1469542 Envelopes, manifolding devices DADAMATION COMMUNICATIONS Inc 5 April 1974 15165/74 Heading B6A A mailing device comprises the combination of first, second, and third sheets attached together by top stub portions 19, the first sheet 11 having means thereon defining a section 14 for the receipt of the sender's name and address and a section 16 for the recipient's name and address, an original mailing envelope (30), Fig. 2 (not shown), having two windows (31), (32), located so as to be in registry with the two sections when the intermediate second sheet is removed and the first 11 and third 13 are folded at a fold indicator 24 and inserted in the envelope, and a reply envelope 33 having a window 34 located so as to be in registry with the sender section 14 when the third sheet is removed from the first, the stub portions are removed and the folded first sheet 11 is inserted into the reply envelope. The first, second and third sheets form a manifolding unit in which data written on the first sheet is copied on to the second and third sheets, the second sheet being removed along a perforated line 20 definining the stub portion before the first and third are folded at 24 and mailed, together with the reply envelope 33 in the original envelope. Upon receipt, the third sheet is removed together with the remaining stub portions and the first sheet is sent back in the reply envelope. Two fold indicators may be provided so that the sheets may be folded along two fold lines.
