Class 20(ii) Forms for anchor bolts in concrete bases are located in position by means of a foursided adjustable metal frame b, c, e, f, Fig. 1, supporting at each corner a sheet metal form a. The forms a have squared upper portions a<SP>1</SP>, Fig. 3, whereby they are secured to two opposed frame members b, c, by bolts d operable in longitudinal slots b<SP>1</SP>, c<SP>1</SP>, the ends of the members b, c, entering slots e<SP>1</SP>, f<SP>1</SP> in the frame members e, f, which are secured to the upper portions a<SP>1</SP> of forms a by bolts g and clamp plates h. A bolt locating cradle member is disposed in the upper portion a<SP>1</SP> of each form a and has an apertured base a with vertical sides a<SP>3</SP>, a<SP>4</SP> each with vertical slots a<SP>5</SP> through which the securing bolts d pass. The cradle member and hence the anchor bolt j which is carried by an apertured. channel member a<SP>10</SP> supported on the cradle, are thus vertically adjustable relative to the form position a<SP>1</SP>. The frame is supported on the sides of the cavity to be filled with concrete by the ends of the frame members e, f or on cross bars and after concreting the frame is lifted vertically taking the forms a with it and leaving the anchor bolts j secured by their heads and plates j<SP>1</SP>, the apertures left being subsequently grouted in. The frame and forms may be galvanized and the frame members graduated to facilitate spacing of the forms a.
