Description
ステントにはさらに、組織の内方成長を防止するためのシリコーンカバーのようなカバーが設けられてもよい。
同一出願人による特許文献1(その全内容は参照により本願に組み込まれる)は、トリガ式の生体接着剤を備えた医療用デバイスを開示している。
1つの態様では、本発明は、少なくとも1つの生体接着剤を含んでなる第1のコーティング組成物と、第1のコーティング組成物を覆って配置された、少なくとも1つの生分解性ポリマーを含んでなる第2のコーティング組成物と、を備えたステントに関する。
本発明の実施形態は数多くの形をとりうるが、本明細書中で詳細に説明されているのは本開示の特定の実施形態である。この説明は本開示の原理の例示であり、本開示を本明細書中に例証された特定の実施形態に限定するように意図するものではない。
図3は、ステント壁12、生体接着層14および外側生分解層16を有するステント10の部分拡大図である。
図7は、本発明によるカバー付きステント10の粘着性ポリマーコーティングを例証する図6の区画7で得られる拡大部分断面図である。
ヒドロゲルは本明細書中における有用性があり、該ヒドロゲルには、限定するものではないが、ポリビニルピロリドン(PVP)、ポリアクリラート、キトサン、ポリエチレングリコール(PEG)のようなポリアルキレングリコール、ポリエチレングリコール/デキストランアルデヒド、ポリエチレンオキシドおよびポリプロピレンオキシドのようなポリアルキレンオキシド、ポリビニルエステル、例えばポリ酢酸ビニル、ポリヒドロキシエチルメタクリラート、ポリビニルアルコールおよびポリビニルエーテル、高い荷電密度を有しているポリアニオン、例えば(メタ)アクリル酸の架橋型または非架橋型のホモポリマーおよびホモポリマー、例えばジビニルグリコールで架橋されたアクリル酸ポリマーであるNoveon(登録商標)AA‐1ポリカルボフィル、アクリル酸のコポリマー、例えばアクリル酸‐コ‐アクリルアミドコポリマー(P(AA‐co‐AM))、ならびにポリアクリル酸の誘導体、例えばポリアクリル酸の塩類およびエステル、例を挙げると、米国オハイオ州ウィクリフのルーブリゾール(Lubrizol)から入手可能なポリアルケニルポリエーテルで架橋されたアクリル酸のホモポリマーおよびコポリマーであるCarbopol(登録商標);(メタ)アクリル酸エステルのポリマーおよびコポリマー;などが挙げられる。これらのポリマーのうち多くの誘導体も本明細書中で使用されうる。例えば米国特許第6,391,033号明細書(その全内容は参照により本願に組み込まれる)を参照されたい。
ポリアクリル酸(polyacylic acid)粘膜接着についての議論に関しては、ヘサン・パーク(Haesun Park)およびジョゼフR.ロビンソン(Joseph R. Robinson)、「ポリ(アクリル酸)ヒドロゲルの粘膜接着のメカニズム(Mechanisms of Mucoadhesion of Poly (acrylic Acid) Hydrogels)」、ファーマシューティカル・リサーチ(Pharmaceutical research)、1987年、第4巻、第6号、p.457−464を参照されたい。
接着層を形成しない任意の生分解性材料が本明細書中で使用可能である。これらの生分解性材料は、in vivoで分解してその完全性を喪失する。適切な生分解性ポリマーの例には、限定するものではないが、ポリ(アミド)、例えばポリ(アミノ酸)およびポリ(ペプチド)、ポリ(エステル)、例えばポリラクチド、例えばポリ(DL‐ラクチド)およびポリグリコリド、ならびにこれらのコポリマー、例えばポリラクチド‐コ‐グリコリド、例えばポリ(DL‐ラクチド‐コ‐グリコリド)、ポリ(L‐ラクチド‐コ‐グリコリド)、ポリ(カプロラクトン)およびポリラクチド‐コ‐カプロラクトン、例えばポリ(DL‐ラクチド‐コ‐カプロラクトン)およびポリ(L‐ラクチド‐コ‐カプロラクトン)、ポリ(無水物)、ポリ(オルトエステル)、ポリ(カーボネート)、例えばチロシン誘導型ポリカーボネート、ポリヒドロキシバレラート、ポリヒドロキシブチラート、ポリヒドロキシブチラート‐コ‐バレラート、ならびにこれらの化学的誘導体(置換、化学基の付加、例えばアルキル、アルキレン、ヒドロキシル化、酸化、および当業者によって日常的になされるその他の修飾)、これらのコポリマーおよび混合物、が挙げられる。
生体接着剤のうちの一部はステント上に恒久的なコーティングを形成する一方、他の生体接着剤はステント上に一時的なコーティングを形成することになる。治療後にステントを除去することが望ましい場合には、一時的な生体接着剤を選択することが望ましい。
恒久的生体接着剤は「活性化するために混合する」プロセスによって生成される架橋材料であってよい。
生体接着層を覆う生分解性コーティング層を使用することが好ましいが、一部の生体接着剤については生分解性コーティング層を伴わずに使用することが可能な場合もある。
治療がなされる病状に応じて本明細書中で様々な治療薬が使用される。本明細書中で使用されるように、用語「治療薬」、「薬物」、「薬学的活性作用薬」、「薬学的活性物質」、「有用な作用薬」、「生物活性作用薬」、およびその他の関連用語は、本明細書中で互換的に使用可能であり、かつ、遺伝子治療薬、非遺伝子治療薬および細胞を含む。薬物は単独で使用されてもよいし、他の薬物と組み合わせて使用されてもよい。薬物には、遺伝物質、非遺伝物質、および細胞が含まれる。
適用するために、治療薬は、生体接着性または生分解性のポリマー材料と共に溶媒または共溶媒混成物中に溶解せしめることが可能である。
その他の適切な溶媒には、テトラヒドロフラン、クロロホルム、トルエン、アセトン、イソオクタン、1,1,1‐トリクロロエタン、またはこれらの混合物が挙げられる。アセトンは極性系または非極性系のいずれとも共に使用することが可能であり、いずれの種類の溶媒とも容易に混成される。
本明細書中で使用することができる具体的なコーティング技法の例には、限定するものではないが、ドロップ・オン・デマンド型(Drop−on−Demand)コーティング技法、マイクロペン型(MicroPen)コーティング技法およびM3Dダイレクト・ライト(M3D direct−write)コーティング技術が挙げられる。
ステントはその後、従来のステント装荷技法を使用してステント送達システムに装荷される。
ステントが患者の体内で展開配置されると、生分解性ポリマー層が腐食し、所望の治療部位でステントの接着をもたらす生体接着層を露出させ、よって望ましからぬステント位置移動を防止する。
上記のリストは例示を目的としたものにすぎず、本発明の範囲を限定するものとして意図されたものではない。
Claims (19)
- ステントであって、
少なくとも1つの生体接着剤を含んでなる第1の層と、
第1の層を覆って配置されている、少なくとも1つの生分解性材料を含んでなる第2の層と、
ステント上に配置されているシリコーン管状構造物であって、前記生体接着剤を含んでなる第1の層の下方に配置されているシリコーン管状構造物と、
を含んでなるステント。 - 前記生体接着剤は粘膜接着剤である、請求項1に記載のステント。
- 前記粘膜接着剤は、アクリル酸および架橋アクリル酸のホモポリマーおよびコポリマー、多糖類、セルロースならびにこれらの混合物で構成されている群から選択された少なくとも1つのメンバーを含んでなる、請求項2に記載のステント。
- 前記粘膜接着剤はヒドロゲルを含んでなる、請求項2に記載のステント。
- 前記生分解性材料は、ポリラクチドおよびポリラクチド‐コ‐グリコリドで構成されている群から選択された少なくとも1つのメンバーを含んでなるポリマーである、請求項1に記載のステント。
- 前記生分解性材料は生分解性の金属元素を含んでなる、請求項1に記載のステント。
- 前記生分解性の金属元素は、鉄およびマグネシウムならびにこれらの合金のうち少なくとも1つを含んでなる、請求項6に記載のステント。
- 前記ステントは、食道ステント、膵臓ステント、胆管ステント、十二指腸ステント、結腸ステントおよび気道ステントで構成されている群から選択されたメンバーを含んでなる、請求項1に記載のステント。
- 前記ステントは相対する終端部分を有し、前記終端部分のうち少なくとも1つはフレア状部を含んでなる、請求項1に記載のステント。
- 前記ステントは基端部および先端部を有し、前記基端部および先端部は相対する終端部分を含んでなり、前記先端部は、前記フレア状部を含んでなるステントの相対する終端部分のうち少なくとも1つを含んでなる、請求項9に記載のステント。
- 前記シリコーン管状構造物は、前記生体接着剤を含んでなる第1の層の下方においてステントの内側表面に配置されている、請求項1に記載のステント。
- 前記ステントは生体接着剤によって包み込まれている、請求項1に記載のステント。
- 前記ステントは外側表面および内側表面を含んでなり、生体接着剤はステントの外側表面の少なくとも一部分に配置されている、請求項1に記載のステント。
- 前記生体接着剤はステントの外側表面の全体に配置されている、請求項13に記載のステント。
- 前記生体接着剤は前記フレア状部に適用されている、請求項9に記載のステント。

前記シリコーン管状構造物の少なくとも一部分上に生体接着剤を適用するステップと、
前記生体接着剤を覆って生分解性層を適用するステップと、
を含んでなる方法。 - ステントであって、
ステント本体と、
ステント本体上に配置されている、少なくとも1つの生体接着剤を含んでなる第1の層であって、前記生体接着剤は天然ポリマー系材料または糖質から形成された合成ポリマー材料である、第1の層と、
第1の層を覆って配置されている、少なくとも1つの生分解性材料を含んでなる第2の層と、
シリコーン管状構造物であって、ステント本体がシリコーン管状構造物に埋め込まれるようにステント本体に配置されており、さらに前記生体接着剤を含んでなる第1の層の下方に配置されているシリコーン管状構造物と、
を含んでなるステント。
