The invention relates to method and device for osseous tissue growth. In the method osseous tissue is uncovered by excising a flap of soft tissue over the site where the osseous tissue growth shall take place. A domed spacer is located at said site, and the flap is put back over the spacer which maintains a cavity between the flap and the osseous tissue. The device comprises a shape permanent domed spacer which is intended to be located over the uncovered portion of the osseous tissue in order to define said cavity over the surface of the osseous tissue.

Description

503 400 2 10 15 20 25 30 35 DE-Al-42 23 153 describes an implant for attachment of a denture to the jaw. This device comprises a stand designed as a grid, which attached to the surface of exposed bone tissue by pointing pins on the stand are driven into the bone tissue. The stand is covered with a membrane of porous polytetrafluoroethylene or other equivalent material, and this membrane abuts with its outer peripheral portion against the surface of the exposed bone tissue.

The stand comprises a central portion with one disposed therein threaded bore for attaching a threaded element.

The stand and the membrane placed over it are covered soft tissue. The purpose of this known device is that inside the frame under the diaphragm there should be a bone growth for anchoring the stand to the jawbone.

Den täta anslutningen vid membranets periferi är till för att för- hindra inväxningen av bindvävnad mellan membranet och benvävnadens yta, varigenom bentillväxten i kaviteten skulle kunna äventyras.WO-A1-94 / 03121 describes a surgical element and a method of selective tissue regeneration, wherein a cavity in the bone tissue is covered with a dimensionally stable ment, which closely adheres to the surrounding surface of the bone tissue the cavity. The membrane is covered with soft tissue, and in it thus the cavity separated from the soft tissue becomes a bone tissue growth to fill the cavity. The dense the connection at the periphery of the membrane is there to prevent the ingrowth of connective tissue between the membrane and the surface of the bone tissue, whereby the bone growth in the cavity could be jeopardized.

Ändamålet med föreliggande uppfinning är att åstad- komma en anordning av detta slag för åstadkommande av benvävnadstillväxt på en benvävnadsyta utan infiltration av 15 20 25 30 3 503 400 mjukvävnad, och i detta syfte har anordningen enligt uppfinningen erhållit de kännetecken, som framgår av patentkravet 1.EP-A2-0 574 091 describes a dimensionally stable dome formed spacers, which is impermeable to cells and is intended to be placed over an exposed portion of bone tissue and at its base is attachable to the bone tissue below complete seal between this and the spacer element for to define a cavity over the surface of the bone tissue. The object of the present invention is to provide come a device of this kind for the purpose of bone tissue growth on a bone tissue surface without infiltration of 15 20 25 30 3 503 400 soft tissue, and for this purpose the device according to the invention has obtained the features which appear from claim 1.

For a more detailed explanation of the invention, reference is made to attached drawings, on which Fig. 1 is a vertical sectional view of a dome format spacer elements in an embodiment according to the invention, Fig. 2 is a vertical sectional view of the spacer element in Fig. 1 in implanted condition and Fig. 3 is a vertical sectional view of the dome-shaped the spacer element according to the invention in another performance.

The spacer element shown in FIG. 1 comprises a spherically curved cap 10 and a cylindrical base portion 11.

På det sistnämnda partiet är pà avstånd ovanför baskanten anordnad en radiellt utskjutande runtomgáende fläns 12.On the latter part is at a distance above the base edge arranged a radially projecting circumferential flange 12.

The spacer element must be made of a tissue belt material, and a suitable material is titanium. Mate- however, the material may also be a resorbable material, for example an aliphatic polyester. The spacer element shall be dimensionally stable, which in the present case means that it can be but does not have to be stiff but can be off such a condition that its shape can be changed and that it then the imparted shape retains the time required for the bone tissue to grow below the spacer element and fill the cavity, Finally, the spacer element should be unbroken in order to bounded by the same. form an impenetrable barrier but it is also possible to have a perforated spacer with so dimensioned perforation, that cells from surrounding tissue cannot pass through the same. It is not necessary, that the spacer element is rotationally symmetrical, but it may have oval or oblong shape. 503 400 4 W U 20 25 30 35 When implanting the spacer element, FIG. 2, loosely cut a lambda 13, comprising bone membrane and connective tissue, from bone tissue 14, so that its surface 15 is exposed. IN the bone tissue is created by means of a trepan drill a circular groove 16 in the surface of the bone tissue, which is so dimensioned that the cylindrical base portion 11 can be received therein the flange 12 in abutment with the surface of the bone tissue, as shown in FIG. 2. The spacer element can be clamped in the groove to be held in place and to abut against one or the other second side surface of the track. It can also be arranged one sealing element between the flange 12 and the bone tissue surface 15.

The essential thing in this context is that the spacer element completely tightly adheres to the bone tissue surface 15. When the spacer the element is applied in this way, it is covered by the lambàn 13 is placed over the spacer element and sewn on surrounding soft tissue. Below the spacer element is now delimited placed a completely closed cavity towards the environment over the laid bone tissue surface, and in this cavity comes growth of bone tissue to occur, so that the space under the spacer will be completely supplemented with bone tissue 17.

10 15 20 25 30 35 5 503 400 Därefter lades lambån tillbaka över distanselementet och fastsyddes vid omgivande mjukvävnad.EXAMPLE On a rabbit, a central incision was made down to the surface of the rabbit's skull and a lamb was lifted off connective tissue and bone membrane for exposing the skull bone on both pages about the cut. A spacer element according to FIG. 1, made of titanium with an inner diameter of 4.5 mm and an inner height of 3.0 mm was applied to the skull bone through it by means of a tripod drill with an inner diameter of 5.0 mm was taken up a 0.6 mm circular groove. The spacer element was fitted with the base portion ll in the groove and obtained a tight fit in this on due to the difference in inner diameter between the tripod drill and baspartiet ll. Through the close abutment between the inside of the base portion and the inner boundary wall of the groove secure fixation of the spacer element on the skull bone is achieved. 10 15 20 25 30 35 5,503,400 Then the lamb was put back over the spacer and was attached to surrounding soft tissue.

After a healing period of three months was killed rabbit and examined histological cross-sections through the skull with the spacer in place. These studies showed complete filling of the cavity below the spacer element with bone tissue without any signs of ingrowth of another type of tissue. This gives at hand, that the use of a spacer that is completely tight has sufficient rigidity and stability and is on reliable method attached to and sealed to the bone tissue at its periphery provides a predictable bone growth in spaces outside the skeletal surface. Although the spacer used in the above specified experiments, was completely dense, it has shown that bone growth can be further stimulated and thus considerably accelerated by the fact that the spacer element has perforation in the cap 10, this perforation being necessary be so dimensioned that it allows nutrient flow through perforation at the same time as penetration of cells from soft tissue is prevented. It should also be possible to stimulate bone growth by mechanically penetrating bone the tissue surface to thereby facilitate the migration of bone-forming cells into the space delimited by the spacer element the space.

In the embodiment of the spacer element according to the finding, shown in FIG. 3, is that of a spherical one arched cap 10 ' seal 18 of soft material, for example silicone rubber, at consisting of the spacer element arranged with a the base edge, the edge portion being received in a groove in the seal. A screw 19 is passed through a central hole in the spacer element with the screw head recessed in it, and by screwing this screw into the bone tissue, the the tensile element is attached to the bone tissue with the seal 18 pressed to a tight abutment against the surface of the bone tissue.
