Abstract
In surgery in which an implant stem 10 is located in a medullary canal 12 by cement 20, a plastics spacer 26 has resilient fingers 36 to keep the stem 10 away from the canal wall. The spacer has a stopped bore 32 to receive the stem 10 with a void 34 beneath it. This allows the stem 10 to sink if the cement contracts, without producing cracks in the cement. <IMAGE>
Description
SPECIFICATION
Spacer for intra-medullary implant
This invention relates to surgery in which an implant is placed in the medullary canal of a bone. For example, when a patient is given an artificial hip joint, an implant is necessary in the medullary canal of a femur (see for example U.K. Patent No. 1409054). Such an implant is located in the medullary canal by use of a bone cement, e.g. polymethylmethacrylate cement. It has been demonstrated by laboratory tests as well as by clinical experience that there is considerable advantage in keeping the whole of the implant stem, its tip in particular, away from touching the bone of the host and completely enclosed in a mantle of cement. The present inventors have previously proposed a spacer for this purpose (see
U.K. No. 1409053).
Fig. 1 of the accompanying drawings shows such an implant 10 located in the medullary canal 12 of a femur 14. The spacer of U.K.
Patent No. 1409053 is shown at 16, and comprises an annular base member around the implant stem, and a plurality of spring members 18 diverging therefrom to contact the wall of the canal. The implant stem 10 is surrounded by a mantle of acrylic cement 20, and a plug 22 as described in U.K. Patent
Specification No. 2054383 prevents penetration of the cement further down the medullary canal.
Clinical experience with this intra-medullary implant has shown that some sinkage of the implant within the femur can sometimes take place. Such sinkage is usually caused by relatively poor cement technique, but nevertheless it occurs in practice and is usually accompanied by cement cracking (shown at 24 in Fig. 1). It is also known that some expansion and contraction of the bone cement takes place during its curing cycle and during the first few weeks of its existence in the patient. It is postulated by many experimenters and clinicians that the result of this change in size of the cement is a very small gap between stem and cement.Where the stem is tapered as shown in Fig. 1, this gap can be taken up by the stem sinking slightly relative to the cement. ! However, the tip of the stem is not able to move through the cement below it without causing cracks such as at 24, and this obviously results in unreliability of the anchorage provided by cement.
The present invention provides a spacer for the stem of an intra-medullary implant, comprising a body having a stopped bore for receiving the tip of the implant stem, and a plurality of generally radially extending resilient members for engaging the sides of the medullary canal to hold the body away from the sides of the medullary canal. It is accordingly possible to insert the tip of the implant stem in the bore, leaving a void in the bore beneath the tip, into which the tip of the stem can sink.

Figure 3 is a perspective view of the spacer shown in Fig. 2.
Referring to Figs. 2 and 3 an intra-medullary implant of the type shown in Fig. 1 and in British Patent No. 1409054 has a tapering stem 10. It is located in the medullary canal 12 of a femur 14 by polymethylmethacrylate bone cement 20 in a similar manner to that shown in Fig. 1, except for the form of the spacer. Instead of an annular spacer 16 fitting over the end of the stem 10, so that the stem 10 protrudes slightly on the underside of the spacer, there is provided a spacer 26 having a cylindrical body 28. The bottom end 30 of the body 28 is domed, and a longitudinal bore 32 opens at the top end of the spacer.
The bore 32 extends most of the way through the body 28, but is stopped just before the domed end 30. In use, as shown in Fig. 2, the bore 32 is a push-fit on the tip of the stem 10, and is not fitted right the way onto the tip, but rather a void 34 is left at the inner end of the bore 32. The bore 32 should be a relatively tight push-fit on the stem 10, so that that this void remains even after the tip of the stem and the spacer have been inserted in the medullary canal.
Although the bore 32 is stopped as described above, a small air vent passage 40 is provided, extending longitudinally from the inner end of the bore 32 to the domed end 30 of the body 28. This assists in fitting the spacer onto the tip of the implant stem, since it allows air to escape from the void 34.
As best seen in Fig. 3, four resilient members 36 extend radially and upwardly from a point on the body 28 near the domed end 30. These are linked to the body 28 by respective thin webs 38, forming four wings, fins or ribs projecting from the side of the body 28. In use, the members 36 resiliently engage with the sides of the medullary canal, and the webs 38 collapse to take up tolerances in the diameter of the cavity in the canal.
In use, the implant is secured in the medullary canal as is conventional, with the spacer 26 in position on the tip of the stem 10 in order to hold the stem away from the sides of the medullary canal. The void 34 is of course left in the body 28. If there should be contraction of the bone cement 20, or if a gap should develop between the implant 10 and bone cement for any other reason, the tapered stem will sink and settle in the cement, and this is accommodated by the tip of the stem sinking into the void 34. It will be appreciated that such sinkage into the void 34 does not put any pressure on cement underneath the tip of the stem 10, and so such sinkage can occur without causing cracking of the cement.
The spacer is moulded from a surgical grade of plastics material, for example ultra high molecular weight polyethylene. This is because there is some doubt as to the suitability of metal as a spacer material, as corrosion between the spacer 26 and the stem 10 can occur in theory although it has not been observed in practice. Furthermore, if it is ever necessary to remove the implant and cement from a patient (during the course of a revision operation for example) a metal material could make extraction of the cement more difficult. The plastics material used in the present embodiment, however, can be drilled out with ease.
So that the spacer device is visible on any subsequent X-rays, a thin band 42 of a radio opaque material such as metal is provided around the body 28, to act as an X-ray marker.
The body 28 could have a truncated conical shape if desired, instead of the cylindrical shape shown. It would also be possible to omit the webs 38 if desired, leaving just the four resilient fingers 36.
Claims (7)
1. A spacer for the stem of an intramedullary implant, comprising a body having a stopped bore for receiving the tip of the implant stem, and a plurality of generally radially extending resilient members for engaging the sides of the medullary canal to hold the body away from the sides of the medullary canal.
2. A spacer according to claim 1, including an air vent passage leading from the inner end of the stopped bore to permit the escape of air from the bore.
3. A spacer according to claim 1 or 2 wherein the bore extends downwardly from an upper end of the spacer, and the resilient members extend both radially and upwardly.
4. A spacer according to claim 1, 2 or 3 wherein collapsible webs extend between the resilient members and the body.
5. A spacer according to any one of the preceding claims, moulded from plastics ma trial.
6. A spacer according to claim 5 having an X-ray marker made from a radio-opaque material.
7. A spacer for the stem of an intramedullary implant, substantially as described herein with reference to Figs. 2 and 3 of the accompanying drawings.
