(19) 狭窄病変部及び閉塞性血栓を治療するための方法であって、
前記狭窄病変部及び前記閉塞性血栓を有する血管内にアクセスカテーテル及びマイクロカテーテルを位置付ける工程であって、前記アクセスカテーテル及び前記マイクロカテーテルは中空内部ルーメンを含む、工程と、
前記病変部に外向きの半径方向力を及ぼすようにサイズ決めされた自己拡張型外側ケージを前記マイクロカテーテル内に位置付ける工程であって、前記外側ケージは、複数の大セルと、前記血管内に前記外側ケージを埋め込むように構成された分離点と、を含む、工程と、
拡張可能な捕捉セクションを前記外側ケージと共に位置付ける工程であって、前記捕捉セクションは、折り畳まれた送出構成と拡張された展開構成とを有する、工程と、
前記血栓及び/又はその断片が前記外側ケージのセルを通って半径方向内向きに通過することを可能にするために、前記マイクロカテーテルから前記外側ケージ及び前記捕捉セクションを展開する工程と、
前記血栓を捕捉するために前記マイクロカテーテルから前記捕捉セクションを展開する工程と、
前記分離点で前記外側ケージを分離し、前記拡張された構成で前記血管内にステントとして前記外側ケージを残す工程と、
前記血栓と共に前記アクセスカテーテルの中へと前記捕捉セクションを引き込む工程と、
捕捉された血栓と共に患者から、前記マイクロカテーテル、前記アクセスカテーテル、及び前記捕捉セクションを引き抜く工程と、を含む方法。
(20) 遠位断片防護要素を設ける工程を更に含む、実施態様19に記載の方法。
Claims (18)
- 血管系内の閉塞部及び狭窄病変部を治療するための装置であって、
捕捉セクションであって、
細長本体と、
近位端部と、
遠位端部と、
折り畳まれた送出構成と、
拡張された展開構成と、を備える捕捉セクションと、
折り畳まれた送出構成と拡張された展開構成とを備えるステントケージと、を備え、
前記ステントケージは、前記捕捉セクションが前記病変部から除去されるとき、前記病変部内に埋め込まれたまま残存するために、前記装置から分離可能となるように構成されている、装置。 - 前記ステントケージはステントである、請求項1に記載の装置。
- 前記捕捉セクションは、前記折り畳まれた送出構成から前記拡張された展開構成への運動の際に、前記閉塞性血栓の1つ以上の部分に係合するように構成されたステントリーバーである、請求項1に記載の装置。
- 前記ステントケージと前記捕捉セクションは共通の近位シャフトを共有する、請求項1に記載の装置。
- 前記ステントケージは、閉鎖されたセルの外周ネットワークを形成する複数の相互連結されたストラットを更に備える、請求項1に記載の装置。
- 前記複数の相互連結されたストラットのうちの少なくとも2つは長手方向に整列されている、請求項5に記載の装置。
- 前記捕捉セクションは、閉鎖されたセルの外側ネットワークを形成するストラットフレームワークを備え、前記ストラットフレームワークの少なくとも一部分は、前記捕捉セクションが外側カテーテルの中へと引き戻される際に、前記拡張された展開構成において前記血栓の少なくとも一部分を埋め込み、挟持するように構成されている、請求項1に記載の装置。
- 捕捉セクションの前記遠位端部にある断片防護要素を更に備え、前記断片防護要素は、
連結部材と、
前記拡張された展開構成にある前記捕捉セクションと少なくとも同じ半径方向サイズに拡張可能な遠位断片フィルタと、を備える、請求項1に記載の装置。 - 前記ステントケージは、病変部内で拡張するように展開されたときに、外向きの半径方向力を及ぼすように構成されている、請求項1に記載の装置。
- 第1の近位シャフトは前記捕捉セクションを関節運動させることが可能であり、第2の近位シャフトは前記ステントケージを関節運動させることが可能である、請求項1に記載の装置。
- 血管系内の閉塞性血栓及び狭窄領域を治療するための装置であって、前記装置は、
近位シャフトと、
外側ケージであって、
折り畳まれた送出構成、拡張された展開構成、及び最大半径方向寸法を有する実質的にチューブ状の構造体、を備える外側ケージと、を備え、
前記外側ケージは、狭窄領域内で拡張するように展開されたときに外向きの半径方向力を及ぼすように構成されており、
前記外側ケージは、前記狭窄部に埋め込まれたまま残存するために、前記装置から分離可能となるように更に構成されている、装置。 - 前記拡張された展開状態において、前記外側ケージの拡張時の前記最大半径方向寸法は直径が約6.0mmである、請求項11に記載の装置。
- 前記外側ケージの前記遠位端部は、前記外側ケージの前記最大半径方向寸法よりも大きい半径方向寸法まで張り出す、請求項11に記載の装置。
- 前記外側ケージは埋め込み型ステントである、請求項11に記載の装置。
- 前記外側ケージは、相互連結されたストラットの構造を更に備える、請求項11に記載の装置。
- 連結部材によって前記近位シャフトに固定的に連結された遠位断片防護要素を更に備える、請求項11に記載の装置。
- 前記外側ケージの前記相互連結されたストラットは、前記拡張された展開構成において前記血栓の少なくとも一部分と係合するように構成されている、請求項15に記載の装置。
- 前記外側ケージの前記相互連結されたストラットは、前記拡張された展開構成において前記血栓の少なくとも一部分を埋め込み、挟持するように構成されている、請求項15に記載の装置。
