Abstract
Description
dispositif"
La présente invention se rapporte à un dispositif pour l'occlusion et l'exposition de vaisseaux lors des anastomoses chirurgicales."Device for occlusion and exposure of vessels
during surgical anastomoses and method
occluding and exposing a vessel using this
device"
The present invention relates to a device for occluding and exposing vessels during surgical anastomoses.
One of the difficulties of vascular surgery is to see clearly the incision which has been made on a vessel when it is desired to make an anastomosis of another vessel or of a tube thereon.
Indeed, when the vessel has been incised and when its walls are soft, it is often collapsed; the precise vision of the different layers is fundamental; it is sometimes difficult, especially if the surgeon is bothered by an oozing of blood.
To overcome these drawbacks, practitioners are used to using clamps which are a kind of forceps used to seal the vessels to prevent blood from returning to the anastomosis area.
These clamping methods are not able to give satisfaction since they risk traumatizing the vessels at the place where their wall is already weakened, and cause them to flatten, thereby creating conditions which are not very favorable for sewing. In addition, such methods are not always usable on small vessels.
The object of the present invention is to remedy these drawbacks by proposing a device for occluding and exposing vessels during surgical anastomoses which is not aggressive for the walls of the vessels and which, in parallel, allows hemostasis while creating conditions likely to assist the surgeon in his subsequent sewing operation. This process allows sewing of undissected vessels.
To this end, the device according to the invention is characterized in that it is constituted by a suture thread crimped at its two ends and carrying, in its central part, two balls whose dimensions correspond to those of the # aisseau on which one wants to do the anastomosis.
The term ball must, of course, be considered in a broad sense and can just as easily apply to spheres as to non-angular volumes of other shape: olive or ellipsoid for example.
The distance between the balls and their dimensions can also vary to a large extent depending on the "work" to be done, all the more since this device can be adapted to both the vascular and coronary.
The balls can be made of plastic, or any other material provided that it is biologically compatible or inert (metal, alloys).
The thread is generally constituted by a single-strand suture, such as for example 6-0 single-strand.
The mode of connection of the wire and the balls can also be arbitrary without departing from the scope of the invention, and, for example being constituted by simple knots or any other method allowing immobilization.
According to another characteristic of the invention, the device comprises at least one derivation constituted by a second suture thread similar to the first to which it is fixed in its central part, between the two balls, crimped at its end, and carrying a third ball analogous or not to the first two.
This characteristic, which can give rise to very sophisticated configurations, is used to make it possible to close several vessels at the same time or a vessel and its collaterals.
The invention also relates to a method for occluding and exposing one or more vessels.
This process is characterized in that an incision is made on the vessel (s) at the place where one wishes to perform the anastomosis, the balls of a device as described above are introduced into this incision, the walls of the vessel (s) are pricked by their internal part beyond the incision using the crimped ends of the suture (s) and one pulls on these ends so as to put the balls in place inside of the vessel (s).
The establishment of the balls inside the vessels is, in fact, relatively easy and we therefore arrive, in accordance with the invention, to make the hemostasis while opening the vessel and giving it a favorable cylindrical shape for the sewing; moreover, the process according to the invention allows, thanks to the presence of the balls inside the vessel, to orient it in the most convenient way for the practitioner who can, thus, carry out his anastomosis in the as adequate and as quick as possible, avoiding any trauma to the vessels by any clamp or traction.
The characteristics of the device as well as of the method which are the subject of the invention will be described in more detail with reference to the accompanying drawings in which: - Figure 1 shows a device with two balls
according to the invention, - Figure 2 shows a device with three balls, - Figure 3 shows, on an enlarged scale, the implementation
place of the device shown in Figure 1 to
inside a ship.
According to FIG. 1, the device for occluding and exposing vessels during surgical anastomosis consists of a suture wire 1 which is crimped at its two ends 2, 2 ′ and carries, in its central part, two balls 3, 3 'whose dimensions correspond to those of the vessel on which we want to make the anastomosis.
The balls 3, 3 ′ which can be spherical but also in the form of an olive or an elipsoid are made of any biologically compatible material and are connected to the wire 1 by means of nodes 4.
The thread 1 is most often made up of a single-strand suture, in particular a 6-0 thread. The distance d between the balls 3 and 3 'depends on the operation that one wishes to carry out.
According to Figure 2, the wire 1 carries, in its central part 10 located between the two balls 3, 3 ', a second wire 1' forming a bypass; this wire 1 'is also provided with a ball 3 "similar to the balls 3 and 3', fixed by means of knots 4 and, is, moreover, crimped at its end 2". We thus obtain a triangular configuration making it possible to close both a vessel and a collateral vessel. We could provide more complicated geometries carrying several branches without departing from the scope of the invention.
In all cases, the diameter of the balls 3, 3 ', 3 "is, like the distance d between them, a function of the work to be carried out; in ceremonial surgery, the sizes of the most commonly used balls are 1 mm, 1.5 mm and 2 mm; these dimensions increase notably in the field of surgical surgery.
Selon la figure 3, le dispositif susmentionné est destiné à être utilisé lorsque l'on veut réaliser une anastomose d'un vaisseau ou d'un tube B représenté schématiquement sur un vaisseau A. According to FIG. 3, the above-mentioned device is intended to be used when it is desired to perform an anastomosis of a vessel or of a tube B shown diagrammatically on a vessel A.
For this purpose, one begins to make on the vessel A an incision 5 at the place where one wishes to make the anastomosis then, one introduces by this one according to arrow I the device constituted by the suture thread 1 carrying the two balls 3 and 3 'before stitching the walls of the vessel A at points 6 and 6' beyond the incision 5 by the crimped ends 2 and 2 'of the wire 1. It then suffices to pull according to the arrows Il on these ends to arrive to correctly place the balls 3 and 3 'in the vessel A so as to make the hemostasis, that is to say to prevent the blood from returning to the area of the anastomosis. The blood represented in hatched lines in FIG. 3 thus remains behind the balls 3 and 3 ′, and the surgeon is in optimal conditions for sewing, especially since the presence of the balls 3 and 3 ′ allows to open the vessel A to give it a cylindrical shape and to orient it in the most convenient way for the operator.
