Description
Technical Field
The application relates to a medical appliance, in particular to a bone wax smearing device for hemostasis in posterior cervical surgery.
Background
In bone surgery, the phenomenon of bleeding from cancellous bone is often caused, and it is common practice to apply bone wax to the bone surface to stop bleeding. The traditional method is that after hard bone wax is kneaded to a softer state, the hard bone wax is smeared on the surface of bleeding bone by fingers, gauze or a stripper. However, when the bone surface is uneven, the smearing and hemostasis effects are often poor, and the gloves and fingers of operators are easily scratched, so that the pollution probability is increased. In addition, a special gauze "peanut kernel" is clamped by a pair of bending pliers, bone wax is laid on the gauze, and then the gauze is coated. However, the gauze is not firmly fixed and is easy to fall off, so that the operation is complicated and the operation efficiency is low. The cervical spine canal expansion and shaping operation is one of the most common cervical spine operations, a plurality of bone grooves (operation steps necessary for expanding the space of the cervical spine canal) are formed between a cervical spine lamina and a side block by using rongeur forceps or abrasive drills in the operation, but because the bone grooves are quite special (usually V-shaped) and are deeper, the common method of smearing bone wax with fingers, gauze or stripper is difficult to seal bleeding points, the operation speed is reduced, the operation time is prolonged, the intraoperative bleeding is increased, more importantly, the cervical spinal cord is beside the bone grooves, and the repeated operation of smearing the bone wax for many times can injure the cervical spinal cord or nerve roots by mistake, so that the irreparable consequences such as nerve injury are caused.
Therefore, there is a need for a bone wax applicator that is specifically directed at posterior cervical spinal canal expansion and shaping, is capable of improving the application efficiency, is adaptable to various bone fracture surfaces, and avoids scratching the operator and damaging the spinal nerve root of the patient.

According to embodiments of the present application, the cross-section of the grip lever may comprise a circular, rectangular, square, regular hexagonal, regular octagonal shape. When the grip lever is cylindrical, it may be 8-12 cm in length and 2-4 mm in cross-section. Preferably, the length of the grip lever may be 10cm and the diameter of the cross section of the grip lever may be 3 mm.
According to an embodiment of the present application, the included angle between the first working surface and the second working surface may be 20 degrees to 70 degrees. Preferably, the included angle between the first working surface and the second working surface is 30 degrees, 45 degrees or 60 degrees.
According to an embodiment of the application, the length of the first applicator head may be 8-12 mm.
According to an embodiment of the application, the first and second working surfaces may be 10 mm in length and 5mm in width.
According to an embodiment of the application, the diameter of the third working surface of the second applicator head may be 4-6 mm. Preferably the diameter of the third working surface of the second applicator head may be 5 mm.
According to an embodiment of the application, the grip lever is perpendicular to the first joint surface of the first applicator head and is connected to each other at the geometric center of the first joint surface.
According to an embodiment of the application, the grip lever may comprise a corner near its first end, which may be at an angle of 30-90 degrees. Furthermore, the grip lever may comprise a detachably connected two-piece grip lever.
According to an embodiment of the application, corners of the first and second applicator heads may include rounded corners.
According to embodiments of the present application, the grip lever, the first applicator head and the second applicator head may be integrally produced and may comprise the same material, including medical stainless steel, titanium alloy or medical plastic, for example.
According to an embodiment of the application, the holding rod and the second application head may be produced in one piece, the first application head is produced separately and connected to the first end of the holding rod by means of a screw thread, and the first application head comprises three first application heads which are exchangeable, wherein the angle between the first working surface and the second working surface is 30 degrees, 45 degrees or 60 degrees, respectively.
The beneficial effects of the application are that
The cervical spine canal expansion and shaping operation is the most common operation of cervical spine, wherein the most important step is to use rongeur or abrasive drill to open a plurality of bone grooves between cervical spine lamina and lateral mass, as a portal shaft to open the lamina, expand the spine canal space and relieve the compression of cervical spinal cord. However, this step has many bleeding points or bleeding surfaces because it bites into the cortical bone and exposes the cancellous bone. The bone groove formed in the operation is usually in a V shape of about 45 degrees, so that the common method of smearing bone wax with fingers, gauze or stripper is difficult to seal bleeding points, not only reduces operation speed, prolongs operation time and increases bleeding in the operation, but also is more important to neck spinal cord beside the bone groove, and repeated operation of smearing bone wax for many times can damage neck spinal cord or nerve root by mistake, and cause irreparable consequences such as nerve injury.
The bone wax application device according to the present application comprises: a grip lever having a first end and a second end opposite the first end and including a grip section between the first end and the second end adapted for grasping by a physician; the first smearing head is in a triangular frustum shape and comprises a first working surface, a second working surface and a first combining surface, and the first combining surface is connected with the first end of the holding rod; and the second smearing head is in a disc shape and comprises a circular third working surface and a second combining surface opposite to the third working surface, the second combining surface is connected with the second end of the holding rod, and the first working surface and the second working surface of the first smearing head and the third working surface of the second smearing head respectively correspond to physiological characteristics of bones of a part needing to smear bone wax. The shape of the first smearing head corresponds to a bone groove, the shape of the second smearing head corresponds to a bone eye, and under the condition of holding the rod by hand, the bone wax can be smeared uniformly by the first smearing head or the second smearing head to enable the bone wax to seal the bleeding point, so that the operation is simple, and time and labor are saved. When the first working surface and the second working surface of the first smearing head and the third working surface of the second smearing head comprise roughness, such as texture, the friction force between the bone wax and the first working surface, the second working surface or the third working surface is increased, and the bone wax can be prevented from falling off. Because the first working surface and the second working surface of the first smearing head form a proper included angle, so that the first working surface and the second working surface correspond to the shape of the bone groove, and the third working surface of the second smearing head corresponds to the bone eye, the first smearing head can well fit the shape of the bone groove, the second smearing head can well fit the shape of the bone eye, the bleeding surface can be easily sealed by pressing bone wax, the operation speed is improved, the operation time is shortened, the bleeding in the operation is reduced, and the risk that the neck spinal cord or nerve root is injured by repeated operation of smearing the bone wax for a plurality of times is avoided. Furthermore, the third working surface of the second smearing head is easier to install bone wax, and aiming at the bleeding point of the bone eye, the bleeding point can be flexibly pressed under uniform stress so as to be sealed.
In addition, according to the situation that the bone groove can be cut into different shapes, the included angle between the first working surface and the second working surface of the first smearing head can be made into an angle suitable for the included angle, for example, three types of 30 degrees, 45 degrees and 60 degrees, and different first smearing heads or different bone wax smearing devices can be selected in operation according to different cases, and the operation is performed respectively or simultaneously.

Fig. 3 is a schematic view showing the connection relationship of three first application heads of the bone wax application device according to the present application to the grip lever. Fig. 4 is a schematic view showing a bone groove and a bone eye in need of application of bone wax. Fig. 5A, 5B and 5C are schematic views showing that the grip lever of the bone wax application device according to the present application has a corner. Fig. 6A, 6B and 6C are schematic views showing the texture of a grip lever of a bone wax application device according to the present application.
As shown in fig. 1, according to an embodiment of the present application, there is provided a bone wax application device including a grip lever 10, a first application head 20, and a second application head 30.
As shown in fig. 1 and 2, the grip lever 10 has a first end 11 and a second end 12 opposite the first end 11, and includes a grip section 13 between the first end 11 and the second end 12 that is suitable for grasping by a physician.
As shown in fig. 2 and 3, the first applicator head 20 has a triangular-frustum shape, including a first working surface 21, a second working surface 22, and a first bonding surface 23. The first engagement surface 23 of the first applicator head 20 is connected to the first end 11 of the grip lever 10.
The second applicator head 30 has a disc shape including a circular third working surface 31 and a second bonding surface 32 opposite the third working surface 31. The second engagement surface 32 of the second applicator head 30 is connected to the second end 12 of the grip lever 10.
The first working surface 21 and the second working surface 22 of the first application head 20 and the third working surface 31 of the second application head 30 correspond to the physiological characteristics of the bone of the part to be coated with bone wax, respectively.
The smearing device according to the application is a medical instrument designed for intraoperative hemostasis for spinal bone surgery, in particular for cervical bone surgery. As shown in fig. 4, the bone slot 40 referred to herein refers to a bone defect of about 30-60V-shape that is cut between the cervical lamina and the cervical lateral mass on both sides of the surgical segment with bone forceps/grinding drill/ultrasonic bone knife, respectively. The bone eyes mentioned herein refer to various bleeding points on the surface of the bone during operation, and can be arranged on any operation wound surface, so long as the operation is performed, a point-like (very small diameter) bleeding point can appear on the surface of the bone (blood can continuously bleed without sealing the bone). Bone eyes often appear at the junction of bone and muscle, or are created by the operator for surgical purposes such as securing instruments.
Thus, according to an embodiment of the present application, the physiological characteristics of the bone corresponding to the first working surface 21 and the second working surface 22 of the first application head 20, where the bone wax is to be applied, may be V-shaped bone grooves artificially formed between the cervical vertebral lamina and the lateral mass. The physiological characteristic of the bone of the portion where the bone wax is to be applied corresponding to the third working surface 31 of the second application head 30 may be bone eyes of the cervical vertebrae surface.
According to an embodiment of the application, the first and second working surfaces 21, 22 of the first applicator head 20 and the third working surface 31 of the second applicator head 30 may comprise roughened surfaces, and the roughened surfaces may comprise uniform texture.
As shown in FIG. 6A, the texture may be a "well" grid. As shown in fig. 6B, the texture may be punctiform roughness. As shown in fig. 6C, the texture may be a "diagonal" glyph grid. The application is not limited thereto but the texture may also be any other suitable way known to a person skilled in the art, for example a "herringbone grid. The height of the texture protruding outwards from the base surface may be, for example, about 1 mm or so, and the pitch of the texture grid may be, for example, about 1 mm or so. The distance between the texture grids may be, for example, the distance between the grids or the distance between the punctiform asperities.
According to an embodiment of the application, the diameter of the third working surface 31 of the second applicator head 30 may be greater than the diameter of the second bonding surface 32. Thus, the second applicator head 30 may be formed in a generally frustoconical shape.
According to embodiments of the present application, the cross-section of the grip lever 10 may include a circular, rectangular, square, regular hexagonal, regular octagonal shape. When the grip lever 10 is cylindrical, its length may be 5-20 cm, preferably 8-12 cm; the diameter of the cross section may be 2-4 mm. Preferably, the length of the grip lever 10 may be 10 cm, and the diameter of the cross section of the grip lever 10 may be 3 mm. The present application is not limited thereto and the length and cross section of the grip lever 10 may be sized to suit the needs of the actual surgical needs without departing from the teachings of the present application.
According to an embodiment of the present application, the included angle between the first working surface 21 and the second working surface 22 may be 20 degrees to 70 degrees. Preferably, the included angle between the first working surface 21 and the second working surface 22 is 30 degrees, 45 degrees or 60 degrees. The angle between the first working surface 21 and the second working surface 22 can be flexibly selected according to the shape of the bone groove to be cut during the operation. For example, when the cut shape of the bone groove is a 45-degree V-groove, the first application head 20 of 45 degrees may be selected to apply bone wax, and the application device according to the present application may be used to apply force to the bone groove to achieve a good hemostatic effect. Alternatively, it is also possible to uniformly spread the bone wax using only one first spreading head 20 of 45 degrees, and then apply force to the bone groove with the spreading device according to the present application through a gauze pad or a medical rubber pad to achieve a good hemostatic effect.
According to an embodiment of the application, the length of the first applicator head 20 may be 8-12 mm, preferably 10 mm. The length of the disposable bone wax coating is not too long, so that the hemostatic effect is poor; conversely, when the length of the disposable application of bone wax is too short, the second applicator head 30 may be used to complete the application of bone wax and hemostasis. Thus, the first and second working surfaces 21, 22 may be about 10 millimeters in length and about 5 millimeters in width. The present application is not limited thereto and the dimensions of first applicator head 20 may be suitably adjusted according to the needs of the procedure and the improvements in the procedure without departing from the teachings of the present application.
According to an embodiment of the application, the diameter of the third working surface 31 of the second application head 30 may be 4-6 mm. Preferably the diameter of the third working surface 31 of the second applicator head 30 may be 5mm. The present application is not limited thereto and the dimensions of second applicator head 30 may be suitably adjusted according to the needs of the procedure and the improvements in the procedure without departing from the teachings of the present application.
As shown in fig. 1, 2 and 3, according to an embodiment of the present application, the grip lever 10 is perpendicular to the first coupling surface 23 of the first applicator head 20 and is connected to each other at the geometric center of the first coupling surface 23 with the first end 11 thereof. The connection between the first end 11 of the grip lever 10 and the first engagement surface 23 of the first applicator head 20 may be of any form. When the grip lever 10 is integrally manufactured with the first applicator head 20, it is a natural connection of homogenous material. When the grip lever 10 is manufactured separately from the first applicator head 20, it may be connected to each other by means of threads, snaps, interference fit, etc. As shown in fig. 3, preferably, the first end 11 of the grip lever 10 may include external threads 11a, and the first engagement surface 23 of the first applicator head 20 may include internal threaded holes 23a, which are threadably coupled to one another. This may facilitate quick replacement when multiple replaceable first applicator heads 20 are employed.
As shown in fig. 5A, 5B and 5C, the grip lever 10 may include a corner 14 near its first end 11. The angle of the rotation angle may be 30-90 degrees. Fig. 5A shows a case where the grip lever 10 has a 30-degree turn angle, fig. 5B shows a case where the grip lever 10 has a 45-degree turn angle, and fig. 5C shows a case where the grip lever 10 has a 90-degree turn angle. In particular, in the bone wax application device according to the present application, the corner may be located near one third of the grip lever 10 near the first end 11, with degrees of bending of 30 degrees, 45 degrees, 90 degrees, respectively. The remainder of the grip lever 10 remains a straight lever. The grip lever 10 having a corner may have the following beneficial effects:
1. During operation, the hand of the operator cannot block the vision of other doctors (such as a main knife) because of the front bending design, so that the other doctors can conveniently and continuously finish the operation.
2. Because of different angle designs, different first smearing heads 20 can be completely selected according to the position, angle and size of each bone groove, the hands of an operator are ensured not to be required to be moved greatly, physical strength is reserved, the risk of double-hand pollution caused by in-operation displacement is avoided, and the risk of medical errors of other operators is avoided when the operator is in displacement.
3. Because of the corner design, when the bone wax is smeared, a downward acting force can be applied, so that the bone groove can be better sealed, and the compression is more uniform and sufficient.
In the case of a grip lever 10 with a corner, the application device according to the application will become a rake operation, which, as mentioned above, is more advantageous for the application of bone wax. However, during hemostatic procedures, additional force is facilitated by hand at the corners rather than directly to the bone groove by grasping the rod 10.
Referring to fig. 5A, 5B and 5C, the grip lever 10 of the applicator device according to the application may be divided into two sections, delimited by a dividing line 15. In the case of a two-piece grip lever 10, the application device according to the application is divided into two tools, namely two separate tools with the first application head 20 as the working head and the second application head 30 as the working head, for waxing and hemostasis of bone grooves and bone eyes, respectively. According to the requirements of the operation, the two sections of holding rods can be connected together, and the two sections of holding rods are changed into a smearing device with two ends being working heads. The connection of the two sections of gripping bars is a detachable connection and may comprise a plug-in connection, a threaded connection, a snap-in connection or any other connection possible.
The sectional design of the holding rod improves the use flexibility of the smearing device according to the application, and in the case of detachment, the device is two independent tools for waxing and hemostasis of bone grooves and bone eyes respectively; in the case of bonding, the two tools are bonded together, and the first applicator head 20 or the second applicator head 30 on one end thereof can be used to apply wax and stop bleeding, which increases the length of the grip lever. The segmented design of the grip lever thus provides the user (surgeon) with the option of splitting and combining, improving the functionality of the application device according to the application.
In order to avoid any damage to human tissue by the application device, corners of the first and second applicator heads 20, 30 may include rounded corners according to embodiments of the present application. The radius of the radius may be 0.5-1 mm.
According to embodiments of the present application, the grip lever 10, the first applicator head 20 and the second applicator head 30 may be integrally produced and may comprise the same material, including medical stainless steel, titanium alloy or medical plastic, for example. When the applicator device according to the application is designed for single use, it is preferably manufactured entirely from medical grade plastic, and any roughness or texture thereon is also formed at one time. When the applicator device according to the application is designed for multiple uses, it is preferably manufactured entirely from medical stainless steel material, with strict sterilization after each use for subsequent re-use. Alternatively, when the applicator device according to the present application is manufactured from medical stainless steel, any roughened or textured portion thereon may be realized from a medical plastic or medical rubber material coated thereon.
Preferably, according to an embodiment of the present application, the grip lever 10 and the second application head 30 may be integrally produced, the first application head 20 is separately produced and is connected to the first end 11 of the grip lever 10 by screw threads, and the first application head 20 includes three replaceable first application heads 20, the first working surface 21 and the second working surface 22 of which have an included angle of 30 degrees, 45 degrees or 60 degrees, respectively. In this case, it is preferable that the first end 11 of the grip lever 10 includes external threads 11a, and the first coupling surface 23 of the first application head 20 includes internal threaded holes 23a, and that the connection and disconnection between the grip lever 10 and the first application head 20 is achieved by the fitting and supporting of the external threads 11a and the internal threaded holes 23 a.
The bone wax application device according to the present application comprises: a grip lever 10 having a first end 11 and a second end 12 opposite the first end 11 and comprising a grip section 13 between the first end 11 and the second end 12 adapted for gripping by a physician; the first smearing head 20 is in the shape of a triangular frustum and comprises a first working surface 21, a second working surface 22 and a first combining surface 23, and the first combining surface 23 is connected with the first end 11 of the holding rod 10; and a second applying head 30 having a disc shape including a circular third working surface 31 and a second bonding surface 32 opposite to the third working surface 31, the second bonding surface 32 being connected to the second end 12 of the grip lever 10, the first and second working surfaces 21 and 22 of the first applying head 20 and the third working surface 31 of the second applying head 30 corresponding to physiological characteristics of bones of a portion to be coated with bone wax, respectively. The shape of the first coating head 20 corresponds to a bone groove, and the shape of the second coating head 30 corresponds to a bone eye, and in the case of holding the rod 10 by hand, the bone wax can be easily coated uniformly by the first coating head 20 or the second coating head 30 to seal the bleeding point, so that the operation is simple, and time and labor are saved. When the first and second working surfaces 21, 22 of the first application head 20 and the third working surface 31 of the second application head 30 comprise roughness, such as texture, friction between the bone wax and the first, second or third working surfaces 21, 22, 31 is increased, and bone wax falling off can be avoided. Because the first working surface 21 and the second working surface 22 of the first smearing head 20 form a proper included angle so as to be corresponding to the shape of the bone groove, and the shape of the third working surface 31 of the second smearing head 30 is corresponding to the bone eye, the first smearing head 20 can be well matched with the shape of the bone groove, the second smearing head 30 can be well matched with the shape of the bone eye, and the bleeding surface can be easily sealed by pressing bone wax, so that the operation speed is improved, the operation time is shortened, bleeding in operation is reduced, and the risk of injury to cervical spinal cord or nerve root caused by repeated operation of smearing the bone wax for multiple times is avoided. Furthermore, the third working surface 31 of the second smearing head 30 is easier to install bone wax, and can flexibly press the bleeding point to seal the bleeding point under uniform stress aiming at the bone eye bleeding point.
In addition, according to the situation that the bone groove may be cut into different shapes, the included angle between the first working surface 21 and the second working surface 22 of the first application head 20 may be made to be suitable for the same, for example, three types of 30 degrees, 45 degrees and 60 degrees, and different first application heads 20 or different bone wax application devices may be selected in the operation according to different cases, and the operations may be performed separately or simultaneously.
The above is only an exemplary embodiment of the present application and is not intended to limit the scope of the present application, which is defined by the appended claims.
