Abstract

Ce dispositif est remarquable en ce qu'il comprend : - une cupule métallique (2) apte à être fixée par du ciment dans le fond de la cavité cotyloïdienne et délimitant une surface interne cylindro-sphérique (2e); - la surface interne cylindro-sphérique de la cupule présente un chanfrein (2f) en sortie pour retarder l'effet came du col contre le bord de la cupule; - un noyau en polyéthylène (3) monté à l'intérieur de la surface de la cupule (2) avec capacité de pivotement multidirectiormel;- le centre (01) de courbure des surfaces interne et externe du noyau (3) est confondu avec le centre de courbure de la surface interne de la cupule métallique (2); - la surface externe de la cupule (2) est composée de deux portions de sphère, de rayon différent, tangentes au pôle; le plus grand de ces deux rayons constituant le diamètre nominal de la cupule; - le centre (03) de la sphère de plus grand rayon étant décalé vers l'extérieur par rapport au centre (01) de la sphère intérieure, le centre articulaire est médialisé de ce décalage;- la cupule (2) présente une collerette (2b) de pressurisation du ciment, le pourtour de ladite collerette étant au diamètre nominal de la cupule (rayon de la sphère extérieure le plus grand).

DISPOSITIF DE RECONSTRUCTION DE LA CAVITE COTYLOIDIENNE DE L'ARTICULATION DE LA HANCHE
L'invention se rattache au secteur technique des implants orthopédiques pour l'articulation de la hanche et plus particulièrement des implants cotyloïdiens de reprise.
De plus en plus aujourd'hui, du fait de l'ancienneté des premières arthroplasties de hanche, les chirurgiens utilisent des implants de reprise pour remplacer les implants fémoraux ou cotyloïdiens descellés ou usés. Plus particulièrement, l'invention concerne les stades avancés de reprise cotyloïdienne pour lesquels sont nécessaires des cotyles dits de reconstruction. A ce jour, différentes solutions de reconstruction sont proposées.
Par exemple, il est utilisé une technique associant des greffes osseuses, un renfort métallique et une cupule cimentée.
Les greffes osseuses prises directement sur le patient ou provenant de banques d'os sont compressées dans les différentes zones détruites du cotyle osseux et un élément métallique de soutien, par exemple une plaque en forme de croix du type de celle connue à ce jour, est placée en fond de cavité.
Cette plaque, en forme de croix, est fixée dans le trou obturateur au moyen d'un crochet, et dans le toit du cotyle par des vis, tandis qu'une cupule est cimentée en son centre.
Cette solution même si elle permet, outre de renforcer l'os receveur, de centrer automatiquement la cupule dans une position anatomique, n'est pas sans complication possible, en particulier sans risque de luxation.
Le taux de luxation relativement élevé (par rapport à celui existant après une intervention de première intention) peut s'expliquer par : - le délabrement osseux et musculaire de l'articulation de hanche reprise une ou plusieurs fois,

- le manque de parade anti-luxation des cupules cimentées du marché.
Une autre technique consiste à utiliser une cupule sans ciment au niveau de la destruction osseuse acétabulaire. Cette solution n'est pas toujours possible, notamment si les pertes osseuses sont importantes, ni souhaitables si le recouvrement d'une cavité régulière hémisphérique entame trop le stock osseux restant.
En cas de destruction importante, il est néanmoins possible, comme pour une reconstruction cimentée, de greffer la cavité acétabulaire et de fixer, approximativement de la même manière que les renforts en forme de croix, une armature métallique en périphérie de cavité ; sur cette armature est adaptée par fixation vissée la cupule sans ciment.
Cette solution n'est pas non plus vraiment satisfaisante, étant donné qu'il n'est pas possible d'avoir simultanément un ancrage périphérique de la cupule via l'armature métallique et son appui en fond de cavité sur les greffes osseuses.
Ce système conduit souvent à un manque de consolidation des greffes par manque de sollicitation. A défaut, des risques de fracture du soutien métallique, une mobilisation de la cupule et des luxations secondaires, peuvent apparaître.
L'invention s'est fixée pour but de remédier à ces inconvénients, de manière simple, sûre, efficace et rationnelle.
Le problème que se propose de résoudre l'invention est de pouvoir associer un matériau tel que greffons osseux, céramique d'hydroxy-apatite ou ciment, mis en compression dans le fond de l'acétabulum, à des éléments prothétiques, en ayant pour objectif : de diminuer les risques de luxation ; - d'obtenir une liberté de positionnement de la cupule par un cimentage ;

d'optimiser la compression du fond de la cavité cotyloïdienne en supprimant tout risque de suspension du matériel.
Pour résoudre un tel problème, il a été conçu et mis au point un dispositif de reconstruction de la cavité cotyloïdienne de l'articulation de la hanche, qui comprend : une cupule métallique apte à être fixée par du ciment dans le fond de la cavité cotyloïdienne et délimitant une surface interne cylindro-sphérique ;

- la surface interne cylindro-sphérique de la cupule présente un chanfrein en sortie pour retarder l'effet came du col contre le bord de la cupule ; un noyau en polyéthylène monté à l'intérieur de la surface de la cupule avec capacité de pivotement multidirectionnel ; - le centre des courbures des surfaces interne et externe du noyau est confondu avec le centre de courbure de la surface interne de la cupule métallique ; - la surface externe de la cupule est composée de deux portions de sphère, de rayon différent, tangentes au pôle ; le plus grand de ces deux rayons constituant le diamètre nominal de la cupule ; - le centre de la sphère de plus grand rayon étant décalé vers l'extérieur par rapport au centre de la sphère intérieure, le centre articulaire est médialisé de ce décalage ; - la cupule présente une collerette de pressurisation du ciment, le pourtour de ladite collerette étant au diamètre nominal de la cupule (rayon de la sphère extérieure le plus grand).
Bien évidemment, sans pour cela sortir du cadre de l'invention, le dispositif n'est pas limité à une utilisation en tant qu'implant cotyloïdien de reprise, mais peut également être utilisé en tant qu'implant cotyloïdien, pour une arthroplastie de la hanche de première intention.

Avantageusement, et notamment dans le cadre d'une reprise, le dispositif comprend une croix métallique de soutien du type de celle connue pour un homme du métier, et fixée dans le fond de la cavité cotyloïdienne avant mise en place du ciment.
L'invention est exposée ci-après plus en détail à l'aide des figures des dessins annexés dans lesquels : - la figure 1 est une vue en perspective de l'implant cotyloïdien conforme aux caractéristiques du dispositif selon l'invention ; - la figure 2 est une vue en coupe longitudinale de l'implant cotyloïdien ; - la figure 3 est une vue en coupe longitudinale montrant la mise en place du dispositif pour la reconstruction de la cavité cotyloïdienne ; - la figure 4 est un exemple de réalisation d'un fond de cotyle sous forme d'une croix adaptée à l'implant cotyloïdien.
Le dispositif de reconstruction de la cavité cotyloïdienne de l'articulation de la hanche comprend, à la base, un implant cotyloïdien composé essentiellement d'une cupule métallique (2) et d'un noyau en polyéthylène (3).
La cupule (2) est conformée pour être fixée par ciment (C) ou tout matériau composite, tel que os-ciment, dans le fond de la cavité cotyloïdienne de l'os iliaque. La cupule (2) se présente essentiellement, sous la forme d'une demisphère, en délimitant une cavité interne de forme générale hémisphérique.
La surface externe de la cupule (2) présente une pluralité de rainures parallèles entre elles et au plan équatorial (2a) et de rainures orthogonales selon quatre méridiens (2d).

Cette cupule métallique (2) délimite une surface interne cylindro-sphérique (2e) apte à recevoir, avec capacité de pivotement multidirectionnel, le noyau en polyéthylène (3).
Cette surface cylindro-sphérique (2e) présente au-delà de la portion cylindrique, dont la hauteur est de 2,5 mm environ, un chanfrein (2f) retardant l'effet came du col de tige contre le bord de la cupule.
D'une manière connue, le noyau (3) est destiné à recevoir, avec capacité de rétention, la tête prothétique (T) de l'implant fémoral correspondant. Le noyau (3) est largement plus qu'hémisphérique pour satisfaire cette fonction de rétentivité.
Selon une caractéristique importante de l'invention, le centre (01) de courbure des surfaces interne (3a) et externe (3b) du noyau (3) est confondu avec le centre de courbure de la surface interne hémisphérique de la cupule métallique (2). La surface externe de la cupule (2) est délimitée par deux portions de sphère, de rayons différents, tangentes aux pôles. Les origines (02) et (03) de ces deux rayons sont décalées vers l'extérieur par rapport au centre (01) qui correspond également au centre de rotation de la tête fémorale (T). Les différents centres (01), (02) et (03) sont disposés en alignement, selon l'axe de symétrie des éléments à assembler (2) et (3). Le décalage des centres (01) et (03) correspond à la médialisation du centre de l'insert et donc de la tête fémorale par rapport au centre anatomique du cotyle. Cette médialisation fait office de système antiluxation de l'insert mobile par rapport à la cupule comme peut le faire un débord extérieur à l'hémisphère sur d'autres cotyles concentriques connus.
A titre d'exemple indicatif nullement limitatif, la cupule métallique (2) est d'épaisseur variable, d'environ 3 mm à partir de son bord circulaire ouvert, pour se raccorder progressivement au niveau de son fond avec une épaisseur d'environ 1 mm.

Selon une autre caractéristique importante de l'invention, la cupule (2) présente une collerette (2b) de pressurisation du ciment. Cette collerette (2b) est inscrite dans une enveloppe de centre (03) décalé vers l'extérieur par rapport au centre (02). On renvoie à la figure 2 qui montre le décalage des différents centres (01), (02) et (03).
La collerette (2b) présente périphériquement des empreintes semicirculaires (2c) pour l'échappement du ciment. Avantageusement, les empreintes (2c) sont régulièrement réparties sur la circonférence de la collerette (2b).
La surface interne de la cupule métallique (2) est pleine et polie pour éviter toute abrasion du polyéthylène mobile.
Selon une autre caractéristique importante de l'invention, et notamment dans la cas d'une reprise, le fond de la cavité cotyloïdienne peut recevoir un organe métallique connu sous le nom de fond de cotyle, en forme de croix (4) (figure 4) et fixé par ses quatre branches (4a, 4b, 4d et 4e) dans la cavité osseuse : une branche supérieure (4a) terminée par une palette de vissage avantageusement découpée en forme de trèfle pour diminuer sa rigidité ; une branche inférieure (4b) terminée par un crochet (4c) inséré dans le trou obturateur. Ce crochet (4c) est avantageusement orienté de 10 vers l'avant pour suivre la ligne directrice du bord du trou obturateur ; deux branches latérales (4d) et (4e) de longueur asymétrique pour tenir compte du placement anteversé de la croix dans l'acétabulum.
La combinaison d'un implant cotyloïdien composé des deux éléments (2) et (3) et du fond de cotyle (4), est donc particulièrement bien adapté dans le cas

d'une reconstruction de la cavité cotyloïdienne de l'articulation de la hanche (figure 3).
A noter également que la cupule (2), ou le fond de cotyle métallique (4), peut présenter des agencements pour la fixation d'éléments rapportés et débordants (5), aptes à faire office de centreurs, en coopérant dans le fond de la cavité cotyloïdienne, en ayant pour objectif d'assurer un espace constant entre la cupule (2) et le fond du cotyle métallique (4) ou directement le fond de la cavité cotyloïdienne.
Les avantages ressortent bien de la description.

Claims (1)

  1. REVENDICATIONS - 1-Dispositif de reconstruction de la cavité cotyloïdienne de l'articulation de la hanche, caractérisé en ce qu'il comprend : une cupule métallique (2) apte à être fixée par du ciment dans le fond de la cavité cotyloïdienne et délimitant une surface interne cylindro-sphérique (2e) ; - la surface interne cylindro-sphérique de la cupule présente un chanfrein (2f) en sortie pour retarder l'effet came du col contre le bord de la cupule ; un noyau en polyéthylène (3) monté à l'intérieur de la surface de la cupule (2) avec capacité de pivotement multidirectionnel ; - le centre (01) de courbure des surfaces interne et externe du noyau (3) est confondu avec le centre de courbure de la surface interne de la cupule métallique (2) ; - la surface externe de la cupule (2) est composée de deux portions de sphère, de rayon différent, tangentes au pôle ; le plus grand de ces deux rayons constituant le diamètre nominal de la cupule ; - le centre (03) de la sphère de plus grand rayon étant décalé vers l'extérieur par rapport au centre (01) de la sphère intérieure, le centre articulaire est médialisé de ce décalage ; - la cupule (2) présente une collerette (2b) de pressurisation du ciment, le pourtour de ladite collerette étant au diamètre nominal de la cupule (rayon de la sphère extérieure le plus grand).
    - 2-Dispositif selon la revendication 1, caractérisé en ce que les différents centres (01), (02) et (03) sont disposés en alignement selon l'axe de symétrie du noyau (3) de la cupule métallique (2).

    - 3-Dispositif selon la revendication 1, caractérisé en ce que la collerette de pressurisation (2b) présente périphériquement des empreintes semi-circulaires (2c) pour l'échappement du ciment.
    - 4-Dispositif selon la revendication 1, caractérisé en ce que la cupule (2) présente des rainures externes (2a) et (2d) pour sa fixation par ciment.

    - 5-Dispositif selon la revendication 1, caractérisé en ce que la surface interne de la cupule métallique (2) est pleine et polie.
    - 6-Dispositif selon la revendication 1, caractérisé en ce que la cupule métallique (2) est d'épaisseur variable d'environ 3 mm à partir de son bord circulaire jusqu'à environ 1 mm au niveau de son fond.
    -7-Dispositif selon la revendication 1, caractérisé en ce qu'il présente un fond de cotyle métallique (4) en forme de croix intégrant un crochet de fixation dans le trou obturateur orienté de 10 vers l'avant et une palette supérieure de vissage découpée en forme de trèfle.
    - 8-Dispositif selon la revendication 1, caractérisé en ce que la cupule (2) présente des agencements pour la fixation d'éléments rapportés et débordants (5), aptes à faire office de centreurs en coopérant avec le fond de la cavité cotyloïdienne.
    - 9-Dispositif selon la revendication 7, caractérisé en ce que le fond de cotyle métallique (4) présente des agencements pour la fixation d'éléments rapportés et débordants (5), aptes à faire office de centreurs en coopérant avec le fond de la cavité cotyloïdienne.
