血管系を介して組織切断用具を血管の所望の位置に挿入すること、及び、前記所望の位置にて組織切断用具の形状の変化を提供して前記血管に隣接する心臓組織に貫入することを含むこと。
Claims (48)
- 心房性細動を治療するために、心臓組織における望ましくない信号伝達を低減するように構成された組織切断用具であって、
当該組織切断用具は、一時的な配送形状にて血管系を介して挿入されると共に、治療することが意図されている心房に少なくとも部分的に位置決めされ、且つ、それに続いて前記一時的な配送形状から、拡張された配送形状を介して、前記心房組織の外側表面を少なくとも越えて伸びるところの更に拡張された形状への形状変化に供されるように構成されており、
当該組織切断用具は、前記心房を含む前記心臓組織の切断のための切断作用を生み出すように構成されており、更に、当該組織切断用具は、前記切断作用によって作られた前記組織の切断片が、電気信号を伝えない瘢痕組織に連続的に置き換えられて、前記心房性細動を生じさせる前記心臓組織の異所性部位を隔離するようなやり方で、前記切断作用により前記心臓組織の壁を貫通するように構成されており、
当該組織切断用具は、少なくとも部分的に球形状をなしている、組織切断用具。 - 前記用具が、左又は右の心房の上部又は下部に位置決めされるように構成されている請求項1に記載の組織切断用具。
- 前記用具が、少なくとも1本の線材又はワイヤからできている請求項1又は2に記載の組織切断用具。
- 前記用具が、2本以上のワイヤからできており、それらのワイヤがお互いに連結され又は編まれている請求項3に記載の組織切断用具。
- 前記用具が、前記心房の壁を通り抜ける切断の前に、治療することが意図されている前記心房の少なくとも一部を取り囲むような形での網状パターンに形成されている請求項1〜4のいずれか一項に記載の組織切断用具。
- 前記網状パターンが前記心房の大部分を取り囲む球形状を形成する請求項5に記載の組織切断用具。
- 前記網状パターンが前記心房の大部分を取り囲むような楕円形のセグメントを形成する請求項5に記載の組織切断用具。
- 前記網状パターンがカップ形を形成する請求項5〜7のいずれか一項に記載の組織切断用具。
- 前記用具は、前記心房の内側表面の少なくとも一部と同一であるように形成されると共に、当該用具が心臓の壁全体を通り抜けて心房の外側に来ることで当該心房を細片に切断することを許容する程度に大きくなるように形成されており、
電気信号を伝える組織を含む心房壁組織の島々が次の島に異常な電流を伝達できないように隔離されるように、前記切断片が前記電気信号を伝えない瘢痕組織に連続的に置き換えられように構成されている請求項1〜8のいずれか一項に記載の組織切断用具。 - 前記用具は、前記心房の内側に配置されるような球形状をしている請求項5に記載の組織切断用具。
- 前記組織切断用具は、心臓での望ましくない電気信号の伝播に対してブロックを形成するように意図された切断パターンを作り出すように構成されている請求項1〜10のいずれか一項に記載の組織切断用具。
- 前記用具は、一時的な配送形状から前記心臓の心房での拡張された配送形状への前記形状変化を達成するように構成された形状記憶材を備えている請求項1〜11のいずれか一項に記載の組織切断用具。
- 前記形状記憶材は、吸収性の形状記憶ポリマーである請求項12に記載の組織切断用具。
- 前記ポリマーの吸収時間は、ポリマーの成分によって決定可能である請求項13に記載の組織切断用具。
- 前記ポリマーの吸収時間は、X線放射線、超音波、電子ビーム、または所定波長の光によるような外部的な変更作用によって決定可能である請求項13に記載の組織切断用具。
- 前記用具が生分解性である請求項1〜15のいずれか一項に記載の組織切断用具。
- 前記用具は、管状部と結合されている請求項1〜16のいずれか一項に記載の組織切断用具。
- 前記用具の管状部は、一時的な配送形状にて血管系を介して心臓に隣接する血管に挿入されると共に、それに続いて、前記心臓組織及び/又は前記血管を切断するような切断作用を生み出すために、前記一時的な配送形状から、拡張された配送形状を経由して、前記組織の外側表面を少なくとも越えて伸びるところの更に拡張された形状への形状変化に供されるように構成されており、
前記用具の管状部は、当該管状部の長手方向において互いに接続される複数のセグメントを備え、前記複数のセグメントのうちの第1のセグメントは、少なくとも前記拡張された配送形状において、その管状部の前記長手方向に対し直交する方向における寸法が第2のセグメントにおける当該寸法より大きなものである請求項17に記載の組織切断用具。 - 前記第2のセグメントが、前記第1のセグメントよりも小さな直径を有する肺静脈系の枝部に合うように構成されている請求項18に記載の組織切断用具。
- 前記用具が更に、心臓の心房壁に挿入されるために、当初、当該組織切断用具の管状部から、前記長手方向に対し略直角に伸びるように構成された少なくとも1つの切断アームを備えており、前記切断アームが、当該組織切断用具の管状部から放射状に伸びるように形状を変化させるように構成されている請求項18または19に記載の組織切断用具。
- 前記少なくとも1つの切断アームが、少なくとも1つの閉鎖ループを備えた形を有している請求項20に記載の組織切断用具。
- 前記用具が挿入されるところの前記血管が、冠状静脈洞である請求項18〜21のいずれか一項に記載の組織切断用具。
- 前記用具の外側表面には鋭利な刃が設けられている請求項1〜22のいずれか一項に記載の組織切断用具。
- 前記用具の外側表面には薬剤が設けられている請求項1〜23のいずれか一項に記載の組織切断用具。
- 前記薬剤が、組織を通り抜ける切断効果を高めるような薬剤を含んでいる請求項24に記載の組織切断用具。
- 前記切断効果を高めるような薬剤が、アルコール、グルタールアルデヒド、ホルムアルデヒド、及び、タンパク質分解酵素からなる群のうちのいずれか一種である請求項25に記載の組織切断用具。
- 前記タンパク質分解酵素がコラーゲンである請求項26に記載の組織切断用具。
- 前記薬剤が、当該用具が挿入される血管の壁の肥厚化を妨げるような薬剤を含んでいる請求項24に記載の組織切断用具。
- 前記肥厚化を妨げるような薬剤が、シクロスポリン、タキシフェロール、ラパマイシン及びタクロリムスからなる群のうちのいずれか一種である請求項28に記載の組織切断用具。
- 前記薬剤が、内皮増殖因子、ヘパリン、及び、アミオダロン、ソタロール又はそのほかの抗不整脈剤からなる群のうちのいずれか一種である請求項24に記載の組織切断用具。

- 心拍調節系における障害の治療のために、請求項1〜34のいずれか一項に記載の少なくとも一つの組織切断用具を備えた、形状が変化する切断用具のキットであって、
当該キットは、複数の形状が変化する切断用具を備えており、
前記形状が変化する切断用具の各々が、第1の配送状態および第2の配送状態を有しており、前記第1の状態の用具は血管系内での所望の位置に挿入可能であるような寸法を有すると共に、その用具は前記所望の位置に配置されるとほぼ前記第2の状態に形状を変えることが可能であり、前記第2の状態の組織切断用具は、少なくとも部分的に球状の形状を有しており、それは所望の位置での血管の直径よりも大きな直径を目指すものであり、これにより、当該組織切断用具が前記所望位置で当該用具を取り囲む組織に埋め込まれて、電気信号の伝達を妨げるべく組織を破壊するものであり、
前記形状が変化する切断用具の少なくとも1つが、心臓の肺静脈の開口部での所望の位置に挿入されるように構成され、且つ、前記形状が変化する切断用具の少なくとも1つが、冠状静脈洞での所望の位置に挿入されるように構成されており、
実際の治療位置での解剖学的条件に適合し且つ当該用具の切断作用による前記組織の切断パターンを作り出すようにデザインされた用具から、完全なキットが組み立てられると共に、これらの用具は、切断片を、電気信号を伝えない瘢痕組織に連続的に置き換えるように構成されている切断用具キット。 - 前記切断パターンは、各個別のケースにおいて、心拍調節系における前記障害を生じさせる異所性部位の配置から独立している請求項35に記載の切断用具キット。
- 前記切断パターンが実質的に、
左上肺静脈(LSPV)と左下肺静脈(LIPV)の周りでの第1の切断(8)、及び、右上肺静脈(RSPV)と右下肺静脈(RIPV)の周りでの対応する第2の切断(10);
肺静脈(PV)の周りでの前記第1の切断(8)と前記第2の切断(10)とを接続する第3の切断(12);
前記第3の切断(12)から冠状静脈洞(CS)までの第4の切断(14);
左肺静脈から左心耳までの第5の切断(16);
下大静脈(IVC)から上大静脈(SVC)までの第6の切断(18);
右肺静脈の周りの前記第2の切断(10)と、前記下大静脈(IVC)及び上大静脈(SVC)間の前記第6の切断(18)とを接続する第7の切断(20);

から選択される複数の切断を備えたメイズIIIパターンである、請求項35又は36に記載の切断用具キット。 - 前記形状が変化する切断用具の一つが、肺静脈に挿入されるように構成されると共に、アームを備えており、そのアームは、前記第2の状態では、冠状静脈洞における別の形状が変化する切断用具に接触するように設けられている請求項35に記載の切断用具キット。
- 前記アームが、冠状静脈洞における前記形状が変化する切断用具に接触する領域で溝を備えている請求項38に記載の切断用具キット。
- 前記形状が変化する切断用具の少なくとも1つが、下大静脈に挿入されるように構成されている請求項35〜39のいずれか一項に記載の切断用具キット。
- 前記形状が変化する切断用具の少なくとも1つが、上大静脈に挿入されるように構成されている請求項40に記載の切断用具キット。
- 上大静脈に挿入されるように構成された前記形状が変化する切断用具および下大静脈に挿入されるように構成された前記形状が変化する切断用具のうちの少なくとも1つがアームを備えており、そのアームは、前記第2の状態では、これらの形状が変化する切断用具の間での接続を形成するように設けられている請求項41に記載の切断用具キット。
- 当該キットが、形状が変化する切断用具を4つ備え、各々がそれぞれの肺静脈に挿入されるように構成されている請求項35〜42のいずれか一項に記載の切断用具キット。

- 前記形状が変化する切断用具の少なくとも1つが左心耳に挿入されるように構成されている請求項35〜44のいずれか一項に記載の切断用具キット。
- 左心耳に挿入されるように構成された前記形状が変化する切断用具がアームを備え、そのアームは、前記第2の状態では、肺静脈での前記形状が変化する切断用具に接触するように設けられている請求項45に記載の切断用具キット。
- 左心耳に挿入されるように構成された前記形状が変化する切断用具が被膜を備え、その被膜は、前記第2の状態では、当該用具の管状形状の端部を覆うものである請求項45又は46に記載の切断用具キット。
- 前記形状が変化する切断用具の少なくとも1つが、右心耳に挿入されるように構成されている請求項35〜47のいずれか一項に記載の切断用具キット。
