Abstract

Cupule comportant une partie sensiblement hémisphérique (7) prolongée par une courte partie cylindrique (8) délimitant l'ouverture pour l'introduction de la pièce d'articulation de la tête de la prothèse, cette cupule étant équipée de deux pattes allongées (10) convergeant au milieu de la cupule, et destinées à réaliser une fixation sur le bassin, au-dessus de la cavité cotyloïdienne, ainsi qu'un crochet (12) et/ou une paire de pattes secondaires et/ou des pions d'ancrage (13). Les pattes allongées (10) sont fixées sur la cupule en retrait du plan de l'ouverture de la partie cylindrique (8), tandis que le crochet (12) et/ou les pattes secondaires (15) sont fixées sur la partie cylindrique (8) de la cupule de façon à sensiblement affleurer le plan de l'ouverture de la partie cylindrique.

Description

La présente invention a pour objet une cupule métallique de reconstruction
pour prothèse de hanche.
Lorsque la cavité cotyloïdienne d'un bassin est fortement détériorée, il n'est pas possible de l'équiper directement d'une cupule en polyéthylène servant à l'articulation de la tête de la prothèse de hanche. Il est donc nécessaire de placer dans la cavité cotyloïdienne une cupule métallique de soutien ou cupule de reconstruction. Compte tenu de la forte détérioration et de l'état osseux déficient du patient, il convient de fixer cette cupule de soutien dans des zones dans lesquelles l'os est sain. A cet effet, une cupule de reconstruction comprend des pattes allongées, des plots d'ancrage et/ou des crochets.
La cupule peut également comporter une paire de pattes secondaires inclinées par rapport aux pattes allongées.
De façon connue, les axes des pattes et des plots passent par le centre de la sphère de la cupule, ce qui permet de réaliser indifféremment un montage à droite ou un montage à gauche, sans nécessiter de cupules spécifiques pour la droite et pour la gauche.
La cupule selon l'invention est du type comprenant une partie hémisphérique prolongée par une partie cylindrique de faible hauteur, dans laquelle débouche l'ouverture d'introduction de la pièce destinée à recevoir la tête de la prothèse.
Dans les cupules métalliques connues, toutes les pattes et/ou crochet sont fixées sur la cupule au niveau du plan extérieur de la partie cylindrique ou face de la cupule. Cette solution présente l'inconvénient que l'angle formé par le plan de la cupule avec l'horizontal est important. Il en résulte des possibilités de luxation de la prothèse. Pour remédier à ces inconvénients, il est connu de procéder à une certaine torsion des pattes allongées qui sont situées au dessus de la cavité cotyloïdienne, pour réduire l'angle formé par le plan d'ouverture de la cupule avec l'horizontal.
Le problème technique à la base de l'invention est donc la réalisation d'une cupule du type précité qui permet de réduire l'angle que forme le plan d'ouverture de la cupule avec l'horizontal.
A cet effet, la cupule qu'elle concerne, comportant une partie sensiblement hémisphérique prolongée par une courte partie cylindrique délimitant l'ouverture pour l'introduction de la pièce d'articulation de la tête de la prothèse, cette cupule étant équipée de deux pattes allongées convergeant au milieu de la cupule, et destinées à réaliser une fixation sur le bassin, au-dessus de la cavité cotyloïdienne, ainsi qu'un crochet et/ou une paire de pattes secondaires et/ou des pions d'ancrage, est caractérisée en ce que les pattes allongées sont fixées sur la cupule en retrait du plan de l'ouverture de la partie cylindrique, tandis que le crochet et/ou les pattes secondaires sont fixées sur la partie cylindrique de la cupule de façon à sensiblement affleurer le plan de l'ouverture de la partie cylindrique.
Cet agencement permet, sans avoir à recourir à une torsion des pattes allongées, de positionner la cupule métallique de telle façon que son plan d'ouverture forme avec l'horizontal un angle inférieur à l'angle formé dans le cas traditionnel. Il en résulte un meilleur appui de la tête de prothèse dans la cupule en polyéthylène destinée à être placée dans la cupule de reconstruction, et par suite une limitation des risques de luxation.
Suivant une caractéristique de l'invention, les pattes allongées sont 15 fixées sensiblement au niveau du plan équatorial de la partie hémisphérique de la cupule.
Ainsi, la distance entre le point de fixation de chaque patte allongée et le plan de l'ouverture de la partie cylindrique est de l'ordre de 3 mm. Avantageusement, le bord délimitant l'ouverture de la cupule comporte un trou formant repère, situé sur l'axe de symétrie des deux pattes allongées.
Ce trou permet au chirurgien de mieux repérer le positionnement angulaire du cotyle à l'intérieur du bassin.
Suivant une autre caractéristique de l'invention, l'angle que forment 25 deux pattes allongées est de l'ordre de 60 pour un diamètre de cupule inférieur ou égal à 57 mm et est inférieur à 60 pour des diamètres de cupule supérieurs à 57 mm.
Avantageusement, l'angle que forment deux pattes allongées est de l'ordre de 52,4 pour des diamètres de cupule compris entre 59 et 65 mm et de l'ordre de 48,84 pour des diamètres de cupule compris entre 67 et 73 mm.
Cette diminution de l'angle que forment deux pattes allongées lorsque le diamètre de la cupule augmente permet, quel que soit le diamètre de la cupule, de réaliser une fixation des pattes à peu près dans la même zone du bassin, qui correspond à une zone dans laquelle l'os est solide.
De toute façon, l'invention sera bien comprise à l'aide de la description qui suit, en référence au dessin schématique annexé, représentant, à titre d'exemples non limitatifs, deux formes d'exécution de cette cupule métallique.
Figure 1 est une vue en perspective éclatée d'une prothèse de hanche comportant une première cupule selon l'invention.
Figure 2 est une vue de cette prothèse positionnée par rapport au bassin.
Figures 3 à 5 sont des vues respectivement en perspective, en élévation, et de côté de la cupule de figures 1 et 2.
Figures 6 et 7 sont deux vues très schématiques en coupe 10 représentant, pour la figure 6, une cupule métallique traditionnelle et pour la figure 7, la cupule métallique selon l'invention.
Figures 8 à 10 sont trois vues respectivement en perspective, en élévation et de côté d'une second cupule.
La figure 1 représente une prothèse de hanche comprenant une cupule métallique de reconstruction 2 destinée à recevoir une cupule en polyéthylène 3 à l'intérieur de laquelle est destinée à venir se monter de façon articulée, une tête 4 montée à l'extrémité d'un col 5 la reliant à une tige 6 destinée à être engagée dans le fémur. La cupule représentée au dessin comprend, comme mieux montré aux figures 3 à 5, une partie hémisphérique 7, prolongée par une partie cylindrique 8 de faible hauteur. La surface extérieure de la cupule comporte une zone 9 crantée, destinée à favoriser l'accrochage sur l'os.
La cupule représentée aux figures 1 à 5, comprend deux pattes allongées 10 convergeant vers le milieu de la cupule, et formant entre elles un angle A de 60 , dans la mesure où le diamètre extérieur de la cupule est inférieur à 57 mm.
Sur l'axe de symétrie des deux pattes allongées 10 est fixé, du côté opposé de la cupule, un crochet 12.
Des plots d'ancrage 13 sont prévus, qui convergent également vers 30 le centre de la partie hémisphérique 7.
Comme montré à la figure 2, la cupule métallique 2 est destinée à être engagée dans la cavité cotyloïdienne du bassin 14, les pattes allongées 10 étant situées vers le haut, et le crochet 12 situé vers le bas par rapport à la cavité du bassin. La fixation est réalisée par l'intermédiaire de vis non représentées au dessin.
Suivant la caractéristique essentielle de l'invention, les deux pattes 10 sont fixées en retrait par rapport au plan d'ouverture de la prothèse, avec un décalage D de l'ordre de 3 mm, les pattes 10 étant fixées sensiblement au niveau du plan équatorial de la partie hémisphérique 7. Au contraire, le crochet 12 est fixé sur la cupule de façon à affleurer le plan d'ouverture de celle-ci. II en résulte, comme montré aux figures 6 et 7 qui sont deux vues en coupe représentant respectivement le montage d'une cupule traditionnelle et le montage d'une cupule selon l'invention, que l'angle X formé par le plan d'ouverture de la cupule et un plan horizontal est inférieur dans le cas d'une cupule selon l'invention au même angle Y obtenu avec une cupule traditionnelle. II en résulte un meilleur appui de la tête de la prothèse dans la cupule en polyéthylène et de celle-ci dans la cupule métallique, et par suite une limitation des risques de luxation.
Les figures 8 à 10 représentent une variante de cette cupule dans laquelle les mêmes éléments sont désignés par les mêmes références que précédemment. Cette seconde cupule se caractérise en ce qu'elle ne comporte pas de crochet, mais un jeu de deux pattes secondaires 15, diamétralement opposées qui, comme le crochet 12, sont fixées au niveau de la partie cylindrique de la cupule, de façon à effleurer le plan d'ouverture de celle-ci.
Dans tous les cas, la cupule présente dans son bord délimitant l'ouverture, un trou 16 de repérage facilitant pour le chirurgien le positionnement de la cupule par rapport au bassin.
Comme il ressort de ce qui précède, l'invention apporte une grande amélioration à la technique existante en fournissant une cupule de structure simple, permettant un bon positionnement vis-à-vis du bassin et évitant ainsi les risques de luxation.
Comme il va de soi, l'invention ne se limite pas aux seules formes d'exécution de cette prothèse, décrites ci-dessus à titre d'exemples, elle en embrasse, au contraire, toutes les variantes. C'est ainsi, notamment, que cette cupule pourrait ne pas comporter de plot d'ancrage, sans que l'on sorte pour autant du cadre de l'invention.

Claims (6)

REVENDICATIONS
1. Cupule métallique de reconstruction pour prothèse de hanche, comportant une partie sensiblement hémisphérique (7) prolongée par une courte partie cylindrique (8) délimitant l'ouverture pour l'introduction de la pièce d'articulation de la tête de la prothèse, cette cupule étant équipée de deux pattes allongées (10) convergeant au milieu de la cupule, et destinées à réaliser une fixation sur le bassin, au-dessus de la cavité cotyloïdienne, ainsi qu'un crochet (12) et/ou une paire de pattes secondaires (15) et/ou des pions d'ancrage (13), caractérisée en ce que les pattes allongées (10) sont fixées sur la cupule en retrait du plan de l'ouverture de la partie cylindrique (8), tandis que le crochet (12) et/ou les pattes secondaires (15) sont fixées sur la partie cylindrique (8) de la cupule de façon à sensiblement affleurer le plan de l'ouverture de la partie cylindrique.
2. Cupule métallique selon la revendication 1, caractérisée en ce que les pattes allongées (10) sont fixées sensiblement au niveau du plan équatorial de la partie hémisphérique (7) de la cupule.
3. Cupule métallique selon l'une des revendications 1 et 2, caractérisée en ce que la distance entre le point de fixation de chaque patte allongée (10) et le plan de l'ouverture de la partie cylindrique (7) est de l'ordre de 3 mm.
4. Cupule métallique selon l'une des revendications 1 à 3, caractérisée en ce que le bord délimitant l'ouverture de la cupule comporte un trou (16) formant repère, situé sur l'axe de symétrie des deux pattes allongées (10).
5. Cupule métallique selon l'une des revendications 1 à 4, caractérisée en ce que l'angle que forment deux pattes allongées (10) est de l'ordre de 60 pour un diamètre de cupule (2) inférieur ou égal à 57 mm et est inférieur à 60 pour des diamètres de cupule supérieurs à 57 mm.
6. Cupule métallique selon la revendication 5, caractérisée en ce que l'angle que forment deux pattes allongées (10) est de l'ordre de 52,4 pour des diamètres de cupule (2) compris entre 59 et 65 mm et de l'ordre de 48,84 pour des diamètres de cupule compris entre 67 et 73 mm.
