(57)【要約】 【課題】 脊椎椎骨の椎弓形成手術の手術時間が短縮で
きる。 【解決手段】 脊椎椎骨１の椎弓を切除して形成される
切除部分６または椎弓部分の切り起こし部分４ｂ，４ｂ
の間に装着される骨補填片１１と、脊椎椎骨１の切除部
分６を橋絡するように配置されたプレート１２と、骨補
填片１１をプレート１２に固定するための固定手段１３
とから構成される。

Description

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】本発明は、たとえば頸椎椎骨
の椎弓形成手術に用いられる脊椎インプラントに関す
る。
【０００２】
【従来の技術】人間の脊柱は７個の頸椎と１２個の胸椎
と５個の腰椎と５個の仙椎が融合した仙骨と融合した尾
骨から構成されている。正常な頸椎椎骨１は、図５に示
すように、椎管が通る部分２と神経が通る椎孔３を有
し、椎孔３に隣接する椎弓４には棘突起４ａが形成され
ており、椎弓４は椎孔３を通る神経に影響を与えない構
造をなしている。ところで、正常な頸椎椎骨１に異常が
発生すると、頸椎椎骨１は、図６に示すように、点線で
示す椎弓４が椎孔３の方向に張り出し、椎弓４の張り出
し部分５が椎孔３を通る神経を圧迫し、運動機能に影響
を与えることになる。椎弓４の張り出し部分５が椎孔３
を通る神経を圧迫する場合の治療法として椎弓形成手術
が知られている。
【０００３】上記椎弓形成手術には、頸椎椎骨１の椎弓
４の部分および張り出し部分５を切除し、張り出し部分
が椎孔３を通る神経を圧迫しないようにする方式のもの
と、棘突起４ａを切除し椎弓４を切り起こすことで張り
出し部分５が椎孔３を通る神経を圧迫しないようにする
方式のものとがある。
【０００４】頸椎椎骨１の椎弓４の部分を切除する椎弓
形成手術は、図７に示すように、頸椎椎骨１の椎弓４の
部分を切除し、この切除部分６に図８に示すような骨補
填片７を装着し、骨補填片７を頸椎椎骨１に糸により縫
合し、骨補填片７を頸椎椎骨１の椎弓４の部分として用
いるものである。
【０００５】頸椎椎骨１の椎弓４を切り起こす椎弓形成
手術は、図９に示すように棘突起４ａを切除し、図示し
ないサージェアトームを用いて棘突起４ａを切除した椎
弓４に中央部に切れ目９ａと両端側に切り込み９ｂ，９
ｂを図１０に示すように設け、切れ目９ａにより分断さ
れた椎弓部分４ｂ，４ｂを切り込み９ｂに沿って切り起
こし、この切り起こし部分４ｂ，４ｂの間に図１１に示
すように骨補填片７を装着し、骨補填片７を椎弓４の切
り起こし部分４ｂ，４ｂに糸により縫合し、骨補填片７
を頸椎椎骨１の椎弓４の部分として用いるものである。

【発明が解決しようとする課題】上記頸椎の椎弓形成手
術に用いられる骨補填片は、頸椎椎骨の切除部分または
椎弓の切り起こし部分の間に嵌合するような形状に成形
されているが、骨補填片は、セラミック成形品であるた
め高価であり、しかも、骨補填片の頸椎椎骨への固定は
糸縫合手段による行なわれるため、この操作に熟練度と
相当の時間を必要とする。
【０００７】本発明は上記した点に鑑みてなされたもの
で、脊椎椎骨の椎弓形成手術の手術に要する時間を短縮
できる脊椎インプラントを提供することを目的とする。
【０００８】
【課題を解決するための手段】本発明の脊椎インプラン
トは、脊椎椎骨の椎弓部分を切除して形成される切除部
分または椎弓部分の切り起こし部分の間に装着される骨
補填片と、脊椎椎骨の切除部分を橋絡するように配置さ
れたプレートと、骨補填片をプレートに固定するための
固定手段とから構成される。
【０００９】本発明の脊椎インプラントは、骨補填片の
脊椎椎骨に接する面にピンを設けることで骨補填片の脊
椎椎骨への固定を確実にしたり、プレートの脊椎椎骨に
接する面に係止部を設けることでプレートを脊椎椎骨の
所定の位置に固定することができる。

【発明の実施の形態】以下本発明の実施の形態を図面を
参照して説明する。なお、図１および図２において図５
および図６と同一部材については同一符号を付す。
【００１１】図１は本発明による脊椎インプラントを頸
椎椎骨の椎弓形成手術に適用した例を示すものであり、
脊椎インプラント１０は、頸椎椎骨１の椎弓４の切除部
分６に取付けられている。
【００１２】上記脊椎インプラント１０は、図２に示す
ように、頸椎椎骨１の椎弓を切除して形成される切除部
分６に装着される骨補填片１１と、頸椎椎骨１の切除部
分６を橋絡するように配置されたプレート１２と、骨補
填片１１をプレートに固定するための固定手段１３とか
ら構成されている。
【００１３】上記骨補填片１１は、ステンレス（ＳＵＳ
３１６系）またはチタン合金を材料として頸椎椎骨１の
切除部分６の形状に対応した形状に成形された全体形状
を扇形状とした本体１４と、この本体１４の両側面に突
設されたピン１５，１５とを有する。この本体１４の大
径面にはねじ１３が外方に延びるように突設されてい
る。このねじ１３にはロックナット１３ａが螺着され
る。
【００１４】上記プレート１２は、ステンレス（ＳＵＳ
３１６系）またはチタン合金の薄板を弓形に成形して形
成されるものであり、両端面に鋸歯のような係止部１６
が形成されている。この係止部１６はプレート１２のス
リップを防止し、プレート１２を頸椎椎骨１の所定の位
置に固定する。このプレート１２の中央部分にはねじ１
３が通る孔１７が形成されている。
【００１５】つぎに作用を説明する。脊椎インプラント
１０を頸椎椎骨１に取付けるには、骨補填片１１を頸椎
椎骨１の椎弓を切除して形成される切除部分６に装着
し、プレート１２をプレート１２に設けた孔１７に骨補
填片１１のねじ１３を通すようにして頸椎椎骨１の切除
部分６を橋絡するように配置し、プレート１２より突き
出る骨補填片１１のねじ１３にロックナット１３ａを螺
着することで行なう。この場合、骨補填片１１に突設さ
れたピン１５，１５が頸椎椎骨１の切除部分６に係止さ
れるので、骨補填片１１は、頸椎椎骨１の所定の位置に
保持され、また、プレート１２に設けた係止部１６が頸
椎椎骨１に係止されるので、プレート１２が固定された
後スリップすることがなく、所定の位置に固定される。
【００１６】図３および図４は本発明の他の実施の形態
を示し、この実施の形態に用いられる脊椎インプラント
１０は、図２に示す脊椎インプラントと同じ構造のもの
であるが、骨補填片１１は頸椎椎骨１の椎弓４の切り起
こし部分４ｂ，４ｂの間に形成される空間に嵌合される
形状をなしている。
【００１７】脊椎インプラント１０を頸椎椎骨１に取付
けるには、骨補填片１１を頸椎椎骨１の椎弓４の切り起
こし部分４ｂ，４ｂの間に装着し、プレート１２をプレ
ート１２に設けた孔１７に骨補填片１１のねじ１３を通
すようにして頸椎椎骨１の椎弓４の切り起こし部分４
ｂ，４ｂを橋絡するように配置し、プレート１２より突
き出る骨補填片１１のねじ１３にロックナット１３ａを
螺着することで行なう。
【００１８】上記実施の形態では、脊椎インプラントを
頸椎椎骨に適用したものについて説明したが、脊椎イン
プラントは胸椎椎骨や腰椎椎骨に適用できるのはもちろ
んである。
【００１９】なお、上記実施の形態では、骨補填片１１
をプレート１２に固定する固定手段として、ねじとロッ
クナットを用いたが、骨補填片１１をプレート１２にゆ
るむことなく固定できるものであれば、スナッブビンの
ような固定手段であってもよい。

【発明の効果】以上述べたように本発明によれば、脊椎
椎骨の椎弓形成手術の手術時間を短縮することができ
る。

    【特許請求の範囲】
  1. 【請求項１】脊椎椎骨の椎弓部分を切除して形成される
    切除部分または椎弓部分の切り起こし部分の間に装着さ
    れる骨補填片と、脊椎椎骨の切除部分または椎弓部分の
    切り起こし部分を橋絡するように配置されたプレート
    と、骨補填片をプレートに固定するための固定手段とを
    有する脊椎インプラント。
  2. 【請求項２】骨補填片の脊椎椎骨に接する面にピンを設
    けたことを特徴とする請求項１に記載の脊椎インプラン
    ト。
  3. 【請求項３】プレートの脊椎椎骨に接する面に係止部を
    設けたことを特徴とする請求項１に記載の脊椎インプラ
    ント。
  4. 【請求項４】固定手段がねじ手段であることを特徴とす
    る請求項１に記載の脊椎インプラント。
  5. 【請求項５】固定手段がスナップピン手段であることを
    特徴とする請求項１に記載の脊椎インプラント。
