背景技术
结扎组织(例如,血管、淋巴结、神经、输卵管和心脏组织)是许多外科手术的惯常做法。这一做法的具体方式是,用手术夹闭合血管或用手术缝线缝合血管。使用手术缝线需要复杂地操作缝针和手术缝线来形成固定血管所需的结。这样复杂的操作十分耗时且执行困难,特别是在以空间和/或可见度有限为特点的内窥镜外科手术中。相比之下,手术夹相对更快又更易于施加。据此,在内窥镜和开放性外科手术中使用手术夹展现急剧增长。
发明内容
本发明人认识到需要改进施夹器和/或手术夹的一个或多个特征,例如手术夹在施夹器中的稳定性。通常由具有一对相对钳口的施夹器来施加手术夹。目前可购得的施夹器通常以例如相对钳口接合手术夹的远端上的凸台的两个接触点固定手术夹。然而,两个接触点无法为手术夹提供充分的稳定性,这可能在外科手术过程中导致手术夹相对于施夹器错位或甚至脱落。本发明的方法和系统旨在缓解或解决上述一个或多个问题和/或现有技术中的其他问题。

具体实施方式
现将结合附图阐述本发明,其中相同的附图标记通篇指代相同的元素。根据常规实践,如本文所用,术语“近侧部分”是指装置或其部件中一般更靠近操作或操纵装置按原样使用的医务人员的特定部分,而术语“远侧部分”应指装置或其部件中与近侧部分相反的特定部分,除非本文另作说明。
本发明总体上涉及一种施夹器,其配置成在医疗手术过程中提高手术夹的稳定性。施夹器可以包括部署在第一钳口构件与第二钳口构件之间的稳定构件。稳定构件以及第一钳口构件和第二钳口构件可以提供至少三个与手术夹的接触点,以防止手术夹在医疗手术过程中的相对运动。稳定构件可以具有从远侧部分延伸出的竖壁。竖壁可以定位在手术夹的近侧部分的两对两侧上并减少手术夹的横向移动。稳定构件具有竖壁的远侧部分的宽度可以大于从远侧部分延伸出的杆体的宽度。远侧部分的宽度增大可以增加手术夹在钳口构件之间的稳定性,并防止远侧部分通过施夹器的杆体缩回。稳定构件可以进一步具有在竖壁之间延伸并配置成减少竖向移动的侧向突起。
稳定构件可以偏置到至少部分位于第一钳口构件与第二钳口构件之间的第一远侧位置,以便于施夹器的正面装载。稳定构件可以在手术夹装载到第一钳口构件与第二钳口构件之间时接合手术夹。稳定构件也可以在手术夹在第一钳口构件与第二钳口构件之间送给到第二近侧位置时缩回。第二近侧位置可以至少部分位于第一钳口构件与第二钳口构件之间。稳定构件可以例如通过配置销和通道和/或增大远侧部分的宽度而纵向约束在第一位置与第二位置之间。销可以是第一钳口构件和第二钳口构件的枢轴销,并且通道可以纵向延伸穿过稳定构件的杆体。稳定构件可以从第一位置移动到第二位置并从第二位置移动到第一位置,但稳定构件不能移动到第一位置的远侧或第二位置的近侧。因此,稳定构件可以配置成:在从夹盒正面装载手术夹期间,当手术夹接纳在施夹器的第一钳口构件与第二钳口构件之间,施加足够的远侧稳定力。施夹器也可以配置成不用任何凸台也能保持手术夹。当将手术夹施加到组织上(例如,结扎血管),施夹器可以稳定手术夹,以防止手术夹拖尾。
图1至图4示出根据本发明配置成施加手术夹200的施夹器100。施夹器100可以包括近侧部分100A和远侧部分100B。施夹器100可以进一步包括手柄机构102、杆体104和钳口结构106。钳口结构106可以能枢转的方式连接到杆体104并由手柄机构102致动。
钳口结构106可以包括第一钳口构件108和第二钳口构件110,它们以能枢转的方式耦接于具有枢轴销114的铰接机构114。钳口结构106可以在第一钳口构件108与第二钳口构件110之间接纳手术夹200,并且第一钳口构件108和第二钳口构件110可以在第一夹脚构件202和第二夹脚构件204的远侧部分上的接触点稳定手术夹200。钳口构件108、110可以在远侧部分处具有接合表面111。接合表面111可以基本平坦和/或呈钩形并且可以配置成以能释放的方式接合手术夹200的夹脚构件202、204的远侧部分,而不具有凸台。

第一夹脚构件202和第二夹脚构件204中每一个夹脚构件也可以包括配置成接合施夹器100的第一钳口构件108和第二钳口构件110的接合表面228、230。如图6所示,接合表面228、230可以包括在第一夹脚构件202和第二夹脚构件204的外表面212、214上形成的基本平坦的面远表面。接合表面228、230可以配置成固定在施夹器100中时接合钳口构件108、110的接合表面111。因此,手术夹200可以由施夹器100固定,而不具有任何圆环形、圆形和/或侧向突出的凸台。稳定构件116施加的远向力引起手术夹200的形状配合联锁,因此施夹器100无需手术夹200的远端处的凸台。取消手术夹200上的凸台,能够缩小手术夹200的竖向和/或侧向轮廓,从而减小所需的空间和/或减少手术夹200造成的创伤。
第一夹脚构件202也可以包括尖端构件232,并且第二夹脚构件204可以包括弯钩构件234。当手术夹200闭合时,弯钩构件234可以围绕尖端构件232弯转,以将手术夹200固定在闭锁构型。在闭锁过程中,第一夹脚构件202和/或第二夹脚构件204可以伸直和/或伸长。第一夹脚构件202的接合表面228可以定位成紧邻尖端构件232,并且第二夹脚构件204的接合表面230可以定位成紧邻弯钩构件234。
图7示出根据本发明的第二实施方案的手术夹300。手术夹300可以包括连接于铰接部分306的第一夹脚构件302和第二夹脚构件304。第一夹脚构件302可以具有带凸曲的内表面308,并且第二夹脚构件304可以具有带凹曲的内表面210。第二夹脚构件304的凹曲可以配置成在闭锁和/或闭合构型时接纳第一夹脚构件302的凸曲。每个内表面308、310可以进一步包括多个齿部324、326,它们配置成进一步将组织固定在第一夹脚构件302与第二夹脚构件304之间。
第一夹脚构件302和第二夹脚构件304中每一个夹脚构件也可以包括配置成接合施夹器100的第一钳口构件108和第二钳口构件110的接合表面328、330。如图6所示,接合表面328、330可以包括在第一夹脚构件302和第二夹脚构件304的外表面312、314上形成的基本平坦的面远表面。接合表面328、330可以配置成固定在施夹器100中时接合钳口构件108、110的接合表面111。因此,如上所述,手术夹200可以由施夹器100固定,而不具有任何凸台。
第一夹脚构件302也可以包括尖端构件332,并且第二夹脚构件304可以包括弯钩构件334。当手术夹300闭合时,弯钩构件334可以围绕尖端构件332弯转,以将手术夹300固定在闭锁构型。在闭锁过程中,第一夹脚构件302和/或第二夹脚构件304可以伸直和/或伸长。第一夹脚构件302的接合表面328可以定位成紧邻尖端构件332,并且第二夹脚构件304的接合表面330可以定位成紧邻弯钩构件334。
图8A至图8B示出从夹盒400装载手术夹200的施夹器100。图8A示出施夹器100接合定位在夹盒400中的手术夹200时的剖视图,而图8B示出施夹器100从夹盒400中移取手术夹200不久之后的等距视图。如图所示,施夹器100与手术夹200之间的三点接合增加手术夹200的安全性。手术夹200上的两个接触点出现在第一夹脚构件202和第二夹脚构件204的远侧表面,而第三个接触点出现在手术夹200的近侧部分(例如铰接部分206)。当拾取手术夹200时,弹簧构件118的压缩可以通过稳定构件116提供远向力,以便尽管有外力,手术夹200也保持形状配合地接合在稳定构件116与第一钳口构件108和第二钳口构件110的接合表面111之间。仅当施夹器100完全闭合和/或闭锁手术夹200时,手术夹200才脱离第一钳口构件108和第二钳口构件110的接合表面111。然后,第一钳口构件108和第二钳口构件110可以打开并移开,以将手术夹200留在适当位置。尽管图2、图3和图8A至图8B示出装载有手术夹200的施夹器100,但容易理解,施夹器100可以与手术夹300一起使用。
本发明的手术夹200、300的各种实施方案可以制成任何合适的尺寸并且可以应用于任意多个组织,例如血管、淋巴结、神经、输卵管或心脏组织。手术夹200、300的各种实施方案可以由任何合适的生物相容材料构成,例如金属和聚合物。但实践中,本发明特别适用于聚合物夹。因此,手术夹200、300的各种实施方案优选由适合的强生物相容性工程塑料形成的单件式整体聚合体组成,如通常用于外科植入物的类型。示例性材料包括均聚物或共聚物聚缩醛、聚对苯二甲酸乙二醇酯(PET)、聚对苯二甲酸丁二醇酯(PBT)、聚甲醛或具有类似性质的其他热塑性材料,这些材料可以注塑、挤出或以其他方式加工成类似制品。
本发明的许多特征和优点参阅本说明书将显而易见,因此所附权利要求旨在涵盖本发明落入本发明真实精神与范围内的全部特征和优点。此外,本领域技术人员容易设想许多修改和变型,因此本发明不局限于图示和描述的确切构造和操作,可以采用本发明范围内的全部适当修改和等效方案。
Claims (20)
1.一种配置成将手术夹施加到组织的施夹器,所述施夹器包括:
配置成接合手术夹的第一钳口构件和第二钳口构件;以及
配置成接合手术夹的稳定构件,所述稳定构件配置成相对于所述施夹器从至少部分位于所述第一钳口构件与所述第二钳口构件之间的第一位置纵向移动到至少部分位于所述第一钳口构件与所述第二钳口构件之间的第二位置,所述稳定构件具有杆状部分和位于杆状部分的横向相反两侧的第一侧壁和第二侧壁,所述第一侧壁和第二侧壁在所述第一钳口构件和所述第二钳口构件之间竖向延伸,并配置成在其间容纳所述手术夹并且定位在所述手术夹的横向相反两侧上以在横向上稳定手术夹,
其中,所述稳定构件的纵向移动约束为所述第一位置与所述第二位置之间的纵向移动。
2.根据权利要求1所述的施夹器,其中,所述稳定构件配置成随着所述第一钳口构件和所述第二钳口构件压缩所述手术夹而缩回。
3.根据权利要求1或2所述的施夹器,进一步包括:铰接销,所述铰接销配置成以能枢转的方式固定所述第一钳口构件和所述第二钳口构件,其中,所述稳定构件具有配置成容纳所述铰接销的纵向通道,并且所述铰接销配置成将所述稳定构件的纵向移动约束在所述第一位置与所述第二位置之间。
4.根据权利要求1或2所述的施夹器,其中,所述稳定构件进一步包括近侧部分上的管状部分,所述杆状部分在所述管状部分远侧,并且所述稳定构件的远侧部分的宽度大于所述杆状部分的宽度。
5.根据权利要求1或2所述的施夹器,进一步包括:定位在所述稳定构件的近侧部分上的弹簧,所述弹簧配置成将所述稳定构件偏压到所述第一位置,所述第一位置位于所述第二位置远侧。
6.根据权利要求5所述的施夹器,进一步包括:固定到所述第一钳口构件和所述第二钳口构件的近端的杆体,其中,所述杆体沿其长度包括至少一个细长狭槽,并且通过所述至少一个细长狭槽可见所述弹簧。
7.根据权利要求5所述的施夹器,进一步包括:邻接所述弹簧的近端的管状构件,其中,所述弹簧的远侧部分邻接所述稳定构件的近侧部分上的管状部分。
8.根据权利要求1或2所述的施夹器,其中,所述稳定构件包括远侧部分上的多个突起,并且所述多个突起配置成竖向上稳定所述手术夹。
9.根据权利要求1或2所述的施夹器,其中,所述第一钳口构件配置成接合所述手术夹的第一夹脚构件的远侧部分,所述第二钳口构件配置成接合所述手术夹的第二夹脚构件的远侧部分,并且所述稳定构件配置成接合所述手术夹的近侧部分。
10.根据权利要求9所述的施夹器,其中,所述第一钳口构件配置成接合所述第一夹脚构件的远侧部分上的第一基本平坦表面,并且所述第二钳口构件配置成接合所述第二夹脚构件的远侧部分上的第二基本平坦表面。
11.一种将根据权利要求1所述的施夹器装载手术夹的方法,所述方法包括:
将所述手术夹的近侧部分接纳在所述施夹器的所述第一钳口构件与所述第二钳口构件之间;
将所述手术夹的近侧部分接合到所述第一位置中的所述稳定构件;
将所述稳定构件纵向缩回;以及
将所述手术夹稳定在所述稳定构件与所述第一钳口构件和所述第二钳口构件之间。
12.根据权利要求11所述的方法,进一步包括将所述稳定构件的纵向移动约束为所述第一位置与所述第二位置之间的纵向移动。
13.根据权利要求11或12所述的方法,其中,稳定手术夹包括借助所述稳定构件的远侧部分上的第一壁和第二壁在横向上稳定手术夹。
14.根据权利要求11或12所述的方法,其中,稳定手术夹包括借助所述稳定构件的远侧部分上的多个突起在竖向上稳定手术夹。
15.根据权利要求11或12所述的方法,进一步包括:从夹盒中拾取手术夹。
16.一种用于施夹器的稳定构件,所述稳定构件包括:
具有近侧部分和远侧部分的细长本体;
所述细长本体的远侧部分上的竖向第一侧壁和竖向第二侧壁,竖向所述第一侧壁和竖向所述第二侧壁配置成在其间容纳手术夹的近侧部分并且定位在横向相反两侧上以在横向上稳定所述手术夹的近侧部分;
设置在所述第一侧壁和所述第二侧壁之间的远侧表面;以及
所述细长本体从所述远侧部分延伸出的杆状部分,其中,
包括所述第一侧壁和所述第二侧壁的远侧部分的宽度大于所述杆状部分的宽度,并且所述第一侧壁和所述第二侧壁位于杆状部分的横向相反两侧。
17.根据权利要求16所述的稳定构件,进一步包括:延伸穿过所述细长本体的纵向通道。
18.根据权利要求16或17所述的稳定构件,其中,所述杆状部分具有大致矩形的横截面。
19.根据权利要求16或17所述的稳定构件,进一步包括:所述细长本体的管状部分形成所述近侧部分。
20.根据权利要求16或17所述的稳定构件,进一步包括:所述远侧部分上的多个突起,所述多个突起配置成在竖向上稳定所述手术夹的近侧部分。
