Abstract
Description
DisPositif Dour la réduction de fractures.Device for the reduction of fractures.
The present invention relates to a device for reducing fractures, in particular fractures of the greater trochanter.
Pertrochanteric fractures, the fracture lines of which interest the upper part of the greater trochanter, can present different anatomical types basicervical, comminuted pertrochanteric, subtrochanteric are today treated by a screw-plate device. This device consists of a plate kept fixed, pressing against the external face of the femur whose profile it follows, thanks to cortical screws penetrating into openings distributed along its longitudinal axis, this plate being extended at its upper end. by a generally cylindrical barrel extending in a direction which forms an obtuse angle with the longitudinal axis of said plate so as to penetrate by per-trochanteric or sub-trochanteric implantation as far as the neck of the femur. A nail or a screw pass in this barrel to anchor in the neck and the head of the femur.
This device is efficient and ensures perfect compression when the invoice line is less than the upper insertion point of the screw-plate. On the other hand, when the line reaches the point of penetration or seat above it, there occurs a secondary displacement in time of the diaphysis inwards with protrusion of the nail and ascent or displacement outside the greater trochanter.
Another device ensuring the maintenance of the greater trochanter is described in patent EP-A-217,317. This device consists of a shaped plate to marry the femur over a large part of its length from the greater trochanter, this plate having a threaded through bore allowing the insertion of a pin capable of penetrating into the head of the femur , this pin being removable by simple unscrewing when the bone is consolidated. Such a device, because of the height of the plate which must adapt perfectly to the profile of the bone, seems in practice not very applicable without having available a plurality of plates capable of adapting to any morphology. In addition, such a device requires a large number of anchoring points increasing trauma and not facilitating the work of the surgeon.
The object of the present invention is therefore to propose a device for reducing fractures of the greater trochanter constituting an improvement of the screw-plate system known to date by eliminating the major drawback of the latter, namely the drift of the greater trochanter with respect to to the rest of the femur during the osteosynthesis process.
The subject of the invention is to this end a device for repairing, in particular the reduction, of comminuted fractures of the greater trochanter of the type comprising a plate consisting of a plate held fixed in abutment against the external face of the femur which it marries profile thanks to cortical screws penetrating into openings distributed along its longitudinal axis, this plate being extended at its upper end by a generally cylindrical shaft extending in a direction which forms an obtuse angle with the longitudinal axis of said plate so as to penetrate by per-trochanteric or sub-trochanteric implantation into the neck of the femur, and the head of the femur, in which it is anchored by means of a nail or a screw introduced by an axial bore passing through said barrel , characterized in that it comprises a trochanteric plate which is interposed between the external face of the femur and the plate of the assembly visplate, and is held in abutment against the bone, on the one hand by said screw-plate assembly whose plate has a profile complementary to said trochanteric plate and whose barrel passes through at least one housing formed in the plate, on the other part by an extension of its upper part constituting at least one hook-shaped branch shaped to match the profile of the greater trochanter and fixing by simple pressure on the latter so that an ascent of the trochanter relative to the rest of the femur is impossible.
According to a preferred embodiment of the invention, the trochanteric plate comprises a third anchor point on the bone of the femur constituted by a through housing, held facing a housing of the screw-plate assembly intended to receive a cortical screw by means of guides arranged in relief on the visible face of said trochanteric plate, said guides preventing any pivoting of the screw-plate assembly around the barrel forming a pivot. In addition, to perfect the anchoring, the trochanteric plate is extended in its upper part by at least two branches forming a hook, said hooks being terminated by claws.

The present invention relates to an improvement to the screw-plate device commonly used to date for reducing fractures of the greater trochanter. It is recalled that the greater trochanter 2 constitutes one of the parts of the femur 1 and adjoins in particular the neck of the femur 3 which separates the greater trochanter from the head of the femur 4.
This screw-plate device shown in FIGS. 2A and 2B comprises a plate 6 formed in one piece with a barrel 7, all having an angled profile. Indeed, the longitudinal axis yy 'of the plate 6 forms with the longitudinal axis xx' of the barrel an obtuse angle a. The barrel 7 is implanted in a sub-trochanteric or per-trochanteric position and extends as far as the neck of the femur 3, or even into the head of the femur 4, where it is kept immobilized by means of a nail or a screw 8. This nail 8 is inserted through one end of the through bore 10 of the barrel 7 to project at its other end and immobilize in the femur head 4. The plate part 6 of the screw-plate 5 assembly includes, to perfect this anchoring on the femur, through holes or housings 9 distributed along its longitudinal axis yy 'and intended to allow the insertion of so-called cortical screws in the femur in the shaft of the bone.
To perfect the compression of the bone with respect to this known screw-plate device, between the plate 6 of the screw-plate and the external face of the femur 1 is inserted a so-called trochanteric plate 11, an embodiment of which is shown in the figures. 3A and 3B. This plate 11 is shaped so as to adapt perfectly on the one hand to the external surface of the femur, on the other hand to the internal face of the plate 6. The plate 6 having a concave internal face; it is the same for the trochanteric plate 11. It is this shape which best adapts to the external profile of the femur. This trochanteric plate 11 is held in abutment against the external face of the femur by means of fixing points arranged in several places. The trochanteric plate is first of all held by the barrel 7 of the screw-plate assembly 5 which passes through a through bore 12 formed on said trochanteric plate 11 before entering the femur. Therefore, due to the elbow formed by the barrel 7 with the plate 6, the plate 6 comes to hold by pressing the trochanteric plate 12 against the external face of the femur. This through housing 12 for receiving the barrel 7 formed in the trochanteric plate 11 has an ovoid shape enlarged in its lower part to allow the introduction and positioning of any screw-plate assembly 5. Indeed, depending on the type of screw-plate assembly used, the angle a between the plate and the barrel varies.
A second anchor point of the trochanteric plate 11 on the femur is located at the level of the greater trochanter so as to ensure a per-trochanteric connection. This anchoring point is produced by means of claws 15 which is provided with the plate 11. These claws 15 are arranged at the end of branches 14 formed in the upper part of the trochanteric plate. These branches 14 are extensions of the upper part of the trochanteric plate 11. The claws 15 which, in the example shown in FIG. 3B, have a triangular shape in profile, allow the greater trochanter 2 to be held integral with the rest of the femur 1 by simple pressure when tightening the other means for fixing said trochanteric plate 11. In fact, these claws easily pass through the insertion of the large external at the crest of the large trochanter 2 before being compressed by the screw lower.
The third anchor point of the trochanteric plate 11 on the femur 1 is located in the lower part of the trochanteric plate and allows a connection in the cortical area. This third anchor point is constituted by a through housing 13 disposed in the lower part of the trochanteric plate 11 along its longitudinal axis.
This through housing 13 is held opposite a housing 9 of the screw-plate assembly intended to receive a cortical screw by means of guides 16 arranged in relief on the visible face of the trochanteric plate 11, said guides 16 preventing any pivoting. of the screw-plate assembly around the barrel forming a pivot on either side of the plate 11. Consequently, these guides, constituted by two wings coming to bear on either side of the lateral edges of the plate part 6 of the screw-plate assembly, allow the screw-plate assembly to be kept in constant alignment with the plate.
The invention is not limited to the abovementioned embodiments but on the contrary encompasses a large number of other embodiments remaining within the scope of the invention.
