Abstract
本发明为一种人体颈椎鞍状关节。由上、下两个半关节配合构成,每个半关节由一个半关节体和基座以燕尾槽结构形式嵌合组成,上、下两个半关节体之间为鞍状面配合接触;基座背面均设有若干钉刺 和/或珍珠面,和/或羟基磷灰石涂层,以及镂空插片,以便于将关节与颈椎骨固定结合。本关节可通过外科手术植入患者颈椎间盘病变部位。使用本鞍状面关节可适用于具有适应症的退行性病变所致的颈椎不稳症、颈椎间盘突出症和颈椎病等。
Description
技术领域
本发明属医疗器械技术领域,具体涉及一种人体颈椎鞍状关节。
背景技术
人体颈椎是脊柱中体积最小、但灵活性最大、活动频度最高之节段。由于不断地承受各种负荷、劳损,以及人均寿命的延长,因退行性变所致的颈椎不稳症、颈椎间盘突出症和颈椎病的发病率成为骨科门诊最常见的疾病,并且很多患者可能因病变侵犯脊髓神经组织而引起肢体功能障碍,甚至导致四肢瘫痪,给患者本人、亲属造成巨大痛苦。目前,临床上治疗上述疾病多采用颈椎间隙减压融合术,即植骨钢板内固定术或界面内植物植骨融合术。上述疗法在治疗的同时,破坏了颈椎间盘的生理活动功能(特别是多节段颈椎病),使融合上节段和下节段承受的力增加而诱发上、下椎间隙的退行性变。
发明人曾设计一种人颈椎关节,它由上、下两个半关节配合构成,两个半关节之间为鞍状面接触。可通过外科手术将关节植入患者颈椎间盘病变部位。鞍状面相对合。使用该颈椎关节,可对颈椎间隙减压,并可恢复该间隙的生理活动功能和生理高度。但是,该关节的基座为齿形,与上、下颈椎骨结合的紧密度尚显不足,影响相互之间的定位固定,从而影响患者恢复生理功能的速度。
发明内容
本发明的目的在于提出一种改进的人颈椎鞍状关节,以增加关节与上、下颈椎骨的紧密结合度,提高患者恢复生理功能的速度。
本发明设计的颈椎关节,由上、下两个半关节配合构成。其中,上、下两个半关节均由一个半关节体和一个基座嵌合组成,半关节体与基座之间通常可以燕尾槽结构形式嵌合,而上、下两个半关节体之间为鞍状面配合接触;基座的背面均设有若干钉刺和/或者珍珠面,和/或羟基磷灰石涂层,以便于将关节与颈椎骨固定结合。此外,在基座背面还增设有中间镂空的插片,以增加关节与颈椎骨结合的紧密度。其结构如图1-图2所示。
本发明中,上、下两块基座的燕尾槽嵌合的一侧端部还分别设有一个90度左右的折边,以便将基座定位及固定。并且在该折边处,设有销钉孔,通过销钉进一步将关节体与基座固定。该销钉孔与基座折边垂直为宜。
本发明中,一对鞍状面关节体分别在矢状面和冠状面有不同半径的弧度,各弧度面配合接触,上下关节面相对应的弧度半径一致,其结构如图2和图3所示。上下关节面的背面分别设有燕尾形凸槽,该凸槽正好分别嵌合于两个基板正面的凹槽内。
本发明设计的颈椎鞍状关节可采用医用钛合金或医用不锈钢,或/和医用聚乙烯等材料制作。
按照人颈椎骨的生理活动特性,它可以作前后屈伸、左右侧弯、单侧旋转等动作。而且,不同的颈椎骨节段,其屈伸、侧弯、旋转的角度均有差别。通常情况下,有下表的参考数据:

说明:C表示颈椎骨,下标表示节段。屈伸、侧弯、旋转的单位为度。
根据颈椎间盘的病变位置及其活动范围,颈椎间盘假体可以设计成大小不同的多种规格尺寸,以满足不同部位的颈椎的屈伸、侧弯和旋转的要求。
本发明设计的颈椎鞍状关节,可通过外科手术将其植入患者颈椎间盘病变的部位,鞍状关节面相对合,两个基座将鞍状关节面上下两部分固定。基座通过刺钉固定于上下两个颈椎椎体节段上。通过销钉进一步将关节体与基座固定。使用本颈椎鞍状关节,可对颈椎间隙减压,并可恢复该间隙的生理活动功能和生理高度,本关节可适用于具有适应症的退行性病变所致的颈椎不稳症、颈椎间盘突出症和颈椎病等。
根据颈椎间盘关节活动的要求,本鞍状关节的活动角根据参考数确定。

具体实施方式
下面通过实施例进一步描述本发明,但不限于该实施例。
实施例,如附图所示,上关节的基板为长方形,其长度为13mm,宽度为12.30mm,厚度为2mm,燕尾形凹槽的底部宽度为7.75mm,凹槽口部6mm,深度1.25mm,挡块的高度为3.5mm,刺钉的高度为1mm,共数个布于基板的背面,刺钉的基座为正方形,边长1mm。上鞍状面关节背面的燕尾形凸槽的底部宽度为6mm,顶部7.75mm,深度1.25mm。上鞍状面关节背面宽度为11mm,长度为15mm,厚度为2mm,上鞍状面关节背面矢状面半径为7.44mm,冠状面半径为13.45mm。下关节的基板为长方形,其长度为14mm,宽度为15mm,厚度为2mm,燕尾形凹槽的底部宽度为7.75mm,凹槽口部6mm,深度1.25mm,挡块的高度为3.5mm,刺钉的高度为1mm,共数个布于基板的背面,刺钉的基座为正方形,边长1mm。下鞍状面关节背面的燕尾形凸槽的底部宽度为6mm,顶部7.75mm,深度1.25mm。下鞍状面关节背面宽度为15mm,长度为16mm,厚度为3mm,下鞍状面关节背面矢状面半径为7.44mm,冠状面半径为13.45mm。上述关节的基座背面中间部位分别设置一个镂空的插片,该插片与基座背面垂直,其长度为7mm,高度4mm,插片中间镂空为十字形。采用医用不锈钢材料,或医用钛合金材料,和/或医用聚乙烯材料,经过一定的工艺加工制作,即得一种型号的人颈椎鞍状关节。
Claims (3)
1、一种人颈椎关节,由上、下两个半关节配合构成,其特征在于上、下两个半关节均由一个半关节体和基座以燕尾槽结构形式嵌合,上、下两个半关节体之间为鞍状面配合接触;基座的背面均设有若干钉刺和/或珍珠面,和/或羟基磷灰石涂层,并设有镂空的插片,以便于将关节与颈椎骨固定结合。
2、根据权利要求1所述的人颈椎关节,其特征在于上、下两块基座的燕尾槽嵌合的一侧端部还分别设有一个90度左右的折边,该折边处设有与折边垂直的销钉孔,通过销钉以便将嵌合在基座正面的关节体定位及固定。
3、根据权利要求1所述的人颈椎关节,其特征在于上、下一对鞍状面关节体分别在矢状面和冠状面有不同半径的弧度,各弧度面配合接触,上下关节面相对应的弧度半径一致。
