Description
Technical Field
The invention relates to the field of medical instruments, in particular to a fallopian tube bracket.
Background
The fallopian tube is divided into 4 parts from inside to outside according to the shape: interstitial part, isthmus part, ampulla part, umbrella part. Clinically, 80% of infertility patients are due to fallopian tube factors. Salpingemphraxis is often caused by adhesion, congestion and edema of the salpingemphraxis due to artificial abortion, spontaneous abortion, drug flow, induced labor, unclean sexual intercourse, pelvic infection and the like, so that sperms and ova cannot be combined, and finally infertility is caused. The medicine has no effect on treating the oviduct blockage, an operation is needed or a test tube baby is needed to solve the fertility problem, and the proportion of the test tube baby is selected as the proportion of the test tube baby due to the infertility caused by the oviduct blockage, which accounts for about 60 percent of the total number of the test tube babies.
The uterine tube has narrow lumen and high requirement on the fineness of the operation, and the laparoscopic operation has certain effect on treating pelvic adhesion and distal obstruction of the uterine tube, but most of the postoperative patients form adhesion and obstruction again, and the postoperative pregnancy rate is low; the hysteroscopic tubal ostium intubation dredging operation treats the proximal occlusion of the fallopian tube and the adhesion of the tubal lumen part, and the probability of the adhesion again after the operation is larger than that of the distal operation of the fallopian tube. Solves the problem of secondary adhesion after operation, improves the success rate of the operation, and becomes the key of the oviduct operation.

Detailed Description
Example 1
As shown in fig. 1, the fallopian tube stent of this embodiment comprises an elastic hollow tube body, wherein the elastic hollow tube body comprises a straight tube section 3, a first curled section 1 arranged at the head end of the straight tube section 3 and a second curled section 2 arranged at the tail end of the straight tube section 3.
It should be noted that the head end of the straight tube section 3 is the end close to the umbrella part of the fallopian tube in use, i.e. the end relatively far away from the uterus; the end of the straight tube section 3 is the end near the interstitial part of the fallopian tube, i.e. the end relatively closer to the uterus. The head and tail ends of other parts are also standardized except for special descriptions.
This device relies on hysteroscope when using, insert the entry of oviduct interstitial part with the head end of first curled section 1, reach the umbrella portion after the isthmus of oviduct and ampulla, later because oviduct umbrella portion space is progressively enlarged, first curled section 1 just correspondingly produces to bend and becomes the roll-like because of self elastic retraction, and second curled section 2 is also curled in uterus this moment, thereby make the curled section at 3 both ends of straight tube section play the effect of card solid straight tube section 3, make straight tube section 3 play the supporting role to the oviduct pipe wall, prevent the postoperative adhesion. Meanwhile, the support tube is integrally made of an elastic hollow tube, the elasticity of the support tube ensures that the straight tube section 3 is adaptive to the irregular shape of the fallopian tube, and the hollow support tube can ensure that the liquid between the umbrella part of the fallopian tube and the uterus is smoothly conveyed, plays a role in drainage and prevents obstruction.
It should be noted that, inserting the first curled segment 1 into the fallopian tube under natural state may be inconvenient due to its elastic retraction, and in case of this situation, a hard guide wire (such as a thin steel wire) may be inserted into the elastic hollow tube before the operation to straighten the curled segments at both ends, and then the fallopian tube stent is sent to a predetermined position by means of the guide wire, and then the guide wire is withdrawn to complete the insertion.
In order to facilitate the access of the liquid to the umbrella part of the fallopian tube and the uterus and prevent the single nozzle from being blocked accidentally, the embodiment is provided with a first through hole 6 on the first curling section 1 and a second through hole 7 on the second curling section 2. In addition, a certain amount of liquid secretion exists in the fallopian tube section corresponding to the straight tube section 3, so that the straight tube section 3 is also provided with a third through hole 5, so that the liquid can flow away along the hollow tube cavity and cannot be closed after the fallopian tube wall is tightly attached to the straight tube section 3.
In the process of placing the device, in order to more accurately know the insertion amount to judge whether the device is moved in place, the straight pipe section 3 and the second curled section 2 are further provided with the length measuring scales 4, the first curled section 1 is completely inserted into the oviduct, so the scales are not required to be arranged, although the straight pipe section 3 mostly occupies the oviduct under the general condition, the patient possibly has a case that the oviduct is too long due to different physiques of the patient, the partial section of the second curled section 2 is also possible to enter the oviduct, and therefore the scales are preferably arranged on the second curled section 2 besides the straight pipe section 3.
In order to facilitate the detachment of the device after the retention period, the end of the second curved section 2 is connected with a nylon thread 8, so that the entire stent tube is pulled out from the fallopian tube and taken out through the uterine orifice by pulling the nylon thread 8.
Example 2
The present embodiment is substantially the same as the previous embodiment, except that the straight tube section 3 has a different structure: the straight tube sections 3 of the previous embodiment are all composed of smooth sections, while the straight tube sections 3 of the present embodiment are provided with folded sections 9.
As shown in fig. 2, the folded section 9 of the present embodiment has a bellows-like wall 10. The accordion shape is just like the bellows of the accordion, and has a plurality of teeth-shaped folding structures when viewed from the side section. It should also be noted that the third through-opening 5 can also be provided in the folding section 9, but that it is stretched in the unfolded configuration compared to the folded configuration. While the length gauge 4 can also be realized by applying different colors on different ring teeth.
The provision of the folding section 9 has the following effects:
1. the liquid in the fallopian tube has the characteristic of one-way flow from the umbrella part to the interstitial part, so after long-term liquid washing, the fallopian tube bracket is washed and displaced, and the folding section 9 is provided with a plurality of transverse stripes, so that the friction force between the fallopian tube bracket and the fallopian tube wall is increased, and the fallopian tube bracket is prevented from sliding.
2. Because the folding section 9 has a contraction state and an extension state (see the contraction state folding section 13 and the extension state folding section 14 in fig. 2), the maximum outer diameter of the extension state is smaller than the maximum outer diameter of the contraction state, when the stent tube is detached at the later stage, the contraction state folding section 13 with the larger outer diameter in the inner part (close to the umbrella part) of the fallopian tube is clamped by the fallopian tube wall, and the folding section 9 close to the outer part (close to the uterus) under the pulling force of the nylon thread 8 is stretched into the extension state folding section 14, the process is sequentially carried out layer by layer from the inner part according to the gear ring arrangement sequence of the folding section 9, and the outer diameter of the extension state folding section 14 is reduced, so that the folding section can pass through the fallopian tube with the diameter not much larger than the contraction state tube diameter, and is convenient to detach; and if the adhesion between the fallopian tube wall and the folding section 9 is generated before, because of the layer-by-layer peeling effect of the folding section 9, compared with the integral pulling-out, the pulling force is reduced, the operation is more gentle and relaxed, the fracture injury, especially the large-area fracture injury, generated by pulling-out the adhesion epidermis is reduced, and the pain of a patient is relieved.
Example 3
As shown in fig. 3 and 4, the present embodiment is substantially the same as the previous embodiment, with the following differences:
the accordion folded tube wall 10 of the previous embodiment is replaced with a spiral folded tube wall 11 in this embodiment. The spiral folding pipe wall 11 means that in the tooth-shaped folding structure of the pipe wall, a single tooth of the pipe wall does not form a closed loop like an organ shape, but forms a cylindrical spiral line, and grooves among the teeth are communicated from beginning to end instead of the organ shape. The nylon wire 8 of the present embodiment is wound along the groove formed by the spirally folded tube wall 11, the winding direction of the nylon wire 8 is from the tail end of the straight tube section 3 to the head end of the straight tube section 3, and the nylon wire 8 located in the groove is clamped by the adjacent tube walls at two sides. The above-mentioned "the nylon thread 8 is clamped by the adjacent pipe walls on both sides" means that, because the pipe walls have certain elasticity, the nylon thread 8 embedded in the groove has the acting force of spreading the pipe walls on both sides, so the nylon thread 8 is clamped under the effect of the resilience force of the pipe walls.
So arranged, the nylon thread 8 held by the tube wall follows the straight tube section 3 into the fallopian tube during the procedure. In the later detaching process, the nylon wire 8 is pulled first, because the deeper nylon wire 8 is still clamped by the pipe walls at two sides, the nylon wire 8 is difficult to be released along the spiral groove, namely the nylon wire 8 is difficult to be integrally slid out in the groove (bottom), and only the nearby nylon wire 8 can cross the tooth top of the spiral groove to be peeled and released. The top of each ring of teeth of the spiral groove is scraped and cut once by the adjacent nylon thread 8, if the fallopian tube wall and the folding section 9 are adhered, the scraping can sequentially cut off and separate the adhesion part from shallow to deep in a micro-scale (determined by the pitch of the teeth), and the micro-cutting of the layer by layer enables the operation to be soft and gentle, is easy to control, enables the surface of the wound to be small, enables the healing after the operation to be completed more quickly, and relieves the pain of a patient. After all the nylon wires 8 are scraped and cut, the nylon wires 8 become a traction rope for drawing the whole elastic hollow pipe body, and the spiral folding pipe wall 11 of the embodiment also has the corresponding technical effects of the organ-shaped folding pipe wall 10 of the previous embodiment, for example, the layer-by-layer peeling effect realized by contraction and extension, so that the drawing of the device becomes smooth, and the pain of a patient is relieved. Of course, it is also possible to divide the string inside the spirally folded tube wall 11 and the string at the end of the second curled segment 2 into two separate strings, and in this embodiment, a nylon thread 8 is used to play the role of both strings, because it can prevent the two strings from winding and being difficult to distinguish.
In addition, the end of the nylon thread 8 of the present embodiment is located inside the groove, and the marker bead 12 is disposed on the nylon thread 8 outside the groove.
This is because the end nylon thread 8 closer to the head end of the straight tube section 3 is held in the groove, which prevents the end thread from snagging during the insertion of the device and preventing it from obstructing the surgical operation. And the nylon thread 8 at the end of the second curled segment 2 is stained due to the contamination of menstrual blood because the nylon thread is positioned in the uterus, so that the nylon thread is close to the color of the inner wall of the uterus and is difficult to be found in the detaching operation, so the nylon thread 8 which is not wound into the groove at the end of the second curled segment 2 (because the end of the nylon thread 8 is all in the groove, the nylon thread 8 can also be described as the nylon thread 8 outside the groove) is fixed with the marking bead 12 which plays a role in marking, so that the nylon thread 8 can be easily found in the detaching operation and the subsequent operation is unfolded.
The present invention has been disclosed in terms of the preferred embodiments, but the invention is not limited to these embodiments. Any equivalent changes or modifications made without departing from the spirit and scope of the invention are also within the protection scope of the invention. The scope of the invention should therefore be determined with reference to the appended claims.
