Description
[0001]本出願は、内容全体が参照により本明細書に組み込まれており依拠される、2017年11月3日出願の「Polymeric Compositions,Delivery Devices,and Methods」という名称の米国仮特許出願第62/581,453号に対する優先権を主張する。
[0002]多くの医療用途において、過度の出血を防止することが重要である。過度の出血を防止し、心臓血管手術を含む手術中の輸血率及び軽い合併症を低減させるために、多数の処置及び材料が提案されている。そのような処置は、金属、ポリマー、及び天然材料などのバリア材を出血部位へ導入することを含む。しかしこれらの生成物は、下の組織にうまく適合しないことがある。実施された他の材料には、ナイロン、セロハン、ポリテトラフルオロエチレン、ポリエチレン、シロキサン、エラストマ、及びポリ乳酸コポリマー膜が含まれる。しかし残念ながら、これらの材料の多くは生分解性ではなく、したがって体内に残り、予測できない、場合により望ましくない結果をもたらす可能性がある。
[0007]治療を改善するために、特に過度の出血を防止するために、ポリマー組成物、送達デバイス、送達システム、及び送達方法が本明細書に記載される。本開示は、材料の粘性、水性キャリア要件、活性剤要件など、現在の組成物の望ましくない特徴をなくすポリマー組成物を実施することを求めている。本開示は、一実施形態において粉末状材料を創部に局所的に投与することによって出血を抑制する方法について記載する。粉末状材料は、水性キャリア、又はトロンビンなどの活性剤を必要としない。本開示はまた、送達箇所及び送達率の両方に関して高度のユーザ制御によってポリマー組成物を患者へ送達するデバイス及び方法について記載している。本開示は、粉末状材料を創部に局所的に送達するデバイス、システム、及び方法をさらに提供する。
[0068]上記で簡単に論じたように、本開示は、様々な実施形態において、粉末状材料を創部に局所的に投与することによって出血を抑制するシステム、方法、及びデバイス、並びにそのような粉末状材料自体を対象とする。粉末状材料は、トロンビンなどの活性剤を含んでおらず、すぐに使用できる効果的な粉末止血剤である。同様に、本開示は、粉末止血剤などのポリマー組成物の精密な投与のための送達デバイス、送達システム、及び関連方法を対象とする。
[0069]本明細書に開示する材料は、架橋又は非架橋の生物学的に適合しているポリマーを含むことができ、そのような材料は、比較的持続性があり、通常はたとえば10日~120日の範囲内の傷の治癒のための時間と同等の分解時間を有する。比較として、本開示の非架橋の生物学的に適合しているポリマーは断片化されており、すなわち個別の乾燥微粒子として材料内に存在し、したがって水和されると(たとえば、血液中)、ポリマーは、0.01mm~5mm、たとえば0.05mm~1mmの範囲内のサブユニットサイズを有するヒドロゲルを形成する。いくつかの場合、生物学的に適合しているポリマーは膨潤性があり、完全に水和されると、200%~5,000%の範囲内、たとえば400%~5,000%及び/又は500%~1000%の範囲内の平衡膨潤を有する。
[00101]図1を次に参照すると、上記で論じた組成物のいずれかを正確に送達するための1つの好適な送達システム、並びに関連する方法及びデバイスが、システム100によって示されている。送達システム100は、シリンジ110と、シリンジ110に動作可能に結合されたばねキャップアセンブリ120とを含む。本開示の場合、本明細書のばねキャップアセンブリ120を区別なく送達デバイスと呼ぶことができる。シリンジ110は、プランジャ112及び胴部114を含む。プランジャ112は、胴部114の円筒形内面に同心円状(concentrically)に係合して封止するように位置し、したがってプランジャ112は、胴部114の長さに沿って並進することができる。プランジャ112及び胴部114はそれぞれ、ポリプロピレン、PVC、非DEHPのPVC、ポリエチレン、ポリスチレン、ポリプロピレン混合物、又は他の類似の材料などの任意の好適なプラスチック材料から構築することができる。プランジャ112及び胴部114はそれぞれ、ポリプロピレンから構築されることが好ましい。

[00134]図14A~図14Cに示すように、送達デバイスは、送達キット400内に提供することができる。送達キット400は、ばねアセンブリキット402及び粉末状顆粒キット404の両方を含むことができる。一実施形態では、ばねアセンブリキット402はばねキャップアセンブリ120を含み、粉末状顆粒キット404はシリンジ110を含み、シリンジ110には粉末状顆粒(たとえば、架橋牛ゼラチン及び非架橋牛ゼラチンの乾燥微粒子)が事前充填されている。異なる実施形態では、ばねアセンブリキット402は、ばねキャップアセンブリ120及び事前充填シリンジ110の両方を含み、粉末状顆粒キット404は、追加の事前充填シリンジ、シリンジ110を再充填するための追加の粉末状顆粒、シリンジ110を介して分注すべき他の材料などを含む。ばねキャップアセンブリ120、事前充填シリンジ110、及び粉末状顆粒を含む送達キット400の構成要素は、上記でより詳細に説明したように構成される。
(実施例)
[00136]ヘパリン化されたオスの豚の外科的に誘発された肝臓外傷を治療するための粉末止血剤架橋ゼラチンの止血効果について評価した。
[00149]実施例1に記載した粉末止血剤顆粒の膨潤度を判定した。血液又は他の流体に接触すると、微粒子は直径が最高70%まで膨潤し、最大膨潤体積が約10分で実現された。対照的に、フロシール(登録商標)VH S/D微粒子(トロンビンと組み合わせたゼラチンを含有する)は、血液又は他の流体に接触すると、直径が約10~20%膨潤し、最大膨潤体積は約10分で実現された。粉末止血剤顆粒によって実証された膨潤度の改善により、周辺組織に力を加えることによって止血を容易にし、以て強化された機械的タンポン作用を提供することが有利である。
[00150]ばねキャップアセンブリ120を使用する前述の方法の1つ又は複数を使用して、順次圧搾における粉末圧搾の効率及び予測可能性を評価した。
Claims (4)
- プランジャ及び胴部を含み、前記プランジャが、前記胴部の内面に沿って係合及び並進するように構成される、シリンジと、
前記シリンジの前記胴部内に配置された送達すべき粉末状物質と、
送達デバイスと、を備え、
前記送達デバイスが、
リテーナリングと、
キャップと、
前記リテーナリングと前記キャップとの間に配置されたばねと、を含み、
前記リテーナリング及び前記キャップが、前記ばねの両端を捕捉するように構築され、したがって前記ばねが、前記リテーナリング及び前記キャップによって保持され、前記キャップ、前記ばね、及び前記リテーナリングが、一体アセンブリを形成し、前記ばねが、圧縮ばねであり、したがって前記ばねが、前記キャップを前記リテーナリングから離れる方へ付勢し、
前記キャップが、
前記シリンジの前記プランジャ内へ延びて係合するように構成されたポストと、
前記キャップの内面から延びるように構成された複数のラッチアームとを含み、前記複数のラッチアームのそれぞれの端部が、傾斜カム面及び掛止ショルダを含み、前記掛止ショルダが、前記キャップの前記内面に対向するラッチ面を画定し、前記キャップが、前記キャップの前記内面から突出する隆起ステップ面をさらに含み、前記ラッチ面と前記隆起ステップ面との間で前記プランジャのフランジ付き端部を協働してロックし、

の間で可動である、送達システム。 - 前記シリンジ胴部が、前記胴部の遠位端にオリフィス、吐出し口、又はアパーチャを画定し、前記シリンジが、前記オリフィスから材料を放出するように構成される、請求項1に記載の送達システム。
- 放出される前記材料が、血液に露出されるとヒドロゲルを形成する生物学的に適合しているポリマーである、請求項2に記載の送達システム。
- 架橋牛ゼラチン及び非架橋牛ゼラチンの乾燥微粒子を含む事前充填シリンジと、
送達デバイスとを備え、
前記送達デバイスが、
リテーナリングと、
キャップと、
