Abstract

L'implant cotyloïdien (1) suivant la présente invention comporte un cotyle fémoral (2) présentant une face interne de glissement (11) et une face externe (13) délimitées par un bord périphérique (10) destiné à être fixé dans une cavité cotyloïdienne (3), ledit cotyle fémoral (2) comportant une paroi (9) dont l'épaisseur (e, e') augmente progressivement entre le bord périphérique (10) et le pôle P dudit cotyle.

présente invention est relative à un implant cotyloïdien pour calotte de resurfaçage de tête fémorale appartenant au demandeur, qui peut, mais de manière non obligatoire coopérer avec un implant pour prothèse fémorale.
10 L'implant cotyloïdien suivant la présente invention comporte un cotyle fémoral présentant une face interne de glissement et une face externe délimitées par un bord périphérique, destiné à être fixé dans une cavité cotyloïdienne, ledit cotyle fémoral comportant une paroi dont l'épaisseur (e, e') augmente progressivement entre le bord périphérique et le pôle P dudit cotyle. 15 L'implant cotyloïdien suivant la présente invention comporte un cotyle fémoral comprenant un profil en forme de croissant de lune de manière que la paroi soit d'une part au niveau du bord périphérique de faible épaisseur (e) et d'autre part au niveau du pôle P d'une épaisseur plus importante (e') que celle prévue au 20 niveau dudit bord périphérique.
L'implant cotyloïdien suivant la présente invention comporte un cotyle fémoral comprenant une face externe dont le profil hémisphérique se prolonge en direction du bord périphérique par une face conique externe qui est inclinée par 25 rapport à un axe vertical d'un angle a.
L'implant cotyloïdien suivant la présente invention comporte un cotyle fémoral comprenant au niveau de sa face conique externe une série de stries.
30 L'implant cotyloïdien suivant la présente invention comporte un cotyle fémoral comprenant au niveau de son bord périphérique un bord périphérique incliné.
L'implant cotyloïdien suivant la présente invention comporte un bord périphérique qui est incliné d'un angle f3 par rapport au plan contenant le bord périphérique de 35 manière à réduire sur une partie du pourtour du cotyle fémoral la longueur de la face conique externe et le nombre de stries.
L'implant cotyloïdien suivant la présente invention comporte un bord périphérique et un bord périphérique incliné qui présentent un profil arrondi ou émoussé. L'implant cotyloïdien suivant la présente invention comporte une paroi dont la variation de l'épaisseur permet d'éloigner progressivement la face interne de glissement de la face externe du cotyle fémoral.
45 La description qui va suivre en regard des dessins annexés, donnés à titre d'exemples non limitatifs, permettra de mieux comprendre l'invention, les caractéristiques qu'elle présente et les avantages qu'elle est susceptible de procurer : 40
Figure 1 est une vue montrant un implant cotyloïdien pour prothèse fémorale suivant la présente invention.
Figures 2 et 3 sont des vues représentant l'implant cotyloïdien suivant la présente invention.
Figures 4 et 5 sont des vues illustrant l'implant cotyloïdien avant et après sa fixation dans une cavité cotyloïdienne d'un bassin.
On a montré en figures 1 à 5 un implant cotyloïdien 1 suivant la présente invention comportant un cotyle fémoral 2 qui est destiné à être fixé dans une cavité cotyloïdienne 3 d'un bassin 4 et dont la surface articulaire de ce dernier est détériorée. L'implant cotyloïdien 1 est prévu pour coopérer avec un implant fémoral 5 comportant par exemple une calotte fémorale 6 qui est fixée sur la tête fémorale 7 d'un fémur 8.
20 L'implant cotyloïdien 1 est constitué d'un jeu ou d'une gamme de cotyle fémoral 2 pour pouvoir s'adapter aux dimensions de la cavité cotyloïdienne 3 à réparer.
Le cotyle fémoral 2 est constitué d'une paroi 9 dont l'épaisseur varie entre le bord périphérique 10 et le pôle P dudit cotyle fémoral 2. 25 Le cotyle fémoral 2 présente une face interne de glissement 11 à profil sensiblement hémisphérique coopérant avec celle externe 12 de l'implant fémoral 5. Le cotyle fémoral 2 comporte une face externe 13 à profil partiellement hémisphérique destinée à coopérer avec la surface articulaire de la cavité 30 cotyloïdienne 3.
Le cotyle fémoral 2 comporte un profil en forme de croissant de lune de manière que la paroi 9 soit d'une part au niveau du bord périphérique 10 de faible épaisseur e et d'autre part au niveau du pôle P d'une épaisseur plus importante 35 e' que celle prévue au niveau dudit bord périphérique.
Ainsi, le cotyle fémoral 2 présente une paroi 9 dont l'épaisseur augmente progressivement entre le bord périphérique 10 et le pôle P.
40 La faible épaisseur e de la paroi 9 au niveau du bord périphérique 10 du cotyle fémoral 2 permet de sous dimensionner le diamètre de fraisage de la cavité cotyloïdienne 3 tout en respectant au maximum le capital osseux périphérique.
La variation de l'épaisseur e, e' de la paroi 9 permet d'éloigner progressivement la 45 face interne de glissement 11 de la face externe 13 du cotyle fémoral 2.
En effet le fraisage permet de calibrer la cavité cotyloïdienne 3 en abrasant le cartilage de la surface articulaire puis l'os spongieux, jusqu'à trouver un fond d'os dur dans le bassin 4. 15 Ce fraisage est obligatoire, mais n'est pas sans inconvénient puisqu'il approfondit la cavité cotyloïdienne 3 et décale vers l'intérieur du bassin 4 le centre de rotation articulaire de l'articulation fémorale.
Dans ces conditions pour que l'articulation fémorale fonctionne correctement, il est nécessaire de pouvoir retrouver, après le fraisage de la cavité cotyloïdienne 3, le centre de rotation articulaire C de l'articulation fémorale.
Pour cela, le cotyle férnoral 2 suivant la présente invention permet d'extérioriser le centre de rotation articulaire C par l'augmentation progressive de l'épaisseur de la paroi 9 afin que celle-ci soit au niveau du fond ou du pôle P d'une épaisseur e' la plus importante.
D'autre part le cotyle fémoral 2 comporte une face externe 13 dont le profil hémisphérique se prolonge en direction du bord périphérique 10 par une face conique externe 14 qui est inclinée par rapport à un axe vertical d'un angle a.
On note que la face interne de glissement 11 du cotyle fémoral 2 est décalée vers l'extérieur par rapport à la face externe 13 venant en appui contre le fond de la cavité cotyloïdienne 3.
Le cotyle fémoral 2 comporte au niveau de sa face conique externe 14 une série de stries 15 qui sont destinées, lors de la fixation dudit cotyle fémoral 2, à pénétrer dans l'os périphérique de la cavité cotyloïdienne 3 préalablement calibrée. Le cotyle fémoral 2 comporte au niveau de son bord périphérique 10 un bord périphérique incliné 6 prolongeant ledit bord périphérique 10. Le bord périphérique 16 est incliné d'un angle f3 par rapport au plan contenant le bord périphérique 10.
30 Le bord périphérique 16 est incliné en direction de l'intérieur du cotyle fémoral 2 réduisant sur une partie du pourtour de ce dernier la longueur de la face conique externe 14 et le nombre de stries 15 et permettant le passage du muscle du psoas .
35 On note que le bord périphérique 10 et le bord périphérique incliné 16 du cotyle fémoral 2 présentent un profil arrondi ou émoussé.
Ainsi le cotyle fémoral 2 présente un bord périphérique 10, 16 qui permet lors de son introduction dans la cavité cotyloïdienne 3 de ne pas être en contact avec le 40 muscle du psoas.
On remarque que le cotyle fémoral 2 comporte une face interne de glissement hémisphérique 11 qui est présente un aspect de surface polie miroir, tandis que les faces externes 13, 14 et les stries 15 sont recouverts d'un substrat ostéo- 45 conducteur.
Il doit d'ailleurs être entendu que la description qui précède n'a été donnée qu'à titre d'exemple et qu'elle ne limite nullement le domaine de l'invention dont on ne sortirait pas en remplaçant les détails d'exécution décrits par tout autre équivalent. 50

Claims (8)

REVENDICATIONS
1. Implant cotyloïdien comportant un cotyle fémoral (2) présentant une face interne de glissement (11) et une face externe (13) délimitées par un bord périphérique (10) destiné à être fixé dans une cavité cotyloïdienne (3), caractérisé en ce que le cotyle fémoral (2) comporte une paroi (9) dont l'épaisseur (e, e') augmente progressivement entre le bord périphérique (10) et le pôle P dudit cotyle.
2. Implant cotyloïdien suivant la revendication 1, caractérisé en ce que le cotyle fémoral (2) comporte un profil en forme de croissant de lune de manière que la paroi (9) soit d'une part au niveau du bord périphérique (10) de faible épaisseur (e) et d'autre part au niveau du pôle P d'une épaisseur plus importante (e') que celle prévue au niveau dudit bord périphérique.
3. Implant cotyloïdien suivant la revendication 1, caractérisé en ce que le cotyle fémoral (2) comporte une face externe (13) dont le profil hémisphérique se prolonge en direction du bord périphérique (10) par une face conique externe (14) qui est inclinée par rapport à un axe vertical d'un angle a.
4. Implant cotyloïdien suivant la revendication 3, caractérisé en ce que le cotyle fémoral (2) comporte au niveau de sa face conique externe (14) une série de stries (15).
5. Implant cotyloïdien suivant la revendication 1, caractérisé en ce que le cotyle fémoral (2) comporte au niveau de son bord périphérique (10) un bord périphérique incliné (16) permettant la passage du muscle du psoas.
6. Implant cotyloïdien suivant la revendication 5, caractérisé en ce que le bord périphérique (16) est incliné d'un angle (3 par rapport au plan contenant le bord périphérique (10) de manière à réduire, sur une partie du pourtour du cotyle fémoral (2), la longueur de la face conique externe (14) et le nombre de stries (15).
7. Implant cotyloïdien suivant la revendication 5, caractérisé en ce que le bord périphérique (10) et le bord périphérique incliné (16) présentent un profil arrondi ou émoussé.
8. Implant cotyloïdien suivant la revendication 1, caractérisé en ce que le cotyle fémoral (2) comporte un profil en forme de croissant de lune de manière que la paroi (9) soit d'une part au niveau du bord périphérique (10) de faible épaisseur (e) et d'autre part au niveau du pôle P d'une épaisseur plus importante (e') que celle prévue au niveau dudit bord périphérique de manière à ce que la variation de l'épaisseur (e, de la paroi (9) permette d'éloigner progressivement la face interne de glissement (11) de la face externe (13) du cotyle fémoral (2).
