Abstract
Description
Unter dem Aspekt der steigenden Lebenserwartung der Bevölkerung nimmt insbesondere die Zahl von degenerativen Erkrankungen der Wirbelsäule zu. Im Vordergrund stehen hierbei Abnutzungserscheinungen der Bandscheiben mit erheblichen Therapiekosten. Neben der operativen Stabilisierung nach vorheriger Bandscheibenentfernung und Ersatz durch einen rigiden knöchernen und/oder Metall-Spacer (knöcherne Fusion) wurden in der zurückliegenden Zeit verschiedene Techniken zum Bandscheibenersatz vorgestellt. Der Nachteil der aktuell verfügbaren Bandscheibenprothesen bzw. Kunststoffkissen/Kunststoffspacer ergibt sich aus dem komplizierten Aufbau und der unzureichenden Fixierbarkeit im Bandscheibenzwischenraum nach Entfernung der krankhaft veränderten Bandscheibe. In der Grenzschicht zum Knochen kann es auf Grund der dynamischen Fixierung des Implantates mit permanenten Druck und Scherkräften zu keiner Osteo- Integration kommen, so dass häufig Dislokationen mit Funktionsverlust der Prothesen auftreten. Andererseits erfordert die Implantationstechnik einiger Zwischenwirbelscheibenprothesen die Eröffnung des subkortikalen Knochens, so dass es zu einer Freisetzung osteogener Faktoren und nachfolgend zur knöchernen Fusion des Bandscheibenfaches kommt. Konstruktiv bedingt sind die meisten Zwischenwirbelscheiben -Endoprothesen nur im frühen Stadium einer krankhaften Veränderungen an der Bandscheibe verwendbar, da mit Fortschreiten der Erkrankung die Begleitstrukturen (Wirbelgelenke, Bänder, Muskulatur) nachhaltig geschädigt werden. Bei ausreichender Stabilität kann auf eine zusätzliche Fixation im Grund- und Deckplattenbereich mit Eröffnung des spongiösen Knochens erzichtet werden. Durch spezielle mechanische Eigenschaften ist die anteilige Wiederherstellung des Banscheibenzwischenraumes mit konsekutiver Entlastung des Facettengelenke möglich.
