Description
本出願は、2016年5月16日に出願された、表題「半月板被膜の破損および修復(Meniscal Capsular Disruption and Repair)」の米国仮特許出願62/337,059号、および2017年3月13日に出願された、表題「膝関節包の破損および修復(Knee Joint Capsular Disruption And Repair)」の米国仮特許出願62/470,473号の優先権を主張する。米国仮特許出願62/337,059号および米国仮特許出願62/470,473号の全ての開示は、参照により本明細書に組み込まれる。
用語「被膜破損(capsule disruption)」または「被膜破損(capsular disruption)」は、被膜が半月板および/または骨膜(すなわち、大腿骨および/または脛骨)から脱離している状態を指す。被膜が他の組織への結合を失うとき、半月板は、その解剖学的位置からずれる可能性がある。
本開示の例示的な実施形態に従って実施された3つの実施例研究を、下に記載する。特に、第1の実施例は、例示的な実施形態による内側半月板被膜破損に関する研究を詳述する。第2の実施例は、例示的な実施形態による内側半月板被膜修復に関する研究を詳述する。第3の実施例は、例示的な実施形態による内側半月板被膜修復の効化を検査する生体力学試験に関する研究を詳述する。
死体に実験を実施して、多くの場合に半月板突出の病理に関連している、膝の前下内側被膜の病変または破損を作り出す可能性を決定した。
最初に緩和状態の試料により出発し、次に外反および内反負荷をかけ、続いて内部および外部回転して、臨床ステップを模倣しながら、死体の膝の診断用超音波を実施した。内側および外側半月は、超音波によって確証されるように無傷であった(図17)。
上記の手順は、半月板突出をもたらした(図18)。その区域の注意深い切開を完了させて、被膜および半月板を可視化した。被膜を、半月板または被膜組織に損傷を与えることなく、脛骨から効率的に上昇させた。
実施例1によって生じた死体の半月板突出の被膜の修復を、2つの無結節SutureTak(登録商標)アンカーを使用して完了した。第1のアンカーを、前に配置し、第2のアンカーを第1のアンカーの後に配置した。経皮的に完了され得る修復を再現する試みにおいて、それぞれのアンカーに関連する縫合糸を、被膜組織の中に注意深く通した。このステップが完了すると、第1のアンカーからの縫合糸を第2のアンカーに装填して、縫合糸の橋を作り出した。これを、第2のアンカーからの縫合糸を第1のアンカーに戻すように繰り返し、切り口を閉じた。
死体の膝を試験して、自然の突出の量を決定し、実験的病変によって突出を生じ、実験的病変の修復効果を試験した。
Claims (11)
- 脛骨の少なくとも一部、大腿骨の少なくとも一部、半月板、前記半月板に結合し前記脛骨から脱離した膝被膜を含み、前記膝被膜が前記半月板に結合し前記脛骨から脱離した複数の冠状線維を有する半月脛骨靱帯を含み、構成要素の全てが合成である膝モデル。
- 前記半月板が、半月板ルートの断裂を模倣することができる、請求項1に記載の合成膝モデル。
- 前記半月板が突出位置にある、請求項1に記載の合成膝モデル。
- 前記複数の冠状線維が前記脛骨に結合することができる、請求項1に記載の合成膝モデル。
- 前記膝被膜が前記大腿骨の少なくとも一部から脱離可能である、請求項1に記載の合成膝モデル。
- 前記膝被膜が合成内側膝被膜を含み、前記合成内側膝被膜と合成内側側副靱帯の集合点で肥厚化する表層、中間層、および深層を含み、これらの層は裸眼でも見ることができる、請求項1に記載の合成膝モデル。
- 超音波可能な膝モデルである、請求項1に記載の合成膝モデル。
- 合成皮膚を含む、請求項7に記載の合成膝モデル。
