Description

  • das Zentriereinrichtung für freie Ende eines Verankerungshaftes einer Gelenkendoprothese Die Erfindung betrifft eine Zentriereinrichtung für das freie Ende eines Verankerungsschaftes einer Gelenkendoprothese, insbesondere einer Hüftgelenkprothese, die mit Hilfe von Knochenzement im Knochen verankert wird, wobei für den Abschluss des zementgefüllten Knochenhohlraumes gegen den weiteren Markraum eine Markraum-Zementsperre vorgesehen ist.
  • Es ist eine bekannte Technik, im Markraum eines Röhrenknochens den mit Knochenzement gefüllten, künstlich geschaffenen Operationshohlraum, der der Aufnahme des Verankerungsschaftes einer Fndoprothese dient, gegen den daran anschliessendn Tcil des Markraumes mit Hilfe einer Markraum-Zementsperre abzuschliessen (CH-PS 625 119).
  • Besonders bei relativ weiten Markräumen haben sich bei dieser Art der Verankerung von Prothesen Schwierigkeiten ergeben, im Knochen die richtige Lage des Schaftes während der Operation zu finden und damit die korrekte Endstellung der Prothese zu gewährleisten. Fcl!lstellungen des Schaftes führen aufgrund falsch und unnatürlich wirkender Belastungsmomente zu örtlichen Belastungsspitzen des Knochens, beispielsweise durch Anliegen des Schaftes arm kortikalen Knochengewebe; diese Belastungsspitzen haben Knochenschäden unc eine Knochenabbau, sowie überdurchschnittliche Be- lastungsspitzen am Prothesenschaft zur Folge.
  • Aufgabe der Erfindung ist es daher, die Gefahr derartiger Fehlstellungen möglichst zu beseitigen, zumindest aber zu verringern.
  • Diese Aufgabe wird gemäss der Erfindung durch einen becherartigen Behälter gelöst, dessen Rand aufspreizbare und für ein Verhaken im Knochengewebe nach aussen abcjewinkelt ist.
  • Beim Einführen der Prothese wird das Schaftende in den Becher des Behälters eingelagert, dessen aufspreizbarer Rand sich an der aus kortikalen Knochengewebe gebildeten Wand des Operationshohlraumes abstützt und damit eine Zentrierung des Bechers bewirkt. Damit wird gleichzeitig eine Zentrierung des Schaftendes im Operationshohlraum erreicht, durch die vermieden wird, dass der Schaft auch nur entlang eines Teils seiner Länge an der Wand der lateralen Aussenseite des Operationshohlraumes direkt anliegt.
  • Die Fixierung des becherartigen Behälters kann in Verbindung mit der Markraum-Zementsperre in einfacher Wei@ auf zweierlei Art erfolgen. Entweder weist der Beche@bo@ dabei einen Haltestift zum Einstecken in die Markraum-Sperre auf, oder diese ist direkt an den t3odcn des Bechers angesetzt.
  • Wie schon die bekannte Markraum-Sperre besteht die Zentriereinrichtung mit Vorteil aus einem in der Implantattechnik üblichen Kunststoff, insbesondere aus Polyäthylen.
  • Im folgenden wird die Erfindung anhand von Ausführungsbeispielen im Zusammenhang mit der Zeichnung näher erläutert.
  • Fig. 1 zeigt ein Ausführungsbeispiel der Zentriereinrichtung vergrössert; Fig. 2 ist ein axialer Längsschnitt durch das obere Endc eines Femurknochens, in den der Schaft einer Hüftgelenksprothese eingesetzt und mit Hilfe der Zentriereinrichtung nach Fig. 1 zentriert ist; Fig. 3 gibt in gleicher Darstellung wie Fig. 2 eine zweite Ausführungsform der Erfindung wieder.
  • Der in seiner Grundform kreiszylindrische, becherartige Behälter 1 (Fig. 1), der zweckmässigerweise aus Kunststoff, insbesondere aus Polyäthylen, besteht, besitzt an seinem Boden einen Haltestift 2; dieser dient dazu, den Behälter 1 in eine zentrale Bohrung einer bekannten Markraum-Zementsperre 3 (Fig. 2) zu stecken.
  • Selbstverständlich ist es auch möglich, statt der Steckverbindung zwischen der Markraum-Sperre 3 und dem Stift 2 eine Schraub- oder Klebeverbindung vorzusehen. Weiterhin ist cs möglich, die Markraum-Sperre 3 und die Zentriereinrichtungals gemeinsames Element mit einem Becher 10 und einer an dessen Boden angesetzten Zementsperre 1 aus einem Stück zu fertigen; eine derartige Ausführungsform zeige Fig. 3.
  • Der freie Rand4 des Behälters 1 ist nach aussen abgewkelt und läuft in einer Spitze aus, mit der der Rand 4 sich in der kortikalen Wand 5 (Fig. 2) des operativ vorbereiteten Markhohlraumes 6 verhaken kann.
  • Auf seniem ganzen Umfang ist der Rand 4 mit parallel zur Mittelachse verlaufenden Einschnitten 7 versehen, die etwa bis zur halten Becherhöhe verlaufen. Sie dienen dazu, den Rand 4 des Behälters 1 aufspreizbar zu machen.
  • Beim Einsatz der neuen Zentriereinrichtung geht man zweckmässigerweise so vor, dass zunächst der Markhohlraum 6 bis zur gewünschten Tiefe operativ ausgeräumt und vorbereitet wird; anschliessend fixiert man in bekannter Weise die Markraum-Zementsperre 3 bzw. das Element lO, 11 aul der vorgesehenen "Niveau"-ilöhc und verbindet - bei der ersten Ausführungsform - die Markraum-Sperre mit dem becherartigen Behälter 1. Nun wird der Markhohlraum oberhalb des Behälters 1 bzw. 10 mit Knochenzement gefüllt; in diesen wird vor seiner Aushärtung der Schaft 9 der zu verankernden Prothese hineingedrückt, bis das freie Schaftende auf dem Boden des Behälters 1 aufliegt.
  • Die Fixierung der Zentriereinrichtung in der "richtigen" Höhe erfolgt dabei nicht nur durch Verhaken der Lappen 12 der Zementsperre 3 bzw. 10 in der kortikalen Knochenwand 5 des Markhohlraumes 6, sondern auch - insbesondere nach Einpressen des Schaftes 9 in den knochenzementgefüllten Markhohlraum 6 - duch Einrasten des aufspreizbaren Randes 4 des Behälters 1 bzw. 10 in dieser Wand 5. wie Fig. 2 und erkennen lassen, ist das freie Ende des Schaftes 9 duch die Zentriereinrichtung derart im Markhohlraum 6 zentriert, dass der Schaft 9 auf seiner ganzen Länge von einem Zementbett 8 umgeben, frei im Markhohlraum 6 verläuft, ohne dass er direkt an dieser Wand 5 aus kortikalem Knochengewebe anliegt.

Claims (3)

  1. Patentansprüche 2 .7Zentriereinrichtung für das freie Ende eines Verankerungsschaftes einer Gelenkendoprothese, insbesondere einer Hüftgelenkprothese, die mit Ililfe von Knochenzement im Knochen verankert wird, wobei für den Abscilluss des zementgefüllten Knochenhohlraumes gegen dcn weiteren Markraum eine Markraum-Zementsperre vorgesehen ist, gekennzeichent durch einen becherartigen Behälter (1, 10), dessen Rand (4) aufspreizbar und für ein Verhaken im Knochengewebe (5) nuch aussen abgewinkelt ist.
  2. 2. Zentriereinrichtung nach Anspruch 1, dadurch gekennzeichnet, dass am Becherboden ein Haltestift (2) zum Einstecken in die Markraum-Zementsperre (3) vorgesehen ist.
  3. 3. Zentriereinrichtung nach Anspruch 1, dadurch gekennzeichnet, dass am Boden des Bechers (10) eine Markraum-Zementsperre (11) angesetzt ist.
