Claims (19)
- 患者の心臓に心臓再同期療法を提供するための心臓植込み型電気装置であって、
前記心臓における心臓電気活動を示す信号を受信する入力装置と、
前記心臓に電気インパルスを送って前記心臓に対し心臓再同期療法を提供するインパルス供給システムと、
表面心電図信号由来の全心電気活動モデルおよびペーシング調節パラメータを記憶するメモリと、
前記メモリと連絡するプロセッサと、を備え、
前記プロセッサは、
−前記受信された信号を受信し、
−全心電気活動のサロゲートとして前記受信された信号を特徴づける形態学的フレームワークを使用して、前記受信された信号を、前記記憶された全心電気活動モデルと比較し、
−前記受信された信号と前記記憶された全心電気活動モデルとの前記比較に基づいて、前記記憶されたペーシング調節パラメータを調整し、そして、
−前記インパルス供給システムと連絡して、1以上の前記調整されたペーシング調節パラメータに基づいて、前記心臓に心臓再同期療法を提供する
ように構成されている、心臓植込み型電気装置。 - 前記プロセッサが、さらに前記受信された信号における1以上のQRS複合グリフを特定するように構成されている、請求項1に記載の心臓植込み型電気装置。
- 前記プロセッサが、さらに前記特定されたQRS複合グリフを、前記記憶された全心電気活動モデルにおける相応するQRS複合グリフと比較するように構成されている、請求項2に記載の心臓植込み型電気装置。
- 前記プロセッサが、さらに、前記受信された信号と、前記記憶された全心電気活動モデルとの比較から実質的な最大心室再同期の証拠を確認し、そして、実質的な最大心室再同期を確認する際に、前記記憶されたペーシング調節パラメータの前記調整を迂回するように構成されている、請求項1に記載の心臓植込み型電気装置。
- 前記プロセッサが、さらに前記全心電気活動モデルおよび前記受信された信号から許容可能な範囲の房室間隔値を決定すると共に、拡張機能障害の確率が最小となるように、前記決定された許容可能な範囲の房室間隔値を用いて前記記憶されたペーシング調節パラメータを調整するように構成されている、請求項1に記載の心臓植込み型電気装置。
- 前記コントローラが、さらに前記入力装置と連絡するように構成され、前記プロセッサが、さらに前記入力装置の心房感度を調整するように構成され、そして、前記心房感度が、拡張機能障害の確率を最小にしつつ実質的に同期する心室興奮を維持するように調整される、請求項1に記載の心臓植込み型電気装置。
- 前記全心電気活動モデルが、患者特異的であり、そして、基礎およびペース調整条件で取得された表面リードEGG信号に由来する、請求項1に記載の心臓植込み型電気装置。
- 心調律管理(CRM)装置を備えた患者の心臓に心臓再同期療法を提供する方法であって、
a)前記CRM装置に電気的に通じる電極を使用して、前記患者の心臓において心臓電気活動を示す信号を取得するステップと、
b)前記取得された心臓電気活動信号を、表面リード心電図測定値由来の心臓電気活動モデルと比較するステップと、
c)前記取得された心臓電気活動信号および前記心臓電気活動モデル間の前記比較に基づいて1以上のペーシング調節パラメータを設定するステップと、
d)前記1以上のペーシング調節パラメータを使用して、前記患者の心臓に前記心臓再同期療法を提供するステップと、
を含む方法。 - 前記ステップb)が、前記取得された心臓電気活動信号における1以上のQRS複合グリフを特定することを含む、請求項8に記載の方法。
- 前記ステップb)が、前記特定されたQRS複合グリフを、前記心臓電気活動モデルにおける相応するQRS複合グリフと比較することをさらに含む、請求項9に記載の方法。
- 実質的な最大心室再同期の証拠が確認されるまでに、前記1以上のペーシング調節パラメータを調整し続けながら、前記ステップa)〜c)を繰り返し行なう、請求項10に記載の方法。
- 前記実質的な最大心室再同期の証拠が、前記ステップc)において、前記取得された電気活動信号を前記心臓電気活動モデルと比較することで確認される、請求項11に記載の方法。
- 前記ステップb)において使用される前記心臓電気活動モデルが、心室興奮時のペース調整全心電気活動及び基礎全心電気活動に関する情報を含む、請求項8に記載の方法。
- 前記ステップb)において使用される前記心臓電気活動モデルが、心室興奮時間及び房室間隔を含む全心電気活動タイミング指標に関する情報をさらに含む、請求項13に記載の方法。
- 前記ステップc)が、房室間隔、心室−心室間隔、及び、ペーシング刺激出力電圧の中で1以上を設定することを含む、請求項8に記載の方法。
- 前記ステップc)が、許容可能な範囲の房室間隔値を決定するとともに、拡張機能障害の確率が最小となるように、前記決定された許容可能な範囲の房室間隔値を使用して前記1以上のペーシング調節パラメータを設定することを含む、請求項8に記載の方法。
- 前記1以上のペーシング調節パラメータが、心房感度を含み、そして、前記ステップc)が、拡張機能障害の確率を最小にしつつも実質的に同期する心室興奮を維持するように、前記心房感度を設定することを含む、請求項8に記載の方法。
- e)前記心臓電気活動モデル及び前記取得された信号を使用して、逆心室リモデリングの確率を計算するステップと、
f)前記1以上のペーシング調節パラメータを使用して、心臓再同期療法における前記逆心室リモデリングの確率を示すレポートを生成するステップと、
をさらに含む、請求項8に記載の方法。 - 前記ステップe)が、少なくともQRSスコア、左心室興奮時間、R波振幅値の平均変化、及び、全面軸シフトに関する情報を使用して、回帰分析を行うことを含む、請求項18に記載の方法。
