【産業上の利用分野】
本発明は静脈内を循環する血栓を止めるに適合する医用
のフィルター並びに静脈内にフィルターを導入するため
のカテーテルに関し、特に−時的に使用されるフィルタ
ー、即ち患者に対して施された治療が満足なものと判断
されフィルターを存在させることがもはや必要ないと判
定された場合に静脈から引き出せるフィルターカテーテ
ルに関する。 [0002]
【従来の技術】
このような−時的なフィルターは、特に特許FR−A−
2580504により知られている。これらは導入のた
めにカテーテルの末端に固定されている。これらの構造
は、静脈内への導入中は濾過を保証するエレメントがカ
テーテルを延長するシース内に折り曲げられ、且つフィ
ルターが静脈内に置かれた時は、遠隔制御手段によって
上記濾過エレメントが広げられるようになっている。 [0003] FR−A−2580504のフィルターに於いて、濾過
手段は複数の脚から成り、それらは静止時には円筒の母
線に沿って相互にほぼ並行に延在している。また、これ
らを広げる手段は、フィルター脚によって規定された上
記円筒の内側に設けられた可逆的な拡張手段である。変
形することにより、拡張手段は、少くともその末端領域
に於いて、静脈壁に対して加えるに十分な強さで脚を内
側から押し出して、傘のスポークの様にして脚を広げる
。 [0004] これらの公知のフィルターは2つの欠点を有する。一つ
には、これらは、カテーテルの末端に配置され、且つフ
ィルターが静脈内で広げられている時に引き出せるよう
になっている濾過膜を保護する捕捉的なシースを必要と
することである[0005] 他方には、これらが濾過エレメントを広げるための捕捉
的な複雑な拡張手段を必要とすることである。この拡張
手段としては、可逆的に遠隔制御が可能なものである必
要があり、例えば適当な流体を供給する導管を有し、ふ
くらますことができるバルーンを要する。 [0006] これはフィルター及びカテーテル組立体の構造を複雑で
微細なものにしている[0007] 以上で明らかにされた欠点を克服すめために、文献FR
−A−2606642は、カテーテルの末端に設けられ
且つフレキシブルな複数の脚から成る濾過エレメントを
提案している。ここで、これらのフレキシブルな脚の向
かい合う端は、相対位置を修正する手段によって互いに
結合された2つの部材間に置かれる。その結果、縦軸に
沿った2つの部材の接近により、フレキシブルな脚の横
断的な拡張が起ることになる。 [0008] 文献FR−A−2606642に於いて挙げられた実施
例ではフレキシブルな脚はカテーテルの末端部を構成す
る管状体を切り出すことにより得られる。ここで、この
末端部は、管状体の内側を縦方向にスライドすることが
できる内部導管に固定される。 [0009] この実施例は特に単純である。しかしながら、このよう
なフィルターカテーテルが静脈内に存在すると、フィル
ターの直ぐ近くに局在した血栓を生じさせる可能性のあ
ることが出願人により認められた。 [0010]
【発明が解決しようとする課題】
本発明は前記欠点を克服するタイプのフィルターカテー
テルを提案することを目的としている。 [0011] [発明の構成] [0012]
【課題を解決するための手段】
本発明は(1)静脈を部分的かつ少くとも一時的にさえ
ぎるために、末端方向に周囲にかけて対称的に分布され
且つフレキシブルな複数の帯状体を規定する複数の縦の
刻み目をつれる管状体と、 管状体の内側の導管及び管状体をそれらの末端付近で堅
く結合され管状体及び内部の導管を4弗化エチレン製と
するフィルターカテーテル。 [0013] (2)管状体及び導管の末端が熱溶接されるフィルター
カテーテル。 [0014] (3)管状体及び導管の末端間に含められ且つそれらに
熱溶接により固定された放射線の不透過材料製のリング
から構成されて成るフィルターカテーテル。 [0015] (4)管状体の末端は導管の内側の方へ折り曲げられ、
一方ではリングを、他方では導管の末端を被覆するフィ
ルターカテーテル。 [0016] (5)内部導管の末端は、金属リングより外方にフレア
ーにされているフィルターカテーテル。 [0017] (6)導管が帯状体の向かい側に位置する領域において
オリフィスから構成されて成るフィルターカテーテル。 [0018] (7)フィルターの末端部は熱硬化処理を受け、フレキ
シブルな帯状体は変形に対する抵抗が改善されているフ
ィルターカテーテル。 [0019] (8)適状体が広げられた状態で熱硬化処理が行なわれ
、フレキシブルな帯状体を構成する4弗化エチレンが広
げられた状態を記憶するフィルターカテーテル[002
0] (9)帯状体が螺旋状によじられている間に熱硬化処理
が行なわれ、フレキシブルな帯状体を構成する4弗化エ
チレンが螺旋形状を記憶するフィルター力テーチルであ

【作用】
本発明は静脈を部分的且つ少くとも一時的にさえぎるフ
ィルターに関するもので、4弗化エチレン製の管状体と
、この管状体内の4弗化エチレン製の導管から構成され
、この管状体にはその末端方向に、周囲にかけて対称的
に分布され、且つフレキシブルな複数の帯状体を規定す
る複数の縦方向の刻み目がつけられ、管状体と導管はそ
れらの末端付近で、例えば熱溶接によって接合され、好
ましくは、放射線の不透過材料からなるリングが熱溶接
中に、それらの末端間に含められ、 フレキシブルな帯状体は、管状体内をスライドする導管
によって横断的に広げられ、これらは、熱硬化処理を受
け、4弗化エチレンの重合体構造中にその広げられた形
、又は螺旋形を記憶する。 [0022]
【実施例】
本発明は末端方向に、周囲にかけて対称に分布され且つ
フレキシブルな複数の帯状体を規定する縦方向の刻み目
をつける管状体と、この管状体の内側で且つその末端付
近で管状体に固定された導管とから成り、部分的且つ少
くとも一時的に静脈をさえぎるフィルターによって完壁
に十分達成され、管状体及び内部導管は両方共4弗化エ
チレン(ポリテトラフルオロエチレン)製であり、 4弗化エチレンがほんの僅かしか血栓を生じない材料で
あり、この材料がその周囲に予め切り出されたフレキシ
ブルな複数の帯状体の横断的な拡張を生じせしめるため
証明された十分なフレキシビリティを与えるものとして
選択されている[0023] 本発明によるフィルターカテーテルは、好ましくは、そ
の末端方向に血流と接触する、まさしく4弗化エチレン
に他ならない材料から成っており、その場合、管状体及
び内部導管は熱溶接により接合される。 [0024] しかしながら、静脈の内側にカテーテルを貫通して置く
ことができることが望ましく、その目的のために、濾過
エレメントの近くに放射線が不透過なエレメントを設け
られ、本ケースに於いては、フィルターカテーテルは、
管状体と内部導管の間に含められ且つ熱溶接中にそれら
に固定される放射線が不透過な材料、例えば金で作られ
たリングから構成され、このように、放射線が不透過な
エレメントは目に見ることはできないし、且つ静脈壁に
引っかかったり、血栓を形成する原因には成り得ない。 [0025] 前フレキシブルな4弗化エチレンの帯状体は、静脈内で
長時間経過後、横断的に広げられ、フィルター組立体の
固有の重量により変形を受けたことが認められこのよう
な変形は、最も下方の帯状体の離間の増大、従って、フ
ィルターがつぶれるという形で現われ、それは、静脈の
横断面に於ける複数の帯状体の不均一な分布を引き起こ
すと共に、特に特定の帯状体の周りに血栓を生じさせる
血流の乱れ現象を起こすものである。 [0026] この欠点を克服するために、本発明によればフィルター
カテーテルの少くとも末端は予め熱硬化処理を受けるが
、この目的は、4弗化エチレンのフレキシブルな帯状体
の機械的な変形に対する抵抗を強めることにあり、この
熱硬化処理は、例えば電子レンジで4〜5分ベーキング
することにより行うことが可能であり、この熱硬化の間
、上記フレキシブルな帯状体は、好ましくは、それらの
広げられた状態に置かれ、このようにして、上記4弗化
エチレンは、その重合体構造内にこの広げられた状態を
記憶するある種の焼きを入れられる。別の改良によれば
、管状体は刻み目の上流で保持され、かつ刻み目の下流
ではフレキシブルな帯状体によじれた形を付与するため
に管状体は自転され、その後、この形状でフィルターカ
テーテルの末端部に対し熱処理が行われ、フレキシブル
な帯状体は静止又は広げられた状態になり、この場合、
フィルターカテーテルの使用中に、フレキシブルな帯状
体は自然に再び螺旋形をとることになる。 [0027] 特開平4−12ex61(8) 次に図面について説明する。本発明のフィルターカテー
テル(よ、本質的Gこ、中空の管状体2と、この管状体
2の内側に同軸的に設置される中空の導管3カ・ら構成
されている。これらの2つのエレメント、即ち管状体2
及び導管3(よ4弗化エチレン(ポリテトラフルオロエ
チレン)製である。 [0028] 本実施例に於いて、「基部」および「末端」とXJ)う
用語は、フイルターカ(導入される患者の人体の部分を
引用して用いられてν)る。フィルター1の末端(よ、
即ち、図1及び図2において右側に示きれた人体の上記
部分から最も離れた端であり、また基部端は近い端であ
る。 [0029] 管状体2及び導管3の末端2a、3aは、それぞれ互い
に熱溶接され、ステンレス鋼、白金もしくは金製の金属
リング4の周りにある。 [0030] このリング4は、放射線を透過しない材料で製作されて
おり、上記フィルターの位置決め巾検出可能である。 [0031] 図1及び図2に於いて明確に示されている通り、リング
4は管状体2と導管3の末端3aの間に設置される。上
記管状体の末端2aは導管3の内側に曲げられて、上記
導管の末端3aを被覆している。これらのエレメントは
、フィルターカテーテルの端15を形成するように互い
に熱溶接されている。 [0032] 図4に図解された別の実施例によれば、内部導管3の末
端3aは金属リング4から突出して外方へフレアーにさ
れ、管状体2の末端2aと接触する状態で熱溶接が行わ
れる。 [0033] 末端2aの上流において、管状体2は、30mmの長さ
dにわたり縦方向につくられた6つの刻み目5から成っ
ている。これらの刻み目5は6個の同一帯状体6を規定
しており、管状体2の60°角のスペースを占めている
。直径3mmの管状体2の場合、各帯状体6は約1.5
mmの幅を有する。 [0034] 導管3は中空で、中央の管路7から成っており、そこに
おいてフィルターカテーテルの導入中案内ロッドとして
作用する金属ロッド8が自由にスライドすることができ
る。 [0035] 基部端の方向で、患者の人体の外側ではあるが、管状体
2は維持及び供給のための装置9に固定されている。こ
の装置9は円筒管10とエンドリング11.12から構
成されている。第1の環状リング11は管状体2の基部
端を円筒管10の前部に接合している。第2のリング1
2は導管3を囲むと共に円筒管10の後部に近接する。 ひじ形部13は、円筒管10本体において、斜めに開は
広げられている。 [0036] 導管3の基部端3bは円筒管10から延在している。リ
ング12はこの末端3bを密封するように囲んでいる。 しかしながら、導管3は、円筒管10に対してリング1
2内を縦に、従って管状体2へ滑ることができる。 [0037] ブロック用リング14は、導管3をリング12に対して
閉止している。 [0038] 円筒管10は、患者の近くで、知られた方法によりフレ
ームに固定される。このフレームは注入される(生産)
物質を供給する2つのシステムを支持するもので、先ず
第1に上記導管の基部端3bにおいて、また第2に円筒
管10のひじ形部13において開けられている。 [0039] 上記フィルターカテーテルは以下の条件下で働く。 [0040] 第1の段階において、案内ロッド8は、その末端8aが
患者の悪い大静脈内の位置になるまで、適当な静脈内へ
経皮的な経路で導入される。 [0041] 第2の段階において、フィルターが植設されるべき大静
脈の位置にリング4がくるまで、フィルターカテーテル
1は患者の人体の外側から案内ロッド8に沿ってスライ
ドされる。この第2段階が実行される間、帯状体6は静
止状態、即ち管状体と並びとなる。 [0042] 図2で図解され且つ上記フィルターの開は広がりに対応
する第3の段階において、導管3をリング12内でスラ
イドさせるようにブロック用リング14が解放される。 導管3の基部端3bは、その後矢印Fの方向へ後方に用
心しながら変位させられる。この段階において、導管3
は全体として、その末端と固定されたリング4と同様に
移動する。円筒管10、従って管状体2の位置が保持さ
れていると、フレキシブルな帯状体6が存在することの
みにより、矢印F方向にリング4が戻ることができる。 各帯状体6は個々に折り曲がり、その結果刻み目5の領
域において上記管状体の均一な横断的な変形が生じる。 [0043] 広げられた位置において帯状体6の中央部分が静脈の内
壁で湾曲りを示すように、帯状体6の変位量及び長さが
選択される。 [0044] 一旦上記フイルターが正しく広げられると、ブロック用
リング14は管状体2に対する導管3の変位を防止する
ために固く締められ、それにより帯状体6を横断的な変
形状態に維持することになる。 [0045] 平面りに沿った断面である図3において明白なように、
6個の帯状体6は管状体2に対して横断的に広げられ、
部分的に静脈18の内部をふさぐ。このようにして、血
が自由に循環する一方、静脈18の内側を移動する血栓
は帯状体6によって阻止される。 [0046] フィルターカテーテルを使用する前に、後者は熱硬化処
理を受けた。この処理は、フレキシブルな帯状体6が図
2において示されたように広げられた状態にある間に、
上記フィルターの末端に熱衝撃を与えることにある。こ
の処理時間及び温度は、上記帯状体がとった形状が、あ
る程度4弗化エチレンの重合体構造において記憶される
ように決定される。この処理は、上記帯状体の変形に対
する抵抗を強め、静脈18の内側のフィルターがつぶれ
たり、静脈内の帯状体6の不均一な配置によって血流に
おいて生じる乱れに起因する血栓の危険を避けるもので
ある。 [0047] 図5に示された熱硬化処理の別の実施例によれば、フレ
キシブルな帯状体6がよじれた形をとる間に、熱処理が
フィルターカテーテルの末端に適用される。その目的の
ため、フィルターカテーテルの末端部を硬直させるよう
に心棒19が内部導管3にはめ込まれる。管状体2の末
端2aは保持用の支持体21の前脚20の位置でとめら
れる。管状体2は自転を受けることになり、それはフレ
キシブルな帯状体6に図5に示すよじれた形状を付与す
る。 [0048] そ、の後、管状体2は支持体21の後脚22内のフレキ
シブルな帯状体6の上流位置でとめられる。支持体21
及びフィルターカテーテルの末端部はベーキング用オー
ブン、例えば電子レンジ内に置かれ、4〜5分の熱処理
を受けることになる。フィルターの使用中、フレキシブ
ルな帯状体6は熱処理の間に与えられた螺旋形をとる傾
向にある。もちろん、支持体21上で、帯状体6は、図
5に示されたような静止位置、あるいは広げられた状態
をとることができる。 [0049] 治療用の(生産)物質が導管3の基部端3bより導管3
を通って上記フィルターの上流で注入することができる
。これは、ヘパリン型または線維素を分解する反凝固剤
である。導管3は、帯状体6の反対領域で、例えば螺旋
状に配置されたオリフィスから構成することができる。 このようにして、導管3内へ導入された治療用の物質は
、上記フィルターの上流で且つそのフィルター自体の水
準に配分することができる。 [0050] 治療用の同様な物質、あるいは可能な別の物質を円筒管
10のひじ形部13がら管状体2を通じて上記フィルタ
ーの直接下流の領域に注入することができる。 [0051] −旦、処置が満足なものと判断されると、上記フィルタ
ーは次のよ“うにして引き出される。 [0052] 即ち、帯状体6が、静止状態で、直線状でかつ互いに並
行な位置を再びとり始める迄、ブロック用リング14が
解放されるとともに、矢印Fと反対の方向に導管3の基
部端3bが押される。リング14はその後再び固く締め
つけられ、導管3/管状体2組立体が静脈から引き出さ
れる。 [0053] 本発明によるフィルターカテーテルについて試験したと
ころ、静脈内にフィルターが存在することに起因した血
栓の形成がないことを認めることができた。 [0054] 本発明は、限定でない例として述べられた上記実施例に
限らず、その種々な変形例の全てに及ぶものである。特
に、案内ロッド8が、中空でかつフィルターの下流でフ
ィルターカテーテル1の末端よりずっと先まで治療用物
質を注入するようにしても良い。 [0055]
【発明の効果】
本発明により静脈内にフィルターが存在することに起因
した血栓の形成が生じないフィルターカテーテルを提供
することができる。

    【特許請求の範囲】
  1. 【請求項1】末端方向に周囲にかけて対称的に分布され
    且つフレキシブルな複数の帯状体を規定する複数の縦の
    刻み目をつける管状体と、この管状体の内側の導管と前
    記管状体を末端付近で結合する結合材とからなるフィル
    ターカテーテル。
  2. 【請求項2】前記管状体及び前記導管の末端が熱溶接さ
    れることを特徴とする請求項1のフィルターカテーテル
  3. 【請求項3】前記管状体及び前記導管の末端間に含めら
    れ、且つ熱溶接により固定された放射線の不透過材料製
    のリングを具備してなる請求項2のフィルターカテーテ
    ル。
  4. 【請求項4】前記管状体の末端が導管の内側の方へ折り
    曲げられ、一方では前記リングを、他方では前記導管の
    末端を被覆することを特徴とした請求項3のフィルター
    カテーテル。
  5. 【請求項5】前記内部導管の末端は、金属リングより外
    方にフレアーにされている請求項3のフィルターカテー
    テル。
  6. 【請求項6】前記導管が帯状体の向かい側に位置する領
    域においてオリフィスから構成されて成る請求項1のフ
    ィルターカテーテル。
  7. 【請求項7】末端部は熱硬化処理を受け、フレキシブル
    な帯状体は変形に対する抵抗が改善されることを特徴と
    した請求項1のフィルターカテーテル。
  8. 【請求項8】前記帯状体が広げられた状態で熱硬化処理
    が行なわれ、フレキシブルな帯状体を構成する4弗化エ
    チレンが前記広げられた状態を記憶する請求項7のフィ
    ルターカテーテル。
  9. 【請求項9】前記帯状体が螺旋状によじられている間に
    熱硬化処理が行なわれ、フレキシブルな帯状体を構成す
    る4弗化エチレンが前記螺旋形状を記憶する請求項7又
    は請求項8のフィルターカテーテル。
