Description

Elément fémoral d'une endoprothèse de hanche. Femoral component of a hip endoprosthesis.

Plus particulièrement, l'invention concerne une tige fémorale multibloquée pour prothèse totale de la hanche. More particularly, the invention relates to a multiblocked femoral stem for total hip prosthesis.

On rappelle que le but de l'arthroplastie totale est de réaliser une articulation stable, mobile et indolore, en mettant en place un implant de longévité suffisante et apte a être changé si nécesaire. Il existe, a ce jour, une très grande variété de prothèses totales de hanche. It is recalled that the goal of total arthroplasty is to achieve a stable joint, mobile and painless, by implementing an implant of sufficient longevity and able to be changed if necessary. There is, to date, a very wide variety of total hip prostheses.

On sait par ailleurs que, pour avoir un bon maintien de 1'élément de prothèse, avec ou sans ciment, le remplissage niétaphyso-diaphysaire est très imprortant.  It is also known that, in order to have a good support of the prosthesis element, with or without cement, the nietaphyso-diaphyseal filling is very unpredictable.

Un des problèmes que se propose de résoudre l'invention, est de réaliser une prothèse susceptible d'être implantée avec ou sans ciment, en étant apte a remplir d'une manière satisfaisante le site prothétique, notamment au niveau de la niétaphyse.  One of the problems to be solved by the invention is to provide a prosthesis that can be implanted with or without cement, being able to satisfactorily fill the prosthetic site, particularly at the level of the niethaphysis.

Pour résoudre ce probleme, il a été réalisé des implants dits personnalisés, en utilisant une conception et fabrication assitées par ordinateur. Une telle méthode nécessite une infrastructure adaptée et présente certains inconvénients :
- cathode de mesure du site prothétique aléatoire par la radiographie conventionnelle : rigueur de la méthode qui la rend difficile å appliquer ; impossibilité de passer d'un intervenant humain au stade de l'interprétation des données radiologiques et de leur transmission å la machine
- méthode de mesure du site prothétique par la toniodensitoniétrie trop onéreuse
- délai entre la mesure et l'obtention de la prothèse assez long, ce qui rend impossible l'utilisation systématique de ce type de prothèse.
To solve this problem, so-called customized implants were made using computer-assisted design and manufacturing. Such a method requires a suitable infrastructure and has certain disadvantages:
- measuring cathode of the random prosthetic site by conventional radiography: rigor of the method which makes it difficult to apply; impossibility of switching from a human worker to the interpretation of radiological data and their transmission to the machine
- method of measurement of the prosthetic site by the too expensive toniodensitoniétrie
