Abstract
The intervention concerns the realization of a very flexible plate used for the treatment of hernias. It has a high elastic memory by virtue of its manufacture made with a perforated mesh fabric impregnated with an elastomer or plastomer in very small quantities.
Description
The invention relates to a reconstruction plate for the field of surgery.
In surgical procedures touching or approaching the brain or the spinal cord, it is necessary to protect or reconstitute the envelopes of these so-called noble tissues and, in particular, the outer membrane called dura mater against postclururgical scar fibrosis. For this purpose, it is usual to use a piece of dura mater taken from a corpse, treated and sterilized, placed in direct contact with the noble tissues by the surgeon. However, this material has several disadvantages: it is expensive because of its origin human and necessary treatments, and above all can transmit diseases (AIDS, hepatitis, Creutzfeldt-jakob ...). These diseases are not necessarily detectable by laboratory tests: there are latency periods during which the carriers of these diseases are already contagious, that is to say can transmit the disease, and where laboratory tests are not yet positive .
The object of the invention is to avoid these risks of transmission of diseases by tissues of human or animal origin, by producing a plate having a fibrillar structure close to the natural connective structure, made of synthetic material, therefore free of disease so protect noble tissue against scar fibrosis.
The invention consists in using a porous textile fabric of tight construction, which can be impregnated with a very small amount of a medical elastomer in order to make the threads of the fabric smoother, less permeable and non-traumatic for the tissues but suitable for letting biological fluids through.
We can also improve the smooth appearance by making a completely smooth side on one side of the product by depositing a uniform layer of elastomer and leaving the other side rougher to allow it to be re-used by the tissues of the host. The smooth side will be placed opposite the noble tissues and the rough side opposite the connective or muscular tissues to allow tissue rehabitation.
The textile fabric can be a knitted fabric, a tight fabric, or more satisfactorily, a so-called non-woven product produced by entangling threads by air jet technique, for example, or by thread-to-thread welding. These products have the advantage of a fibrillar structure close to the natural connective structure.
As an exemplary embodiment of a plate according to the invention, a 100% polyester nonwoven product is used. This product, after cleaning by known techniques, is impregnated with a medical grade silicone by soaking then draining and polymerization in an oven.
The plate obtained is slightly permeable due to the small amount of silicone, very flexible and which can easily deform for the same reasons; it allows the passage of biological fluids.
According to a preferred embodiment of the invention, 8 mg / cm 2 of elastomer has been deposited on a 0.45 mm thick nonwoven weighing 8 mg / cm 2.
If it is desired to obtain a perfectly smooth face, before coating, polymerization is carried out by calendering a thin layer of silicone on one side of the product, the assembly then being polymerized in an oven. From a non-woven fabric with an initial thickness of 0.45 mm, a final product with a thickness of approximately 0.60 mm is obtained after impregnation and calendering of a layer of silicone on one side.
The exemplary embodiment with a nonwoven can, of course, be extended to any type of textile construction, knitting of any type, fabric, or even flat braid. The thicknesses and mass of the fabrics, the quantities deposited can vary depending on the fabrics and applications.
The material of the tissue having a fibrillar structure close to the natural connective structure is a biocompatible polymer which can be polyester, polyamide, polypropylene, polytetrafluoroethylene or any biocompatible material, very flexible and non-traumatic for the tissues.
The biocompatible elastomer used can be silicone, polyurethane or any other absorbable or non-absorbable biocompatible elastomer.
A plastomer can also be used to impregnate the fabric and in particular polyamide, polyvinyl chloride, polyester, polyurethane or polyolefin.
The application describes a plate for neurosurgical treatments touching or approaching the dura mater, but the invention makes it possible to produce plates for other interventions: reinforcement plates for orthopedic surgery, visceral surgery, including the treatment of hernias, gynecological surgery, cardiac and vascular surgery and in general all medical and veterinary applications benefiting from plates with fibrillar structure close to the natural connective structure, impregnated with a very small amount of a medical elastomer in order to make the threads of the tissue more smooth, less permeable and non-traumatic for the tissues.
According to another embodiment of the invention, a tubular textile product is impregnated in order to obtain a flexible and elastic vascular prosthesis, but retaining its tubular shape and slightly permeable to let the biological fluids pass.
