Abstract
Description
The present invention relates to a pin used in surgery, more particularly for hand surgery according to the so-called KAPANDJI method.
Intra-focal skewering according to the KAPANDJI method poses problems not attributable to the technique but more often due to KIRSCHNER pins. It is necessary to overcome two problems: excessive migration and the conflict between the distal end of the spindle and the subcutaneous and cutaneous elements.
According to the invention, this result is obtained with a spindle used in surgery, more particularly for hand surgery according to the so-called KAPANDJI method, characterized in that it has a proximal tip with an anti-migration stopper. and a distal end covered with a protective tip.
The invention will be better understood with the aid of the description given below of an embodiment given by way of nonlimiting example, with reference to the appended drawings in which - FIG. 1 is an elevation view side of a spindle according to the invention; - Figure 2 is a schematic view of the pin positioned in the patient's hand - Figure 2A is an enlarged view of the distal part, once positioned - Figure 2B is an enlarged view of the proximal part, once positioned.
The pin (1) essentially consists of a body (2), a tip (3) in the proximal part and a distal end (4) provided with a protective cap (5).
The tip has an anti-migration stopper (6) which prevents displacement of the spindle, with reconstruction of the bone at the level of a groove (7). This is shown schematically in Figure 2B.
The protection of the distal end by the end piece (5) prevents damage to the corresponding skin tissue.
Preferably the point will be made of diamond, the rest of the pin may be made of steel or titanium.
The operative method of skewering according to KAPANDJI is the variant, for the reduction of wrist fractures.
A pin is forced into place by the diamond point which allows their use with a simple manual handle because the penetration is easy and you feel the pin stop by hand after its penetration at the cortical opposite.
After having installed the 2 or 3 pins depending on the type of fracture to be treated, the radiological control is carried out.
The distal end of the spindle is cut with a pair of pliers, pushing the skin as far as possible.
Then the distal end is covered by the metal tip (5) which is deformed by crushing it between the two jaws of a cutting pliers. It thus becomes adherent to the pin and can be covered by the skin tissue which is closed with one or two stitches.
A plaster splint for 10 to 21 days is often associated according to the operators at the end of this intervention.
The advantages of the device according to the invention are multiple and start from the following chemical observations - bone penetration is easy. The crossing of the opposite cortex is done without great difficulty - the manual sensation of stopping during penetration avoids pushing this pin too far and therefore, damaging the tendon and vascular-nervous elements; moreover, it can dispense with radioscopic control - the distal protective tip plays its role well and avoids secondary injuries to the tendons and skin tissue. It adheres well to the spindle and makes it easier to identify when ablating the material - we often see when ablating these pins that they hold up well.
This comes from different elements
the overflow of the diamond point acting as a retrograde stop,
the diameter of the spindle that enters the cortex is less than that of the pins used until now,
lastly, the distal tip helps, by leaning on the skin, to prevent the recoil of this pin.
Finally, this equipment does not require any particular instrumentation for its installation.
