Abstract
Description
Apparative Erweiteruriy eines Gergte-Systems zur DurchführungApparative expansion of a device system for implementation
The invention relates to an expansion of a device system, which is in connection with the well-known high-frequency surgery devices and suitable modified endoscopic coagulation probes for targeted and tissue-sparing electro-coagulative treatment of abnormal tissue changes.
The possibility of e.g. mucosal lesions with high-frequency electrical Thermally sclerosing currents for the purpose of hemostasis or tissue regeneration, is a world-wide procedure in medicine. Also in operative endoscopy high-frequency surgical devices are used in conjunction with suitable button electrode probes used sporadically for a few years. The further spread of these coagulation systems So far, the following disadvantages in particular stand in the way: 1. Gluing the coagulation probe on the coagulated tissue with the risk of tearing off the coagulum and the possible Initiation of new bleeding.
In almost all cases, bleeding is the actual source of bleeding not visible through covering blood and the coagulation electrode cannot can be applied in a targeted manner.
In the course of coagulation, the tissue was heated to relatively high temperatures heated, dried out and usually also carbonized, which increases the electrical properties were changed in such a way that a targeted dosage of the electrical to be applied High frequency energy became almost impossible. This resulted in many cases a large spatial expansion of the tissues # ek # rose, which is particularly evident in the case of mucosal lesions involved a relatively high risk of primary or secondary perforation.
The invention is based on the object through a targeted apparatus Extension of conventional endoscopic electrocoagulation systems with a device system to realize, which while avoiding the disadvantages of conventional systems mentioned a targeted and tissue-sparing high-frequency coagulation spatially and thermally to a limited extent in operative endoscopy.
This object is achieved according to the invention in that conventional endoscopic high-frequency surgical device systems expanded by the following components will: 1. A coagulation probe, which is connected to its electrical Tissue contact surface has a system of holes through which before, during and, after electrocoagulation, a suitable liquid, e.g. distilled and sterile water, flows out. The liquid flow prevents the excessive heating, especially the carbonization of the tissue, i.e. the temperatures remain practically below the respective boiling point of the liquid. For bleeding lesions the effective electrical contact area of the high-frequency probe is based on its geometric Expansion is limited because that which emerges from the source of bleeding is electrically relative Well conductive blood is thinned or flushed aside. This will also be optimal Visual conditions created, which the targeted and spatially selective coagulative Allow treatment of source of bleeding. The simultaneous presence of mefirerer The aim of drilling in the probe contact surface is that also during the pressing of the electrode tip during coagulation, the liquid flow is not complete is interrupted. In practice, this has resulted in an even distribution of several Drilling through the electrode contact surface has proven to be sufficient.
A mechanism integrated into the electrode tip for automatic Limitation of the contact pressure of the probe on the tissue surface. Here the Probe tip designed as a metallic sleeve, which is against a suitable Spring over a metal pierced for the purpose of supplying liquid Can move the cylinder. Limiting the contact pressure proves to be advantageous, because experiments have shown that the risk of perforation is too strong Probe pressure and the associated reduction in the thickness of the mucous membrane is enlarged.
A pumping device for accurate pressure and flow rate dosable liquid supply into the probe tip, which in a known manner via a PTFE hose, which also contains the electrical supply lines, with the pump connected is. With the help of a peristaltic pump, the sterility of the fluid supply can be made from the reservoir to the probe tip.
The advantages achieved by the invention are that 1. bleeding Lesions are flushed free of interfering blood before and during treatment, 2. the tissue temperature during coagulation is kept suitably low, 3. adhesion of the electrode to the coagulated tissue is prevented, 4. spatial The extent of the necrosis is limited, 5. the contact pressure of the probe tip is small We kept # d and 6. the risk of perforation can be largely avoided.
An extension of the device system of the type described was made by us for the liquid-assisted electrocoagulative endoscopic breastfeeding of gastrointestinal bleeding compiled and already successfully used clinically. According to the invention, the system consists of a controllable pressure and flow rate Peristaltic pump unit, which is distilled water from a sterile reservoir via a sterilized PTFE (Teflon) tube into a tube as described above executed metallic probe with pressure limitation is pumped. This probe with an outer diameter of 2.4 mm can be attached to the working channel of commercially available endoscopes to be introduced. The system is controlled by a two-stage foot switch, i.e. in the first switching stage the water is supplied while in the second switching stage In addition, the high-frequency current via the also routed in the Teflon hose electrical leads is applied.
