Abstract

一种直接穿刺型胆管狭窄扩张引流管属于医疗器械制造技术领域。本发明装置包括引流管、支撑管和穿刺针,引流管中,单通道管4呈锥形,其内通道呈锥形;单通道管5侧壁上有两个侧孔;双通道管6表面呈凹形,包裹聚乙烯球囊膜,两端固定在双通道6上;双通道管7尾部连接注水连接头;单通道管8尾部设有单通道连接头,连接头表面设有螺纹;支撑管头端设有塑料阻力块,尾端设有帽状接头,帽状接头内壁和外壁有螺纹,将该结构旋在引流管尾部接头上;穿刺针尾端设有帽状接头,帽状接头内壁有螺纹,将该结构旋在支撑管尾部接头上。本发明的优点是,能直接经皮穿入胆管引流胆汁及并扩张狭窄胆管。

Description

一种直接穿刺型胆管狭窄扩张引流管
技术领域
本发明属于医疗器械制造技术领域,涉及一种微创医疗技术及手术器械,用于经皮经肝胆管穿刺治疗良性肝外胆管狭窄的微创手术器械,具体地说是一种直接穿刺型胆管狭窄扩张引流管。
背景技术
肝外胆管狭窄是临床一种常见病和多发病,包括炎性胆管狭窄、损伤性胆管狭窄、肝移植术后胆管吻合口狭窄、恶性胆管狭窄等,胆管狭窄可引起狭窄上方胆道压力升高、胆汁排泄不畅,引起寒战高热、黄疸及上腹部疼痛症状,持续性梗阻可引起胆汁淤积性肝硬化、肝功能衰竭,甚至危及患者生命。
对于肝外胆管狭窄的治疗包括外科手术、内镜及介入治疗,其中外科手术风险大,胆肠吻合术后并发症多;内镜手术创伤小,需切开乳头括约肌,存在术后出血、急性胰腺炎、反流性胆管炎等并发症,对于上消化道狭窄、胃肠手术后再次行内镜手术难度较大,失败率较高。三种治疗方案中,介入治疗创伤最小,在局麻下即可完成,即可作为首选防范,也可作为外科手术及内镜手术治疗失败的替补防范。
介入治疗胆管狭窄的方法包括胆道外引流、胆道塑料支架置入或胆道金属支架置入,以解除狭窄,恢复胆汁的通畅引流。三种引流方式中胆道外引流管只能解除黄疸,不能解除狭窄,大量胆汁的丢失会导致纳差,水、酸碱及离子紊乱;胆道塑料置入次之,但是取出支架后再狭窄率较高;胆道金属支架置入是治疗肝外胆管狭窄可以获得更大的扩张直径,一般用于恶性肝外胆管狭窄的治疗,很少用于良性胆管狭窄的治疗,因为经皮经肝穿刺途径难易将置入的金属支架取出。内镜治疗胆管狭窄确定置入胆道金属支架通畅时间更长、狭窄复发率更低,效果优于塑料支架。
发明内容
本发明的目的就是提供一种用于引流胆汁及扩张胆管狭窄的直接穿刺型胆管狭窄扩张引流管。
本发明由引流管1、支撑管2和穿刺针3构成,其中,
a.引流管1由单通道管4、单通道管5、双通道管6、双通道管7和单通道管8构成;单通道管4呈锥形,其内设置锥形通道9;单通道管5内设置的通道 10与通道9相通,单通道管5侧壁上有侧孔23;双通道6内设置有通道11和通道12,通道11与通道10相通,通道12穿过双通道6侧壁与球囊13相通,双通道6外表面呈凹形处包裹的球囊13膜固定在双通道6上,球囊13未充盈时呈膜状;双通道管7内设置有通道14与通道15,通道14与通道11相通,通道15与通道12相通,双通道管7底部侧壁设置有与通道12相通的单通道连接头16,双通道管7设置有侧孔17;单通道管8内设置有与通道14相通的通道 18,尾部设置的连接头19外表面设有螺纹;
b.支撑管2为单通道管,头端设有阻力块20,尾端设置的帽状接头21内壁和外壁有螺纹,支撑管2置于引流管1内,帽状接头21旋在引流管1尾部连接头19上;
c.穿刺针3为金属针,尾端设有帽状接头22,帽状接头22内壁有螺纹,穿刺针3置于支撑管2内,帽状接头22旋在支撑管2尾部的帽状接头21上;
引流管1长60-100cm;其中,单通道管4顶端外径7Fr,内径6Fr,长 1.5cm;单通道管5外径8.5Fr,内径7Fr,长2.5cm;双通道管6凹部外径 8Fr,内径7Fr,长7-11cm;双通道管7外径8.5Fr,内径7Fr,长44-77cm;单通道管8外径8.5Fr,内径7Fr,长2-3cm,连接头19长3-5cm;
单通道管5侧壁上的侧孔23是2个;
双通道管7设置有的侧孔17是3-5个;
球囊13长度6-10cm,球囊直径6-20mm;
球囊13是聚乙烯球囊;
球囊13两端由金属环固定在双通道6上;
支撑管2外径6Fr,内径5Fr,长59-99cm,其中,帽状接头21长2cm;
支撑管2为金属管,头端设有的阻力块20是塑料阻力块20;
穿刺针3外径4Fr,长61-101cm,其中,帽状接头22长1cm。
本发明所达到的有益效果是:
1.本发明组装后可直接穿刺进入扩张的胆管内;
2.本发明中充盈柱形球囊可扩张狭窄,引流管可内外引流胆汁,损伤小、并发症少;
3.本发明中引流管外径固定,球囊直径多样,更容易穿刺进入胆管内。

下面结合附图和具体实施方式对本发明作进一步详细的说明。
本发明装置由引流管1、支撑管2和穿刺针3构成,其中,
a.引流管1由单通道管4、单通道管5、双通道管6、双通道管7和单通道管8构成;单通道管4呈锥形,其内设置锥形通道9;单通道管5内设置的通道 10与通道9相通,单通道管5侧壁上有2个侧孔23;双通道6内设置有通道11 和通道12,通道11与通道10相通,通道12穿过双通道6侧壁与聚乙烯球囊13 相通,双通道6外表面呈凹形处包裹聚乙烯球囊13膜,两端由金属环固定在双通道6上;双通道管7内设置有通道14与通道15,通道14与通道11相通,通道15与通道12相通,双通道管7底部侧壁设置有与通道12相通的单通道连接头16,双通道管7设置有5个侧孔17;单通道管8内设置有与通道14相通的通道18,尾部设置的连接头19外表面设有螺纹;
b.支撑管2为单通道管金属管,头端设有塑料阻力块20,尾端设置的帽状接头21内壁和外壁有螺纹,支撑管2置于引流管1内,帽状接头21旋在引流管1尾部连接头19上;
c.穿刺针3为金属针,尾端设有帽状接头22,帽状接头22内壁有螺纹,穿刺针3置于支撑管2内,帽状接头22旋在支撑管2尾部的帽状接头21上;
引流管1长100cm;其中,单通道管4顶端外径7Fr,内径6Fr,长1.5cm;单通道管5外径8.5Fr,内径7Fr,长2.5cm;双通道管6凹部外径8Fr,内径 7Fr,长11cm;双通道管7外径8.5Fr,内径7Fr,长77cm;单通道管8外径 8.5Fr,内径7Fr,长3cm,其中,连接头19长5cm;
球囊13长度10cm,球囊直径20mm;
支撑管2外径6Fr,内径5Fr,长99cm,其中,其中,帽状接头21长2cm;
穿刺针3外径4Fr,长101cm,其中,帽状接头22长1cm。
截面A和截面B之间是胆管狭窄段,截面B和截面C之间是胆管。
1.直接胆管穿刺:
本装置组装后,在超声或X线监视下将本装置穿刺进入到胆管内,退出穿刺针3,经支撑管2注入造影剂完整显示肝外胆管及狭窄部位,经支撑管2置入导丝24并进入狭窄远端胆管或十二指肠内,固定导丝24及引流管1,退出支撑管2,X线透视下调整引流管1位置,使狭窄位于球囊13中央,充盈部分球囊13 来固定引流管1,退出导丝24,此时将腹壁外引流管1缝合固定在穿刺点旁腹壁上,接引流袋引流胆汁。
2.更换引流管:
在超声或X线监视下经原引流管置入导丝24,使导丝24位于狭窄远端,保留导丝24并退出原引流管,将组装的本装置中的穿刺针3退出,在导丝24引导下将带有支撑管2的引流管1置入到胆管内,固定导丝24及引流管1,退出支撑管2,使引流管1越过狭窄,X线透视下调整引流管1位置,使狭窄位于球囊13中央,充盈部分球囊13来固定引流管1,退出导丝24,此时将腹壁外引流管1缝合固定在穿刺点旁腹壁上,接引流袋引流胆汁。

1.一种直接穿刺型胆管狭窄扩张引流管,其特征在于,由引流管(1)、支撑管(2)和穿刺针(3)构成,其中,
a.引流管(1)由单通道管(4)、单通道管(5)、双通道管(6)、双通道管(7)和单通道管(8)构成;单通道管(4)呈锥形,其内设置锥形通道(9);单通道管(5)内设置的通道(10)与通道(9)相通,单通道管(5)侧壁上有侧孔(23);双通道(6)内设置有通道(11)和通道(12),通道(11)与通道(10)相通,通道(12)穿过双通道(6)侧壁与球囊(13)相通,双通道(6)外表面呈凹形处包裹的球囊(13)膜固定在双通道(6)上;双通道管(7)内设置有通道(14)与通道(15),通道(14)与通道(11)相通,通道(15)与通道(12)相通,双通道管(7)底部侧壁设置有与通道(12)相通的单通道连接头(16),双通道管(7)设置有侧孔(17);单通道管(8)内设置有与通道(14)相通的通道(18),尾部设置的连接头(19)外表面设有螺纹;
b.支撑管(2)为单通道管,头端设有阻力块(20),尾端设置的帽状接头(21)内壁和外壁有螺纹,支撑管(2)置于引流管(1)内,帽状接头(21)旋在引流管(1)尾部连接头(19)上;
c.穿刺针(3)为金属针,尾端设有帽状接头(22),帽状接头(22)内壁有螺纹,穿刺针(3)置于支撑管(2)内,帽状接头(22)旋在支撑管(2)尾部的帽状接头(21)上。
2.根据权利要求1所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,引流管(1)长60-100cm;其中,单通道管(4)顶端外径7Fr,内径6Fr,长1.5cm;单通道管(5)外径8.5Fr,内径7Fr,长2.5cm;双通道管(6)外径8Fr,内径7Fr,长7-11cm;双通道管(7)外径8.5Fr,内径7Fr,长44-77cm;单通道管(8)外径8.5Fr,内径7Fr,长2-3cm,连接头(19)长3-5cm。
3.根据权利要求1或2所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,单通道管(5)侧壁上的侧孔(23)是2个。
4.根据权利要求1或2所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,双通道管(7)设置有的侧孔(17)是3-5个。
5.根据权利要求1所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,球囊(13)长度6-10cm,球囊直径6-20mm。
6.根据权利要求1或5所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,球囊(13)是聚乙烯球囊。
7.根据权利要求1、5或6所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,球囊(13)两端由金属环固定在双通道(6)上。
8.根据权利要求1所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,支撑管(2)外径6Fr,内径5Fr,长59-99cm,其中,帽状接头(21)长2cm。
9.根据权利要求1或8所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于,支撑管(2)为金属管,头端设有的阻力块(20)是塑料阻力块(20)。
10.根据权利要求1所述的一种直接穿刺型胆管狭窄扩张引流管,其特征在于穿刺针(3)外径4Fr,长61-101cm,其中,帽状接头(22)长1cm。
