Abstract

Bei einem Hüftgelenkschaft für eine zementfreie Hüftgelenkprothese mit einem länglichen, in den Knochenmarkraum einzuschlagenden Grundkörper und einem am proximalen Ende seitlich wegstehenden Ansatz mit einem Konus zum Aufsetzen einer Gelenkkugel, ist vorgesehen, daß der Querschnitt des Schafts im proximalen Bereich eine Mehrzahl von parabelförmigen, in Längsrichtung verlaufender Nuten (10) aufweist, welche zwischen sich Stege (11) ausbilden, die sich in die Spongiosa einschneiden, wobei die Hüllkurve (12) der Stege ein asymmetrisch nach außen aufgebauchtes Rechteck umschreibt, während die Hüllkurve (12) der Böden der parabelförmigen Nuten (10) annähernd ein Rechteck ausbildet.

Description

Die Erfindung richtet sich auf einen Hüftgelenkschaft für eine zementfreie Hüftgelenkprothese mit einem länglichen, in den Knochenmarkraum einzu­ schlagenden Grundkörper und einem am proximalen Ende seitlich wegste­ henden Ansatz mit einem Konus zum Aufsetzen einer Gelenkkugel. Ein derartiger Schaft ist beispielsweise aus der DE 197 46 984 A1 bekannt.
Dabei wird grundsätzlich angestrebt, die Querschnittgeometrie eines sol­ chen Schaftes und die Oberflächenstruktur durch das Vorsehen von Stegen so auszubilden, daß eine möglichst gute Rotationsstabilität und ein primär fester Sitz erreicht werden.
Der Erfindung liegt die Aufgabe zugrunde, durch eine neuartige Formge­ bung Primärstabilität und Rotationssicherheit noch zu erhöhen.
Diese Aufgabe wird erfindungsgemäß dadurch gelöst, daß der Querschnitt des Schafts im proximalen Bereich eine Mehrzahl von querschnittlich pa­ rabelförmigen, in Längsrichtung verlaufende Nuten aufweist, welche zwi­ schen sich Stege ausbilden, die sich in die Spongiosa einschneiden, wobei die Hüllkurve der Stege ein asymmetrisch nach außen aufgebauchtes Rechteck umschreibt, während die Hüllkurve der Böden der parabelförmi­ gen Nuten annähernd ein Rechteck ausbildet.
Mit Vorteil kann weiterhin vorgesehen sein, daß die Stege zum distalen Ende hin auslaufen.
Günstigerweise weist der Hüftgelenkschaft im Bereich des distalen Endes einen rechteckigen Querschnitt auf.
An der proximalen Stirnseite kann eine kugelförmige Einsenkung zum An­ setzen eines Einschlaginstruments ausgebildet sein.
Eine besonders vorteilhafte Legierung aufgrund ihrer Körperverträglichkeit für den Hüftgelenkschaft ist die verwendete Titan-Aluminium6-Niob7- Schmiedelegierung.
Um ein optimales Einwachsen zu gewährleisten, kann vorgesehen sein, daß der Schaft eine Oberflächenrauhigkeit von Rz 40 bis 60 µm aufweist.
Zusammenfassend läßt sich feststellen, daß durch einen Hüftgelenkschaft der in Betracht stehenden Art eine zementfreie Verankerung mit hoher Primärstabilität und Rotationssicherheit bei proximaler Verankerung er­ reicht werden kann.
Der Querschnitt des konischen Schaftes vermeidet das Füllen des diaphysä­ ren Knochenabschnittes und somit den diaphysären Kontakt mit hoher Vor­ spannung, wodurch eine Verlagerung der Krafteinteilung nach proximal erfolgt. Durch diese proximale Krafteinteilung in die metaphysäre Spongio­ sa und Kortikalis wird eine proximale Knochenatrophie verhindert.
Die Längsstege im anterioren und posterioren proximalen Schaftbereich verleihen dem Schaft eine bauchige Außengeometrie.
Die Stege erhalten durch ihre parabelförmigen Zwischenräume die Spon­ giosa im proximalen Femur, was eine gute Versorgung der tragenden Kno­ chenstrukturen gewährleistet. Beim Einschlagen des Implantats schneiden sich diese Längsstege einem Spongiosagewinde ähnlich in die proximale metaphysäre Spongiosa ein, verdichten diese und gewährleisten eine ma­ ximale Rotationsstabilität. Ferner erlaubt diese Geometrie dem Operateur den Rückgriff von einem größeren auf den nächstkleineren Prothesenschaft, ohne daß die großflächige Verankerung in der proximalen Metaphyse ver­ mindert wird.
Nachfolgend wird die Erfindung anhand eines bevorzugten Ausführungs­ beispiels in Verbindung mit der Zeichnung näher erläutert. Dabei zeigen:
Fig. 1 eine Seitenansicht eines erfindungsgemäßen Hüftgelenkschaftes,
Fig. 2 eine um 90° gegenüber Fig. 1 gedrehte Ansicht,
Fig. 3 einen Schnitt längs der Schnittlinie III-III in Fig. 1
Fig. 4 einen Schnitt längs der Schnittlinie IV-IV in Fig. 1, und
Fig. 5 einen Schnitt längs der Linie V-V in Fig. 1.
Ein in der Zeichnung dargestellter Hüftgelenkschaft 1 weist eine konische Grundform auf, d. h. er verjüngt sich vom proximalen Ende 2 hin zum distalen Ende 3 konisch.
Am proximalen Ende 2 ist ein sich schräg zur Seite wegerstreckender An­ satz 4 mit einem genormten Konusabschnitt 5 zum Aufsetzen einer in der Zeichnung nicht dargestellten Gelenkkugel vorgesehen.
Die proximale Stirnseite 6 weist eine kugelförmige Einsenkung 7 zum An­ setzen eines Einschlaginstruments auf.
Im proximalen Ende 2 sind in die Seitenflächen 8 und in die dem Ansatz 4 abgewandte Stirnseite 9 parabelförmige Nuten 10 eingebracht, welche zwi­ schen sich zur Außenseite hin spitz zulaufende Stege 11 ausbilden. Die Hüllkurve 12 der Stegaußenkanten bzw. des Grundkörpers 13 weist die Form eines asymmetrisch nach außen aufgebauchten Rechtecks auf, wäh­ rend die Hüllkurve der Böden der Nuten 10 im wesentlichen ein Rechteck bilden.
Wie eine Gegenüberstellung der Fig. 3 bis 5 zeigt, laufen die Stege 11 zum distalen Ende 3 hin aus und der Querschnitt nähert sich immer stärker einer Rechteckform.

Claims (6)

1. Hüftgelenkschaft für eine zementfreie Hüftgelenkprothese mit einem länglichen, in den Knochenmarkraum einzuschlagenden Grundkörper und einem am proximalen Ende seitlich wegstehenden Ansatz mit einem Konus zum Aufsetzen einer Gelenkkugel, dadurch gekennzeichnet, daß der Querschnitt des Schafts im proximalen Ende (2) eine Mehrzahl von para­ belförmigen, in Längsrichtung verlaufender Nuten (10) aufweist, welche zwischen sich Stege (11) ausbilden, die sich in die Spongiosa einschneiden, wobei die Hüllkurve (12) der Stege (11) ein asymmetrisch nach außen auf­ gebauchtes Rechteck umschreibt, während die Hüllkurve (12) der Böden der parabelförmigen Nuten (10) annähernd ein Rechteck ausbildet.
2. Hüftlenkschaft nach Anspruch 1, dadurch gekennzeichnet, daß die Ste­ ge (11) zum distalen Ende (3) hin auslaufen.
3. Hüftgelenkschaft nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß er im Bereich des distalen Endes (3) einen rechteckigen Querschnitt aufweist.
4. Hüftgelenkschaft nach Anspruch 1, dadurch gekennzeichnet, daß an der proximalen Stirnseite (6) eine kugelförmige Einsenkung (7) zum Anset­ zen eines Einschlaginstruments ausgebildet ist.
5. Hüftgelenkschaft nach Anspruch 1, dadurch gekennzeichnet, daß er aus einer für medizinische Zwecke geeigneten Titanschmiedelegierung besteht.
6. Hüftgelenkschaft nach Anspruch 1, dadurch gekennzeichnet, daß er eine Oberflächenrauhigkeit von Rz 40 bis 60 µm aufweist.
