Abstract
Description
Osteoarthritis of the knee is essentially painful due to the trabecular micro-compression of the center of the tibial plateau.
To remedy this osteoarthritis of the knee, implants were used, generally made of large surface metal in order to distribute the loads.
However, these implants did not allow perfect stability during knee operation.
The present invention overcomes these drawbacks by allowing rapid placement of smaller implants, horizontal positioning of the support for the sliding surface, mechanical or biological fixation and, above all, a modular and revisable system to remedy the risks of 'wear.
In addition, the so-called sliding surface is versatile for the various basic varieties proposed in the invention.
According to the invention, the base surface constituted by an upper block provided on its lower part with fixing members in the upper face of the tibia comprises laterally, along these large ribs, grooves intended to receive beads, sliding surfaces of various shapes thus allowing, if necessary, after wear or other anomaly the reappearance of the patient.
According to another characteristic of the invention, the fixing members of the block carrying the sliding surfaces are constituted either by small metal rods distributed in sufficient number, or by longitudinal frames and transverse frames pierced with various openings for placing in place of conventional osteosynthesis screws
According to another feature of the invention.
Various other characteristics of the invention will also emerge from the detailed description which follows.
Embodiments of the object of the invention are shown, by way of nonlimiting examples, in the accompanying drawings.
Fig. 1 is a perspective view of a first embodiment of the base surface of the implant.
1. Fig. 2 is an inverted perspective view of the base surface of FIG. 1.
Fig. 3 is an inverted perspective view of a second embodiment of the base surface of the implant.
Fig. 4 is an inverted perspective view of FIG. 3 but comprising a sliding surface.
Figs. 5a, 5b, 5c and 5d show, in front elevation, various embodiments of the sliding surface usable with the implant of the invention.
Fig. 6 is a partially sectional view of the placement of an implant in accordance with the invention on the upper part of the tibia.
Figs. 7, 8 and 9 are anterior medial plan views of implants according to the invention on a tibia during the end of the operative phase.
In fig. 1. there is shown, in perspective, a first embodiment of a base surface of implants which is constituted by an upper block 1 of rectangular parallelepiped shape having, on these two long sides, grooves 2 intended to receive beads 3 provided in cavities 4 of sliding surface 5 (see fig. 5a to 5d).
The underside of the base surface 1 is provided, as can be seen in FIGS. 1 and 2, small metal rods 6 in sufficient number to allow the attachment, by introduction of the metal rods 6 in the upper part of the tibia, of the base surface 1 of the implant.
These rods are made of biologically compatible material with the human body but, most often, metal.
With regard to the base surface 1, its shape has been indicated rectangular but, for specific anatomical reasons, it may be round, oval or have any other simple or complex geometric shape.
Its upper face la is flat and smooth and can even undergo a polishing treatment in order to minimize friction.
When this base surface is in place on the upper part of the tibia in the medial tibial plateau and / or in the lateral tibial plateau (see fig. 7, 8 and 9), the practitioner sets up, by sliding, a surface of slide 5 (see fig. 5a to 5d) whose upper face 55 can be flat (see fig. 5a), hollow 55a (concave) (see fig. 5b), gently sloping 55b (concave with raised edges) (see fig . 5c), inclined plane 55c (oblique) (see fig. 5d) depending, on the one hand, on the location on the tibial plateau and, on the other hand, the particular configuration of the patient's knee.
In certain cases, the upper face of the sliding surfaces can be adapted anatomically to the patient, especially when it is a question of repairing a knee having already received a prosthesis.
Of course, again, the sliding surfaces are made of materials biologically compatible with the human body and, in particular, high density polyethylenes, ceramic products, carbon-based products, etc. are used.
Finally, there is shown, in section, in FIG. 6, the head of a tibia 20, the upper face of which has been worked to receive a base surface 30, the lower part of which, comprising two parallel frames 31, allows large screws to be put in place, because of oblong holes 32 33 has conventional cancellous osteosynthesis for fixing by compression of the assembly in the bone.
In addition, there are provided, transversely below the base surface, cross members 34 drilled with at least one hole 34a to receive one or more osteosynthesis screws 34b for fixing by traction.
It is also possible to provide openings 35 (see FIGS. 3 and 4) so that the bone can recolonize the implant in order to ensure its biological fixation.
For further information, it is stipulated that the base surfaces as well as their annexes (see fig. 1, 2, 3, 4 and 6) can be coated with porous metal with or without coating of PAH (hydroxylapatite).
As can be seen from the foregoing description, the positioning of the sliding surfaces (see fig. 5a to 5d) on the base surfaces, by penetration of the beads 3 in the grooves 2 thereby forming a snap mechanical, achieves a simple fixing ensuring the versatility of the sliding surface on the base surface and allows a later revision, if necessary, and a change of sliding surface in an easy way in the case of premature wear.
The fact of using compatible materials and products, on the one hand, and making small implants, on the other hand, allows practitioners to intervene quickly, not only tiring the patient but also the perfect delivery of knee joint ligaments thus ensuring a surprising success for interventions which, until then, were painful for the patient and sometimes had a disappointing result.
