Description
Technical Field
The application relates to a medical instrument, in particular to a hip joint prosthesis for osteolysis treatment.
Background
Osteolysis, also known as massive osteolysis, is a rare bone lesion characterized by massive osteolysis; if pathological fracture and dysfunction occur in the diseased region, surgical treatment such as internal fixation, bone cement reconstruction, prosthesis replacement and the like can be performed.
When the ilium is subjected to pathological fracture due to bone dissolution, the steel plate, namely the fixing plate, is fixed on the ilium through the metal screw, namely the fixing screw; an acetabulum cup is integrally formed on the surface of the fixing plate far away from the ilium, a spherical femoral head, namely a ball head, is in ball joint with the inside of the acetabulum cup, and a femoral stem inserted into a femoral medullary cavity is fixed on the ball head; furthermore, in order to enable the ball head to freely rotate in the acetabular cup, a larger notch for the ball head to freely rotate in the acetabular cup needs to be formed at the end part of the femur close to the ilium when the femur is cut, and the space of the notch is at least larger than the volume of the ball head, so that the ball head can freely rotate in the acetabular cup to prevent the femur from colliding with the acetabular cup during movement; however, due to the opening of the notch, the position of the femoral stem in the femoral medullary cavity needs to be fixed, so that the ball head is prevented from slipping to the inside of the femur through the notch; if the ball head is directly fixed on the femur, the rotation of the ball head in the acetabular cup is influenced.
In view of the above-mentioned related art solutions, the inventors found that: because the femur is provided with the notch for the rotation of the ball head, and the notch is larger relative to the ball head, when the position relation between the ball head and the acetabular cup is adjusted, the femoral stem needs to be fixed on the femur in time to prevent the ball head from moving along the axial direction of the femoral stem, the fixing operation of the positions of the ball head and the femoral stem is difficult, namely the mounting difficulty of the hip joint prosthesis is larger.
Disclosure of Invention
In order to facilitate the fixation of the positions of the acetabular cup and the ball head and the installation of the hip joint prosthesis, the application provides the hip joint prosthesis for osteolysis treatment.
The osteolysis treatment hip joint prosthesis adopts the following technical scheme:
a hip joint prosthesis for osteolysis treatment comprises a fixing plate; a ball head is integrally formed on the fixing plate, an acetabular cup is hinged to the ball head upper ball, and the acetabular cup is arranged on one side of the ball head away from the fixing plate; the outer surface of the acetabulum cup is integrally formed with a femoral stem.
Through adopting above-mentioned technical scheme, offer in advance on the thighbone and supply thighbone handle male breach, because the diameter of acetabular cup is greater than the bulb, and then the breach of seting up on the thighbone need not provide the space for the rotation of bulb, only need satisfy with the thighbone handle insert to the thighbone inside can, the breach diameter of seting up on the thighbone like this be less than the diameter of acetabular cup can, the surface overlap joint of acetabular cup is on the breach edge of seting up on the thighbone this moment, and then realize the preliminary fixed to acetabular cup position, the operation is comparatively simple.
Preferably, the fixing plate is provided with a fixing rod and a fixing component for fixing the fixing rod on the fixing plate, and the fixing rod extends towards one side of the fixing plate far away from the femoral stem; a fastening screw is arranged through the fixed rod.
Through adopting above-mentioned technical scheme, pass through fixed subassembly with the dead lever and fix on the fixed plate to make the dead lever extend along the extending direction of lumbar vertebrae, pass inside dead lever and screw in lumbar vertebrae through fastening screw, and then realize the fixed of dead lever and lumbar vertebrae, because the dead lever has been fixed on the fixed plate, and then realize further supporting fixed to the fixed plate, realized the multiple spot of fixed plate promptly and supported the location, improved the holistic steadiness of false body and support intensity.
Preferably, the ilium mop is formed by bending one end of the fixing plate far away from the fixing rod to the direction far away from the ball head.
Through adopting above-mentioned technical scheme, when carrying out the installation of fixed plate, make the ilium drag the laminating on the tip that the lumbar vertebrae was kept away from to the ilium, the ilium is dragged and is held up by the ilium when the human body stands promptly, plays the supporting role for the ilium on the one hand, and on the other hand makes the position between ilium and the fixed plate fixed more reliable.
Preferably, the fixed subassembly is including fixing the supporting seat on the fixed plate, offers the one end male mounting hole that supplies the dead lever to keep away from the fixed plate from the supporting seat on the supporting seat, and the one end screw in that the fixed plate was kept away from to the supporting seat has the pressure cap that hinders the dead lever and drop from the mounting hole.
Through adopting above-mentioned technical scheme, after the fixed plate is fixed on the ilium, make the dead lever from the tip embedding to the mounting hole inside of supporting seat, the tip that will press cap screw in supporting seat after that supports tightly on global at the dead lever until pressing the cap, and then realizes compressing tightly the dead lever on the supporting seat.
Preferably, a supporting screw is fixed on the fixing plate, a hinged cavity for placing the head of the supporting screw is formed in the supporting seat, the cavity wall of the hinged cavity is an arc-shaped surface, the head of the supporting screw is an arc-shaped bulge matched with the cavity wall of the hinged cavity, and the head of the supporting screw is in sliding connection with the cavity wall of the hinged cavity; a gap is formed between the supporting seat and the fixing plate.
By adopting the technical scheme, in the process of small-amplitude movement of a human body, the lumbar vertebra also moves along with the human body and drives the fixed rod to move with a certain amplitude, and the position of the fixed rod relative to the fixed plate also changes to a certain extent; by arranging the supporting screw, the arc-shaped head of the supporting seat is matched with the hinge cavity, so that the head of the supporting screw is in ball hinge connection with the supporting seat, and the supporting seat can rotate on the fixing plate in a certain range in space; when the lumbar vertebrae moves, if the fixing rod warps, the fixing rod drives the supporting seat to rotate on the fixing plate.
Preferably, an intramedullary pin is fixed to the fixing plate, and a surface of the intramedullary pin is treated to promote bone ingrowth.
By adopting the technical scheme, the osteolysis causes partial disappearance of bone marrow, so that the fixing screw is difficult to be reliably fixed in the ilium, and the intramedullary needle is needed to ensure that the fixing between the fixing plate and the ilium is more reliable; the intramedullary pin is more stably fixed in the ilium by the bone ingrowth effect.
In summary, the present application has the following technical effects:
1. the fixing plate, the ball head, the acetabular cup and the femoral stem are arranged, the ball head is integrally formed on the fixing plate, the acetabular cup is integrally formed on the femoral stem, a gap formed in the femur does not need to provide a space for rotation of the ball head, and only the femoral stem needs to be inserted into the femur, so that the diameter of the gap formed in the femur is smaller than that of the acetabular cup, the outer surface of the acetabular cup is lapped on the edge of the gap formed in the femur, and further preliminary fixation of the position of the acetabular cup is achieved, and the operation is simple;
2. through having set up dead lever, fixed subassembly and fastening screw, realize further supporting fixed to the fixed plate, realized the multiple spot of fixed plate promptly and supported the location, improved the holistic steadiness of false body and supported intensity.

Detailed Description
The present application is described in further detail below with reference to the attached drawings.
Referring to fig. 1 and 2, the present application provides an osteolytic iliac fixation prosthesis, including a fixation plate 1 fitted on a surface of an ilium 13 and a plurality of fixation screws 3 for fixing the fixation plate 1 on the ilium 13; wherein, fixed plate 1 is customized through 3D printing technique according to the shape on patient's ilium 13 surface and forms, and set screw 3 runs through fixed plate 1 and screw in inside ilium 13, and then realizes the fixed of position between fixed plate 1 and the ilium 13.
Referring to fig. 1 and 3, a ball head 10 is integrally formed on the surface of the fixing plate 1 far away from the ilium 13, a hemispherical shell-shaped acetabular cup 7 is hinged to the ball head 10, the opening of the acetabular cup 7 faces the fixing plate 1, a femoral stem 9 extending in the direction far away from the lumbar vertebra 12 is integrally formed on the outer surface of the acetabular cup 7, a gap for inserting the femoral stem 9 is formed on the femur 14, and the ball head 10 faces the gap on the femur 14; because the diameter of acetabular cup 7 is greater than bulb 10, and then the breach of seting up on thighbone 14 need not provide the space for the rotation of bulb 10, only need satisfy and insert femoral stem 9 to thighbone 14 inside, the breach diameter of seting up on thighbone 14 like this is less than acetabular cup 7's diameter can, acetabular cup 7's surface overlap joint is on the breach edge of seting up on thighbone 14 this moment, and then realize the preliminary fixed to acetabular cup 7 position, follow-up accessible metal material's screw with femoral stem 9 fix on thighbone 14 can.
In the installation method of the femoral stem 9 in the related art, the position relation between the ball head 10 and the femur 14 needs to be fixed in advance, but because the gap with the diameter larger than that of the ball head 10 is arranged on the femur 14, the relative position between the ball head 10 and the femur 14 is difficult to fix, and the installation method of the femoral stem 9 in the application is simpler and more convenient; another installation method of the femoral stem 9 in the related art is to cut off one end of the femur 14 close to the ilium 13 along the horizontal plane of the human body, so as to facilitate the insertion of the femur 14 and provide a large amount of space for the rotation of the head of the femur 14, but the cut-off amount is large by the method; the arrangement mode of the ball head 10 and the acetabular cup 7 related in the application only needs to provide a notch for inserting the femoral stem 9 on the femur 14, and the osteotomy amount of the femur 14 is reduced.
Referring to fig. 1, due to the influence of osteolysis, the bone marrow in the ilium 13 partially disappears, and thus the fixing effect of the fixing screw 3 is not ideal, in order to enable the fixing plate 1 to be more stably fixed on the ilium 13, the surface of the fixing plate 1 near the ilium 13 is provided with an intramedullary needle 6 inserted into the ilium 13, and the surface of the intramedullary needle 6 is treated to promote the bone ingrowth effect, which is further explained as the treatment method of the surface of the intramedullary needle 6 includes, but not limited to, 3D printing of a trabecular bone-imitating porous structure, coating with a titanium/tantalum spraying coating or coating with a hydroxyapatite coating.
