Abstract
本发明涉及营养食品技术领域,具体涉及一种含中链甘油三酯高蛋白营养粉组合物及其应用。所述的组合物中含有两者以上混合保存或者各自独立保存的以下组分:碳水化合物、脂肪、蛋白质、膳食纤维、矿物质、维生素和A组分;各组分重量份为脂肪15~60份、蛋白质20~90份、膳食纤维10~30份、矿物质0.5~30份、维生素0.02~3份,A组分1~15份;所述A组分选自山药、茯苓、百合、银耳、玉竹、蒲公英、枸杞、红枣、蓝莓提取物中的任意一种或几种的组合物。本发明提供的营养粉组合物弥补了普通营养制剂中无药食同源物质和植物化学物的不足,多种有益成分与营养素一切协同对结核患者起到改善营养状况、缓解肠胃不适、提高免疫等作用,能对结核患者起到更加积极的营养支持和干预作用。

技术领域
本发明涉及营养食品技术领域,具体涉及一种含中链甘油三酯高蛋白营养组合物及其应用。
背景技术
结核病(TB)是一种传染性疾病,是全球主要死因之一,也是单一感染源的主要死因。营养不良在结核病患者中极为常见,特别是贫穷相关的营养不良是结核病发病的高危因素之一,患者的营养状态不仅与结核病的发病和进展有着直接的关系,对结核病治疗方案的选择和疗效也将产生直接影响。因此,对于结核患者应积极纠正其营养不良状态。
结核患者营养风险发生率均较高,一般在35%以上,最高可达86.1%,而且有营养风险患者的生理机能、生理职能、躯体疼痛、疲劳和一般健康状况要比无营养风险患者差。结核患者营养素缺乏主要表现在:易出现蛋白质和能量供应不足,蛋白质、氨基酸代谢异常,血脂异常,低胆固醇血症较为普遍,维生素A、维生素B6、维生素C普遍缺乏,50%以上的患者存在维生素D缺乏,血清锌、硒、铁浓度普遍偏低。吴晓光等人研究中对62例肺结核患者进行NRS2002评估,且随机分为肠内营养组和肠外营养组,结果显示两组病例治疗后理想体质量百分比、血清白蛋白、血红蛋白、淋巴细胞总数与治疗前相比差异有显著性;治疗后肠内营养组理想体质量百分比及T淋巴细胞较肠外营养组有明显改善,并且NRS2002评估结果显示,两组营养状况均有好转,肠内营养组患者营养状况改善明显好于肠外营养组。
可见,结核患者存在营养风险的比例较高,对于存在营养风险的结核患者应实施五阶梯营养治疗模式,其中肠内营养支持是临床上首选的营养支持方式和营养治疗手段。
其中,口服是最理想的肠内营养支持给予途径,这种方式最符合生理特点,患者容易接受,依从性好。并且,口服营养补充能明显改善患者的营养状态,如提高营养不良患者体内血红蛋白、转铁蛋白、白蛋白及前白蛋白水平等。然而,对于肺结核患者而言,脂肪的呼吸商相对较低,适当提升脂肪的供能比,可以保护肺功能,但是,脂肪增加又会增加肝脏和胃肠道负担,结核药物本身也对肝功能有一定的损伤。
因此,开发出一种既能够提高脂肪供能比,又不会增加肝脏和胃肠道负担的肺结核病人用营养产品,对于肺结核患者具有重要意义。
发明内容
本发明的目的在于提供一种既能够提高脂肪供能比,又不会增加肝脏和胃肠道负担,可以有效改善存在营养风险结核病患者营养状态的含中链甘油三酯的高蛋白营养产品,对于肺结核患者具有重要意义。
为实现上述目的,本发明采用的技术方案如下。
第一方面,本发明提供一种含中链甘油三酯高蛋白营养组合物。
所述的组合物中含有两者以上混合保存或者各自独立保存的以下组分:碳水化合物、脂肪、蛋白质、膳食纤维、矿物质、维生素和A组分;各组分重量份为脂肪15~60份、蛋白质20~90份、膳食纤维10~30份、矿物质0.5~30份、维生素0.02~3份,A组分1~15份;
所述的脂肪中含有中链甘油三酯;中链甘油三酯的含量占所述脂肪总重量的30~50wt%;
所述A组分选自山药、茯苓、百合、银耳、玉竹、蒲公英、枸杞、红枣、蓝莓提取物中的任意一种或几种的组合物,上述组分可为结核病患者提供药食同源成分或植物化学物,弥补了普通营养制剂中缺乏此类营养成分的不足,并可起到一定的补脾、润肺、提高免疫力等作用。
在一个具体的实施例中,所述脂肪中还含有n-3多不饱和脂肪酸、植物油脂、动物油脂中的至少一种。
在一个具体的实施例中,所述的植物油脂选自玉米油、菜籽油、大豆油、花生油、椰子油、茶油、红花油、橄榄油中的至少一种;所述动物油脂选自乳脂肪、鱼油微囊粉、亚麻籽油微囊粉中的至少一种。
亚麻籽油是含有多种不饱和脂肪酸的植物油脂,如α-亚麻酸,亚油酸,油酸等,其中α-亚麻酸含量最高可达53%,是其它植物油所无法匹及的。因为不饱和脂肪酸对氧的敏感性及自身特有的气味,让其在食品方面的应用受到了很大的限制,而亚麻籽油微囊粉可以有效阻止不饱和脂肪酸的氧化且能掩盖其不良气味。亚麻酸及其代谢产物DHA、DPA、EPA等有免疫调节、抗炎支持等作用。其中,n-3系列脂肪酸可抑制局部肺组织炎症介质的释放,降低炎症介质所导致的不良反应,例如肺动脉高压、肺泡膜的高通透性、肺水肿等,保护肺泡上皮细胞,避免肺部病变加重,改善患者的临床症状扩张血管、促进血液循环。
在一个具体的实施例中,所述的碳水化合物选自麦芽糊精、玉米粉、燕麦粉、小麦粉、经过酶解处理的大米粉中的至少一种。
在一个具体的实施例中,所述的蛋白质选自大豆蛋白、大豆分离蛋白、水解大豆蛋白肽、浓缩乳清蛋白、分离乳清蛋白、水解乳清蛋白肽中的至少一种。
在一个具体的实施例中,所述的膳食纤维选自低聚果糖、低聚异麦芽糖、菊粉、低聚木糖、抗性糊精中的至少一种。
在一个具体的实施例中,所述矿物质选自铁、钙、锌、硒、镁、钾、铜、锰、磷、碘中的至少一种;所述的维生素选自维生素A、维生素D、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、叶酸、泛酸、维生素C、生物素中的至少一种。
优选的一种含中链甘油三酯高蛋白营养组合物,所述的组合物中含有两者以上混合保存或者各自独立保存的以下组分:碳水化合物、脂肪、蛋白质、膳食纤维、矿物质、维生素和A组分;
各组分重量份为脂肪15~60份、蛋白质20~90份、膳食纤维10~30份、矿物质0.5~30份、维生素0.02~3份,A组分1~15份;

在所述辅料中,相对于100重量份的碳水化合物,所述脂肪的含量为15~60重量份,所述蛋白质的含量为20~90重量份,所述膳食纤维的含量为10~30重量份,所述矿物质的含量为0.5~30重量份,所述维生素的含量为0.02~3重量份,所述A组分的含量为1~15重量份;

中链甘油三酯(MCT)在机体内消化、吸收、转运和代谢不同于长链甘油三酯(LCT),其在体内消化代谢的速度与葡萄糖相当,且产生的能量高于葡萄糖产生的能量2倍。进入机体的MCT对胆盐和胰酶的依赖性很小,在肠道中被脂肪酶迅速水解成MCFA和甘油,MCFA不再合成甘油三酯,也不组成乳糜微粒而是经肠道上皮细胞吸收后与蛋白质结合,不需要通过淋巴系统,直接通过门静脉转运到肝脏,转运到辅酶肝细胞的MCFA不需要依赖肉碱转移酶作用,直接进入线粒体内进行β-氧化。MCFA产生的乙酰辅酶A可在线粒体内进入三羧酸循环彻底氧化成二氧化碳和水,或生成酮体及延长脂肪酸碳链,也可进入细胞液参与脂质从头合成。MCT快速消化吸收,氧化代谢的特点,使得产生大量的乙酰CoA,过多的乙酰CoA合成了酮体。
第二方面,本发明提供一种制备含中链甘油三酯高蛋白营养组合物的方法,该方法包括:将含中链甘油三酯高蛋白营养组合物中的各组分进行混合。
所述的组合物制备为粉、片、棒等口服剂。
第三方面,本发明提供一种含中链甘油三酯高蛋白营养组合物在对结核病患者的辅助治疗中的应用。
营养不良是影响结核病患者治疗效果的重要因素。结核患者营养风险发生率均较高,一般在35%以上,最高可达86.1%。据世界卫生组织(WHO)2021年全球结核病报告显示,2020年全球新发结核病患者987万,发病率为127/10万,其中估计有230万结核病病例可归因于营养不良。结核病与营养之间存在着复杂的内在联系,营养不良可增加患者治疗失败、死亡和获得性耐药等风险,尤其是耐药结核病(MDR-TB)的发展会使得结核病疫情的控制现状更加严峻,是导致结核病流行的众多因素之一。由于MDR-TB自身疾病的特点使得患者营养不良程度更为严重,营养不良又可能使病情进一步恶化导致治疗失败。
对于肺结核患者而言,脂肪的呼吸商相对较低,适当提升脂肪的供能比,可以保护肺功能,但是,脂肪增加又会增加肝脏和胃肠道负担,
本发明提供的一种含中链甘油三酯高蛋白营养组合物中,含有中链甘油三酯,既能够提高脂肪供能比,又不会增加肝脏和胃肠道负担;又含有蛋白质、维生素、及微量矿物元素等营养物质,营养均衡,能有效防治营养不良,能明显改善患者的营养状态,如提高营养不良患者体内血红蛋白、转铁蛋白、白蛋白及前白蛋白水平等,对于肺结核患者具有重要意义。
本发明提供的含中链甘油三酯高蛋白营养组合物中含有药食同源物质或植物提取物,多种有益成分与营养素一切协同对结核患者起到改善肠胃不适、提高免疫等作用,弥补了普通营养制剂中无药食同源物质和植物化学物的不足。
与现有技术相比,本发明的有益效果如下:
针对肺结核患者的特殊营养代谢特点和营养需求,有针对性的开发适用于结核患者的营养产品。为结核患者提供合理、适当的营养支持和干预。

本发明所述蛋白质选自大豆蛋白、大豆分离蛋白、水解大豆蛋白肽、浓缩乳清蛋白、分离乳清蛋白、水解乳清蛋白肽中的至少一种。
本发明所述膳食纤维选自低聚果糖、低聚异麦芽糖、菊粉、低聚木糖、抗性糊精中的至少一种。
本发明所述矿物质选自铁、钙、锌、硒、镁、钾、铜、锰、磷、碘中的至少一种。
根据一种特别优选的具体实施方式,所述维生素选自维生素A、维生素D、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、叶酸、泛酸、维生素C、生物素中的至少一种。
本发明制备含中链甘油三酯高蛋白营养粉的方法,该方法包括:将第一方面所述组合物中的各组分进行混合。
优选地,将所述组合物中的各组分进行混合的操作包括:
(1)将所述膳食纤维进行粉碎,以获得过20~60目筛的膳食纤维粉料;
(2)将所述矿物质、所述维生素、所述碳水化合物进行第一接触混合,得到第一混合物;
(3)将所述第一混合物、所述膳食纤维粉料、所述脂肪、所述蛋白质和所述A组分进行第二接触混合,得到第二混合物,并将所述第二混合物过20~60目筛。
优选地,所述第一接触混合的条件至少包括:搅拌速度为100~1500rpm,温度为20~30℃,时间为1~5min。
以下将通过实例对本发明进行详细描述。以下实例中的各种原料均为市售品。以下实例中,植物油脂为含量重量比为1:1:2的菜籽油、大豆油和椰子油的组合;1重量份表示1g。
实施例1
本实施例用于说明本发明所述含中链甘油三酯高蛋白营养粉按照表1中的配方和工艺参数,并按如下所述的方法制备含中链甘油三酯高蛋白营养粉。
制备含MCT高蛋白营养粉的方法包括以下步骤:
(1)将所述膳食纤维进行粉碎,以获得过20目筛的膳食纤维粉料;
(2)将所述矿物质、所述维生素、所述碳水化合物进行第一接触混合,得到第一混合物;其中,第一接触混合的条件为:搅拌速度为500rpm,温度为室温,时间为3min;
(3)将所述第一混合物、所述膳食纤维粉料、所述脂肪、所述蛋白质和所述A组分进行第二接触混合,得到第二混合物,并将所述第二混合物过20目旋转振动筛;其中,第二接触混合的条件为:搅拌速度为1500rpm,温度为室温,时间为3min。
在没有特别说明的情况下,其余实例采用与实施例1相同的流程进行,不同的是所采用的含MCT高蛋白营养粉中各组分的投入量不同,具体参见表1中。

实施例2
理化性质测试
对实施例1营养粉1制备得到的高蛋白营养粉的蛋白质含量、脂肪含量、膳食纤维、水分、灰分和微生物指标进行测试,具体测试结果见表2。
其中,采用国家标准GB 5009.5对制得的营养粉中蛋白质含量进行测试,采用国家标准GB 5009.6对制得的营养粉中脂肪含量进行测试,采用国家标准GB 5009.3对制得的营养粉中水分进行测试,采用国家标准GB 5009.4对制得的营养粉中灰分进行测试。采用国家标准GB 5009.88或规定的检测方法对制得的营养粉中膳食纤维进行测试。根据测得的蛋白质、脂肪、膳食纤维、水分、灰分计算制得的营养粉中碳水化合物、能量。采用国家标准GB29922中规定的方法对制得的营养粉中微生物指标进行测试,具体测试结果见表2。

测试结果表明,前述实施例制备得到的高蛋白营养粉的理化指标、营养成分指标和微生物指标均符合国家标准。
测试例3:人体试验测试
191例骨结核患者分试验1组、试验2组和对照组,对照组:给予常规治疗和手术治疗,包括抗感染、抗结核、手术等临床治疗,饮食选择食堂提供的营养餐。试验1组:常规饮食及手术、治疗的基础上,术后给予实施例1中的营养粉,每日3次,每次50g,干预2周。试验2组:常规饮食及手术、治疗的基础上,术后给予实施例2中的营养粉,比较三组试验起点和终点的营养状况:

从表3中可以看出,本发明提供的高蛋白营养粉实施例1能够使结核患者术后白蛋白水平升高更快、CRP水平降低更快、且术后2周时血红蛋白可略有升高,其他两组血红蛋白则呈现下降趋势,本营养粉实施例1可有效改善结核患者术后的营养状况和炎症反应。
测试例4:人体试验测试
在三家医院纳入76例确诊为肺结核且存在营养风险的患者,随机分为试验组和对照组,排除退出试验及数据不全的患者,最终试验组34例,对照组27例。对照组:常规治疗基础上给予安素1日3次,每次50g。试验组:常规治疗基础上给予实施例1中的营养粉,每日3次,每次50g,干预2周。比较两组试验起点、干预1周及终点的营养状况:

从表4中可以看出,本发明提供的高蛋白营养粉实施例1能够使肺结核患者血红蛋白略有升高,而对照组的血红蛋白在持续下降,CRP水平降低比对照组多。与人体测试试验1结果相同,本营养粉可改善肺结核患者的营养状况和炎症反应。
以上详细描述了本发明的优选实施方式,但是,本发明并不限于此。在本发明的技术构思范围内,可以对本发明的技术方案进行多种简单变型,包括各个技术特征以任何其它的合适方式进行组合,这些简单变型和组合同样应当视为本发明所公开的内容,均属于本发明的保护范围。
Claims (10)
1.一种含中链甘油三酯高蛋白营养组合物,其特征在于,所述的组合物中含有两者以上混合保存或者各自独立保存的以下组分:碳水化合物、脂肪、蛋白质、膳食纤维、矿物质、维生素和A组分;各组分重量份为脂肪15~60份、蛋白质20~90份、膳食纤维10~30份、矿物质0.5~30份、维生素0.02~3份,A组分1~15份;
所述的脂肪中含有中链甘油三酯,中链甘油三酯的含量占所述脂肪总重量的30~50wt%;
所述A组分选自山药、茯苓、百合、银耳、玉竹、蒲公英、枸杞、红枣、蓝莓提取物中的任意一种或几种的组合物。
2.一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述的组合物中含有两者以上混合保存或者各自独立保存的以下组分:碳水化合物、脂肪、蛋白质、膳食纤维、矿物质、维生素和A组分;
各组分重量份为脂肪15~60份、蛋白质20~90份、膳食纤维10~30份、矿物质0.5~30份、维生素0.02~3份,A组分1~15份;
所述A组分为含量重量比为1:1~3的山药和百合的组合;
所述脂肪为中链甘油三酯、植物油脂和动物油脂,且以所述脂肪的总质量为基准,所述中链甘油三酯的含量为30~50wt%,所述植物油脂的含量为35~60wt%,所述动物油脂的含量为10~15wt%;
所述蛋白质选自大豆分离蛋白、浓缩乳清蛋白、水解乳清蛋白肽中的至少一种;
所述膳食纤维为含量重量比为1:1~3的菊粉和抗性糊精的组合;
所述矿物质选自铁、钙、锌、镁中的至少一种;
所述维生素选自维生素A、维生素E、维生素B1、维生素B2、维生素B12、维生素C、烟酸、叶酸、泛酸中的至少一种。
3.根据权利要求1或2所述的一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述脂肪中还含有n-3多不饱和脂肪酸、植物油脂、动物油脂中的至少一种。
4.根据权利要求1或3所述的一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述的植物油脂选自玉米油、菜籽油、大豆油、花生油、椰子油、茶油、红花油、橄榄油中的至少一种;
所述动物油脂选自乳脂肪、鱼油微囊粉、亚麻籽油微囊粉中的至少一种。
5.根据权利要求1或3所述的一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述的碳水化合物选自麦芽糊精、玉米粉、燕麦粉、小麦粉、经过酶解处理的大米粉中的至少一种。
6.根据权利要求1或3所述的一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述的蛋白质选自大豆蛋白、大豆分离蛋白、水解大豆蛋白肽、浓缩乳清蛋白、分离乳清蛋白、水解乳清蛋白肽中的至少一种。
7.根据权利要求1或3所述的一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述的膳食纤维选自低聚果糖、低聚异麦芽糖、菊粉、低聚木糖、抗性糊精中的至少一种。
8.根据权利要求1或3所述的一种含中链甘油三酯高蛋白营养组合物,其特征在于:所述矿物质选自铁、钙、锌、硒、镁、钾、铜、锰、磷、碘中的至少一种;
所述的维生素选自维生素A、维生素D、维生素E、维生素K、维生素B1、维生素B2、维生素B6、维生素B12、烟酸、叶酸、泛酸、维生素C、生物素中的至少一种。
9.一种制备含中链甘油三酯高蛋白营养的方法,其特征在于,该方法包括:将权利要求1~8任意一项所述组合物中的各组分进行混合;
所述的组合物制备为粉、片、棒、片等口服剂。
10.权利要求1~8任意一项所述组合物对结核病患者的辅助治疗中的应用。
