End effector, surgical instrument, purse-string forceps and purse-string nail
Technical Field
The utility model relates to the technical field of medical equipment, especially relate to an end effector, a surgical operation apparatus, a purse-string forceps and a purse-string nail.
Background
Gastrointestinal tract surgery usually requires cutting off a closed tissue lumen and performing purse string suture at the disconnected lumen, wherein the purse string suture is used for being matched with a nail anvil of a circular anastomat to perform subsequent anastomosis and other operations by using the circular anastomat.
Purse-string sutures are a common suturing technique in surgical operations, and two main ways of purse-string suturing are known. One way is to manually perform a purse string suture, as shown in fig. 1a and 1b, the physician first passes the purse string 58 through the tissue surface around the incision in the luminal tissue 59 with a purse string needle, and then tightens the purse string 58 to complete the purse string suture. The manual suture mode is complex to operate, the operation precision is difficult to grasp, and the suture is easy to fail due to thread off, so that the manual suture mode is rarely adopted in the surgical operation at present.
Another way is to use purse-string forceps to assist in making a purse-string suture. As shown in FIG. 2a, a purse-string forceps is also used to perform purse-string suture by threading a needle (i.e., a purse-string needle) with a string (i.e., a purse-string 58).
As shown in FIG. 2b, there is also a purse-string forceps that performs a purse-string suture by attaching a purse-string 58 to the surface of the lumen tissue 59 by means of a purse-string 57, typically by firing the purse-string 57 to pierce and secure it to the lumen tissue 59, and the purse-string 58 is either interconnected with the purse-string 57 or pressed between the lumen tissue 59 and the purse-string 57 by the purse-string 57 to attach the purse-string 58 to the lumen tissue 59 to perform the purse-string suture.
Generally, when a surgical operation is performed on a lumen tissue, besides a purse-string suture is required to be performed on an incision of a remaining end of the lumen tissue, a lesion end of the lumen tissue also needs to be cut and sutured. The existing purse-string forceps have single function, can only complete purse-string suture on the remaining end, and cannot cut and suture the pathological change end. Therefore at least also needs to be fitted
In the prior art shown in fig. 2b, the purse string 57 needs to be fixed on the surface of the lumen tissue 59 by a non-staple anvil forming technology, and the use of the purse-string forceps often causes the phenomenon that the purse string is torn off, thereby causing the purse-string suture to fail, which also increases the operation risk and the pain of the operation patient.
Moreover, endoscopic surgery has emerged in the eighties of the twentieth century, a technique that allows surgeons to perform surgery without causing significant trauma to the patient, primarily through endoscopy and various imaging techniques. Compared with open surgery, endoscopic surgery has the remarkable advantages of small tissue damage, quick postoperative recovery and the like, thereby becoming a development trend. In this case, purse-string forceps for endoscopic surgery appeared, and the existing purse-string forceps for endoscopic surgery also had the problem of single function as described above.

For example, as shown in fig. 4, 5a and 5b, the cutting mechanism may include a cutting blade 23, the cutting blade 23 is capable of moving between the suturing mechanism and the purse string suturing mechanism under the action of a push-type cutter mechanism, such as a push-type cutter rod or other component, to separate the sutured and purse string sutured tissues, respectively, and the cutting blade 23 and the push-type cutter mechanism may be integrated or separated (in this case, the cutting blade 23 is disposed at the far side of the push-type cutter mechanism). The first and second jawarms are provided with a knife slot 24 (shown in fig. 4) for accommodating the movement of the cutting knife 23, so as to provide guidance for the movement of the cutting knife 23, and make the cutting movement more stable and accurate.
A sewing mechanism:
the utility model discloses an end effector's sewing mechanism can adopt the current various technical scheme of the end effector's of cutting stitching instrument sewing mechanism to realize.
For example, as shown in fig. 5a and 5b, the stapling mechanism may be arranged on one side in the longitudinal direction of two jawarms, one arranged with a staple anvil 18 and the other arranged with a staple magazine 19, in which magazine 19 staples 20 are arranged, which staples 20 may be arranged in one or more rows in the longitudinal direction, which staples 20 can be fired by a staple pusher mechanism. The staple pusher mechanism for firing the staples 20 may comprise a staple pusher bar (in this embodiment, the knife bar of the cutting knife 23 acts as a staple pusher bar), a staple pusher shoe 21 and a staple pusher 22. The firing process may be that the staple pusher bar moves longitudinally distally to drive the staple pusher shoe 21, the inclined surface of the staple pusher shoe 21 acts on the staple pusher 22 to move it laterally, and then the staples 20 are pushed out of the staple magazine 19 by the staple pusher 22, pass through the lumen tissue 59, and press against the staple anvil 18, and the staples 20 are formed into a B-shape by the staple anvil 18, thereby completing the stapling.
The number of rows of staples 20 is not limited, but should be at least one row, and preferably at least two rows, so that at least two sutures can be placed at the diseased end of the luminal tissue, greatly reducing the risk of suture failure. When the staples 20 are arranged in a plurality of longitudinal rows, only one staple pusher shoe 21 may be provided to push the forward plurality of staple pushers 22.
When the cutter arbor of cutting knife 23 pushed out the nail pole as the staple, the position of cutting knife 23 can set up to pushing out the nail slider 21 for the staple and leaning on more (being close to near side more promptly) for sew up earlier the back cutting, can reduce pathological change end internal tissue outflow like this, improve the success rate of operation. In one embodiment, the staple pusher shoe 21 is pushed by the knife bar of the cutting knife 23, and before the cutting blade of the cutting knife 23 cuts the tissue, the inclined surface of the staple pusher shoe 21 already pushes the front staple pusher 22 to fire the staples 20, i.e., to suture the front lumen tissue, so that the cutting knife 23 always cuts after the tissue is sutured.
The purse-string suture mechanism comprises:
the purse string suture mechanism may be provided on the other side of the longitudinal direction of the two forceps arms, that is, on the side opposite to the suture mechanism. Preferably, the first pouch nail magazine arranged on the first forceps arm and the second pouch nail magazine arranged on the second forceps arm are opposite in position when the jaws are closed, so that a good pouch suture effect can be obtained.
As shown in fig. 6a and 6b, the pouch nail pushing mechanism includes: a pouch nail pushing rod, a pouch nail pushing slider 16 and a pouch nail pusher 17 which are arranged aiming at each pouch nail box; for each pouch nail box, when the pouch nail pushing rod drives the pouch nail pushing slider 16 longitudinally towards the far side, the inclined surface of the pouch nail pushing slider 16 drives the pouch nail pusher 17 transversely, so that the pouch nail pusher 17 pushes the pouch nails 57 out of the pouch nail box, and the pouch nails 57 can penetrate into the lumen tissue 59 and are connected with the lumen tissue 59.
The pouch nail pushing mechanism and the staple pushing mechanism can be two mechanisms which are independent from each other and driven respectively, at this time, the pouch nail pushing rod and the staple pushing rod are different parts respectively, the pouch nail pushing slider and the staple pushing slider are different parts respectively, in addition, the pouch nail pushing mechanism and the staple pushing mechanism can be driven synchronously, as shown in fig. 6b, the pouch nail pushing rod and the staple pushing rod are integrated with a cutter bar of the cutting knife (the pouch nail pushing rod, the staple pushing rod and the cutter bar can also be different parts connected with each other), the pouch nail pushing slider 16 and the staple pushing slider 21 are integrated (or different parts are connected with each other), for example, when the staple pushing slider is driven longitudinally, the inclined surface of the staple pushing slider can drive the pouch nail pusher 17 and the staple pusher 22 transversely, so that the pouch nails and the staples are fired respectively.
As shown in fig. 4, the positioning manner of the purse wire 58 on the surface of the nail box is not limited, and preferably, the surfaces of the first purse nail box 14 and the second purse nail box facing the jaws are respectively provided with a line card 15, and the purse wire 58 is clamped on the line card 15.
