Description

466 732 _ z 10 15 20 25 30 35 struktivt enkel, hållbar och lättrörlig, att det inte uppstår något slitage, att det erhålles en säker, var- aktig och skonsam förankring i benen och att ledprote- sen är lätt utbytbar.
Ledprotesen enligt uppfinningen har i angivet syfte erhållit de kännetecken som framgår av patent- kravet l.
För närmare förklaring av uppfinningen, varvid även ytterligare därigenom uppnådda fördelar kommer att anges, hänvisas till bifogade ritningar, på vilka FIG l är en sidovy, delvis i vertikalsektion, av en fingerledsprotes enligt uppfinningen, FIG 2 är en planvy, delvis i horisontalsektion av fingerledsprotesen i FIG l, FIG 3 är en sektionsvy, som visar fingerleden in- opererad mellan två ben, FIG 4 är en ändvy av infästningsplattan på tappen i ett utförande och FIG 5 är en motsvarande vy av ett annat utföran- de, samt FIG 6 visar ett modifierat utförande av ledkrop- pen av elastomert material på samma sätt som FIG l.
Med hänvisning till FIG l - 5 innefattar den där visade fingerledsprotesen en ledkropp l0 av en elasto- mer, som kan“ítgöras Denna ledkropp är förbunden med ett par tappar ll av ett biokompatibelt material, företrädesvis titan, som är infästa i ledkroppen genom att denna är formsprutad omkring ändflänsar l2 på tapparna, så att dessa änd- flänsar är fullständigt inbäddade i elastomermateria- let. För att en tillförlitlig infästning av tapparna skall erhållas, är flänsarna utförda antingen med slitsar l3 enligt FIG 3 eller med hål l4 enligt FIG 4, så att elastomermaterialet vid formsprutningen kan 10 15 20 25 30 35 Q 466 752 tränga genom dessa,öppningar. Ledkroppen bi1dar ett par p1attformiga rektangu1ära ansvä11ningar 15 omkring f1änsarna och bi1dar me11an dessa ett re1ativt tunt 1iv 16 med svag väïvning.
Vardera tappen 11 är inskjuten i en rörformig skruv 17, som 1ika1edes ska11 bestå av ett biokompati- be1t materia1 och 1ämp1igen är av samma beskaffenhet som tapparna, dvs den bör bestå av titan. Skruven har en ändf1äns 18 och skruvas in i kana1en i det ben, vari den ska11 förankras, såsom framgår av FIG 3, ti11s f1änsen an1igger mot benet. Skruven växer fast i benet, vi1ket medför att benet inte bryts ner på grund av ojämn be1astning och att protesen kan utsättas för större be1astningar under 1ängre tid, så att även yng- re patienter med aktivt 1iv kan få en protes, som fungerar avsevärt bättre under 1ång tid än proteser av känt utförande. Borrningen i skruven är ti11räck1igt 1ång för att tappen inte ska11 bottna i densamma, då den är he1t inskjuten i borrningen. Monteringen av 1edprotesen under1ättas av att tapparna 11 kan skjutas in i skruvarna 17,_sedan dessa har infästs i benen.
Efter monteringen hå11es tapparna intryckta i skruvar- na av den me11an dem anordnade e1astiska 1edkroppen 10. Eventue11t kan tapparna 1åsas i skruvarna efter monteringen. Eftersom det under användningen av prote- sen inte förekommer någon röre1se me11an tapparna och skruvarna, b1ir s1itaget på protesen minima1t, varige- nom 1ivs1ängden b1ir stor och risken för infektioner och irritationer på grund av att partik1ar från prote- sen kommer ut i kroppens vävnader, är praktiskt taget he1t e1iminerad. En annan förde1 är att 1edkroppen med ti11hörande tappar genom en enke1 operation 1ätt kan bytas ut med skruvarna kvarsittande i benen, om 1ed- protesen av någon an1edning inte sku11e fungera ti11- fredsstä11ande. Tappen kan vara cy1indrisk, så att den 466 732 .4 10 15 20 25 30 35 är vridbar i skruvenïgvarigenom protesen blir lättare att injustera, men vridbarheten medför också risk för att protesen kan vrida sig ur sitt rätta läge under användningen. Även om det vridbara utförandet blir billigare än ett utförande, där medel är anordnade för att förhindra vridning, kan det därför eventuellt vara att föredraga att tappen är så styrd i borrningen, att den inte kan vridas däri. Detta kan uppnås genom att ett utsprång på tappen eller i borrningen förskjutbart upptages i ett spår i borrningen resp tappen eller ge- nom att tappen och borrningen har passande orund form.
Exempelvis kan tappen och borrningen ha sexkantig tvärsektionsform, vilket medför den fördelen, att skruven kan dragas åt vid inskruvningen medelst en i dess borrning ingripande tappnyckel. I annat fall får skruven utformas med skruvmejselskåra eller med sex- kantig fläns, för att man skall kunna få grepp på den vid inskruvningen.
Tack vare den elastiska ledkroppen l0 är ledpro- tesen återfjädrande, vilket är viktigt vid behandling av reumatiska patienter. Utan âterfjädringen tenderar patientens fingrar att stelna i en böjd position.
Dessutom tar den elastiska ledkroppen upp stötar och vibrationer, som.därigenom dämpas med ökning av håll- fastheten hos protesen och dess infästningar som följd.
Ledkroppen l0 kan utformas även på det sätt som framgår av FIG 6, enligt vilken livet är tunnare och valvet på undersidan av livet är högre. En sådan led tjänar bättre än leden i det först beskrivna utföran- det till att ersätta den del av benet i den naturliga fingerleden, som bildar knogen, men kan å andra sidan medföra den olägenheten, att vävnad växer in i det hö- ga valvet på undersidan och därvid kan hämma ledbarhe- ten genom att vävnaden klämmes in under ledkroppen samtidigt som livet på sin översida kan bli utsatt för 10 15 20 25 30 35 § 466 732 större dragpåkännlngar. Det är emellertid möjligt att utforma livet på olika sätt för bästa möjliga anpass- ning av fingerledsprotesen till förhållandena på det ställe, där den skall inopereras.
Den beskrivna ledprotesen är billigare och enkla- re att tillverka än sådana proteser som innefattar element, vilka glider mot varandra, vilket innebär att protesen kan komma till användning inom en större pa- tientkategori än de dyrare lederna.
Uppfinningen har exemplifierats genom beskrivning av en fingledsprotes, men det skall understrykas, att uppfinningen kan tillämpas även på andra leder av gångjärnstyp, såsom handleder, armbågsleder och fot- leder.

Claims (8)

Ledprotes, innefattande en ledkropp (10) mel- lan ett par tappar (ll) fdr infästning i de ben, mel- lan vilka ledprotesen skall vara anordnad, k ä n n e - t e c k n a d av kombinationen av ett som ledkropp (10) mellan tapparna (ll) anordnat liv (I6) av elasto- mert material, vari tapparna (ll) är infästa medelst i materialet inbäddade, mot tapparna tvärställda flänsar (12), och en för vardera tappen (ll) anordnad, i benet infästbar rörformig skruv; vari tappen är inskjuten, varvid tappar och skruvar bestär av ett biokompatibelt material.
