体により産生される。眼房水は、小柱網から解剖学的導管でありシュレム管とし
ても知られる強膜静脈洞に向かって、水晶体のまわりを流れる。次に眼房水は、
この管から強膜上静脈へ放出される。狭角または開放角緑内障として知られる病
気の状態において、小柱膜を通る流れは減少し、これにより眼圧が増加し、視神
経の退化および失明を生じ得る。小柱網の流出は、主に年齢に関係する組織また
は細胞デブリにより通常妨げられる。緑内障の治療において、眼圧を低下させる
ために様々の方法が試され使用されてきた。 【0003】 以下の参考文献およびここに引用されるすべての他の参考文献は、ここに完全

小さい治療を行うことができる。そのような手術装置を使用して、緑内障の治療
のために管に能動的にまたは受動的に物質または構造物を注射または挿入するこ

位置選定または画像装置が含まれる。 【0004】 発明の詳細な説明 図1は、組織を詳細に示す眼の前方部分の断面図を示す。角膜1は、組織の最
も前方にある透明な部分である。眼球の外表面の大部分を含む不透明な強膜3は
角膜1に隣接する。角膜1の下方に、眼房水で満たされた前眼房3が存在する。虹
彩4は、後眼房5の上方に吊るされ、水晶体6に入る光の量を調節する。水晶体6は
、眼球提靭帯7により保持される。眼房水は毛様体8により産生され、この液体は
水晶体6のまわりを、後眼房5を通って前眼房3に流れる。次に眼房水は小柱網9を
通ってシュレム管10に流れる。その後眼房水は毛細管網を通って強膜上静脈に放
出される。 【0005】 図2は、本発明の装置による位置選定装置を示す。この位置選定装置は、画像
によるまたは画像によらない。好ましい実施の形態において、位置選定装置には
、コンピューターからシステムへ連結線を供給する(図示せず)ハンドピース12
に対して適切な角度で取り付けられた、セクター走査超音波変換器または光学検
出器11が含まれる。変換器11の画像平面13により、シュレム管16の位置を走査す
る。変換器11は、強膜14の表面上に配置され、角膜15および縁17から放射状に走
査されてシュレム管16の正確な位置を測定する。変換器11の表面に接触する組織
は、適切に湾曲して胸膜の表面を円滑に走査できる。手術装置はこの図面には記
載されていない。 【0006】 図3は、本発明の装置による位置選定装置および侵襲性の非常に小さい微小手
術装置lを示す。セクター走査超音波変換器または光学検出器18が、シュレム管
の上方に位置する画像平面19とともに示され、管22に導入される微小カニューレ
20を示す。微小手術装置の遠位の先端は、エラストマーの膨張バルーン21を含む
。バルーン21は、微小カニューレ20の遠位端に直接組み込まれてもよく、またそ
こに取り付けられたバルーン21を有する別の装置が微小カニューレ20により導入
されてもよい。バルーン21は、手術前の状態22から膨張した状態の23へ管22を膨
張することが示される。微小カニューレは、管に沿って進行し、連続するセグメ
ントを膨張する。 【0007】 図4は、位置選定装置、侵襲性の非常に小さい微小手術装置および位置選定装
置に関連して微小手術装置を誘導する手術装置を組み込む本発明のある実施の形
態の側面図を示す。超音波セクター走査変換器または光学検出器24は、ハンドル
26に取り付けられた変換器ハウジング25の底側に配置される。変換器ハウジング
25は、軸方向に延在し片持ち梁27を形成する。片持ち梁27は、誘導スリーブ28、
角度調節機構29および微小カニューレ31を進行するための親指ホイール機構30を
有する。微小カニューレ31の貫通の深さは角度調節ノブ32により調節され、微小
カニューレ31の軸方向の動きは、微小カニューレ31上で先導スクリュー33を進行
する親指ホイール30により調節される。微小カニューレ31の隣接端は、注射器な
どを取り付けるための女性ルーアー付属品34を有する。使用において、シュレム
管35は変換器24を使用して位置付けられ、微小カニューレ31は強膜を通って管に
進行される。 【0008】 以下に、非常に侵襲性の小さい眼の手術方法および特に緑内障の治療のための
本発明の方法の実施例を記載する。これらの本発明の例としての記載は、本発明
の装置の図面および前記の説明を参照することにより理解されるであろう。 【0009】 実施例1:実験を行い、高周波数超音波の標的分析の測定を行った。様々の直
径の微小溝に匹敵するように超音波の類似模型を調製した。標準の80mmスチレン
ペトリ皿に10mm間隔を開けて、様々の直径のステンレス鋼管(Small Parts, Inc.
Miami Lakes, FL)を配置することにより類似模型を調製した。ゼラチン、250ブ
ルーム(Woburn, Edible Pork Skin Gelatin)の10%溶液を、完全に溶解するまで
滅菌水柱でゼラチン粉末を加熱することにより調製した。管が約1mmの深さに被
覆されるまでゼラチン溶液をペトリ皿に注いだ。冷却によりゼラチンを固まらせ
、次に管をペトリ皿から取り出し、様々の直径の開いた溝を残した。この方法で
、110、150、205、230および255ミクロンの直径の溝を作製した。ルーアーハブ
