SUBSTANCE: invention concerns medicine, particularly dermatology, and can be applied in psoriatic patient treatment. Method involves basic therapy including administration of desensitising and antigistamine substances and vitamins, physiotherapy and ointment therapy. General systemic medications and symptomatic therapy are prescribed if required. Additionally 10% aqueous solution of purple alfalfa herb extract is administered orally by 5 ml 3 times per day. 10% Luceron ointment of purple alfalfa herb extract is applied onto lesion loci 1-2 times per day. Treatment course comprises 20-35 days. Collagen plates with purple alfalfa extract Luceron are applied onto resorption-resistant psoriatic patches beginning from 12th-17th treatment day twice or thrice for 48-72 hours.

EFFECT: efficient treatment due to anti-inflammatory effect and general immunity activation, reduced duration of treatment.

The invention relates to medicine, namely to dermatology, and can be used to treat patients with psoriasis.

Psoriasis is a chronic inflammatory dermatosis of multifactorial origin with many “triggering” factors. In recent years, psoriatic disease is also considered as an autoimmune disease with an inflammatory reaction in the dermis, changes in various organs and systems.

Many methods for the treatment of psoriasis have been developed, however, the persistence of the course, the lack of reliable methods of treatment for this dermatosis still remain an urgent problem of improving existing methods and finding new drugs for the treatment of psoriasis patients.

A known method of treating psoriasis [Patent RU 2302193, 07/10/2007], including photochemotherapy 4 times a week for 4 weeks. In this case, prolactin and thyroid-stimulating hormone are determined in the blood. In parallel with photochemotherapy, parlodel is prescribed for certain indicators. However, the presence of contraindications, side effects and restrictions on the use of parlodel, as well as the duration of treatment (more than 6 months) limit the use of the method.

There is a method of treating psoriasis [Patent RU 2290926, 01/10/2007], including desensitization, detoxification therapy with enterosorbent, from 10 to 30 general photochemotherapy procedures 3-4 times a week with the introduction of ammifurin at a dose of 20 mg per 20 kg of body weight, and introduce genvevit 2 tablets per day for 3 weeks and additionally introduce a soothing collection of herbs overnight for a month.

With the right integrated approach to the treatment of psoriasis, the method is extremely long, which limits its use.

A known method of treating psoriasis [Patent RU 2281796, 08/20/2006], which provides for the impact on one of the pathogenetic links of psoriasis - impaired intestinal absorption, expressed by malabsorption syndrome. The method is as follows: a group of people with impaired absorption in the small intestine is isolated from psoriasis patients, the skin condition is assessed using the international PASI index, they undergo a course of low-intensity laser therapy, acting on the small intestine against the background of traditional therapy, for a treatment course of 10-12 procedures. The disadvantages of this technique include the possibility of its use to a limited group of patients - only to persons with malabsorption syndrome and the need for a special installation and specially trained personnel.

A known method for the treatment of psoriasis [Patent RU 2278660, 06/27/2006], including laser phoresis with an infrared laser gel form of a drug based on pantogematogen in the projection of large vessels of the upper and lower extremities, alternating every other day in the axillary and cubital areas with irradiation of the popliteal and inguinal areas. Then they conduct a course of baths with a solution of pantohematogen at a concentration of 0.03-0.08 g / l with a gradually increasing water temperature from 36-37 ° to 39-40 ° C and reducing the procedure time from 15 minutes to 8 minutes per 10- 12 treatments every other day. The disadvantage of this method is the short period of remission (4-6 months).

A known method of treating psoriasis [Patent RU 2292215, January 27, 2007], which consists in prescribing drug therapy and options for external treatment of psoriasis by applying three ointment formulations from various compositions in combination with collections of herbs for oral administration in three stages of 30 days in succession, various compositions for each stage. However, the disadvantage of this method is the multicomponentness of all three compositions of ointments and herbs in each stage, the duration of treatment (more than 90 days), which limits the application of the method.

The prototype is a method of treating psoriasis, including detoxification, hyposensitizing, antihistamines, vitamins, peripheral blood circulation enhancers, hepatoprotectors, in severe cases - cytostatics, immunosuppressants, retinoids. Preparations for external use: glucocorticoid agents, zinc pyrithione, salicylic ointment 1%, 2%; ointments containing naphthalan oil, ichthyol, tar birch, etc. Physiotherapy is used. The objective of the invention was the development of a method for the treatment of psoriasis, increasing the effectiveness of treatment.

The technical result when using the invention is the reduction of treatment time, prolonged remission.

The proposed method for the treatment of psoriasis patients is as follows: apply alfalfa grass inoculum in the form of a 10% aqueous solution (drug "Luceron", manufacturer GUP "Immunopreparat", TU 9391-005-04863057-01) 5 ml orally 3 times a day ( 1.5 g), washed down with 50 ml of warm boiled water for 20-30 minutes before meals for 20-35 days. The drug is prescribed in combination with basic therapy, including the introduction of desensitizing, antihistamines, vitamins, physiotherapy, and, if necessary, additionally the introduction of restorative drugs. Topical application of alfalfa grass seed extract for psoriasis: 10% ointment from the alfalfa grass seed extract "Luceron" (Unguentum Luceroni, manufacturer of GUP "Immunopreparat") is applied to the skin with a thin layer on all lesions 1-2 times a day for 20-35 days. From 12-17 days, with the plaque form of psoriasis, collagen plates with an extract of alfalfa sowing grass Lucerne are applied. The Luceron plate (manufacturer of the State Unitary Enterprise Immunopreparat) is tightly applied to poorly absorbable psoriatic plaques, mainly with exudative psoriasis, on the foci in the shins and forearms. The plates are fixed on psoriatic plaques with a bandage for 48-72 hours (until the plate is completely resorbed). The course of treatment with plates is 2-3 times.

For the treatment of psoriasis patients, an alfalfa herb extract was used, produced according to the original technology as a food supplement. Alfalfa grass seed extract contains amino acids, uronic acids, amino sugar, organic acids, flavinoids, microelements and has a pronounced immunotropic, anti-inflammatory, choleretic, hepatotropic, wound healing, antioxidant activity, antiallergic effect. Local application of alfalfa sowing seed extract is known [patent for invention No. 2110272 of 02.20 96, "Means for treating thermal wounds"; Patent for invention No.2116079 dated 02.26.96 “Phytopreparation of biogenic action”; patent for the invention No. 2143925 dated January 10, 2000 “Wound healing coating”], as well as oral administration of an extract of grass alfalfa inoculum in the form of a 10% solution for the treatment of allergic diseases in children [Patent RU 2161978, January 20, 2001]. Earlier alfalfa herb extract was not used in the treatment of psoriasis.

According to this technique, 44 patients with a positive effect were treated.

The proposed method is illustrated by the following examples.

Example 1. Under observation was patient A., 19 years old, with a diagnosis of disseminated psoriasis, a progressive stage. The history of the disease №1979. Sick 4 years. Concomitant diseases: frequent acute respiratory viral infections. Was admitted to the hospital during an exacerbation of the disease. On the trunk, upper, lower extremities, in the pubic region there were pinkish-red papules, plaques up to 1-1.5 cm in diameter, rash on the legs was draining and plaques up to 3-7 cm in diameter formed. Against the background of basic therapy (10% solution of calcium chloride iv, 30% solution of sodium thiosulfate iv, 1% solution of nicotinic acid i / m, aloe extract i / m, solution of tavigil i / m, physiotherapy - total UFO) the patient received 5 ml 3 times a day (1.5 g) of alfalfa sowing herb extract 30 times before meals, drinking 50 ml of warm boiled water. Local treatment: once a day, a thin layer of 10% ointment from the extract of alfalfa grass (Lucerne) was applied to the elements of the rash. In the patient, on the 4th day of treatment, psoriatic rashes turned pale, on the 7th day of treatment, papules and plaques flattened, peeling decreased. The state of health and mood improved. However, skin infiltration in the region of plaques on the legs remained, therefore, from the 12th day of treatment, wound healing collagen plates with seed alfalfa extract were applied to the plaques in the legs, tightly bandaging them to the skin. The bandage was not removed within 3 days, during which time the plates completely absorbed, then new plates were applied. After 2 rounds of dressings, the plaques on the legs were significantly paled, flattened. On the 21st day of treatment, the patient was discharged with significant improvement (PASI before treatment 28, after treatment - 1.8). Remission 8 months.

Example 2. Patient G., 45 years old. He was admitted to the hospital with a diagnosis of advanced exudative psoriasis in a progressive stage, medical history No. 1473. Sick 5 years. From the patient's history: often has colds (ARVI), pneumonia in 2004; acute renal failure in 2004; during pre-hospital examination, the patient revealed chronic gastritis, chronic cholecystitis. The skin of the trunk (chest, back, stomach), upper and lower extremities, mainly extensor surfaces, where papular-plaque rashes (from 0.5 to 2-3 cm in diameter), covered with silver-white scales, along with which were observed single plaques covered with layering of a thick layer of flakes soaked with exudate, flakes, when removed, a wet surface was exposed. From the first day of treatment, the patient was prescribed general and local treatment, after two weeks - physiotherapy (UFO). General treatment, in addition to antihistamines, desensitizing drugs, detoxification therapy, vitamins, included oral administration for 25 days of a 10% solution of sowing alfalfa herb extract 5 ml 3 times a day (1.5 g) 30 minutes before meals with 50 ml warm boiled water. Local therapy consisted in the appointment of rash elements - 10% ointment from the extract of grass alfalfa (Lucerne). The ointment was applied in a thin layer once a day. A week later, the patient had abdominal pain; the rash on the skin turned pale. By the 16th day of treatment, the papules and plaques had flattened, the plaques took the form of garlands; feeling better. The patient was discharged on the 25th day of treatment with good treatment results (PASI before treatment 30.3, after treatment 4.6). Remission 7 months.

Example 3. Patient F., aged 40, was admitted to the hospital with a diagnosis of disseminated exudative psoriasis with lesions of large folds, a progressive stage, psoriatic arthropathy. Sick for 17 years. Case history No. 1926. Concomitant diseases: chronic gastritis, chronic cholecystitis, chronic pancreatitis. On the skin of the body there were rashes in the form of single papules, multiple large psoriatic plaques (up to 5-10 cm in diameter), covered with a layering of yellowish, exudated flakes of scales, with violent rejection of the latter, a bright red wet surface is exposed. The same plaques are localized in the inguinal, axillary cavities, on the skin of the lower legs, and the upper third of the forearms. The patient was prescribed the following treatment: vitamin therapy - a / m solution of vitamin B ₆ , B ₁₂ , restorative - aloe vera extract / m, antihistamine - tavegil solution / m, suprastin tablets inside, desensitizing - 10% solution of calcium chloride iv, 30 % solution of intravenous sodium thiosulfate, physiotherapy - selective phototherapy in the regressing stage, application of ozokerite to the area of the knee joints, from the 19th day - physiotherapy (UV, UHF). In addition, the patient took orally for 35 days a 10% solution of alfalfa sowing herb extract 5 ml 3 times a day (1.5 g) 30 minutes before meals, washed down with 50 ml of warm boiled water. Local therapy was as follows: to dry the exudation in the foci in the first 4 days, the plaques were smeared with a solution of fucorcin, then 10% ointment from the extract of alfalfa sowing grass (Lucerne) was applied to the rash elements. The ointment was applied in a thin layer twice a day for the first week, then once a day until the end of treatment. Three weeks after the hospitalization, abdominal pain decreased significantly, a feeling of nausea passed, the tongue began to clear of plaque, the appetite, and stools returned to normal. In the foci of lesion peeling became less, papules and plaques began to flatten. To accelerate the process of resorption of plaques on the legs and forearms from the 17th day of treatment, wound healing collagen plates with alfalfa seed were applied to the plaques, tightly bandaging them to the skin. The bandage was not removed for 3 days, then new plates were applied (the previous ones absorbed in 3 days). After 3 rounds of dressings, the plaques on the legs and forearms completely disappeared. By the end of treatment, biochemical blood tests returned to normal. The patient was discharged on the 38th day of treatment in satisfactory condition with significant improvement (PASI before treatment 47.5, after treatment - 6.2). Thus, a patient with a common form of exudative psoriasis, arthropathy, who received drugs from an extract of alfalfa grass inoculum (inside as a 10% solution and externally as 10% ointment and collagen plates) did not receive hormones, cytostatics, retinoids, was discharged in satisfactory condition with significant improvement. Remission of 6 months.

A method of treating patients with psoriasis, including the introduction of desensitizing, antihistamines, vitamins, physiotherapy and ointment therapy, characterized in that it is additionally orally administered a 10% aqueous solution of alfalfa sowing herb extract 5 ml 3 times a day, and as an ointment therapy is applied to the foci of 10% ointment from the extract of alfalfa grass Lucerne sowing 1-2 times a day, the course of treatment is 20-35 days. 2.
