Description
[1]ラパマイシン、テムシロリムス、エベロリムス、リダフォロリムス、TAFA93、ウミロリムス、オルコロリムス、ゾタロリムス、及び薬学的に許容されるこれらの塩からなる群から選択される少なくとも1つの化合物を有効成分として含有する進行性骨化性線維異形成症の予防剤又は治療剤。
[2]上記有効成分として、ラパマイシン、テムシロリムス、エベロリムス、リダフォロリムス、及び薬学的に許容されるこれらの塩からなる群から選択される少なくとも1つの化合物を含有する、上記[1]に記載の予防剤又は治療剤。
[3]上記有効成分が、ラパマイシン又は薬学的に許容されるその塩である、上記[1]に記載の予防剤又は治療剤。
[4]上記有効成分が、エベロリムス又は薬学的に許容されるその塩である、上記[1]に記載の予防剤又は治療剤。
[5]アクチビンA受容体タイプI(ACVR1)にアミノ酸変異を有する患者用である、上記[1]〜[4]のいずれかに記載の予防剤又は治療剤。
[6]上記アミノ酸変異が、上記ACVR1のGSドメイン又はキナーゼドメインにおけるアミノ酸変異を含む、上記[5]に記載の予防剤又は治療剤。

[9]フレアアップ又は異所性骨化の発症を回避又は抑制する薬剤である、上記[1]〜[8]のいずれかに記載の予防剤又は治療剤。

[20]進行性骨化性線維異形成症の予防剤又は治療剤が、フレアアップ又は異所性骨化の症状の治癒、症状の改善、症状の軽減、又は症状の進行を抑制する薬剤である、上記[18]の使用。
(1)フレアアップ又は異所性骨化を発症する前から日常的にFOP予防剤/治療剤を投薬する。
(2)外傷及び感染等のフレアアップを発症する要因が生じた際にFOP予防剤/治療剤を投薬し、フレアップの発症を回避又は抑制する。
(3)炎症、痛み、フレアアップ等が生じたときにFOP予防剤/治療剤を投薬し、異所性骨化の発症を回避又は抑制する。
(1)フレアアップの症状の抑制、フレアアップが発症している期間の短縮。
(2)フレアアップを発症した後の異所性骨化の症状の抑制。
(3)通常、進行性の経過をとる異所性骨化の症状又は進行の抑制。
(方法)
山中らの方法(Cell,2007,131(5),8611−8672.)に従って、FOP患者由来のiPS細胞(以下、「患者由来iPS細胞株」という場合がある。)を樹立した。この患者の細胞は、ACVR1遺伝子にFOPの原因となる典型的な変異を有することを確認した。この変異は、ACVR1のアミノ酸配列(配列番号2に記載のアミノ酸配列)における第206番目のアルギニン残基をヒスチジン残基へ置換する変異である。また、コントロールとして使用する目的で、BAC(Bacterial Artificial Chromosome、大腸菌人工染色体)ノックインベクターによる遺伝子修復によって、上記ACVR1遺伝子の変異を正常型(野生型)に修復したiPS細胞株(以下、「変異修復iPS細胞株」(gene−corrected (rescued) iPS cell clone)という場合がある。)を樹立した。
FOP患者由来の細胞をアクチビン−Aで処理すると、軟骨分化が促進された。このとき、ラパマイシン、テムシロリムス、エベロリムス、リダフォロリムスはいずれも軟骨分化を抑制することが明らかになった(ラパマイシン0.1、1、10、100nM、エベロリムス及びテムシロリムス1、10、100nM、P<0.0001、ヒトアクチビン−Aのみで処理した場合に対するDunnett型多重比較)。
(方法)
実施例1と同様の方法によって、患者由来iPS細胞株及び変異修復iPS細胞株から間葉系幹細胞様の細胞をそれぞれ分化誘導した。これらの細胞とヒトアクチビン−Aを発現するC3H10T1/2細胞とをそれぞれ混合し、免疫不全マウス(NOD/SCIDマウス)に移植した。具体的には、免疫不全マウスの右足側腓腹筋に患者由来iPS細胞株から分化した間葉系幹細胞様の細胞とC3H10T1/2細胞とを混合したものを移植し、免疫不全マウスの左足側腓腹筋に変異修復iPS細胞株から分化した間葉系幹細胞様の細胞とC3H10T1/2細胞とを混合したものを移植した。移植後、週に5回、ラパマイシンを経口(3、10又は30mg/kg)又は腹腔内(5mg/kg)投与した。移植から6週間後に、マイクロCT(商品名inspeXio SMX−100CT、株式会社島津製作所製)を用いて異所性骨の形成を観察し、TRI/3D−BON(ラトックシステムエンジニアリング社製)により異所性骨量(cm3)を算出した。右足側腓腹筋内に形成された異所性骨量の算出結果を図2に示す。図2において、棒グラフは各投与群の平均値±標準偏差(SD)を示す(n=4〜5)。
ラパマイシン非投与群では全てのマウスにおいて、患者由来iPS細胞株から分化した間葉系幹細胞様の細胞とヒトアクチビン−Aを発現するC3H10T1/2細胞を移植した部位(右足側腓腹筋)に異所性骨化が認められた。一方、ラパマイシン投与群では、右足側腓腹筋における異所性骨化が有意に抑制された(腹腔内投与のラパマイシン5mg/kg投与群、P<0.01、ラパマイシン非投与群に対するスチューデントt検定。経口投与の3、10及び30mg/kg投与群、P<0.0001、ラパマイシン非投与群に対するDunnett型多重比較)。なお、いずれの投与群においても、変異修復iPS細胞株から分化した間葉系細胞様の細胞とヒトアクチビン−Aを発現するC3H10T1/2細胞を移植した部位(左足側腓腹筋)に異所性骨化は認められなかった。
Claims (7)
- ラパマイシン、テムシロリムス、エベロリムス、リダフォロリムス、TAFA93、ウミロリムス、オルコロリムス、ゾタロリムス、及び薬学的に許容されるこれらの塩からなる群から選択される少なくとも1つの化合物を有効成分として含有する進行性骨化性線維異形成症の予防剤又は治療剤。
- 前記有効成分として、ラパマイシン、テムシロリムス、エベロリムス、リダフォロリムス、及び薬学的に許容されるこれらの塩からなる群から選択される少なくとも1つの化合物を含有する、請求項1に記載の予防剤又は治療剤。
- 前記有効成分が、ラパマイシン又は薬学的に許容されるその塩である、請求項1に記載の予防剤又は治療剤。
- 前記有効成分が、エベロリムス又は薬学的に許容されるその塩である、請求項1に記載の予防剤又は治療剤。
- アクチビンA受容体タイプI(ACVR1)にアミノ酸変異を有する患者用である、請求項1〜4のいずれか一項に記載の予防剤又は治療剤。
- フレアアップ又は異所性骨化の発症を回避又は抑制する薬剤である、請求項1〜5のいずれか一項に記載の予防剤又は治療剤。
- フレアアップ又は異所性骨化の症状の治癒、症状の改善、症状の軽減、又は症状の進行を抑制する薬剤である、請求項1〜5のいずれか一項に記載の予防剤又は治療剤。
