Abstract
Description
Claims (36)
- カテーテルを患者のリンパ系の内部にまたは近くで進めること;および
前記リンパ系から体液を排出すること
を含む、患者のリンパ系から余分な体液を除去する方法。 - カテーテルを前記リンパ系内に進めることは、左鎖骨下静脈、大腿静脈、内頸静脈、右鎖骨下静脈、尺側皮静脈、または上腕静脈を通って、かつ胸管内に、前記カテーテルを進めることをさらに含む、請求項1に記載の方法。
- カテーテルを前記リンパ系内に進めることは、前記カテーテルの遠位端に位置する拡張型部分を径方向に拡張させることをさらに含み;前記拡張型部分は、前記カテーテルよりも大きな径に拡張する、請求項1に記載の方法。
- 前記拡張型部分は、円錐形に自己拡張する複数の編組ワイヤと、前記複数の編組ワイヤを覆って設けられた体液非透過性層とをさらに含む、請求項3に記載の方法。
- 前記拡張型部分は、レーザーカットチューブ、編組ワイヤ、またはポリマースリーブをさらに含む、請求項3に記載の方法。
- カテーテルを前記リンパ系の近くで進めることは、左鎖骨下静脈を通じて前記胸管の開口に隣接して前記カテーテルを進めること;および前記左鎖骨下静脈内で前記カテーテルの1つまたは複数のバルーンを膨らませて、前記胸管の開口を隔てることをさらに含む、請求項1に記載の方法。
- 前記左鎖骨下静脈内で前記カテーテルの1つまたは複数のバルーンを膨らませることは、前記胸管開口の近位の第1のバルーンを膨らませることと、前記胸管開口の遠位の第2のバルーンを膨らませることとを含む、請求項6に記載の方法。
- 第1のカテーテルを患者のリンパ系の内部にまたは近くで進めること;
植込み型排出デバイスを前記リンパ系内に送り出すこと;
第2のカテーテルを前記植込み型排出デバイスの近くに進めて、前記第2のカテーテルを前記植込み型排出デバイスに接続すること;および
前記第2のカテーテルを通じて前記リンパ系から体液を排出すること
を含む、患者のリンパ系から余分な体液を除去する方法。 - 前記植込み型排出デバイスを送り出すことは、前記胸管内でステント部分を拡張すること、および前記胸管を通ってかつ前記左鎖骨下静脈内に、前記ステント部分に接続されたガイドワイヤを配置することを含む、請求項8に記載の方法。
- 前記第2のカテーテルを進めることは、前記ガイドワイヤを覆ってかつ前記胸管内に前記第2のカテーテルを進めることをさらに含む、請求項9に記載の方法。
- 前記植込み型排出デバイスは、前記胸管内でステント部分を拡張することを含む、請求項8に記載の方法。
- 前記第2のカテーテルを前記植込み型排出デバイスに接続することは、前記ステント部分上の第1のねじ筋の組を前記第2のカテーテル上の第2のねじ筋の組に係合させることを含む、請求項11に記載の方法。
- 前記第2のカテーテルを前記植込み型排出デバイスに接続することは、前記第2のカテーテルと前記ステント部分との間にフックを係合させることを含む、請求項11に記載の方法。
- 前記第2のカテーテルを前記植込み型排出デバイスに接続することは、少なくとも1組の磁石を介して前記第2のカテーテルを前記ステント部分に磁気により係合させることを含む、請求項11に記載の方法。
- 前記第2のカテーテルの一方の側上または前記第2のカテーテルの遠位端上に位置する第1の磁石の組をさらに含む、請求項14に記載の方法。
- 前記ステント部分は、拡張されると曲がる遠位部分を有する、請求項14に記載の方法。
- 前記第2のカテーテルを前記植込み型排出デバイスに接続することは、前記植込み型排出デバイスを通じてバルーンを送り出すこと、ならびに前記胸管からの排出速度を増加させるように前記植込み型排出デバイスの遠位で前記バルーンを迅速に膨らませることおよび萎ませることをさらに含む、請求項11に記載の方法。
- リンパ系に植込み型排出デバイスの遠位端を係留すること;
細長い管状部分を配置すること;
前記近位端を皮下でアクセス可能とするかまたは前記患者の外側に配置させるように、前記患者の外皮の表面に隣接して、前記植込み型排出デバイスの近位端を配置すること
を含む、患者のリンパ系から余分な体液を除去する方法。 - 前記遠位端はステント様部分である、請求項18に記載の方法。
- 前記近位端は皮下でアクセス可能なポートである、請求項19に記載の方法。
- 内に配置され通路を遮断する前記細長い管状部分 取り外し可能なスタイレットをさらに含む、請求項20に記載の方法。
- 前記近位端は、皮下でアクセス可能な拡張型体液リザーバである、請求項19に記載の方法。
- 前記近位端は、皮下でアクセス可能なポートをさらに含む、請求項22に記載の方法。
- 前記細長い管状部分は、前記細長い管状部分内で排出路に接続している複数の排出孔を含む、請求項19に記載の方法。
- 前記ステント様部分を覆って設けられたポリマーフィルムカバーをさらに含む、請求項19に記載の方法。
- リンパ系において植込み型排出デバイスを送り出すこと;
前記リンパ系内の圧力をモニタリングすること;
前記リンパ系内の圧力が所定値を達成するまで、前記植込み型排出デバイスを用いて前記リンパ系を排出すること
を含む、患者のリンパ系から余分な体液を除去する方法。 - カテーテルにより前記リンパ系に入ること;
前記リンパ系にシャントの遠位端を係留すること;
前記シャントの第1端を前記リンパ系内に配置すること; および
前記シャントの第2端を前記リンパ系の外側の場所に配置して、前記リンパ系から体液を排出させること
を含む、患者のリンパ系から余分な体液を除去する方法。 - カテーテルにより前記リンパ系に入ることは、乳糜槽内に穿孔することを含む、請求項27に記載の方法。
- 前記シャントの第2端を配置することは、前記シャントの第2端を膀胱、十二指腸、または腸の内部に配置することを含む、請求項28に記載の方法。
- 前記シャントは、体液を前記リンパ系からのみ排出させるように構成された一方向弁を含む、請求項29に記載の方法。
- 前記シャントは、複数の透析繊維を含有する外部管状壁を含む、請求項29に記載の方法。
- 前記上大静脈を通り、前記上大静脈の開口を通って、右肺静脈内に前記シャントを配置することをさらに含む、請求項27に記載の方法。
- 前記上大静脈を通って心臓の左心房内に前記シャントを配置することをさらに含む、請求項27に記載の方法。
- 少なくとも部分的に胸管の弁を通して治療デバイスを配置し、前記胸管の尖部が閉鎖するのを防ぐこと;および
前記胸管からの体液を患者の血管に排出させること

前記細長い管状部分の第1端に接続された拡張型ステント様構造体;および
前記細長い管状部分の第2端に接続され、患者の体内で皮下に留置するために構成された弁またはリザーバ
を含む、リンパ系排出デバイス。
