(57)【要約】 椎骨インプラント（10）が、これに隣接して配置された別のインプラントの外周部に合致した形状の凹状切除部分（16）を備えている。椎骨間スペースを伸延させる先端部（21）を備えた専用伸延器（20）が、本発明のインプラントを受け入れるよう断面がオーバーラップした全体として円形の輪郭の２つのチャネル（26,27）を有している。

【発明の詳細な説明】
【０００１】 〔発明の分野〕 本発明は、薄型固定ケージ及び薄型固定ケージのための挿入セットに関する。
【０００２】 〔発明の背景〕 公知の脊椎インプラント、たとえば、椎骨固定に用いられるインプラントは、
均等に分布された状態の適当な支持及び固定誘導手段となるよう対をなして用い
られる場合が多い。植込み及び外科手技のための空間が狭いので、望ましい寸法
形状及び特質を有する一対のインプラントを施すことが困難又は不可能な場合が
ある。或る既存の設計のインプラントは、他のインプラントに密接して配置でき
るよう形づくられているが、最適な性能、融通性又は挿入しやすさを達成したも
のはない。
【０００３】 〔発明の概要〕 本発明の目的は、隣り合うインプラントを最適な性能をもって密接して配置で
きるような最適形状をもたらす設計のインプラント及び関連の器具及び方法を提
供することにある。これら目的及び他の目的は、本発明のインプラントの形状並
びに関連の器具及び方法によって達成される。
【０００４】 好ましい実施形態では、本発明の固定インプラントは、細長いインプラントの
円周面に凹状切除部分を備えている。凹状部分は、対応関係をなす凹面を備えた
隣接配置のインプラントの外形と合致している。新規なデュアルタング伸延器又
は伸延ツールが、本発明のインプラントを密接させた状態で挿入配置しやすくす
るような２つのオーバーラップする断面形状を備えている。
【０００５】 本発明の好ましい実施形態を図面を参照して説明する。
【０００６】 〔好ましい実施形態の詳細な説明〕 図１、図２及び図３Ａはそれぞれ、本発明の薄型固定ケージ１０の斜視図、側
面図及び端面図である。本発明のケージ１０は、サージカル・ダイナミックス・
インコーポレーテッドによって現在市販されている Ray TFC（登録商標）固定ケ
ージとして一般に知られている形式のものである。Ray TFC(登録商標) 固定ケー
ジは、共通譲受人の米国特許第４，９６１，７４０号に開示されており、かかる
米国特許の内容を 本明細書の一部を形成するものとしてここに引用する。
【０００７】 本明細書に開示する固定ケージは、並置状態の２つの従来型固定ケージによっ
て占められる空間の全体の広さを減少させるよう別の固定ケージとを用いること
ができる。固定ケージ１０は、ケージ１０の挿入及び椎間腔内への固定を容易に
する螺旋ねじ山１４を有している。ねじ山１４は、ねじ山の輪郭を減少させるた
めに凹状部分１６，１７を形成するよう設けられている。図示のように、凹状部
分１６，１７は好ましくは、１８０°の間隔を置いて設けられている。所望なら
ば、本発明を実施するのに必要な凹状部分は１つであるに過ぎない。また、所望
ならば３以上の凹状部分を設けてもよい。凹状部分１６，１７を設けることによ
り、２又は３以上のケージ１０，１１を、一方のケージ１１の丸くなった部分を
図３Ｂに示すように隣のケージ１０の凹状部分１７内に配置すると、互いに密接
して配置することができる。理解できるように、このように配置された２つの薄
型ケージ１０，１１の幅の合計（横方向スペース) は、これらのうち少なくとも
一方が凹状部分を備えていない２つの従来型ケージを並置した場合の幅の合計よ
りも小さい 図４〜図７Ｂは、本発明の固定ケージの挿入器具セットを示している。挿入器
具セットは、タング牽引子２０、ガイド３０及びインパクタプレート（衝撃板）
４１付き衝撃体４０を有している。タング牽引子２０は、一対の互いに間隔を置
いたタング２１を有し、これらタングは、椎間腔内へ挿入してこれを伸延できる
ように寸法決めされると共に遠方側端部のところが楔として形づくられている。
タング２１の形状及びタングによる椎間腔の伸延の仕方は、１９９７年７月８日
に出願された係属中の米国特許出願第０８／８８９，６６１号に記載されており
、かかる米国特許出願の内容を本明細書の一部を形成するものとしてここに引用
する。
【０００８】 タング牽引子２０は、インパクタプレート４１を伸延器２０の手元側端部に取
り付ける際、インパクタプレート４１のタブ４２を取り付けるための一対の手元
側に延びるスロット付きタブ２２を有している。タブ４２をタブ２２のスロット
２３に挿入してインパクタプレート４１及びねじ山４３，４５によってインパク
タプレート４１に連結された細長い一体形衝撃体４０をタング牽引子２０に取り
付ける。次に、衝撃体４０の手元側端部４７に適当なツール、たとえばハンマー
で衝撃を与え又は軽くたたいて端部２１を椎骨間スペースに挿入するのがよい。
挿入後、タブ４２を滑らせてスロット２３から離脱させ、衝撃体とインパクタプ
レート４１を分離して取り出し、タング牽引子２０を端部２１が椎骨間スペース
内に挿入された状態で定位置に残す。
【０００９】 次に、遠方側端部のピン３２を牽引子２０の長手方向スロット２５に挿入する
ことによりガイド３０をタング牽引子２０に取り付ける。遠方側端部ピン３２を
スロット２５に嵌め込んでガイド３０を固定状態のタング牽引子２０に対しピン
３２を中心として、タング牽引子２０の２つの開口部２６，２７のそれぞれの整
列位置相互間で回動できるようにする。開口部２６，２７はそれぞれ、従来型ケ
ージ挿入ツール( 図示せず) と一緒に固定ケージを受け入れるように形づくられ
ている。ガイド３０をピン３２を中心として回転させてその軸方向ボア３４が、
開口部のそれぞれを貫通して穴を形成している間、タング牽引子２０の開口部２
６，２７のうちの一方と整列するようにする。適当なツール、たとえば、上述の
米国特許出願第０８／８８９，６６１号に記載されたものをボア３４に挿入して
固定ケージ挿入のためのスペースを作る。次に、たとえば本発明の形式の固定ケ
ージを細長い挿入ツールを用いてボア３４及びそれぞれの対応関係にある端部牽
引子の開口部２６，２７に挿入して椎骨間スペース内に配置できるようにする。
各ケージを、凹状部分１６，１７のうち一方が椎骨間スペースの隣接の開口部又
はボアの方に向くように配置する。次に、ガイド３０を回して軸方向ボア３４が
牽引子２０の他方の開口部２６，２７と整列するようにする。別の固定ケージ(
これは、凹状部分を備えていても、或いは備えていなくてもよい) を上述したの
と同様の方法で挿入してその外周部分が最初に挿入された固定ケージの凹状部分
１６，１７内に嵌まるようにする。
【００１０】 手術野の前処理及び固定ケージの挿入中にガイド３０をタング牽引子２０に対
してその２つの整列位置の各々に保持するために相互係止器具３３を設けてもよ
い。
【００１１】 本発明の変形実施形態、たとえば図８及び図９に示す変形実施形態は、種々の
形状をしたインプラント本体を有し、これらインプラント本体は、追加のインプ
ラント本体との密接配置状態を容易に達成する凹状部分を備えている。たとえば
、図８のインプラント本体１００は、骨の固定を容易にするための中央開口部１
０２及びその隣の長円形インプラント１０８の円周方向凸壁１０６を合致して受
け入れるような形状の凹状側壁１０４を備えた長円形の半部の形態をしている。
図９のインプラント本体２００は全体的に円筒形であり、開口端部２０４，２０
６相互間に延びる長手方向軸線に全体として垂直に整列した凹状チャネル２０２
を有している。
【００１２】 長さと幅が互いにほぼ等しいタング牽引子２０は、ビジビリティ（可視性）を
高めると共に、ユーザが、牽引子２０に挿入される器具、たとえば骨鉗子の易動
性が増大しているので外からの椎間板組織を容易に除去できるようにすることは
理解できよう。これは好ましい実施形態ではあるが、牽引子２０の長さは種々の
利点を奏するために所望に応じて様々なものであってよい。
【００１３】 好ましい実施形態を本明細書において開示したが、本発明の範囲から逸脱する
ことなく設計変更例及び改造例を想到できることは理解されよう。

Claims (12)

    【特許請求の範囲】
  1. 【請求項１】 骨固定インプラント器具であって、骨の固定に対応するよう
    中空の内側室が形成された本体と、前記本体に設けられていて、別のインプラン
    ト器具の次に密接して配置しやすいよう少なくとも１つの直線状に延びる凹状チ
    ャネルが形成された外面とを有することを特徴とする骨固定インプラント器具。
  2. 【請求項２】 前記中空内部室は、全体として円筒形であることを特徴とす
    る請求項１記載の固定インプラント。
  3. 【請求項３】 前記外面は、椎骨間スペース中への挿入及び固定を容易にす
    るようねじ山が設けられていることを特徴とする請求項１記載の固定インプラン
    ト。
  4. 【請求項４】 前記本体は、互いに反対側の端部に開口部を有し、前記開口
    部は、前記中空内部室と連通していることを特徴とする請求項１記載の固定イン
    プラント。
  5. 【請求項５】 前記凹状チャネルは、前記互いに反対側の端部相互間に延び
    るよう整列していることを特徴とする請求項４記載の固定インプラント。
  6. 【請求項６】 前記凹状チャネルは、前記互いに反対側の端部相互間に延び
    る想像上の軸線と全体的に垂直に整列していることを特徴とする請求項４記載の
    固定インプラント。
  7. 【請求項７】 インプラントを隣り合う骨表面間に挿入するような形状の骨
    固定インプラントセットであって、少なくとも１つの凸面を備えた第１の骨固定
    インプラントと、第２の骨固定インプラントとを有し、前記第２の骨固定インプ
    ラントは、前記第１及び第２のインプラントを前記骨表面相互間のスペース内に
    互いに隣接して配置すると、前記第１のインプラントの前記凸面を合致して受け
    入れるようになった少なくとも１つの凹面を有することを特徴とするインプラン
    トセット。
  8. 【請求項８】 隣り合う椎体を伸延させてこれらの間に複数の椎骨インプラ
    ントを受け入れる開口部を作る器具であって、手元側端部及び遠方側端部を備え
    た本体と、前記本体に設けられていて、前記手元側端部と遠方側端部との間に延
    びる開口部とを有し、前記開口部は、断面が全体として円形の２つのチャネルを
    形成して前記チャネルの前記円形の形状の断面がオーバーラップするようになっ
    ており、前記器具は、前記遠方側端部のところに設けられていて、前記椎体を楔
    作用により伸延させる一対の伸延器先端部を更に有していることを特徴とする器
    具。
  9. 【請求項９】 前記手元側端部のところに設けられていて、インプラント挿
    入ガイドを、これが前記チャネルの各々の整列位置相互間で選択的に回動させる
    ことができるような回動方式で前記本体に取り付けるピボット取付け手段を更に
    有していることを特徴とする請求項８記載の器具。
  10. 【請求項１０】 前記手元側端部のところに設けられていて、椎骨間スペー
    ス形成ツールガイドを、これが前記チャネルの各々の整列位置相互間で選択的に
    回動させることができるような回動方式で前記本体に取り付けるピボット取付け
    手段を更に有していることを特徴とする請求項８記載の器具。
  11. 【請求項１１】 椎骨インプラントを受け入れるための椎骨間スペースを作
    る方法であって、少なくとも２つの直線状チャネルを備えた伸延器を用いて隣り
    合う椎骨を伸延する段階と、前記少なくとも２つのチャネルのうち第１のチャネ
    ル及び前記伸延器に取り付けられたツールガイドにより椎骨間スペース形成作業
    を実施する段階と、前記ツールガイドを前記伸延器に対して回動させて前記ツー
    ルガイドが前記チャネルのうちの第２のものと整列するようにする段階と、前記
    第２のチャネルを通して椎骨間スペースの形成作業を実施する段階とを有するこ
    とを特徴とする方法。
  12. 【請求項１２】 複数の椎骨インプラントを椎骨間スペースに挿入する方法
    であって、少なくとも２つの直線状チャネルを備えた伸延器を用いて隣り合う椎
    骨を伸延させる段階と、前記椎骨間スペースを前記伸延器を介して前記インプラ
    ントを受け入れることができるように作る段階と、第１のインプラントを前記少
    なくとも２つのチャネルのうち第１のチャネル中へ挿入すると共に前記伸延器に
    取り付けられた挿入ガイド中へ挿入する段階と、前記挿入ガイドを前記伸延器に
    対して回動させて前記挿入ガイドが前記少なくとも２つのチャネルのうちの第２
    のチャネルと整列するようにする段階と、第２のインプラントを前記第２のチャ
    ネル中へ挿入する段階とを有することを特徴とする方法。
