Abstract

The cage (1) has a longitudinal part in which bulged surface regions are arranged between longitudinal flats (10), where each flat is provided with longitudinal cuts (11, 12). The longitudinal cuts create a recess (13) of the cage. The cut (12) has a corner (15) that is constituted by a cutting element. Diameter of an end (2) is lower than diameter of an end (3), where each end is equipped with a handling seam for permitting the seam to be cooperated with a tool authorizing transmission of axial rotation movement.

Description

The present invention relates to an interbody cage for performing a vertebral arthrodesis. A vertebral arthrodesis consists of fusing the vertebral bodies of two contiguous vertebrae in order to obtain a stabilization of the spine, following the lesion of an intervertebral disk for example. To practice vertebral arthrodesis, one uses, not exclusively, consists of a destiny to be fused. The cages presenting under the invention an intersomatic cage, or several of them, which form a perforated box filled with spongy bone, inserted between the vertebral bodies of the vertebrae

These cages can have various characteristics, some consist of simple tubes having externally reliefs intended to ensure their anchoring, others consist of tubes shaped externally screwed so that their implementation is done by screwing after realizing A drilling. Cages of this type are generally used in pairs, the two cages being arranged substantially parallel between the two bodies of the vertebrae to be fused. The disadvantage of most of these cages is that under the effect of the pressure exerted by the two vertebrae, there is a risk of axial expulsion cages. It is an object of the present invention to provide an intersomatic cage for performing vertebral arthrodesis to overcome this disadvantage. The intervertebral cage for performing a spinal fusion according to the invention consists of a hollow, perforated piece intended to be filled with cancellous bone and to be inserted between the bodies of two contiguous vertebrae to be welded, and it is characterized essentially in that it has the general shape of an elongated ellipsoid having two diametrically opposed longitudinal flats, and in that it comprises on the one hand longitudinally, in each of the curved surface regions disposed between said flats, at least a longitudinal cutout of which at least one edge constitutes a cutting element, and on the other hand, at least one of its ends, a manipulation fingerprint allowing cooperation with a tool allowing the transmission of an axial rotational movement .

During the establishment of a cage, at impaction the two straight faces are positioned parallel to the articular surfaces of the vertebral bodies, then, by printing an axial rotation of 90 ° is positioned each of the curved faces facing a articular surface, in which at least one cutting edge has previously made, by removal of material during said rotation, a profile cavity congruent with that of said convex face, so as to anchor it in the longitudinal direction . According to an additional feature of the interbody cage according to the invention, the end comprising the manipulation fingerprint also comprises a reversible means for fixing a gripping and impaction tool. According to another additional characteristic of the intersomatic cage according to the invention, it is in the shape of an elongated ellipsoid truncated at each of its ends. According to another additional feature of the intersomatic cage according to the invention, it further has a substantially frustoconical shape in that one of the ends is of a larger diameter.

According to another additional feature of the interbody cage according to the invention, it externally has asperities designed to promote anchoring in the bone material. The advantages and characteristics of the intersomatic cage according to the invention will emerge more clearly from the description which follows and which refers to the appended drawing, which represents several non-limiting embodiments thereof. In the accompanying drawing: - Figure 1 shows a schematic side view of a first embodiment of an intersomatic cage according to the invention. - Figure 2 shows a schematic view of another side of the same cage. - Figure 3 shows a schematic view of one end of the same cage. - Figure 4 shows a schematic view of the other end of the same cage. FIG. 5 represents a schematic side view of a second embodiment of an intersomatic cage according to the invention. - Figures 6a and 6b show schematic cross-sectional views of two vertebral bodies during insertion of an interbody cage according to the invention. With reference to FIGS. 1, 2, 3 and 4, we can see a first embodiment 1 of an intersomatic cage according to the invention, and more particularly an intersomatic cage that can be inserted anteriorly or posteriorly. . The intervertebral cage 1 is in the form of a generally elongate ellipsoid-shaped element, truncated at each of its ends 2 and 3. In this embodiment, the cage 1 has a slightly frustoconical shape. end 2 thus having a smaller diameter than that of end 3, which is, however, less than that of the middle part of cage 1. Cage 1 furthermore has two longitudinal flats 10, diametrically opposite, in each of which is made a longitudinal cut 11, while is also practiced, between each flat 10, a longitudinal cut 12. These 35 cutouts 11 and 12 create on the one hand, since they are through, a recess 13 of the cage 1 and, on the other hand, for each of the cutouts 12, in cooperation with the outer wall 14 of the cage 1, two edges 15 which constitute cutting means. Each of the ends 2 and 3 is provided with an axial rotation manipulation footprint, in this case it is a diametral slot, respectively 20 and 30, intended to allow blade adaptation of a screwdriver or equivalent. Each of the ends 2 and 3 also comprises a reversible means for fixing a gripping and impaction tool, in this case this means consists of a tapped hole, respectively 21 and 31, making it possible to secure the cage 1 by screwing at the end of a tool to bring it to the insertion site and allow impaction. Referring now to FIG. 5, there can be seen a second embodiment 4 of an intersomatic cage according to the invention, which is particularly shaped to be implanted by laparoscopy. This cage 4 has substantially the same characteristics of the cage 1. It is elongate ellipsoid shaped, truncated at each of its ends 5 and 6, which are of the same diameter, it comprises two flats 40 diametrically opposed, of which only one is visible, in which are made diametrical holes 41, which could however be replaced by longitudinal cutting, while is performed between each flats 40, a longitudinal cutout 42 which creates with the holes 41 a central recess 43, and with the outer wall 44, edges 45 which constitute cutting means. One of the ends, in this case the end 6, is provided with an axial rotation manipulation footprint, constituted in this embodiment by a diametrical slot 60 intended to allow the blade to be adapted. a screwdriver or equivalent. The end 6 is also provided with a reversible means for fixing a gripping and impaction tool, which is not visible in this figure, and which may consist of a threaded hole making it possible to secure the cage 4 at the end. of a tool by screwing. Referring now to FIGS. 6a and 6b, it is possible to see the manner in which an intersomatic cage 4 is placed according to the invention, knowing that the placement of an intervertebral cage 1 is implemented in the same manner. In these figures is shown schematically two adjacent vertebral bodies 70 and 71 of a vertebral column 7, and separated by an intervertebral space 72.

In FIG. 6a, it can be seen that an intersomatic cage 1 or 4 according to the invention is inserted by impaction in the intervertebral space, this cage 1 is oriented so that the flats 40 are in contact with the articular surfaces, respectively 73 and 74, bodies respectively 70 and 71, so as to promote insertion. Note that in the case of an interbody cage 1, the shape of the wedge allows better insertion. When the cage 4 is in place, it transmits thereto, through the diametrical slot 60, using a tool, an axial pivoting movement over at least 90 °, so that the edges 45 of the cutouts 42 come to cut the articular surfaces 73 and 74 and to dig in each a vault, respectively 75 and 76 with which the wall 44 becomes congruent, thus preventing any axial movement of the cage in the intervertebral space, while introducing into central recess 43 of the bone material to form part of the graft. Note that a pivoting of the order of 120 °, followed by a pivoting in the opposite direction of the order of 30 °, allows a better cutting of the articular surfaces 73 and 74, because the two edges 45 of each of the cutouts 42 come into action.
