆。

蘿蔔硫素亦稱1-isothiocyanato-4-methylsulfinylbutane。蘿蔔硫素之衍生物包含但不限於蘿蔔硫素之次硫酸硫代氨基甲酸鹽類似物(sulfoxythiocarbamate analogues)、6-methylsulfinylhexyl isothiocyanate(6-HITC),以及包含蘿蔔硫素結構,但在異硫氰基(isothiocyanato group)及亞碸基(sulfoxide group)間有不同側鏈及/或不同間距基長度之化合物。蘿蔔硫素衍生物之範例包含以下參考資料所述者,各該參考資料之全部內容茲以此述及方式納入本說明書:Hu等人發表於Eur J Med Chem,2013,64:529-539之文章;Ahn等人發表於Proc Natl Acad Sci USA,2010,107(21):9590-9595之文章;Morimistu等人發表於J.Biol.Chem.2002,277:3456-3463之文章;以及Baird等人發表於Arch Toxicol,2011,85(4):241-272之文章。

在本說明書中,[受試者」一詞係指任何動物,包含哺乳類及鳥類。哺乳動物包含但不限於人類、狗、貓、馬、牛、駱駝、象、獅、虎、熊、海豹,及兔。在較佳實施例中,該些受試者包含非食用類之哺乳動物,例如人類、貓及狗。
以下為一示範性配方:含蘿蔔苷之青花菜萃取物(濃度大約12% w/w),50mg至5g
含芥子酵素之冷凍乾燥青花菜芽粉末,25mg至500mg
抗壞血酸,5mg至500mg
奶薊萃取物(大約20%至35% w/w之水飛薊賓),25mg至5g
一種疏水性交互作用層析(hydrophobic interaction chromatographic,HILIC)法已研究出來,該方法包含以下條件:層析管柱:沃特斯公司(Waters Corp.)之BEH(ethylene bridged hybrid)醯胺,粒徑1.7-μm;內徑2.1mm x 100mm

上述條件允許分離五種典型之十字花科硫代葡萄糖苷,包含蘿蔔硫素前驅
物、蘿蔔苷。
在有可變濃度之抗壞血酸(範圍從0至600μmoles/Liter)環境下,以取自青花菜芽之固定濃度之芥子酵素,將含有約12%(w/w)蘿蔔苷之大約250mg青花菜種子萃取物加以水解。使該些反應混合物保持在38℃之恆溫下60分鐘,並每15分鐘取出等份之實驗試樣(aliquots),以層析方式測定蘿蔔苷之濃度。蘿蔔苷之消耗速率被解釋為蘿蔔苷轉換成蘿蔔硫素之速率。將蘿蔔苷含量減少作為抗壞血酸濃度增加之函數以圖表呈現時產生了一系列之線性圖;該些線性回歸線之斜率反映了蘿蔔苷之消耗速率,其單位為μmoles/minute。明顯的是,在濃度600μmoles/Liter之抗壞血酸環境下,其反應速率較缺少抗壞血酸之調節作用時之反應速率增加到13倍。
茲進行由兩部分組成之一項實驗,以進一步闡明抗壞血酸在調節芥子酵素活性中的作用。所有溶液均以20mM TBS緩衝液(Tris-buffered saline)製備,pH值為7.5,該pH值之前已被指出最適於芥子酵素之活性;每一取樣管內裝有精確稱重之冷凍乾燥青花菜粉末100mg作為芥子酵素之來源。該實驗在38℃下進行2小時,每隔30分鐘移除等分之試樣,且蘿蔔苷及蘿蔔硫素之含量皆以HPLC法測定。一種強酸性之反應停止液被用於即刻抑制被移除等份試樣中芥子酵素之進一步活性。一控制樣本不含抗壞血酸,且未以輔助因子協助酵素轉換之進行。
第一部分。在濃度固定為1mmol/Liter之抗壞血酸環境下,加入青花菜種子萃取物(含大約12% w/w之蘿蔔苷),份量從250mg增加至500mg。
第二部分。將青花菜種子萃取物之量固定在250mg,使抗壞血酸之濃度從0.4mmol/Liter改變至3.8mmol/Liter。
下表將蘿蔔苷(GR)及蘿蔔硫素以μmole為單位表示。明顯的是,在幾乎全部的反應混合物中,蘿蔔苷轉換成蘿蔔硫素在前30分鐘內便告完成。然而,控制樣本在沒有抗壞血酸(AA)之刺激作用下所發生之酵素轉換經仔細檢驗後顯示,蘿蔔苷等莫耳轉換成蘿蔔硫素,亦即所消耗之蘿蔔
苷之量,相當於所產生之蘿蔔硫素之量。
該實驗之第二部分評定濃度漸增之抗壞血酸對芥子酵素活性之調節作用。可觀察到的是,在芥子酵素之促進下,蘿蔔苷轉換成蘿蔔硫素一開始明顯呈線性,直到抗壞血酸濃度達2mmol/L,之後便顯著趨於平穩。
最後,在該實驗第一部分之30分鐘後,檢驗蘿蔔硫素之產出,其顯示在濃度1mmol/Liter之抗壞血酸環境下,100mg冷凍乾燥青花菜芽粉末中所含固定份量之芥子酵素,能夠以可預測之線性方式產生至少200μmole之蘿蔔硫素。圖1、2、3及4呈現了本實驗之結果。
本實例之實驗使用模擬腸液(SIF)粉末,其係一種商業供應之濃縮物,在組成、pH值及離子強度方面十分接近人類之腸內容物。該實驗使用美國藥典(USP)溶離試驗儀2(Dissolution Apparatus 2(槳形)),有500mL之模擬腸液分配至該試驗儀之六個溶解容器,並以150mg之冷凍乾燥青花菜芽粉末作為芥子酵素之來源。在容器1~4中,抗壞血酸之濃度從0.25mmol/Liter至1.00mmol/Liter不等;在容器5中,除濃度1mmol/Liter之抗壞血酸外,還懸浮有3.125g之胰酵素(8x USP);在容器6中,除濃度lmmol/Liter之抗壞血酸及3.125g之胰酵素(8x USP)外,還加有雙倍份量之冷凍乾燥青花菜芽粉末(300mg)。在該些容器溫度達到38℃後,將富含蘿蔔苷(12%,w/w)之250mg青花菜種子萃取物加入每一容器,並將以此得到之懸浮液以75RPM之轉速攪動2小時。每隔15分鐘取出等份試樣並測定其蘿蔔硫素含量。圖4呈現蘿蔔硫素較大產出與抗壞血酸較高濃度間之直接相關,尤其是在該實驗之前期階段。
以下所進行之試驗係為測定蘿蔔硫素及水飛薊賓之組合對麩胺基硫水平之作用。麩胺基硫在體內扮演重要角色,因其可作為一種抗氧化劑、解毒劑及免疫調節劑。低水平之麩胺基硫會使病人易受氧化壓力、疾病及癌症所影響。因此,增加麩胺基硫之水平能帶來有益影響。
在該試驗中,將人類肝癌細胞株HepG2細胞分別以二甲基亞碸(DMSO,為載體控制組(vehicle control))、蘿蔔硫素(SFN)、水飛薊賓(Silib)、蘿蔔硫素及水飛薊賓之組合,處理24小時。將細胞萃取液(cell
lysate)加以收集,並使用鄰苯二甲醛(o-phthalaldehyde,OPT)作為螢光試劑來測量麩胺基硫之水平。圖5及圖6呈現該試驗之結果。
在該試驗之第一部分,將0.5μM蘿蔔硫素之效果與不同濃度之水飛薊賓及不同濃度之0.5μM蘿蔔硫素與水飛薊賓之組合加以比較。詳細而言,該些細胞係以下列其中之一加以處理:(i)DMSO(載體控制組)、(ii)0.5μM蘿蔔硫素(SFN)、(iii)100μM水飛薊賓(Silib)、(iv)200μM水飛薊賓(Silib)、(v)300μM水飛薊賓(Silib)、(vi)0.5μM蘿蔔硫素(SFN)及100μM水飛薊賓(Silib)、(vii)0.5μM蘿蔔硫素(SFN)及200μM水飛薊賓(Silib),以及(viii)0.5μM蘿蔔硫素(SFN)及300μM水飛薊賓(Silib)。其結果顯示,與各成分單獨存在之情況相較,蘿蔔硫素與水飛薊賓之組合在每一測試用量下都具有協同效果。舉例而言,在該些細胞係以單獨成分處理之情況下,與DMSO(載體)控制組相較,單獨以蘿蔔硫素處理時,其麩胺基硫之水平皆相同,而單獨以水飛薊賓處理時,其麩胺基硫之水平稍微降低。然而,當該些細胞係以蘿蔔硫素與水飛薊賓之組合處理時,在每一測試用量下,其麩胺基硫之水平與控制組相較,皆有協同式之增加。麩胺基硫之水平增加為有益之影響。此部分試驗之結果呈現於圖5。
在該試驗之第二部分,將2μM蘿蔔硫素之效果與不同濃度之水飛薊賓,以及不同濃度之2μM蘿蔔硫素與水飛薊賓之組合加以比較。詳細而言該些細胞係以下列其中之一加以處理(i)DMSO(載體控制組)、(ii)2μM蘿蔔硫素(SFN)、(iii)100μM水飛薊賓(Silib)、(iv)300μM水飛薊賓(Silib)、(v)2μM蘿蔔硫素(SFN)及100μM水飛薊賓(Silib),以及(vi)0.5μM蘿蔔硫素(SFN)及300μM水飛薊賓(Silib)。其結果顯示,
與各成分單獨存在之情況相較,蘿蔔硫素與水飛薊賓之組合在每一測試用量下都具有協同效果。舉例而言,在該些細胞係以單獨成分處理之情況下,與DMSO控制組相較,單獨以蘿蔔硫素處理時,其麩胺基硫之水平皆相同,而單獨以水飛薊賓處理時,其麩胺基硫之水平稍微降低。然而,當該些細胞係以蘿蔔硫素與水飛薊賓之組合處理時,在每一測試用量下,其麩胺基硫之水平與控制組相較,皆有協同式之增加。麩胺基硫水平增加有益於細胞之去毒。此部分試驗之結果呈現於圖6。
患有非酒精性脂肪肝疾病(NAFLD)之一受試者經歷之症狀包括不舒服、疲倦及腹部不適。給予該受試者服用含有蘿蔔苷、芥子酵素、抗壞血酸及一種奶薊萃取物之一藥錠。該藥錠為一種腸溶膜衣配方,會將該些內容物在小腸中釋放。每日服用該藥錠一個月後,該受試者感到多種替代性生物標記(surrogate biomarkers),包含與症狀改善相關之麩胺基硫,均獲得了調節。
