Description

別の態様では本開示は1種類以上の薬学的に許容可能な担体と共に本明細書に記載される単離アンタゴニスト性抗原結合タンパク質を含む医薬組成物を提供する。本明細書に記載されるそれらの医薬組成物および使用方法は、下で詳細に説明されるように他の活性薬剤との併用(共投与)実施形態も包含する。
グルカゴン受容体とのそれらの相互作用により、本アンタゴニスト性抗原結合タンパク質は、現在の治療に関連する望まれない副作用のうちの一つまたは複数を低減および/または排除しつつ、糖新生およびグリコーゲン溶解を制御することにより血糖値を低下させるのに、並びにまた、グルカゴンのその受容体との相互作用の遮断が有益である広範囲の状態および障害を治療するのに、有用である。
循環器疾患は、心臓および/または血管に発症する種々様々な疾患、障害および状態の総称である。循環器疾患の種類としては、アンギナ、心発作(心筋梗塞)、アテローム性動脈硬化、心不全、循環器疾患、リウマチ性心疾患、心不整脈(異常な心臓の拍動)、脳血管疾患、先天性心疾患、心筋症、左室肥大、右室肥大、梗塞後心破裂、心臓の感染症、冠動脈疾患、抹消動脈疾患、腎動脈狭窄、大動脈瘤、心筋疾患、心臓弁障害、心筋炎、および心外膜炎が挙げられる。

本実施例において、本発明者らは、抗グルカゴン受容体モノクローナル抗体の、心筋梗塞後心不全におけるLVリモデリングに対する治療法としての可能性を評価した。本実施例では、配列番号8に記載の重鎖配列および配列番号9に記載の軽鎖配列を含む抗GCGR抗体(「REMD2.59C」)のインビボ活性を、心筋梗塞誘導C57BL/6マウスを用いて評価する。
本実施例では、本発明者らは、db/dbマウスにおける糖尿病によって誘導された心筋症に対するヒト抗GCGR抗体の潜在的な予防効果を評価した。本実施例では、配列番号51に記載の重鎖配列および配列番号52に記載の軽鎖配列を含むヒト抗GCGR抗体(「REMD477」)のインビボ活性を、C57BL/6/db/db雄糖尿病マウスおよびdb/+雄対照マウスを用いて評価した。
本実施例では、配列番号8に記載の重鎖配列および配列番号9に記載の軽鎖配列を含む抗GCGR抗体(「REMD2.59C」)のインビボ活性を、ミニブタ心筋梗塞モデル(シンクレア・リサーチ社)における有効性について評価する。
梗塞マウスの作製
MIは、以前の記述(Patten RD et al.,Am J Physiol.,1998;274:H1812−1820.[PubMed:9612394])の通りにマウスに誘導した。簡潔に説明すると、胸部を左開胸にて開いた。実体顕微鏡を用いて左冠動脈を目視にて特定し、7−0縫合糸を左心耳より1〜2mm下にある動脈に巻いた。心電図(ECG)を継続的にモニターした。縫合糸による左冠動脈の結紮によって、左冠動脈の永続的な閉塞が生じた。心臓色の蒼白さおよびST部分上昇によって心筋虚血を確認した。胸部を6−0絹縫合糸で縫合した。自発呼吸が再開した後、気管内チューブを取り除いた。完全に意識が戻るまで動物を監視した。動物をケージに戻した後、標準的な固形飼料および水を与えた。
インビボにおける心機能を、経胸壁心エコー法(Acuson P300,18 MHz transducer;Siemens)により、意識のあるマウスにおいて以前の記述の通りに評価した。マウスの心臓を、乳頭レベルで傍胸骨短軸から2次元的にイメージングした。この画像から、左室寸法を測定した。左室内径短縮率(FS)を、左心室(LV)の拡張末期径(EDD)および収縮末期径(ESD)から算出した。MモードのEDDおよびESDを3〜5回の拍動から平均した。研究および分析は処置を知らされていない研究者が行った。
マウスをペントバルビタール(40mg/kg、腹腔内)で麻酔した。胸部を左開胸にて開いた。梗塞心臓を露出させた。マウス用Mikro−tipカテーテル(SPR1000)を、梗塞を起こしていない心室壁を通じて左心室内に挿入した。左室圧をPowerLabシステム(Adインスツルメント社(Adinstruments))によって測定した。心室収縮能を、Chart 7ソフトウェア(Adインスツルメント社)により算出した。
心臓を、10%緩衝ホルマリンで固定し、パラフィン包埋し、6μm切片にした。1心臓当たり1つの中縦断面(middle longitudinal section)をマッソントリクローム染色した。左心室の非梗塞巣からの、8つの無作為に選択した視野(400×)を、線維症および筋細胞サイズについて、顕微鏡下で調べた。各群は5〜6の心臓を含み、各群において最低40視野を、コンピュータ化された側面法(NIH Image J)によって解析した。線維症を評価するため、線維性である青色の面積および全心筋面積を測定した。線維性面積は心筋面積に対する線維性面積の割合として表した。横断面筋細胞において、筋細胞サイズを測定した。合計100〜150細胞/心臓を解析した。梗塞心臓のサイズを評価するために、2つの方法を使用した。梗塞面積は、心臓写真の表面および裏面を用いて、合計心室面積に対する梗塞心室面積の割合として求めた。梗塞サイズは、心臓切片を用いて、心室壁全長に対する梗塞心室壁の長さの割合として測定した。観測者は心臓切片の由来を知らされていない者とした。
TUNELアッセイを、In Situ Apoptosis Detection Kitを用いて行った。簡潔に説明すると、心臓を10%ホルマリン溶液で灌流固定し、パラフィン包埋し、6μm切片にした。1心臓当たり1つの中央(middle)縦断面をTUNEL染色した。プロテアーゼK(20μg/ml)を各スライドに加えた。切片を2%過酸化水素で覆って、内在性ペルオキシダーゼを不活性化した。固定後、切片をTdT緩衝液と一緒に37℃で30分間インキュベートした。1×SSCで反応を止めた。洗浄後、スライドを使用準備済み(RTU)ストレプトアビジン−HRPと一緒に30分間インキュベートした。DAB溶液を添加して陽性シグナルを発色させた。RTUヘマトキシリンによる対比染色後、スライドを封入剤で覆い、顕微鏡下で解析した。各群は5〜6の心臓を含んだ。1心臓当たり梗塞巣から8つの視野(400×)を、陽性細胞および総細胞について、コンピュータ化された側面法(NIH Image J)を用いて解析した。アポトーシスの程度は総細胞に対する陽性細胞の割合として表した。

Claims (6)
- 心不全の治療において使用するための医薬組成物であって、前記医薬組成物が、(i)ヒトグルカゴン受容体に特異的に結合する単離アンタゴニスト性抗原結合タンパク質および(ii)1種類以上の薬学的に許容可能な担体を含み、
前記単離アンタゴニスト性抗原結合タンパク質が:
(1)配列番号51の重鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列と、配列番号52の軽鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列とを含むヒト抗GCGR抗体;および
(2)配列番号8の重鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列と、配列番号9の軽鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列とを含むキメラ抗GCGR抗体
からなる群から選択されることを特徴とする医薬組成物。 - 請求項1に記載の医薬組成物において、治療対象の心不全が、心筋梗塞後心不全および糖尿病性心筋症性心不全からなる群から選択されることを特徴とする医薬組成物。
- 左室(LV)リモデリングの治療において使用するための医薬組成物であって、前記医薬組成物が、(i)ヒトグルカゴン受容体に特異的に結合する単離アンタゴニスト性抗原結合タンパク質および(ii)1種類以上の薬学的に許容可能な担体を含み、
前記単離アンタゴニスト性抗原結合タンパク質が:
(1)配列番号51の重鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列と、配列番号52の軽鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列とを含むヒト抗GCGR抗体;および
(2)配列番号8の重鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列と、配列番号9の軽鎖アミノ酸配列またはこれと少なくとも90%の配列相同性を有するアミノ酸配列とを含むキメラ抗GCGR抗体
からなる群から選択されることを特徴とする医薬組成物。 - 請求項1〜3のいずれか一項に記載の医薬組成物において、単離アンタゴニスト性抗原結合タンパク質の治療有効量が、1週間当たり、0.001〜100mg/kg体重、0.001〜90mg/kg体重、0.001〜80mg/kg体重、0.001〜70mg/kg体重、0.001〜60mg/kg体重、0.001〜50mg/kg体重、0.001〜40mg/kg体重、0.001〜30mg/kg体重、0.001〜20mg/kg体重、0.001〜10mg/kg体重、0.001〜5mg/kg体重、0.001〜4mg/kg体重、0.001〜3mg/kg体重、0.001〜2mg/kg体重、0.001〜1mg/kg体重、0.010〜50mg/kg体重、0.010〜40mg/kg体重、0.010〜30mg/kg体重、0.010〜20mg/kg体重、0.010〜10mg/kg体重、0.010〜5mg/kg体重、0.010〜4mg/kg体重、0.010〜3mg/kg体重、0.010〜2mg/kg体重、0.010〜1mg/kg体重、0.1〜50mg/kg体重、0.1〜40mg/kg体重、0.1〜30mg/kg体重、0.1〜20mg/kg体重、0.1〜10mg/kg体重、0.1〜5mg/kg体重、0.1〜4mg/kg体重、0.1〜3mg/kg体重、0.1〜2mg/kg体重、0.1〜1mg/kg体重、0.5〜50mg/kg体重、0.5〜40mg/kg体重、0.5〜30mg/kg体重、0.5〜20mg/kg体重、0.5〜10mg/kg体重、0.5〜5mg/kg体重、0.5〜4mg/kg体重、0.5〜3mg/kg体重、0.5〜2mg/kg体重、0.5〜1mg/kg体重、1〜50mg/kg体重、1〜40mg/kg体重、1〜30mg/kg体重、1〜20mg/kg体重、1〜10mg/kg体重、1〜5mg/kg体重、1〜4mg/kg体重、1〜3mg/kg体重、1〜2mg/kg体重、および0.1〜1mg/kg体重からなる群から選択されることを特徴とする医薬組成物。
- 請求項4に記載の医薬組成物において、単離アンタゴニスト性抗原結合タンパク質の治療有効量が、1週間当たり、0.01〜10mg/kg体重であることを特徴とする医薬組成物。
- 請求項1〜5のいずれか一項に記載の医薬組成物において、抗心不全薬を更に含み、前記抗心不全薬が、アンジオテンシン変換酵素(ACE)阻害剤、βアドレナリン遮断薬、アンジオテンシン(angiotension)II受容体遮断薬(ARB)、利尿薬、およびジギタリスからなる群から選択されることを特徴とする医薬組成物。
