Description

Vascular anastomosis fixing and supporting device and operation method thereof
Technical Field
The invention belongs to the technical field of medical vascular suturing lap joint anastomosis instruments, and particularly relates to a vascular anastomosis fixing and supporting device and an operation method thereof.
Background
Vascular anastomosis is a basic operation form clinically, and comprises vascular injury repair, vascular transplantation, vascular lap joint, vascular suturing, repair and the like, and is a commonly used effective treatment method for reconstructing blood flow. The application is very wide, and is commonly used in various surgical operations, such as replantation of the severed limb, vascular accidental injury, organ transplantation, anastomosis of arteriovenous fistula and the like. With the continuous development of microsurgery instruments, microsurgery lines and operation microscopes, the manual small (micro) vessel anastomosis technology is gradually mature, and the caliber of anastomosed vessels is gradually reduced.
However, it is not easy for even experienced surgeons to perform a perfect vascular anastomosis, particularly the small microvascular anastomosis mentioned above. The following takes intracranial vascular suturing as an example to see the difficulties faced: firstly, the surrounding environment where the blood vessels to be anastomosed are relatively complex, a plurality of nerves/fibers, various brain tissues and the like are densely distributed around any blood vessel, meanwhile, the blood vessels are immersed in the cerebrospinal fluid/blood and mixed liquid composed of cleaning liquid, nutrient solution and the like which are continuously injected from outside in the operation process, the blood vessels to be operated with similar colors are not easy to separate, visual fatigue is caused by long-time operation, and the risk of misoperation is increased; the diameters of the blood vessels to be anastomosed are smaller, the blood vessels are different from a few tenths of a millimeter to two or three millimeters, the blood vessels become soft and smooth after the blood flow is clamped, the devices can move when in contact, the positions are difficult to fix, and the suturing is difficult; in addition, while the patient is in a dormant or semi-dormant state, the beating of the heart is still transmitted to various parts of the whole body, particularly cerebral vessels and nerve parts, and is in an unstable state during anastomosis operation, and the operator needs to take great care in suturing operation to avoid puncturing nearby tissues and free nerves and vessels; finally, it is critical that the blood vessel and associated tissue be deprived of nutrient supply after occlusion, and that this condition be too long in duration, resulting in deactivation and even complete necrosis, which is highly desirable for the duration of the suturing procedure. For very fine anastomosis procedures which need to be carried out under a microscope, the external conditions are very unfavorable for judgment of operators, so that the operation difficulty of medical staff is increased, and the operation efficiency and quality are affected.
At present, some flexible sheets or gauzes are usually adopted to assist suture operation, and although partial problems can be solved, the efficiency is still low, the function is single, and the operation is not standard.

The method of operation of the vascular anastomosis fixation support device 10 provided by the present invention will be described in connection with two different embodiments of the blood vessel to be anastomosed.
Referring to fig. 6, in a first embodiment of the present invention, a method for operating a vascular anastomosis fixation support device 10 in a 1mm diameter mini-vascular end anastomosis comprises the steps of:
Separating two blood vessels A and B to be anastomosed, wherein the two ends respectively block blood flow, and the end face 34 to be anastomosed of the blood vessel A and the end face 35 to be anastomosed of the blood vessel B are as same as possible, or can be trimmed into inclined planes to enlarge the anastomotic stoma area, or an internal supporting mode is adopted to obtain similar suture end faces; or can be placed into a vascular support plug to assist in expanding the blood vessel to be anastomosed;
The vascular anastomosis fixing and supporting device 10 provided by the invention is placed at the lower part of a blood vessel to be anastomosed and isolated from other tissues or liquid in the environment where the blood vessel to be anastomosed is positioned;
The positions of the two blood vessels are adjusted to be positioned on the supporting cambered surface 16 of the convex supporting part 15;
The first positioning movable end 28 of the first positioning strip 25 and the second positioning movable end 30 of the second positioning strip 26 are respectively adjusted by an instrument to be bent to be matched with the blood vessel to be anastomosed, the position of the blood vessel to be anastomosed is limited flexibly, and the blood vessel to be anastomosed is prevented from tilting, laterally moving and axially moving in the suturing process;
The position of the vascular anastomosis fixing and supporting device 10 is adjusted by pulling the through hole 31 of the bending part 12, so that the bending part 12 is supported on nearby tissues or instruments such as gauze as much as possible, and the operation position is stabilized; or by means of the through hole 31 of the instrument holding bending part 12.
The liquid 36 at the operation position can be led out through the drainage tube 17 before or during operation, so that the operation platform is in a stable state, that is, the surface of the liquid 36 is level with or slightly lower than the upper surface 13 of the main body supporting part 11; specifically, when the liquid at the blood vessel to be anastomosed, such as cerebrospinal fluid/blood, is continuously increased or heparin saline is continuously required to wash the operation area in operation, so that the liquid level rises to the top surface of the raised support portion 15, the liquid 36 can be drained out of the body by the drainage tube 17 (refer to fig. 7) to avoid the blood vessel from being infiltrated, so that the operation position is kept on a stable plane or the anastomosed part is kept in a relatively dry environment. It will be appreciated that this drain 17 may not be installed if the surgical environment is relatively free of too much fluid disturbance.
When the external injection of the cleaning liquid or the nutrient solution causes the unstable vascular anastomosis fixing and supporting device 10 due to the impact or the rising of the liquid level, the vascular anastomosis fixing and supporting device 10 can conduct the upper liquid and the lower liquid through the overflow holes 20 around the convex supporting part 15, and can play a role of balancing the whole platform.
The medical staff can judge whether the blood vessel size and the position move or not according to the relative positions of the transverse scale 32 and the longitudinal scale 33 on the fixed supporting device 10 for vascular anastomosis during suturing, so that the operation and adjustment are convenient and timely.
Referring to fig. 8, a second embodiment of the present invention is a method for operating a vascular anastomosis fixation support device 10 in 2mm diameter mini-vascular side anastomosis, comprising the steps of:
The operation of the side anastomosis of the second embodiment is similar to the end anastomosis of the first embodiment, except that the placement position and the suturing position of the two blood vessels to be sutured of the side anastomosis are different from the end anastomosis, so that the positioning method can be adjusted as required.
Two blood vessels A and B to be anastomosed are separated, blood flow is blocked by clamping two ends of an anastomosed part by a blood vessel clamp, and a corresponding longitudinal blood vessel A incision 37 and a corresponding longitudinal blood vessel B incision 38 are made. The two suture blood vessels are placed in parallel and are close to each other, the incision positions are placed oppositely, and the direction is slightly upward according to the requirement; or can be placed into a vascular support plug to assist in expanding the blood vessel to be anastomosed;
The first positioning movable end 28 of the first positioning strip 25 and the second positioning movable end 30 of the second positioning strip 26 are respectively adjusted by an instrument and bent to be matched with the blood vessels to be anastomosed, and the difference is that one blood vessel or two blood vessels can be respectively fixed at two sides of the anastomosed blood vessels at the moment according to the requirement;
the position of the vascular anastomosis fixing and supporting device 10 is adjusted by pulling the through hole 31 of the bending part 12, so that the bending part 12 is supported on nearby tissues or instruments such as gauze as much as possible, and the operation position is stabilized; the working pad position can also be fixed by means of the through hole 31 of the clamping bending part 12.
The liquid at the operation position can be led out through the drainage tube 17 before or during operation, so that the operation platform is in a stable state; it will be appreciated that this drain 17 may not be installed if the surgical environment is relatively free of too much fluid disturbance.
When the vessel anastomosis fixing and supporting device 10 is unstable due to impact or liquid level elevation caused by the injection of the cleaning liquid or the nutrient solution from the outside, the overflow hole 20 of the vessel anastomosis fixing and supporting device 10 conducts the liquid at the upper part and the lower part, so that the effect of balancing the whole platform can be achieved.
The medical staff can judge whether the blood vessel size and the position move or not according to the relative positions of the transverse scale 32 and the longitudinal scale 33 on the fixed supporting device 10 for vascular anastomosis during suturing, so that the operation and adjustment are convenient and timely.
In particular, in the second embodiment, the length X of the protruding support portion 15 should be greater than the sum of diameters of two blood vessels, and the width Y of the protruding support portion 15 should be greater than the incision length of the blood vessels.
The invention has at least the following advantages:
the middle part of the upper surface of the main body supporting part extends upwards to form a convex supporting part, namely the convex supporting part is higher than the upper surface of the main body supporting part, so that the sutured blood vessel can be stably placed and is not immersed in liquid such as cerebrospinal fluid or blood nearby, and the sutured blood vessel can be completely exposed in the sight of an operator, and is convenient to operate; bending the first positioning movable end of the horizontally arranged first positioning strip and the second positioning movable end of the second positioning strip to be matched with the blood vessel to be anastomosed by using an instrument, wherein the position of the blood vessel to be anastomosed is limited flexibly, and the blood vessel to be anastomosed is prevented from tilting, laterally moving and axially moving in the suturing process; the vascular anastomosis fixing and supporting device can conduct upper and lower liquid through overflow holes around the convex supporting part, and can play a role in balancing the whole platform; the transverse graduations and the longitudinal graduations are arranged, so that the information such as the diameter of the blood vessel, the relative positions of the two blood vessels, the anastomosis size and the like can be provided for an operator, accurate judgment is facilitated, and the quality of anastomosis operation is improved; the vascular anastomosis fixing and supporting device is made of flexible materials, has the density smaller than that of water and has certain supporting force; the color is set to be green or vivid color which is obviously different from blood vessels; the reflecting lining plays a role of a contrast substance with the transparent vessel wall in the clamping state, is convenient to distinguish, provides a clear visual field, is convenient to operate, and reduces the risk of misoperation.
