2',3'-디데옥시시티딘을 극히 저용량으로 함유하는 약제학적 조성물
본 발명은 활성성분으로서 극히 저용량의 2',3'-디데옥시뉴클레오시드 동족체를 함유하는 항레트로바이러스성 조성물에 관한 것이다.
바이러스성 화학요법제 분야는 최근에 바이러스성 질환의 진단 및 치료와 관련하여 제기된, 특히 해결하고자 하는 문제에 부응하여 발전되어 왔다. 특히 흥미로운 것은 레트로바이러스, 특히 HIV 바이러스 퇴치에 유효한 화합물의 개발이다.
항바이러스성 화확요법제의 효능성은 당연히,특정 바이러스의 동적, 즉, 감염된 숙주내에서의 이의 감염성, 생활주기, 복제, 및 확산정도에 대한 이해를 포함하는 다수의 요인에 따라 좌우된다.
뉴클레오시드 동족체인 이러한 항바이러스성 물질의 대부분은 불행히도, 바이러스 진행과정에만 특이적인 억제제는 아니다. 대부분의 이러한 화합물은 숙주세포의 정상적인 분자성 진행과정을 상당히 또는 경미한 정도로 간섭하기 때문에, 비감염된 세포상에 독성효과를 유발한다.
이러한 사실은 특히 2',3'-디데옥시시티딘(ddc) 및 이의 동족체와 같은 2',3'-디데옥시뉴클레오시드 동족체를 사용하는 경우에 나타난다. ddc는 특히, 유효한 항바이러스 용량으로 투여할 경우, 말초성 시경병증을 야기시켜 얼얼함(tingling), 마비, 및 모르핀과 같은 경력한 진통제를 요구하는 통증을 유발한다.
2',3'-디데옥시아데노신의 경우에 있어서, 유효한 항바이러스이기 위해서는 특히 고용량이 필요하다.
본 발명은 극히 저용량의 디데옥시뉴클레오시드 동족체가 어떠한 신경병증도 야기시키지 않으면서 AIDS 예방 또는 AIDS 레트로바이러스로 감염된 환자의 치료에 유효하다는 사실을 발견하였다.
디데옥시뉴클레오시드 동족체는, 뉴클레오시드 분자의 리보스부분의 2' 및 3' 위치에 두 개의 하이드록시 치환체가 존재하지 않는, 아데노신, 티미, 시티딘, 구아노신, 우리딘 또는 이노신의 2',3'-디데옥시 동족체 이다. 여기에는 뉴클레오시드가 아미노, 할로겐, 알킬, 아지도, 시아노, 및 뉴클레오시드 동족체상에서 통상적으로 발견되는 다른 그룹으로 치환되는 2',3'-디데옥시뉴클레오시드 라디칼이 포함된다. 또한 본 발명의 범위에서는 5'-하이드록시 그롭의 아실 및 포스페이트 에스테르의 사용도 포함된다.
본 발명은 환자에게 0.001 내지 0.10mg/kg/일의 디데옥시뉴클레오시드 동족체를 투여하기에 충분한 양으로 용량단위내에 디데옥시뉴클레오시드 동족체를 함유하는, AIDS를 예방하거나 AIDS 레트로바이러스로 감염된 환자를 치료하는데 적합한 약제학적 조성물을 포함한다. 본 발명은 또한 0.001 내지 0.10mg/kg일의 디데옥시뉴클레오시드 동족체를 환자에게 투여함을 특징으로 하여, AIDS를 예방하거나 AIDS 레트로바이러스로 감염된 환자를 치료하는 방법을 포함단다.
바람직하게는 디데옥시뉴클레오시드 동족체는 2',3'-디데옥시시티딘이다.
0.001 내지 0.05mg/kg의 2',3'-디데옥시시티딘을 1일 1 내지 6회 용량으로 환자에 투여하는 것이 특히 바람직히다.
0.01mg/kg의 2',3'-디데옥시시티딘을 1일 4회로 나누어 환자에 투여하는 것이 가장 바람직하다.
화합물은 경구, 정맥내, 비경구 또는 점막피부내로 투여될 수 있다.
본 발명의 화합물은 용액상태로 단독투여될 수 있다. 그러나, 바람직한 양태로서, 활성성분(들)은 약제학적 제형으로 사용되거나 투여될 수 있다. 이러한 체형은 하나 이상의 약제학적으로 허용되는 담체 및/또는 다른 치료제와 함께, 하나 이상의 활성성분(디데옥시뉴클레오시드)을 함유한다. 본 발명의 범위내에 포함된 바와 같이, "허용되는"이란 용어는 제형의 다른 성분과 사용될 수 있고, 환자 또는 숙주세포에 무해함을 의미한다. 이러한 담체에는 경구, 직장내, 비강내, 국소적,구강내, 설하, 질내, 또는 비경구(피하,근육내,정맥내, 및 경피투여 포함) 투여용으로 적합한, 본 분야의 전문가에게 잘 공지된 화합물이 포함된다. 본 발명에서 사용하기에 적합한 특정의 담체를 추가로 하기에 정의하였다. 약제학적으로 허용되는 유도체의 사용과 관련하여 본 발명의 경우, 본 발명에 따르는 화합물은 또한 바이러스성 감염증의 치료 또는 예방용 약제를 제조하는데 사용될 수도 있는 것으로 인지될 것이다.
제형은 편리하게는 단위용량 형으로 존재할 수 있고, 약제학적 분야에 공지된 방법으로 제조될수 있다. 이러한 방법에는 추가의 의약적으로 활성인 성분을 함유할 수 있는 담체내에서의 활성성분의 제조가 포함된다.
본 발명의 2',3'-디데옥시시티딘의 경구투여의 한 방법은 염화나트륨, 용액중에, 바람직하게는 오렌지쥬스중의 0.9% 염화나트륨중에 유효량의 2',3'-디데옥시시티딘을 용해시키는 방법으로 구성된다. 바람직한 방법은 2',3'-디데옥시뉴클레오시드를 정제형태로 투여하는 것이고, 하기 성분중 하나 이상을 포함할 수 있다 : 유당(함수성, 신속한 유동성), 미세결정상 셀룰로스, 콜로이드성 이산화탄소, 크로스카르멜로스 나트륨, 마그네슘 스테아레이트, 스테아르산, 및 기타 부형제, 착색제, 및 약리학적으로 상용성인 담체, 경구투여용 조성물을 절식 또는 비-절식 상태의 환자에게 투여할 수 있다. 정체 제형의 예를 표 1 내지 7에 표시하였다.
경구투여용으로 적합한 본 발명의 제형(서방성 제형 포함)은 각각 예비측정된 양의 활성성분을 함유하는, 캅셀, 케세(cachet) 또는 정제와 같은 개별단위로서, 산제 또는 과립제 상태로 ; 용액 또는 수용액중의 현탁액 상태로; 유중수형 액체 유재 또는 수중유형 액체유제 상태로 존재 할 수 있다. 활성성분이 또한 존재할 수 있다.
국소투여용으로 적합한 제형에는 향미제, 통상적으로 자당 및 아카시아 또는 트리가칸트 고무중의 활성성분을 함유하는 로젠지제; 젤라틴 및 글리세린, 또는 자당 및 아카시아와 같은 불활성 기제중의 활성성분을 함유하는 패스틸제(pastille) ; 및 적합한 액체 담체중의 활성성분을 함유하는 구강세척액이 포함된다.
직장투여용 제형은 예를 들어 코코아버티 또는 살리실레이트를 포함하는 적합한 기재를 함유하는 좌제상태일 수 있다.
질내투여용으로 적합한 제형은 활성성분 이외에 경우에 따라 본 분야에 공지된 바와 같은 담체를 함유하는 패서리(pessary),탐폰, 크림, 겔, 페이스트, 발포제 또는 분무제로서 존재할 수 있다.
비경구투여용으로 적합한 제형에는 산화방지제,완충제,제균제(bacteriostats) 및 제형이 의도하는 수혈자의 혈액과 등장액을 이루게 하는 용질을 함유할 수 있는 수성 및 비-수성, 등장성 멸균 주사용액 ; 현탁화제 및 농조화제를 포함할 수 있는 수성 및 비-수성 멸균 현탁액이 포함된다. 제형은 1회-투여용량 또는 다수-투여용량의 밀봉된 용기(예 : 앰플 및 바이알)내에 존재할 수 있으며, 사용하기 직전에 멸균액체 담체(예 : 주사용 수)를 첨가하기만하면 되도록 동결-건조상태로 저장할 수 있다. 즉석에서 사용할 주사용액 및 현탁액은 전술한 종류의 멸균된 산제, 과립제 및 정제로부터 제조할 수 있다.
투여된 성분은 또한 다른 항-바이러스성 약제 및 생물학적 제제와 함께, 또는 골수 또는 임파구 이식물 또는 의약제를 포함한 다른 면역 조절요법과 병용하여 치료에 사용될 수 있다.

  1. 2',3'-디데옥시시티딘을 1일 투여용량 0.001 내지 0.05mg/kg/일이 투여될수 있도록 하는 양으로 함유하는 AIDS 예방 또는 치료용 약제학적 조성물
  2. 제1항에 있어서, 1회 투여용량 단위로서 2',3'-디데옥시시티딘 0.001 내지 0.05mg/kg/을 함유하는 약제학적 조성물
  3. 제1항에 있어서, 경구 또는 정맥내 투여용 약제학적 조성물
