DESCRIZIONE
dell'invenzione industriale avente per titolo:
“Protesi d’anca veterinaria modificataâ€
La presente invenzione concerne una protesi d’anca veterinaria di superficie che presenta alcune modifiche rispetto alla protesi d’anca veterinaria oggetto della domanda di brevetto MI2008A001367 depositata il 25 Luglio 2008 a nome dello stesso richiedente.
La protesi d’anca veterinaria descritta nella suddetta domanda comprende una coppa acetabolare fissata al bacino nell’acetabolo, una cupola femorale fissata alla testa del femore con una vite femorale senza necessità di resezione anche parziale della testa stessa, ed una calotta intermedia interposta tra coppa acetabolare e cupola femorale per il montaggio facilitato della protesi durante l’operazione.
Il montaggio della protesi secondo il citato brevetto avviene partendo dal montaggio a caldo, prima dell’operazione, della calotta intermedia dentro la coppa acetabolare; calotta e coppa hanno due fori che hanno una posizione di allineamento, per una data posizione reciproca, che consente il passaggio di due viti per il fissaggio della coppa acetabolare all’acetabolo, e le due viti non sporgono dalla superficie interna della coppa acetabolare, per non interferire con la rotazione libera della calotta intermedia con la coppa acetabolare. La cupola femorale viene poi fissata alla testa del femore con la vite femorale, e la cupola femorale collegata al femore à ̈ inserita nella corrispondente cavità interna della cupola intermedia, e qui trattenuta assialmente tramite l’inserimento di un anello seeger in una apposita cava nella superficie interna della cupola intermedia. Il risultato finale à ̈ che la cupola femorale può ruotare su tre assi perpendicolari tra loro rispetto alla coppa acetabolare e non possono lussarsi tra loro. In questo modo si à ̈ ottenuta una protesi anti lussazione.
Scopo della presente invenzione à ̈ quello di apportare alla protesi d’anca veterinaria sopra descritta alcune modifiche che consentono un diverso e migliorato montaggio ed uso della stessa. In particolare si ritiene opportuno di non vincolare meccanicamente le cupole tra di loro, in modo che la prevenzione della lussazione sia ottenuta grazie all’azione della capsula articolare e delle strutture muscolotendinee contigue all’articolazione coxo-femorale.
In accordo con l’invenzione tale scopo à ̈ raggiunto con una protesi coxofemorale di superficie per uso veterinario, comprendente una cupola femorale, una coppa acetabolare (o cotile) ed un perno femorale di fissaggio a vite, caratterizzata dal fatto che la superficie di contatto della coppa acetabolare con la cupola femorale definisce vani complementari in volume tra loro.
Queste ed altre caratteristiche della presente invenzione saranno rese maggiormente evidenti dalla seguente descrizione dettagliata di un suo esempio di realizzazione pratica illustrato a titolo non limitativo negli uniti disegni, in cui:

Con riferimento alle figure allegate ed in particolare alle figure 1 e 2, si nota una protesi 1 coxo-femorale assemblata, comprendente una prima forma di realizzazione di cupola femorale 2 con un relativo perno femorale 4 di fissaggio a vite, ed una prima forma di realizzazione di coppa acetabolare 3. La superficie interna 12 della coppa acetabolare 3 à ̈ emisferica come la superficie esterna 10 della cupola femorale 2, ad essa complementare, come si può apprezzare anche dalla vista da sotto di figura 2. La superficie di contatto 5 della coppa acetabolare 2 con la cupola femorale 3 definisce vani complementari in volume tra loro.
In figura 3 si nota il perno femorale 4 dotato di un colletto 14 e di una porzione di gambo 15 con un filetto 16 che sopravanza in diametro il colletto 14. Una testa 17 del perno 4 à ̈ dotata di un incasso 18 per il serraggio del perno 4 con una chiave quadrata o a brugola. Il gambo, interamente filettato, à ̈ composto da una parte intermedia cilindrica ed una finale a cono, al fine di consentire un più agevole inserimento nel femore.
Alle figure 4 e 5 si nota la cupola femorale 2 nella prima forma realizzativa che comprende la superficie esterna 10 emisferica, una superficie interna 19 (figura 5) emisferica dotata di tre dentelli 7 aggraffanti disposti su un bordo interno 6 della cupola femorale 2 a 120° tra loro (altre disposizioni e numero di dentelli 7 sono possibili) ed un foro femorale 11 per il passaggio del perno femorale 4 di fissaggio alla testa del femore.
Alle figure 6, 7, 8 si nota la coppa acetabolare 3 nella prima forma realizzativa che comprende una superficie interna 12 emisferica, complementare alla superficie esterna 10 della cupola femorale 2 con la quale va a contatto su una superficie di contatto 5, che per questa forma realizzativa coincide con le superfici 10 e 12. Nella coppa 3 à ̈ praticato un foro filettato 14 per il fissaggio della coppa acetabolare 3 al bacino. Ulteriori fori acetabolari 14 in diverse posizioni possono essere praticati per il fissaggio della coppa 3, ma non sono qui illustrati. Tre appendici 9 aggraffanti sono disposte lungo un bordo esterno 8 della coppa acetabolare 3, distanti tra loro 120°. Il numero e la disposizione delle appendici 9 aggraffanti può differire.
Per il montaggio della prima forma realizzativa, si posiziona la coppa acetabolare 3 nell’acetabolo privato delle cartilagini; le appendici 9 aggraffanti hanno la duplice funzione di posizionare con precisione in tre dimensioni la coppa acetabolare 3 nell’acetabolo e di vincolare la coppa 3 in rotazione all’acetabolo stesso, per una migliore tenuta e stabilità globale della protesi. La coppa acetabolare 3, inserita a pressione (tecnica “press fit†), può essere eventualmente fissata da eventuali viti da osteosintesi, note, da inserire in uno o più fori acetabolari 14 della coppa 3.
Nella testa del femore, privata delle cartilagini, viene praticato un foro pilota (non raffigurato), di diametro di poco inferiore al perno femorale 4, e lunghezza di poco superiore al perno femorale 4, e si posiziona la cupola femorale 2 che viene fissata con il perno femorale 4, avvitato nell’osso guidato dal foro pilota. I dentelli 7 aggraffanti impediscono la rotazione della cupola femorale 2 nella sua sede sulla testa del femore, a tutto vantaggio della stabilità dell’impianto.
In figura 15 sono visibili i componenti della protesi 1, secondo la prima forma realizzativa, impiantati: il perno femorale 4 blocca sulla testa femorale la cupola femorale 2. La coppa acetabolare 3 e la cupola femorale 2 ruotano liberamente tra loro, attorno a tre assi perpendicolari tra loro.
In figura 15 non sono stati visualizzati la capsula articolare e i fasci muscolari che proteggono l’articolazione così ricostruita dalla lussazione.
Alle figure 9 e 10 si nota una seconda forma realizzativa per cupola femorale 20, in cui la superficie esterna 100 della cupola femorale 20 à ̈ una porzione di calotta sferica tale da eccedere il diametro massimo. Il volume cavo interno, definito da una superficie interna 190, à ̈ definibile come ima porzione cilindrica sormontata da una porzione di calotta sferica. La cupola femorale 20 à ̈ dotata anch’essa di tre dentelli 7 aggraffanti disposti su un bordo interno 6 della cupola femorale 20 a 120° tra loro (altre disposizioni e numero di dentelli 7 sono possibili) e di un foro femorale 11 per il passaggio del perno femorale 4. Il montaggio della cupola femorale 20 della seconda forma di realizzazione avviene previa fresatura moderata della testa del femore nella forma del volume cavo interno della cupola femorale 20, ottenibile per esempio con una apposita fresa a tazza, di forma complementare alla forma del volume cavo interno della cupola femorale, e lasciando una porzione di calota sferica, che à ̈ la forma naturale della testa del femore.
Alle figure 11, 12, 13 e 14 si nota una seconda forma realizzativa per coppa acetabolare 3, che presenta una porzione prolungata 13 di bordo esterno 8, per ovviare ad un contenimento acetabolare carente. Anch’essa, vantaggiosamente, à ̈ dotata di un foro acetabolare 14 per il fissaggio della coppa acetabolare 30 al bacino. Ulteriori fori acetabolari 14, anche in diverse posizioni, possono essere praticati per il fissaggio della coppa 30, ma non sono qui illustrati. Tre appendici 9 aggraffanti sono disposte lungo un bordo esterno 8 anche di questa forma di realizzazione di coppa acetabolare 30, distanti tra loro 120°. Il numero e la disposizione delle appendici 9 aggraffanti può differire.
Si vuole qui evidenziare che ogni combinazione à ̈ possibile tra differenti forme di realizzazione di coppe acetabolari 3, 30 e cupole femorali 2, 20. Per esempio, à ̈ possibile montare una coppa acetabolare 3 della prima forma di realizzazione con una cupola femorale 20 della seconda forma di realizzazione.
Il montaggio della seconda forma realizzativa della coppa acetabolare 30 avviene in modo analogo a quanto visto per la coppa acetabolare 3 della prima forma di realizzazione, posizionando però la porzione prolungata 13 del bordo esterno 8 verso il bordo dorsale dell’ articolazione, anatomicamente definito rima dorsale acetabolare.
Si vuole anche evidenziare che la cupola femorale 20 della seconda forma di realizzazione consente, in abbinamento alla coppa acetabolare 3, 30 della prima o seconda forma di realizzazione, una maggiore escursione dell’articolazione coxofemorale, rispeto alla cupola femorale 2 della prima forma di realizzazione.
Vantaggiosamente, il perno 4 e la superficie interna 19, 190 della cupola femorale 2, 20 sono ricoperti di idrossiapatite di calcio, per stimolare la crescita ossea.
Una superficie esterna 25, 250 (figure 1, 6, 7, 11, 12) della coppa acetabolare 3, 30 à ̈ ricoperta con un strato di idrossiapatite di calcio e biossido di titanio; quest’ultimo, essendo poroso, favorisce l’ancoraggio osseo. I tratamenti di rivestimento descriti possono essere suscetibili di modifiche legate all’innovazione tecnologica in materia.

Claims (13)

  1. RIVENDICAZIONI 1. Protesi (1) coxo-femorale di superficie per uso veterinario, comprendente una cupola femorale (2, 20), una coppa acetabolare (3, 30) ed un perno femorale (4) di fissaggio a vite, caratterizzata dal fatto che la superficie di contatto (5) della coppa acetabolare (2, 20) con la cupola femorale (3, 30) definisce vani complementari in volume tra loro.
  2. 2. Protesi (1) secondo la rivendicazione 1, caratterizzata dal fatto che la superficie esterna (10) della cupola femorale (2) Ã ̈ emisferica.
  3. 3. Protesi (1) secondo la rivendicazione 1, caratterizzata dal fatto che la superficie esterna (100) della cupola femorale (20) Ã ̈ una porzione di calotta sferica tale che eccede il diametro massimo.
  4. 4. Protesi (1) secondo la rivendicazione 1, 2 o 3, caratterizzata dal fatto che la cupola femorale (2, 20) presenta una pluralità di dentelli (7) aggraffanti lungo un bordo interno (6).
  5. 5. Protesi (1) secondo una qualsiasi delle rivendicazioni da 1 a 4, caratterizzata dal fatto che la cupola femorale (2, 20) presenta un foro femorale (11) per il passaggio del perno femorale (4) di fissaggio.
  6. 6. Protesi (1) secondo una qualsiasi delle rivendicazioni da 3 a 5, caratterizzata dal fatto che la superficie interna (12) della coppa acetabolare (3) Ã ̈ emisferica.
  7. 7. Protesi (1) secondo una qualsiasi delle rivendicazioni da 1 a 5, caratterizzata dal fatto che la coppa acetabolare (30) presenta una porzione prolungata (13) di bordo esterno (8), per ovviare ad un contenimento acetabolare carente.
  8. 8. Protesi (1) secondo una qualsiasi delle rivendicazioni precedenti, caratterizzata dal fatto che la coppa acetabolare (3, 30) presenta una pluralità di appendici (9) aggraffanti lungo il bordo esterno (8).
  9. 9. Protesi (1) secondo una qualsiasi delle rivendicazioni da 1 a 8, caratterizzata dal fatto che la coppa acetabolare (3, 30) presenta almeno un foro acetabolare (14) per il passaggio di almeno una vite di fissaggio.
  10. 10. Protesi (1) secondo una qualsiasi delle rivendicazioni precedenti, caratterizzata dal fatto che il perno femorale (4) comprende un colletto (14) ed una porzione di gambo (15) con un filetto (16) che sopravanza in diametro trasversale il colletto (14).
  11. 11. Protesi (1) secondo la rivendicazione 10, caratterizzata dal fatto che il perno femorale (4) comprende una testa (17) con un incasso (18) assiale per chiave a brugola.
  12. 12. Protesi (1) secondo una qualsiasi delle rivendicazioni precedenti, caratterizzata dal fatto che il perno (4) e la superficie interna (19, 190) della cupola femorale (2, 20) sono ricoperte da idrossipatite di calcio.
  13. 13. Protesi (1) secondo una qualsiasi delle rivendicazioni precedenti, caratterizzata dal fatto che una superfìcie esterna (25, 250) della coppa acetabolare (3, 30) à ̈ ricoperta da idrossipatite di calcio e biossido di titanio.
