Abstract

Element of the type comprising a diaphyseal part of elongate shape and intended to be engaged in the medullary cavity of the femur, and a metaphyseal part whose free end protruding from the bone is equipped with a ball intended to articulate inside a cup fixed in the acetabular cavity. According to the invention, the diaphyseal part (2) and the metaphyseal part (3) are produced independently of one another and are joined to one another before implantation, taking into consideration the required sizes, respectively, for the diaphyseal part and for the metaphyseal part. Diaphyseal parts and metaphyseal parts of different sizes are equipped with the same assembly means.

Description

FEMALE ELEMENT FOR HIP PROSTHESIS
The present invention relates to a femoral element for hip prosthesis.

In case of deterioration of the head of the femur, or of the acetabular cavity inside which it is mounted, it is common to proceed with the implantation of a prosthesis.

The implantation of a prosthesis is carried out by sawing the neck of the femur or upper part thereof and by replacing it with a femoral element which comprises a tail intended to be engaged in the medullary canal, terminated by a shaped head general spherical engagement in the acetabulum. Depending on the state of this cavity, it may or may not be equipped with a cup of generally hemispherical shape inside which the head of the prosthesis is articulated.

The femoral element comprises a diaphyseal part of elongated shape and intended to be engaged in the medullary canal of the femur, and a metaphyseal part, whose free end projecting from the bone is equipped with the generally spherical head.

It is commonly accepted that the transmission of stresses on the femur must be carried out at the level of the metaphysis, the diaphyseal part essentially having the function of serving as a guide, and of positioning the metaphyseal part, without having to play an important role of transmission of stresses. .

The femoral elements currently constructed are designed to respect these two principles. However, there are often problems due to the relative difference in the sizes of the metaphysis and the diaphysis. A narrow shaft can only admit a small rod, while the metaphysis may require a larger implant.

The object of the invention is to provide a femoral element for hip prosthesis, which allows, whatever the morphology of the individual, to best fulfill the functions which are assigned to the diaphyseal part, to serve as a guide, and with the metaphyseal part having to ensure the main part of the transmission of the stresses.

To this end, in the femoral element that it concerns, the diaphyseal and metaphyseal parts are produced independently of one another and assembled to each other before implantation, taking into account the dimensions required respectively for the diaphyseal part and for the metaphyseal part, diaphyseal and metaphyseal parts of different sizes being equipped with the same assembly means.

In addition, the metaphyseal part is of a shape adapted to the side of the patient, right or left, to be fitted.

It is thus possible to have a sampling of sizes of metaphyseal parts, right and left, for example nine in number, and a sampling of sizes of diaphyseal parts, for example six in number. The means of assembly between these two types of parts being standard, in the example given above it is possible to make nine times six combinations.

It is thus possible to adapt in practically all cases, the metaphyseal and diaphyseal sizes of the femoral element, ensuring excellent positioning of the latter.

According to one embodiment of this element, the diaphyseal part has, on the side of the metaphyseal part, a front face from which protrudes axially a cylindrical rod itself extended by a threaded rod of smaller diameter, while the part metaphyseal presents, on the side of the diaphyseal part, a front face into which opens a smooth cylindrical cavity extended by a tapped cavity, these two cavities being at the diameters of the smooth rod and of the threaded rod of the diaphyseal part respectively.

This method of assembly ensures excellent positioning and excellent behavior of the two parts of the prosthesis relative to one another, the longitudinal guidance being provided by the smooth cylindrical parts, and the front faces being in abutment one against the other. the other.

According to another characteristic of the invention, the metaphyseal part comprises in its upper part and on its internal face a shoulder in the form of a flange section, arranged in a plane substantially perpendicular to the axis of the upper zone of the metaphyseal part. .

This shoulder is used to position the prosthesis, making it move back a little during the impaction, which makes it "sit" and certainly avoids the varus.

The general structure of this femoral component of hip prosthesis, thus ensures
- an exclusively metaphyseal transmission of constraints,
- automatic positioning,
- a directing role for the diaphyseal piece which does not transmit any constraints.

We therefore avoid all risks of rupture due to an excessive fixing of a diaphyseal part rigidly associated with a metaphyseal part which is ill-adapted, too small and sometimes mobile.

According to an embodiment of this element, the diaphyseal part comprises, protruding from its connection zone to a metaphyseal part, three elastic rods or nails called ENDER.

This diaphyseal part allows an elastic but solid mounting, which can block the rotation of the fragments with or without the help of diaphyseal screws. A simple modeling of the elastic nails allows reliable, light and elastic assemblies to be produced.

Advantageously, the metaphyseal part includes notches arranged on its outer face.

In addition, the notches formed on the metaphyseal part have a general orientation perpendicular to the axis of the femur to be fitted.

In accordance with another advantageous characteristic, the femoral element has undergone a surface treatment by diffusion of plasma coated with hydroxyapatite.

These last characteristics favor the secondary rehabitation of this cementless prosthesis.

The femoral element according to the invention comprises, as shown in FIG. 1, a diaphyseal part 2 and a metaphyseal part 3 produced independently of one another.

The diaphyseal part 2 comprises a body 4 intended to be engaged in the medullary canal of the femur, and the end of which situated on the side of the metaphyseal part 3 comprises a front face 5 from which axially protrudes a smooth cylindrical rod 6 itself extended by a threaded rod 7 of smaller diameter.

For its part, the metaphyseal part 3 comprises a head 8 intended for fixing the element of generally spherical articulation shape inside the acetabular cavity, this head being separated from the body 9 by a shoulder 10, in shape of a flange section, disposed in a plane substantially perpendicular to the axis of the head 8. The body 9 of the part 3 comprises, turned towards the side of the diaphyseal part 2, a front face 12 into which a smooth cylindrical cavity opens 13 itself extended by a tapped cavity 14, these two cavities 13 and 14 being at the diameters of the smooth rod 6 and of the threaded rod 7 of the diaphyseal part, respectively.

Two types of metaphyseal implants 3 are provided, a right and a left. In practice, nine sizes are provided for the metaphyseal parts 3, and six sizes for the diaphyseal parts 2. The metaphyseal and diaphyseal parts being interchangeable, it is thus possible to make fifty four combinations. FIG. 3 illustrates respectively in solid lines and in fine lines two types of femoral element obtained from two metaphyseal parts 3 and two diaphyseal parts 2 of different sizes.

In the embodiment shown in FIG. 4, in which the same elements are designated by the same references as above, the diaphyseal part 3 is constituted by three elastic nails 15. These three elastic nails allow a solid mounting which can block the rotation of the fragments with or without the help of diaphyseal screws, after a simple modeling of the nails.

Figures 5 and 6 show an alternative embodiment of the femoral element shown in Figures 1 to 3, wherein the same elements are designated by the same references as above. In this femoral element, the metaphyseal part 3 includes notches 16, which have a general orientation substantially perpendicular to the axis 17 of the femur to be fitted. These notches facilitate bone rehabitation, and consequently the fixing of the prosthesis which is a cementless prosthesis, the surface of which has been coated by diffusion of titanium plasma coated with hydroxyapatite.

As is apparent from the above, the invention brings a great improvement to the existing technique, by providing a femoral element for hip prosthesis allowing perfect adaptation on the one hand, of the metaphyseal part and on the other hand, of the diaphyseal part to the patient's morphology, so that the metaphyseal part transmits all of the stresses, and the diaphyseal part only acts as a guide and for positioning the metaphyseal part.

As it goes without saying, the invention is not limited to the sole embodiments of this element described above by way of example, on the contrary it embraces all the variant embodiments thereof.
